Factitious Total Triiodothyronine(T3) Elevation in a Euthyroid Patient With Paraproteinemia

Abstract Background: We report a rare case of a patient treated with levothyroxine for hypothyroidism who also had paraproteinemia and was found to have a clinically inconsistent elevation of T3 by RIA. Clinical Case: A 72 Year old African American female with a history of hypothyroidism and IgG kappa Multiple Myeloma (MM) was admitted to the hospital for altered mental status. Her hypothyroidism had been well controlled for years on a stable dose of levothyroxine 0.075mg daily. Review of systems were negative pertinent to thyroid dysfunction. Family history: negative for thyroid disease. On p

Cellular electropharmacology of amiodarone.

The complex profile of amiodarone actions on the electrophysiological properties of cardiac cells reviewed in this article may be summarized as follows. As acute effects, amiodarone inhibits both inward and outward currents. The inhibition of inward Na+ and Ca2+ currents is enhanced in a use- and voltage-dependent manner, resulting in suppression of excitability and conductivity in both iNa- and iCa-dependent cardiac tissues. The inhibition is greater in the tissues stimulated at higher frequencies, and in those with less negative resting (or diastolic) membrane potentials. As outward currents

Defective thyroidal iodine concentration in protein-calorie malnutrition.

Although protein-calorie malnutrition (PCM) is known to result in various abnormalities of thyroid function, the exact relationship between the two is not clearly understood. Therefore, the thyroid function of 10 men, 13-55 yr of age, with severe PCM was studied in a clinical research ward before and 3-4 months after protein-calorie repletion. Before repletion, all subjects had low serum T4 (mean +/- SEM, 5.1 +/- 0.5 micrograms/dl) and T3 (74 +/- 6 ng/dl) concentrations. Eight subjects were chemically euthyroid, and their free T4 (1.5 +/- 0.1 ng/dl) and serum TSH (2.9 +/- 1.4 microU/ml) values

Johanson--blizzard syndrome.

Johanson-Blizzard syndrome is an extremely rare ectodermal dysplastic disorder characterized by aplasia or hypoplasia of alae nasi, midline scalp defects, growth retardation, varying degrees of mental retardation, hypothyroidism, exocrine pancreatic insufficiency and congenital deafness. This condition is supposed to be an autosomal recessive disorder. We are reporting a female neonate with the characteristic features and an uncommon less emphasized feature viz. cafe-au-lait spots.

Autoimmune thyroiditis and a rapidly progressive dementia: global hypoperfusion on SPECT scanning suggests a possible mechanism.

We report the clinical, laboratory, EEG, and SPECT findings in a 59-year-old euthyroid woman with previously undiagnosed autoimmune thyroiditis, subclinical hypothyroidism, and rapidly progressive dementia. We made a diagnosis of Hashimoto's encephalopathy based on elevated thyrotropin, abnormal EEG, and clinical improvement after thyroid hormone replacement. SPECT demonstrated global hypoperfusion with normalization on clinical recovery, suggesting a possible mechanism for the pathogenesis of Hashimoto's encephalopathy.

Hyperthyroidism and Down syndrome.

Down syndrome has long been associated with a variety of autoimmune conditions. A significant link has also been described between Down syndrome and autoimmune thyroid disease, usually hypothyroidism. We report two cases of Down syndrome with Graves disease both with significantly elevated titres of thyrotropin receptor antibody and the presence of the HLA Bw46. We also comment on the problems in diagnosis and management of such patients.

Practical management of hyperthyroidism.

There are several causes of hyperthyroidism, and correct diagnosis is essential for management. Graves' disease is most commonly managed with radioactive iodine therapy (131I), antithyroid drugs or surgery. Toxic adenomas (single or multiple) may be treated with 131I or surgery. Most types of thyroiditis are managed expectantly. Pregnant women, children and the elderly deserve special consideration. Follow-up is vital to identify the later development of hypothyroidism.

Pregnancy in three patients with hypergonadotropic ovarian failure.

The authors present three women with early hypergonadotropic ovarian failure. In one patient autoimmune aetiology of the disease was suggested by the co-existing Addison's disease and idiopathic hypothyroidism. In the other two women the causes of hypogonadotropic amenorrhea remained unclear. All three women were delivered of normal children: one after two spontaneous pregnancies and the other two after a pregnancy induced by short-term estrogen therapy.

Thyrotoxic thyroiditis after radiotherapy for Hodgkin's disease.

Exposure of the thyroid gland to ionizing radiation has been associated with a variety of abnormalities. Among these are tardive hypothyroidism and an increased risk of developing thyroid nodules and cancer. Although acute thyroiditis has been known to complicate radioactive iodine 131 therapy, it has rarely been associated with external beam irradiation. Thyrotoxic painless thyroiditis developed in two patients after mantle-field irradiation for Hodgkin's disease.

Causal effect of thyroid function on cortical brain structure: A two-sample Mendelian randomization study.

BACKGROUND: The causal relationship between variations in thyroid dysfunction and brain structure remains elusive. We intended to probe into this causal connection using a Mendelian randomization (MR) approach. METHODS: Aggregated statistics from the genome-wide Association Study (GWAS) on Autoimmune thyroid dysfunction were obtained from the FinnGen database, including autoimmune hyperthyroidism (N = 2,190) and autoimmune hypothyroidism (N = 49,055). Aggregate statistics on cortical area and thickness for GWAS were obtained from the Enhancing NeuroImaging Genetics through Meta-Analysis (ENIG

Exploring the Connection Between Thyroid Health and Psychiatric Disorders: A Comprehensive Review With a Focus on Schizophrenia and Bipolar Disorder.

The interplay between thyroid dysfunction and major psychiatric disorders, particularly schizophrenia and bipolar disorder, represents a critical area of research with significant implications for diagnosis and management. Thyroid dysfunction manifests as either hyperthyroidism or hypothyroidism, each exerting distinct effects on psychiatric conditions. This bidirectional relationship underscores the need for a comprehensive understanding to enhance clinical outcomes. This comprehensive review explores the underlying mechanisms linking thyroid dysfunction and psychiatric disorders, including n

Clinical outcomes and functional preservation of microwave ablation versus surgical resection for benign thyroid nodules: a propensity score-matched retrospective cohort study.

BACKGROUND: Microwave ablation (MWA) is an organ-preserving minimally invasive option for symptomatic benign thyroid nodules. Comparative evidence on thyroid function preservation after MWA versus surgical resection (SR) remains limited, particularly when postoperative hormone replacement is separated by surgical extent. METHODS: We retrospectively analyzed 113 consecutive adults treated with ultrasound-guided MWA or SR for symptomatic or cosmetically concerning cytologically benign thyroid nodules between January 2022 and October 2024. Patients with abnormal preoperative thyroid function, hy

Importance of iodide sufficiency and normal thyroid function in fertility and during gestation.

Appropriate management of thyroid dysfunction in pregnancy is challenging in both its primary, secondary and tertiary forms of the disease. Primary hypothyroidism is by far most prevalent globally. Main causes are insufficiency of iodide supplementation in developing countries and autoimmunity in developed countries. However, after a very successful global implementation by World Health Organisation over decades accompanied by specific recommendations for management of the iodide supplementation during pregnancy, recent studies found that women both in USA and EU are again mild to moderately i

International overview on juvenile-, adult- and elderly-onset rheumatoid arthritis: The age at disease onset as a fundamental determinant of clinical presentation.

BACKGROUND: Elderly-onset rheumatoid arthritis (EORA) may have peculiar findings compared to juvenile-onset RA (JORA). The aim of the work was to present and compare the clinical characteristics of RA patients with JORA and elderly-onset EORA to a group of cases with adult-onset (AORA) and to contrast the findings worldwide. METHODS: The study included 1100 adult RA patients: 209 JORA and 329 EORA, compared with 562 AORA extracted from a big data national study on 10,364 RA patients. Clinical characteristics, laboratory investigations, medications received, and co-morbidities were recorded. T
