Effect of stride length on the running biomechanics of healthy women of different statures.

BACKGROUND: Tibial stress fracture is a debilitating musculoskeletal injury that diminishes the physical performance of individuals who engage in high-volume running, including Service members during basic combat training (BCT) and recreational athletes. While several studies have shown that reducing stride length decreases musculoskeletal loads and the potential risk of tibial injury, we do not know whether stride-length reduction affects individuals of varying stature differently. METHODS: We investigated the effects of reducing the running stride length on the biomechanics of the lower ext

Intertrochanteric fracture with low-energy trauma in a young woman with anorexia nervosa: A case report.

RATIONALE: Anorexia nervosa is a chronic psychiatric disease defined by severe weight loss, due to fear of obesity, and self-imposed semi-starvation. Of the many complications following anorexia nervosa, low bone mineral density (BMD) is a significant risk factor for fractures. Anorexia nervosa is associated with higher risk of incident fracture in females across all age groups, and in males >40 years old. Sites at highest risk of fracture include the hip/femur and pelvis in females, and vertebrae in males with anorexia nervosa. PATIENT CONCERNS: A 29-year-old woman known to have suffered fro

Total Distance Walked After Hip Surgery Identifies Older Patients With Sarcopenia.

This study aims to determine the optimal cut-off point for total walking distance on the day of hospital discharge to identify older adults at high risk of sarcopenia following hip fracture surgery. A prospective observational study included 135 patients aged > 65 years who underwent hip fracture surgery. Logistic regression models were used to predict probable or confirmed sarcopenia based on hospital stay data. Cut-off points were determined using the Youden index, with internal validation through 10-fold cross-validation. The mean age of participants was 81.6 years (SD 7.6), and 72% were wo

[Osteosynthesis of geriatric acetabular fractures].

The aim of the treatment of acetabular fractures is principally an anatomical reconstruction of the joint surface to avoid posttraumatic arthritis. The increasing number of geriatric patients with acetabular fractures of the anterior column type resulting from simple falls on the side and substantial patient-related inherited comorbidities, represent particular challenges for the surgical approach and osteosynthesis due to the osteoporotic bone quality. Due to the establishment of minimally invasive lateral pelvic accesses to the acetabulum and the adaptation of the osteosynthesis technique an

Reverse wedge effect following intramedullary nail fixation of trochanteric fracture, what does it imply?

Lag screw cut-out is the most common cause of fixation failure of trochanteric fractures. Intraoperative assessment of fracture reduction and fixation quality is vital to avoid fracture reduction and achieve good functional outcomes. In a recent study, Zhang et al. reported the occurrence of a reverse wedge effect after intraoperative nail insertion based on a new computed tomography(CT)-guided fracture classification system, which specifically happened to the basicervical facture type and resulted in valgus deformity with gapping at the medial inferior fracture line. Impingement between the r

Arthroscopic treatment of Pipkin type I femoral head fractures: a report of 2 cases.

Pipkin type I fractures historically have required open treatment, with the potential-associated morbidity. Two cases of arthroscopic fracture fragment excision are described. These were completed using standard techniques and introducing the application of Steinman pins, through an accessory anterior portal, for capture of the fragment. Both the patients had satisfactory outcomes. We conclude that hip arthroscopy is a valuable option for the treatment of Pipkin I fractures.

Injuries about the hip in the adolescent athlete.

Athletic injuries in or around the hip in the adolescent athlete encompass possible causes such as a single, traumatic event to those of repetitive microtrauma. The injuries may involve the bone or the soft tissues, with former involving the epiphysis, apophysis, metaphysis, or diaphysis, whereas the latter includes muscles and tendons. With the improvements in surgical technique and instrumentation for hip arthroscopy and the development of magnetic resonance arthrography, clinicians have been able to diagnose and treat labral tears, hip instability, snapping hip, loose bodies, chondral injur

The Stability of Fixation of Vancouver B2 Periprosthetic Femoral Fractures: Effect of Implantation Technique.

BACKGROUND: Due to increasing volume of total hip arthroplasties, periprosthetic femoral fractures have become a common complication with increased revision burden and perioperative morbidity. The objective of this study was to evaluate the fixation stability of Vancouver B2 fractures treated with 2 techniques. METHODS: A common B2 fracture was created by reviewing 30 type B2 cases. The fracture was then reproduced in 7 pairs of cadaveric femora. The specimens were divided into 2 groups. In Group I ("reduce-first"), the fragments were reduced first, followed by implantation of a tapered flute

"It's part of getting old": ageing perceptions and cultural beliefs underpin hip recovery and refracture prevention experiences: a qualitative study.

BACKGROUND AND AIMS: Hip fractures are a leading cause of morbidity and mortality among older adults, with significant consequences for recovery and refracture risk. While recovery experiences have been explored and interventions established, limited understanding exists of how individual, social and cultural factors influence engagement with recovery and secondary prevention in multicultural systems. We explored these factors among culturally diverse older adults and caregivers following low trauma hip fracture. METHODS: A qualitative study embedded within a prospective cohort at a metropoli

Assessment of Force Applied During Examination Under Anesthesia Used to Determine Stability in Posterior Wall Acetabular Fractures.

OBJECTIVES: Determination of hip instability associated with posterior wall acetabular fractures may be difficult. Thus, dynamic stress examination under anesthesia (EUA) was developed as a tool for guiding treatment. EUA uses positioning of the hip and application of force across the hip to detect instability. While aspects of the EUA technique seems consistently described in the literature and practiced by surgeons, some components are ill-defined. The goal of this study was to assess standardization of applied force during EUA among experienced acetabular surgeons. METHODS: Fellowship-trai
