Anesthesia management with single injection paravertebral block for aorta coarctation in infant.

BACKGROUND: Thoracotomy causes severe pain in the postoperative period. Perioperative thoracic paravertebral block reduces pain score and may improve outcome after pediatric cardiac surgery. This prospective study was designed for the efficacy and duration of a single level, single injection ultrasound-guided thoracic paravertebral block (TPVB) for fifteen infants undergoing aortic coarctation repair. METHODS: After approval of the ethical committee and the relatives of the patients, 15 infants who had undergone thoracotomy were enrolled in the study. The patients received 0.5 ml·kg(-1) a bol

Bannayan-Riley-Ruvalcaba syndrome with deforming lipomatous hamartomas in infant--case report.

Bannayan-Riley-Ruvalcaba Syndrome is a rare condition caused by mutations in the PTEN gene. It displays association of multiple lipomas, macrocephaly, hemangiomas, hamartomatous intestinal polyposis, developmental delay and speckled pigmented maculae on the male genitalia. We report the case of a nine-month-old boy who had fast growing and progressive tumors for three months, macrocephaly and lentigines on the penis. Imaging tests showed extensive lipomatosis with invasion of paraspinal muscles, enlargement of the spinal canal and spinal cord compression; after surgical excision of the mass, t

Acquired bartter-like syndrome associated with colistin use in a preterm infant.

Acquired Bartter-like syndrome (BLS), characterized by hypokalemic metabolic alkalosis, hypomagnesemia, hypocalcemia, and normal kidney function, can be induced by diuretics or antibiotics. It is a very rare condition and only anecdotal cases mostly in adults were reported. Although tubulopathy associated with colistin was reported in adults, to the best of our knowledge, colistin-associated BLS neither in adults nor in children has been reported in the literature. We here report a-28-week, 740 g female preterm infant who developed BLS just after colistin treatment for Acinetobacter baumannii

Acute kidney injury after acute gastroenteritis in an infant with hereditary hypouricemia.

UNLABELLED: Hereditary hypouricemia is a rare disorder characterized by extremely low serum uric acid levels caused by excessive urinary excretion due to an inherited tubular defect in urate handling. Exercise-induced acute kidney injury (AKI) is the main complication of this disorder, though AKI may also be induced by other factors. A 7-month-old boy with hereditary hypouricemia developed AKI associated with severe dehydration caused by rotavirus gastroenteritis. He also showed severe hypernatremia and metabolic acidosis and received continuous renal replacement therapy for 3 days. He showed

Remarkably delayed occurrence of normal surfactant composition in an extremely preterm infant.

Reported herein is the first case of a remarkably delayed occurrence of normal surfactant composition in an extremely preterm infant who required a total of 15 doses of artificial pulmonary surfactant (Surfacten®). A male infant, born at 26 weeks gestation, developed respiratory distress at birth. Chest radiography was consistent with respiratory distress syndrome. The infant required repeated doses of surfactant, each resulting in transient periods of decreased ventilator requirement and improved blood gas values. Surfactant proteins (SP)-A, SP-B, SP-C, and SP-D from tracheal aspirate samples

Forecasting respiratory collapse: theory and practice for averting life-threatening infant apneas.

Apnea of prematurity is a common disorder of respiratory control among preterm infants, with potentially serious adverse consequences on infant development. We review the capability for automatically assessing apnea risk and predicting apnea episodes from multimodal physiological measurements, and for using this knowledge to provide timely therapeutic intervention. We also review other, similar clinical domains of respiratory distress assessment and prediction in the hope of gaining useful insights. We propose an algorithmic framework for constructing discriminative feature vectors from physio

Development of icterus gravis in a preterm infant with G71R UGT1A1 polymorphism.

BACKGROUND: Uridine diphosphate-glucuronosyltransferase (UGT) gene family is involved in the detoxification of biomaterials and drugs in the liver. Among the UGT gene family members, only UGT1A1 is involved in bilirubin conjugation. As a result, deficient UGT1A1 activity causes jaundice. One disease that is characterized by reduced UGT1A1 activity is Gilbert's syndrome. Two prevalent UGT1A1 polymorphisms responsible for Gilbert's syndrome have been identified: G71R in exon 1 and A(TA)7TAA in the TATA box of the promoter region. Recently, the G71R polymorphism has been associated with breastfee

Extremely low birth weight and infant mortality rates in the United States.

OBJECTIVE: Infant mortality rates (IMR) and neonatal mortality rates (NMR) in the United States have not decreased recently. The purpose of this study was to determine the contributions of birth weight and gestational age subgroups to the IMR and NMR in the United States. METHODS: We used the most recent (1983-2005) US linked birth and infant death data and simple regression analysis to determine the contributions of specific birth weight and gestational age subgroups to trends in IMR and NMR. RESULTS: IMR and NMR decreased between 1983 and 2005 for all birth weight and gestational age subgr

Glial choristoma of the temporal bone in a 7-month-old infant.

IMPORTANCE: Heterotopic glial tissue is a rare, benign congenital anomaly that can present as a fibrous mass with or without overt signs of neural tissue origin, such as overlying alopecia, pigmentation, or a "hair collar" sign. These lesions are typically found in the midline area, though they have been found laterally involving the mastoid and middle ear regions. As demonstrated by this case report, a lateral scalp lesion may represent glial tissue even if fine-needle aspiration does not demonstrate neural findings. OBSERVATIONS: This single case report represents a 7-month-old white infant

[Acrodermatitis enteropathica-like disease in a full-term exclusively breast-fed infant].

UNLABELLED: Symptomatic zinc deficiency can occur in exclusively breast-fed infants. We report a case in a 5-month-old infant. CASE REPORT: A 5-month-old exclusively breast-fed boy presented with persistent erythema of breech since the age of 2 months as well as perioral and acral erythematous scaly lesions. Laboratory investigations revealed low zinc levels in the infant's serum, the mother's plasma zinc level was normal, and breast milk zinc was low. Rapid healing occurred after oral zinc supplementation. DISCUSSION: Zinc deficiency in breast-fed infants is a rare disease caused by a low l

[Acute osteomyelitis of the clavicle in the newborn infant: a case report].

Acute osteomyelitis of the clavicle accounts for less than 3% of osteomyelitis cases, with its usual location in the middle third. It may be hematogenous, due to contiguity, or secondary to catheterization of the subclavian vein or neck surgery. The diagnosis is often delayed, and clinical symptoms may simulate obstetric brachial plexus palsy in young children. We report a new case of osteomyelitis of the clavicle in a 30-day-old newborn.

Infant meningococcal vaccination: Advisory Committee on Immunization Practices (ACIP) recommendations and rationale.

At its October 2012 meeting, the Advisory Committee on Immunization Practices (ACIP) voted to recommend vaccination against meningococcal serogroups C and Y for children aged 6 weeks through 18 months at increased risk for meningococcal disease. Meningococcal groups C and Y and Haemophilus b tetanus toxoid conjugate vaccine (Hib-MenCY-TT [MenHibrix, GlaxoSmithKline Biologicals]) is licensed for active immunization for prevention of invasive disease caused by Haemophilus influenzae type b (Hib) and meningococcal serogroups C and Y. Hib-MenCY-TT is not indicated for prevention of disease caused

A thrombosing, giant, distal posterior cerebral artery aneurysm in a newborn infant.

Intracranial aneurysms are extremely rare in infancy. No consensus has yet been developed about the exact treatment of this rare situation. The authors report the case of a 47-day-old male infant who had multiple seizures on the same day, leading to the diagnosis of an intracranial aneurysm. The case was managed conservatively with close imaging follow-up, and the patient had a good recovery. The results of neurological examination were completely normal at the 5-year follow-up visit. These rare lesions may be suspected on the basis of clinical findings and correctly diagnosed with current neu

Retinopathy of prematurity: are we missing any infant with retinopathy of prematurity?

AIMS: To evaluate incidence of retinopathy of prematurity (ROP) and validity of current ROP screening criteria adopted in the Armed Forces Hospitals. METHODS: A retrospective study of ROP indices was carried out in infants admitted to the neonatal intensive care unit in Khamis Mushait, Saudi Arabia, from January 2009 to December 2011. All infants who had birth weight (BW) of 1500 g or less and/or gestational age (GA) of 32 weeks or less had their data reviewed for ROP diagnosis and related indices. Some heavier or older infants were also included in the study. RESULTS: 386 infants were scree
