The economic burden of tuberculosis care for patients and households in Africa: a systematic review.

OBJECTIVES: To summarise the state of knowledge on the economic impact and consequences of tuberculosis (TB) diagnosis and treatment for patients/households in Africa, and to highlight any weaknesses in the work conducted to date. METHODS: We systematically searched for published articles in English between 1990 and June 2010 in eight databases and the World Health Organization (WHO) website. Broad search terms were used ('tuberculosis' OR 'tuberculosis/HIV' AND 'costs' AND 'Africa'). Only studies that reported any costs of TB care for patients/households were retained. All costs were convert

Update on Rehabilitation After Stroke: Global Changes and the Continued Importance of Therapy Intensity, Dose, and Timing.

Stroke rehabilitation research has seen an unprecedented expansion in the volume of randomized controlled trials. This sets the stage for conducting meta-analyses and network meta-analyses, which help compensate for the large number of smaller phase 2 studies. Over half of randomized controlled trials are conducted in the chronic phase when neuroplasticity is reduced, and the timing of the intervention is not directly generalizable to clinical care. Meanwhile, research productivity has plateaued in high-income countries over the past decade. Therapy intensity and dosage are regarded as importa

A global, proxy-based assessment of building climatization vulnerability.

Building compactness and construction epoch are relevant proxies of energy efficiency in buildings. By integrating building stock and Earth observation data, we characterize buildings globally and identify vulnerable areas dominated by old building age and unsuitable compactness in relation to climate conditions. Such areas are home to a population of 48 million persons that could increase by 10% to 30% by 2100 under likely socioeconomic pathways, shifting from cold regions of Eurasia to the heat-prone Sahel zone, Gulf countries and Pakistan. Results reveal stark disparities in building energy

Design and technical validation of a low-cost, titanium sheet metal TORP (SM-T) for manufacturing and use in LMICs.

OBJECTIVES: Total ossicular replacement prostheses are commonly used to restore hearing in the conductive hearing impaired. However, total ossicular replacement prostheses are expensive and scarcely available in low- and middle-income countries. Therefore, the aim was to develop and validate an open-source design of a total ossicular replacement prosthesis that can be produced locally for low- and middle-income countries hospitals. METHODS: Based on input from literature and clinicians a design was made and verified. Accuracy, re-bending safety, cadaver ear fitting, sound transmission and pro

Bridging the Gap: Implementing cardiac rehabilitation programmes in pakistan's healthcare system.

Cardiac rehabilitation (CR) is a secondary prevention programme aimed at reducing the mortality and frequency of recurrent cardiac events and contributes to improvement in quality of life of the patients suffering from cardiovascular diseases (CVD). While the healthcare system around the world has revolutionized, provision of CR services is still lacking in its implementation. According to statistics, < 54% of the countries actually provide CR and this percentage drops at an alarming rate of about 8% and 28% in low and middle income countries (LICs/LMICs) respectively, even though the need of

Lean on me: effects of social support on low socioeconomic-status pregnant women.

This study identified how close relationships are related to low-income pregnant women's ability to cope and overall health. Previous research has shown that stress during pregnancy is related to long-term negative physical and psychological health outcomes for both the mother and the infant. Lower socioeconomic status has been related to higher morbidity and mortality across the lifespan. Women typically rely on close relationships for social support to help reduce stress. However, stress levels can be elevated when women engage in co-rumination. Co-rumination is defined as excessive problem

Population mobility and disease progression in people living with HIV: a machine learning analysis of a 10-year dynamic cohort.

BACKGROUND: Inter-regional population mobility poses challenges to the residence-based HIV follow-up system. This study aimed to define source heterogeneity among PLHIV (local incident versus incoming migrant) and to evaluate its predictive value on out-migration (spatial mobility) and its association with disease progression using machine learning models. METHODS: A dynamic longitudinal cohort (N = 5,213) was constructed from monthly follow-up data spanning 116 months (2016-2025) in Chongqing, China. An XGBoost model was developed to predict out-migration risk during management, with the SHA

Regional inequality in infant RSV morbidity and mortality burden in the era of expanded RSV passive immunisation: a global projection modelling study.

BACKGROUND: Respiratory syncytial virus (RSV) is a major cause of acute lower respiratory infection (ALRI) in infants, with low- and middle-income countries (LMICs) bearing a disproportionately high burden. While novel RSV prophylactic products including long-acting monoclonal antibodies and a maternal vaccine are being rolled out for protecting the general infant population in high-income countries, implementation in LMICs is only expected by 2028, potentially exacerbating health inequities. We aimed to assess regional inequality in infant RSV disease burden with and without the implementatio
