Double multielectrode mapping catheters facilitate radiofrequency catheter ablation of focal atrial fibrillation originating from pulmonary veins.

INTRODUCTION: Several reports have demonstrated that focal atrial fibrillation (AF) may arise from pulmonary veins (PVs). The purpose of this study was to investigate the safety and efficacy of using double multielectrode mapping catheters in ablation of focal AF. METHODS AND RESULTS: Forty-two patients (30 men, 12 women, age 65+/-14 years) with frequent attacks of paroxysmal AF were referred for catheter ablation. After atrial transseptal procedure, two long sheaths were put into the left atrium. Two decapolar catheters were put into the right superior PV (RSPV) and left superior PV (LSPV),

[Repetitive monomorphic ventricular tachycardia (Gallavardin type): clinical and electrophysiological characteristics in 20 patients].

Repetitive monomorphic ventricular tachycardia (RMVT) is defined by the presence of numerous monomorphic isolated, premature ventricular complexes, couplets, and runs of unsustained ventricular tachycardia having the same morphology in patients without structural heart disease. Patients with RMVT mostly demonstrate the typical left bundle branch block morphology with normal or rightward axis during tachycardia. At our institution, 20 patients with RMVT have been systematically studied: a syncope had occurred in 35% of our patients, in three cases a syncope was the first manifestation of the RM

Effects of isoflurane on electrophysiological measurements in children with the Wolff-Parkinson-White syndrome.

This study was designed to assess the effects of isoflurane (ISO) on the electrophysiological properties of the accessory pathway, atrium, ventricle, and AV node in children with the Wolff-Parkinson-White (WPW) syndrome. The results of programmed electrical stimulation were analyzed in 51 patients (4 months to 17 years of age) with WPW. The study population was divided into two groups. Twenty-seven patients received local anesthesia and intramuscular injection of meperidine, promethazine, and chlorpromazine (MPC group). Twenty-four patients received general anesthesia with ISO inhalation (ISO

Perinodal cryosurgery for atrioventricular node reentry tachycardia in 23 patients.

Atrioventricular node reentry tachycardia is the most common cause of paroxysmal supraventricular tachycardia. Available nonpharmacologic therapies include (1) catheter ablation or cryosurgical ablation of the His bundle and insertion of a permanent pacemaker and (2) surgical dissection around the atrioventricular node or discrete cryosurgery of the perinodal tissues, in an attempt to divide or ablate only one of the dual atrioventricular node conduction pathways responsible for the tachycardia while leaving the other intact. This report describes 23 consecutive patients who underwent the disc

Guanethidine adrenergic neuropathy: an animal model of selective autonomic neuropathy.

Chronic administration of guanethidine to adult rats induces a selective autoimmune adrenergic neuropathy. Physiological and biochemical features of this disorder in the peripheral nervous system were explored in young adult Sprague-Dawley rats given daily intraperitoneal guanethidine monosulfate for 5 weeks. Control rats received daily saline injections. The guanethidine-treated animals gained less weight, had ptosis, and had a lower mean arterial blood pressure in the supine and upright tilted positions. Norepinephrine was depleted in the peroneal, sural, tibial, and vagal nerves, the nutrie

Evaluation and treatment of survivors of sudden cardiac death or nonbradycardic syncope unrelated to coronary artery disease.

The authors describe their clinical strategy and experience with electrophysiologic testing in a group of patients who have had at least one episode of sudden cardiac death or nonbradycardic syncope in the absence of coronary artery disease. Their study shows that for this population programmed stimulation commonly induces ventricular tachycardia, which can then be suppressed by drug therapy.

Erectile dysfunction, type 2 diabetes, and cardiovascular disease: a narrative review and insights from a global real-world cohort analysis.

BackgroundErectile dysfunction (ED) is highly prevalent among men with type 2 diabetes mellitus (T2DM) and reflects systemic vascular and metabolic dysfunction. Shared mechanisms include endothelial dysfunction, oxidative stress, inflammation, autonomic neuropathy, and hypogonadism. Therefore, ED may function not only as a complication of T2DM but also as an early clinical marker of cardiometabolic disease.ObjectiveThis narrative review summarizes current evidence on the epidemiology, mechanisms, and cardiometabolic implications of ED in men with T2DM, and evaluates the impact of major cardiom

Approach to the Diagnosis and Management of Complex Fascicular Ventricular Tachycardias.

Complex ventricular tachycardias involving the fascicular system (fascicular ventricular tachycardias [FVTs]) can be challenging. In this review, we describe our approach to the diagnosis and ablation of these arrhythmias with 10 illustrative cases that involve (1) differentiation from supraventricular tachycardia; (2) assessment for atypical bundle branch reentry and other interfascicular FVTs; (3) examination of P1/P2 activation sequences in sinus rhythm, pacing, and tachycardia; and (4) entrainment techniques to establish the tachycardia mechanism and aid circuit localization. To summarize,

Effect of naloxone on haemodynamic responses to acute blood loss in unanaesthetized rabbits.

1. We have tested the hypothesis that the pressor action of (-) naloxone HC1 after haemorrhage is due to antagonism of endogenous opiate mechanisms that are activated by haemorrhage, rather than to some more direct vasoconstrictor action of the drug. 2. Six conscious rabbits were treated intravenously with either naloxone (4 mg kg-1, then 0.1 mg kg-1 min-1) or equivalent volumes of saline. In unbled rabbits the naloxone regimen had no effect except to cause a transient bradycardia. After each treatment the rabbits were bled at a rate of 2.45 ml kg-1 min-1 until blood pressure fell to 40 mmHg o

Connections Reduce Rheumatic Heart Disease-Related Mortality in Western Australia: A Mixed Methods Study.

INTRODUCTION: Preventable rheumatic heart disease (RHD) deaths continue to occur in Australia, with Aboriginal people disproportionately affected. Despite research into structural drivers and the lived experience of people with RHD, and national guidelines focusing on RHD prevention and treatment, recent coronial inquests have highlighted that systemic failures are ongoing. Few studies describe RHD service delivery and/or mortality within the Western Australian (WA) context. OBJECTIVE: This study aimed to integrate quantitative information regarding RHD-related deaths in WA between 2012 and 2

Effects of ventricular conduction block patterns on mortality in hospitalized patients with dilated cardiomyopathy: a single-center cohort study.

BACKGROUND: Ventricular conduction blocks (VCBs) are associated with poor outcomes in patients with known cardiac diseases. However, the prognostic implications of VCB patterns in dilated cardiomyopathy (DCM) patients need to be evaluated. The purpose of this study was to determine all-cause mortality in patients with DCM and VCB. METHODS: This cohort study included 1119 DCM patients with a median follow-up of 34.3 (19.5-60.8) months, patients were then divided into left bundle branch block (LBBB), right bundle branch block (RBBB), intraventricular conduction delays (IVCD) and narrow QRS grou

OBJECTIVE: The secundum type of atrial septal defect (ASD) is a common congenital cardiac anomaly. Transcatheter closure of the defect is the preferable therapeutic approach to avoid potential future complications. This work extends the data collected so far regarding the electrocardiographic, morphologic, hemodynamic, and mechanical changes following the procedure. DESIGN: From April 2002 to October 2003, 42 consecutive patients underwent successful transcatheter closure of secundum type ASD. They were sampled for different parameters by transthoracic echocardiography before the procedure an

A molecular pathway including Id2, Tbx5, and Nkx2-5 required for cardiac conduction system development.

The cardiac conduction system is an anatomically discrete segment of specialized myocardium that initiates and propagates electrical impulses to coordinate myocardial contraction. To define the molecular composition of the mouse ventricular conduction system we used microdissection and transcriptional profiling by serial analysis of gene expression (SAGE). Conduction-system-specific expression for Id2, a member of the Id gene family of transcriptional repressors, was identified. Analyses of Id2-deficient mice demonstrated structural and functional conduction system abnormalities, including lef

Personalized intensity modulation radiation therapy auto-planning for esophageal cancer.

Esophageal cancer (EC) is among the most common malignancies worldwide, with radiotherapy as a key treatment. Intensity-modulated radiation therapy (IMRT) offers highly conformal dose distributions while sparing critical organs; however, planning is complex due to the esophagus' central location and proximity to vital structures. This study aimed to develop a personalized, automated IMRT planning system for EC. A retrospective analysis of 301 patients treated with 45 Gy in 25 fractions was conducted. Patient data, including CT images, structure sets, and clinical plan (CP) dose distributions,

The impact of cardiovascular events in bronchiectasis: a systematic review and meta-analysis.

BACKGROUND: Bronchiectasis is a chronic respiratory condition characterised by airway and systemic inflammation with prevalence increasing with age. Given the median age of the patients, it is common to observe the presence of comorbidities, particularly cardiovascular diseases, which have been linked to adverse clinical outcomes. To investigate the pooled estimates of the association between bronchiectasis and coronary heart disease or stroke within this population, we conducted a systematic review and meta-analysis of the available scientific evidence. METHODS: Three investigators independe
