Recurrent postoperative pulmonary hemorrhage complicated by asphyxia-induced cardiac arrest successfully managed with extracorporeal membrane oxygenation in an infant: a case report.

This report describes an infant who experienced two distinct episodes of postoperative pulmonary hemorrhage after a Rastelli-type procedure, each driven by a different mechanism requiring tailored management. The initial right-sided pulmonary hemorrhage was attributed to excessive flow through major aortopulmonary collateral arteries (MAPCAs). Although hemostasis could not be achieved by coil embolization alone, selective temporary occlusion using balloon catheter limited the bleeding to a more segmental distribution. Despite airway obstruction leading to cardiac arrest, bleeding did not worse

Thoracolaparoscopic Esophagectomy for Severe Post-Corrosive Esophageal Stricture in a Young Adult: Implications for Minimally Invasive Surgical Practice.

BACKGROUND Post-corrosive esophageal stricture (PCES) is a severe condition resulting from caustic ingestion that often leads to progressive dysphagia and requires surgical intervention after endoscopic treatment failure. Although minimally invasive esophagectomy is widely used in oncologic practice, its role in PCES remains limited due to the technical challenges posed by dense fibrosis and distorted anatomy. CASE REPORT We present the case of a 21-year-old man with severe PCES after accidental ingestion of potassium permanganate (KMnO₄). The patient developed progressive dysphagia despite re

Early Platelet Dysfunction in Sepsis: An ICU Pilot Study.

Platelets are critical for hemostasis and play an active role in immune responses to infection. While thrombocytopenia in sepsis is associated with poor outcomes, platelet dysfunction remains less explored. This prospective observational pilot study investigated the relationship between platelet dysfunction and sepsis severity using multiple platelet function tests. Ten adults with sepsis or septic shock admitted to the ICU of "Fondazione Policlinico Universitario A. Gemelli" and seven healthy controls were enrolled. Blood samples were collected at admission (T0), after 48 h (T1), and after 7

Liver Embolization for Subcapsular Hematoma in HELLP (Hemolysis, Elevated Liver Enzymes, and Low Platelets) Syndrome: A Case Report.

HELLP syndrome is a severe obstetric complication characterized by hemolysis, elevated liver enzymes, and low platelet count, typically occurring in the third trimester or postpartum period. One of its rare but life-threatening complications is subcapsular hepatic hematoma, which may rupture and cause catastrophic hemorrhage. We report the case of a 38-year-old woman with type 2 diabetes mellitus who developed severe right upper quadrant pain three days after a cesarean delivery complicated by eclampsia. Laboratory findings were consistent with HELLP syndrome. Abdominal ultrasound and contrast

Uterine Preservation Following Catastrophic Hemorrhage From an Undiagnosed First-Trimester Placenta Increta: A Case Report and Review of the Literature.

Placenta accreta spectrum (PAS) in the first trimester is a rare but potentially fatal condition. Preoperative diagnosis during early pregnancy remains extremely challenging, particularly in individuals with a previous cesarean delivery. We describe the case of a 35-year-old woman (gravida 3, para 2) with two prior cesarean deliveries who presented at eight weeks of gestation for the management of a missed abortion. Despite her high-risk status, PAS was not suspected on initial ultrasound. Medical induction with misoprostol resulted in sudden massive hemorrhage and hemorrhagic shock 12 hours l

Designing cryogels through cryostructuring of β-chitin nanofibers to reinforce polyelectrolyte complexes for non-compressible hemorrhage control.

Non-compressible hemorrhage remains a major clinical challenge, requiring hemostatic materials with rapid blood absorption and sufficient mechanical robustness. Conventional carboxymethyl chitosan (CMCS)/sodium alginate (SA) polyelectrolyte complex (PEC) sponges exhibit high swelling capacity but limited uptake kinetics and mechanical strength. Here, β-chitin nanofibers (β-ChNFs) were incorporated into the CMCS/SA system to establish a freeze-thaw reinforcement strategy depended on β-ChNFs. Repeated freeze-thaw cycles had little effect on the storage modulus (G') of CMCS/SA alone, whereas G' p

Acute Appendicitis With Periappendiceal Abscess Caused by Migration of an Endoscopic Clip Into the Appendix: A Case Report.

Acute appendicitis is a common surgical emergency, most often caused by luminal obstruction. However, appendicitis resulting from the migration of an endoscopic clip is extremely rare. Because non-absorbable foreign bodies are unlikely to be expelled spontaneously from the appendix, their presence has important implications for management. An 82-year-old woman presented with right lower abdominal pain. Four years earlier, she had undergone colonoscopic polypectomy with endoscopic clipping for hemostasis. Contrast-enhanced computed tomography revealed a periappendiceal abscess and a high-densit

Influence of Hemostatic Disorder on Type II Endoleak Development After Endovascular Abdominal Aortic Aneurysm Repair.

Endovascular aneurysm repair (EVAR) is a widely used minimally invasive treatment for abdominal aortic aneurysms. However, postoperative type II endoleak (T2EL) remains a relevant complication associated with a risk of aneurysm rupture and the need for repeated imaging follow-up, resulting in exposure to ionizing radiation. Identification of biological factors predisposing to T2EL may improve risk stratification. This pilot study aimed to investigate whether disturbances in hemostasis are associated with early T2EL development after EVAR. A total of 103 patients treated with EVAR for symptomat

Rare Causes of Portal Vein Thrombosis and Occlusion: A Narrative Review.

Portal vein thrombosis (PVT) refers specifically to the presence of a thrombus within the main portal vein trunk or its intrahepatic branches. In contrast, portal vein occlusion encompasses a broader spectrum of conditions, including tumor invasion, external compression and disorders that predispose to thrombosis, such as thrombophilia or inflammatory states. Advanced liver disease, particularly cirrhosis, is the most common cause of PVT, primarily due to portal hypertension, altered hemostasis and hemodynamic changes, followed by malignancies and inherited or acquired thrombophilic conditions

Human placenta as a biological vascular model for endovascular training.

INTRODUCTION: Simulation-based training in endovascular procedures remains limited by the availability of biological vascular models that provide tactile feedback. Existing animal, cadaveric, synthetic, and virtual simulators incompletely reproduce the mechanical behavior of complex neurovascular anatomy. METHODS: Human placentas were prepared by cannulation of the umbilical vessels, vascular flushing, and establishment of continuous gravity-driven flow. Vessel curvature was modified to simulate challenging neurovascular anatomy, including carotid siphon-like configurations, and aneurysm anal

Ruptured pulmonary arteriovenous malformation causing hemothorax during pregnancy: a case report.

BACKGROUND: Pulmonary arteriovenous malformation (PAVM) is a rare pulmonary vascular anomaly. During pregnancy, physiological hemodynamic changes and hormonal fluctuations substantially increase the risk of PAVM rupture. Spontaneous hemothorax secondary to ruptured PAVM is extremely uncommon and often presents with nonspecific manifestations, frequently resulting in misdiagnosis and delayed treatment. CASE PRESENTATION: A 33-year-old woman at 31+ 6 weeks of gestation presented to the emergency department with a 30-minute history of sudden-onset left-sided chest pain radiating to the shoulder

Percutaneous direct carotid puncture using an elastic needle for coil embolization of a ruptured cerebral aneurysm with right-sided aortic arch and carotid stenosis: illustrative case.

BACKGROUND: Endovascular treatment of ruptured intracranial aneurysms can be particularly challenging in patients with a right-sided aortic arch and concomitant carotid artery stenosis, where stable catheterization via conventional transfemoral routes is often impossible. Direct carotid puncture is a potential alternative. However, conventional sheaths can transiently occlude the carotid artery and increase the risk of cerebral ischemia. OBSERVATIONS: An 87-year-old patient with subarachnoid hemorrhage due to a ruptured intracranial aneurysm presented with a right-sided aortic arch and severe

Heart-Specific Spinal and Vagal Afferents: Transcriptomic Signatures and Optogenetically Modulated Functional Coupling With Cardiomyocytes.

Sensory neurons innervating the heart transmit chemical and mechanical cues to the central nervous system via the dorsal root ganglion (DRG) and nodose ganglion (NG). Despite their importance in cardiac pain and cardiovascular reflexes, the molecular and functional properties of heart-specific sensory (HS) neurons remain elusive. Here, DRGHS and NGHS neurons innervating the heart were FACS purified using a retrograde labeling strategy with Di-8-ANEPPQ, then characterized molecularly by bulk RNA sequencing or evaluated functionally in cocultures with neonatal cardiomyocytes. DRGHS and NGHS neur

Measurement of Active Thrombin Bound to Circulating D-Dimers as a Sensitive Biomarker for Prothrombotic Conditions.

BACKGROUND: Thrombin activity plays a crucial role in hemostasis, and it is tightly regulated by a system of activators and inhibitors. Active, unregulated thrombin may initiate clotting without previous vessel injury, causing thrombosis. Thrombin bound to fibrin participates in fibrinogen transformation, while its inhibition by the naturally occurring heparin-antithrombin complex is impeded. Bound thrombin activity has been detected on fibrin degradation products. METHODS: D-dimers were isolated using a commercial ELISA kit. Mass spectrometry was used to confirm the presence of thrombin on c

Intraoperative factors associated with de novo overactive bladder after Holmium Laser enucleation of the prostate.

OBJECTIVES: HoLEP markedly attenuates symptoms in the majority of patients with BPH. However, a subset develops de novo OAB postoperatively, which may significantly compromise QOL. Despite its clinical relevance, evidence regarding the incidence of and perioperative factors contributing to de novo OAB after HoLEP remains limited. METHODS: The present study included 536 patients who underwent HoLEP between April 2013 and September 2023. Among them, 175 patients without preoperative OAB were eligible for analysis. RESULTS: De novo OAB was identified in 18 of 175 patients (10.3%). Baseline demo

Posterior nasal hemorrhage mimicking intra-abdominal hemorrhage after gastrectomy: a case report.

BACKGROUND: Postoperative hemorrhage is a serious complication following gastrectomy, most commonly resulting from anastomotic bleeding or pseudoaneurysm rupture. When conventional diagnostic modalities fail to identify the source of bleeding, establishing an accurate diagnosis can be extremely challenging. CASE PRESENTATION: We report the case of a 77-year-old Asian woman diagnosed with gastric malignancy who underwent Roux-en-Y total gastrectomy in our department. On postoperative day 6, she developed an esophagojejunal anastomotic leak. On postoperative day 28, the patient experienced hemo

New and novel pharmacotherapies for hemophilia A: an update.

INTRODUCTION: Hemophilia A (HA) management has progressed significantly, yet standard and extended half-life (EHL) clotting factors present persistent limitations, including intravenous burden and suboptimal trough levels. AREAS COVERED: This narrative review highlights recently approved agents and phase III candidates for HA. In replacement therapy, efanesoctocog alfa maintains normal-to-near-normal factor VIII (FVIII) levels weekly by bypassing endogenous von Willebrand factor dependence. In non-replacement therapies, the focus centers on rebalancing agents-the anti-tissue factor pathway in

Microparticles Released by Dengue Virus-Infected Monocytes Mediate Endothelial Activation and Vasculopathy.

Dengue is the most prevalent arthropod-borne viral disease, caused by infection with the dengue virus (DENV). Severe dengue is characterized by significant vasculopathy involving a proinflammatory and procoagulant state associated with increased vascular permeability. However, the host-virus interactions driving this process remain incompletely elucidated. Monocytes (Mø) are primary target cells during DENV infection and actively release extracellular vesicles, like microparticles (MPs), mediating intercellular communication, contributing to dengue pathogenesis. Here, we evaluated whether MPs
