Clinical and antibody responses after influenza immunization in systemic lupus erythematosus.

After immunization with A/New Jersey/76 and A/Victoria/75 influenza vaccines, 11 patients with systemic lupud erythematosus were serially evaluated for changes in disease activity, serologic abnormalities, and their capability to generate specific antibodies. One patient, with active disease, developed a diffuse, proliferative glomerulonephritis. None of the other patients or control subjects had significant local or systemic side effects. Significant levels of antibodies were generated to A/New Jersey/76 in eight of the 11 patients and in seven of eight control subjects and to A/Victoria/75 i

Mechanisms and therapeutic prospects of DNA methylation-mucosal innate immunity crosstalk in inflammatory bowel disease.

Inflammatory bowel disease (IBD) comprises a group of chronic and relapsing intestinal inflammatory disorders whose pathogenesis and progression are closely associated with disruption of the intestinal mucosal barrier, dysregulated immune responses, and altered epigenetic regulation. The innate immune system is a central component of mucosal host defense and plays a pivotal role in pathogen recognition, inflammatory signal transduction, immune-cell functional regulation, and maintenance of barrier homeostasis. In recent years, DNA methylation has been increasingly recognized as an important me

Pancytopenia due to lepromatous involvement of the bone marrow: successful treatment with multidrug therapy.

Leprosy is a chronic infectious disease with a wide spectrum of signs and symptoms depending on the ability of the host's immune system to resist the infection. The disease is frequently associated with sensory loss in skin lesions and damage in peripheral nerve trunks leading to nerve function impairment. In lepromatous leprosy, the immune system offers no protection against the multiplying bacilli and this results in heavy infiltration of the internal organs. We report a case of florid lepromatous leprosy with bone marrow suppression due to the disease, presenting with anemia, leukocytopenia

Cholangiopathy with respect to biliary innate immunity.

Biliary innate immunity is involved in the pathogenesis of cholangiopathies in cases of biliary disease. Cholangiocytes possess Toll-like receptors (TLRs) which recognize pathogen-associated molecular patterns (PAMPs) and play a pivotal role in the innate immune response. Tolerance to bacterial PAMPs such as lipopolysaccharides is also important to maintain homeostasis in the biliary tree, but tolerance to double-stranded RNA (dsRNA) is not found. Moreover, in primary biliary cirrhosis (PBC) and biliary atresia, biliary innate immunity is closely associated with the dysregulation of the peridu

Protective immune response against Rhodococcus equi: An innate immunity-focused review.

Rhodococcus equi causes pyogranulomatous pneumonia in foals and immunocompromised people. Despite decades of research efforts, no vaccine is available against this common cause of disease and death in foals. The purpose of this narrative review is to summarise the current understanding of interactions between R. equi and the host innate immune system, to describe features of the immune response that are associated with resistance or susceptibility to R. equi infection, and help guide strategies for developing novel approaches for preventing R. equi infections. Virulence of R. equi in foals has

Systemic Chronic Active Epstein-Barr Virus Disease.

Epstein-Barr virus is a ubiquitous gammaherpesvirus that usually causes an asymptomatic infection followed by lifelong persistence in memory B cells. Virus replication is controlled by a robust antiviral immune response, and EBV-associated lymphoproliferative diseases only develop in immunocompromised individuals. However, systemic chronic active Epstein-Barr virus (CAEBV) disease occurs in individuals without an apparent immunodeficiency or an underlying genetic immune defect. These individuals cannot control EBV infection, leading to life-threatening conditions including haemophagocytic lymp

Shaping the spectrum - From autoinflammation to autoimmunity.

Historically, autoimmune-inflammatory disorders were subdivided into autoinflammatory vs. autoimmune diseases. About a decade ago, an immunological continuum was proposed, placing "classical" autoinflammatory disorders, characterized by systemic inflammation in the absence of high-titer autoantibodies or autoreactive T lymphocytes, at the one end, and autoimmune disorders at the other end. We provide an overview of recent developments and observations, filling in some of the gaps and showing strong interconnections between innate and adaptive immune mechanisms, indicating that disorders from b

[Strategy of persistence (facts, hypotheses)].

The universal phenomenon of virus persistence is considered to be a regular phenomenon primarily meeting the interests of the host. Examinations of 1112 patients with various diseases of the nervous system revealed a certain dependence of macroglobulin measles antibody upon the character of the disease and the intensity of demyelinization process. These data do not suggest the etiological role of the persisting measles virus in the genesis of multiple sclerosis and a large group of other nervous system diseases (SSPE). Similar results were obtained in analogous examinations of patients for mac

Role of the NF-kappaB axis in immune modulation of osteoclasts and bone loss.

NF-kappaB is a vital component of the molecular programs for immune cell development and activation, inflammatory responses, and osteoclast differentiation. This transcriptional regulatory family is activated by diverse immunological and inflammatory stimuli and contributes to both positive feedback of the immune and osteolytic responses as well as their resolution. The ubiquilous expression of NF-kappaB components in osteoclasts and other immune cells creates an opportunity to gain a better understanding of the complex interplay between the immune and skeletal systems in physiological and pat

Unmasking a B-Cell Linkage Phenotype in Severe Acute Meningoencephalitis: A Case Report of IVIG and HydroBox-assisted Immunomodulatory Therapy.

BACKGROUND/AIM: Severe acute meningoencephalitis is a life-threatening neurological condition frequently associated with excessive inflammation, immune dysregulation, and secondary immune dysfunction. Immune exhaustion and abnormal immune cell remodeling may contribute to persistent inflammation and impaired recovery. Molecular hydrogen has emerged as a potential adjunctive therapy due to its selective antioxidant and immunomodulatory properties. CASE REPORT: A previously healthy 30-year-old male developed fulminant acute meningoencephalitis complicated by coma, respiratory failure, and sever

A potential therapeutic target: The role of neutrophils in the central nervous system.

Neutrophils play a critical role in immune defense as the first recruited and most abundant leukocytes in the innate immune system. As such, regulation of neutrophil effector functions have strong implications on immunity. These cells display a wide heterogeneity of function, including both inflammatory and immunomodulatory roles. Neutrophils commonly infiltrate the central nervous system (CNS) in response to varied pathological conditions. There is still little understanding of the role these cells play in the CNS in such conditions. In the present review, we will summarize what is known of n

Immunologic abnormalities accompanying acute and chronic viral infections.

A number of viral infections induce profound alterations in immune function; these changes may or may not be related to direct infection of lymphoid cells. The immunologic abnormalities that occur during acute systemic viral infections such as measles or infectious mononucleosis include suppression of responses to tuberculin skin tests in vivo and suppression of mitogen responses in vitro, evidence of activation of the immune system with up-regulation of activation-associated cell-surface markers and soluble cellular products, and evidence of altered immune regulation with autoimmune disease a

Acute infections of the central nervous system in children and adults: diagnosis and management.

Central nervous system infections are due to different microorganisms such as viruses, bacteria, mycobacteria, fungi, amoebas, and other parasites. The etiology depends on multiple risk factors, and it defines the infection location because some microorganisms prefer meninges, brain tissue, cerebellum, brain stem or spinal cord. The microorganisms induce diseases in the nervous system through direct invasion, neurotoxin production, and the triggered immune response. To determine the infection etiology, there are several diagnostic tests which may be conducted with cerebrospinal fluid, blood, r

The Role of the Plasminogen/Plasmin System in Inflammation of the Oral Cavity.

The oral cavity is a unique environment that consists of teeth surrounded by periodontal tissues, oral mucosae with minor salivary glands, and terminal parts of major salivary glands that open into the oral cavity. The cavity is constantly exposed to viral and microbial pathogens. Recent studies indicate that components of the plasminogen (Plg)/plasmin (Pm) system are expressed in tissues of the oral cavity, such as the salivary gland, and contribute to microbial infection and inflammation, such as periodontitis. The Plg/Pm system fulfills two major functions: (a) the destruction of fibrin dep

Effect of T cells on bone.

Bone and immune systems mutually influence each other by sharing a variety of regulatory molecules and the tissue microenvironment. The interdisciplinary research field "osteoimmunology" has illuminated the complex and dynamic interactions between the two systems in the maintenance of tissue homeostasis as well as in the development of immune and skeletal disorders. T cells play a central role in the immune response by secreting various immune factors and stimulating other immune cells and structural cells such as fibroblasts and epithelial cells, thereby contributing to pathogen elimination a

Minimizing Ischemia Reperfusion Injury in Xenotransplantation.

The recent dramatic advances in preventing "initial xenograft dysfunction" in pig-to-non-human primate heart transplantation achieved by minimizing ischemia suggests that ischemia reperfusion injury (IRI) plays an important role in cardiac xenotransplantation. Here we review the molecular, cellular, and immune mechanisms that characterize IRI and associated "primary graft dysfunction" in allotransplantation and consider how they correspond with "xeno-associated" injury mechanisms. Based on this analysis, we describe potential genetic modifications as well as novel technical strategies that may

Use of psychotropic drugs in patients with HIV infection.

Psychotropic drugs are frequently employed to treat the wide range of neuropsychiatric syndromes that patients infected with the human immunodeficiency virus (HIV) may develop. In order to administer these agents properly, physicians should take certain factors into account: the central nervous systems of these patients are often impaired, the patients tend to suffer from medical illnesses, and they may be taking various other drugs. The possible interactions between substances taken by these patients may sometimes make it necessary to adjust the dosage of psychotropic agents administered. In

Alox12/15 Deficiency Exacerbates, While Lipoxin A4 Ameliorates Hepatic Inflammation in Murine Alcoholic Hepatitis.

Alcoholism is one of the leading and increasingly prevalent reasons of liver associated morbidity and mortality worldwide. Alcoholic hepatitis (AH) constitutes a severe disease with currently no satisfying treatment options. Lipoxin A4 (LXA4), a 15-lipoxygenase (ALOX15)-dependent lipid mediator involved in resolution of inflammation, showed promising pre-clinical results in the therapy of several inflammatory diseases. Since inflammation is a main driver of disease progression in alcoholic hepatitis, we investigated the impact of endogenous ALOX15-dependent lipid mediators and exogenously appl

Therapeutic Time Migration in Hepatocellular Carcinoma: Rethinking the Timing of Systemic Therapy Across the Disease Continuum.

The management of hepatocellular carcinoma has traditionally followed a stage-dependent sequence in which surgery or thermal ablation is prioritized for early disease, transarterial approaches are used for intermediate-stage tumors, and systemic treatment is introduced after progression to advanced disease or loss of suitability for locoregional therapy. This paradigm was developed when systemic treatments had limited antitumor activity. Modern immune checkpoint inhibitors and immune-antiangiogenic combinations have substantially changed these premises. Randomized trials now demonstrate clinic

Epipharyngeal Abrasive Therapy (EAT) Field Theory: A Clinical Framework for EAT in Modulating Psycho-Neuro-Endocrino-Immune (PNEI) Dynamics.

Chronic epipharyngitis/chronic nasopharyngitis has traditionally been understood as a localized inflammatory disease, but clinically, it often presents with systemic symptoms that span the nervous, immune, and endocrine systems, such as headaches, fatigue, low-grade fever, impaired concentration, orthostatic dysregulation, joint pain, anxiety, and sleep disorders. Epipharyngeal abrasive therapy (EAT) is a treatment that applies chemical and physical stimulation to the mucous membranes of the epipharynx/nasopharynx using a cotton swab soaked in a 1% zinc chloride solution. Recently, there have
