Effects of suramin on ocular onchocerciasis.

39 patients (Group A) with ocular onchocerciasis in the Sudan-savanna of north Cameroon were given 4-6 g of suramin and followed in detail over 1-2 years. 39 other patients (Group B) received suramin followed 2 weeks later by a 6-7 day course of diethylcarbamazine (DEC). A further 18 patients (Group C) received placebo injections and were followed in the same way by the same observers. Suramin caused serious general reactions among the 100 patients who started the course - 1 case of stomatitis, 1 exfoliative dermatitis, and several cases of severe prostration, among which 2 ended fatally. Thes

Complement Suppresses the Initial Type 1 Interferon Response to Ocular Herpes Simplex Virus Type 1 Infection in Mice.

The complement system (CS) contributes to the initial containment of viral and bacterial pathogens and clearance of dying cells in circulation. We previously reported mice deficient in complement component 3 (C3KO mice) were more sensitive than wild-type (WT) mice to ocular HSV-1 infection, as measured by a reduction in cumulative survival and elevated viral titers in the nervous system but not the cornea between days three and seven post infection (pi). The present study was undertaken to determine if complement deficiency impacted virus replication and associated changes in inflammation at e

Sustained Release of Tacrolimus From a Topical Drug Delivery System Promotes Corneal Reinnervation.

PURPOSE: Corneal nerve fibers provide sensation and maintain the epithelial renewal process. Insufficient corneal innervation can cause neurotrophic keratopathy. Here, topically delivered tacrolimus is evaluated for its therapeutic potential to promote corneal reinnervation in rats. METHODS: A compartmentalized neuronal cell culture was used to determine the effect of locally delivered tacrolimus on sensory axon regeneration in vitro. The regenerating axons but not the cell bodies were exposed to tacrolimus (50 ng/mL), nerve growth factor (50 ng/mL), or a vehicle control. Axon area and length

The spectrum of oculocutaneous disease: Part I. Infectious, inflammatory, and genetic causes of oculocutaneous disease.

Many skin diseases are associated with ocular findings, emphasizing the need for dermatologists to be fully aware of their presence, and as a result, avoid overlooking conditions with potentially major ocular complications, including blindness. We review important oculocutaneous disease associations with recommendations for the management of the ocular complications and appropriate referral to our ophthalmology colleagues. Part I of this 2-part review focuses on the infectious, inflammatory, and genetic relationships.

[Cycloferon and management of herpes virus infection].

The treatment of patients with various forms of herpes requires a complex approach with using chemo- and immunotropic drugs. The use of Cycloferon, an interferon inductor (12.5% injection solution, 150 mg tablets or 5% liniment) was shown efficient. It had antiviral and immunotropic action in the mono- and combination therapy of herpes simplex of the skin and mucosa, genital herpes, ophthalmoherpes, herpes zoster, infectious mononucleosis. Cycloferon lowered the level and period of the disease clinical signs, prolonged the remission, corrected the immunity shifts, prevented the complications.

Subconjunctival bevacizumab for corneal neovascularization secondary to topical anesthetic abuse.

We report the clinical course of a 56-year-old patient diagnosed with toxic keratopathy due to topical anesthetic abuse. One month later, while a large corneal epithelial defect persisted, the cornea developed circumferential neovascularization that was treated with subconjunctival bevacizumab. Corneal neovascularization (CN) showed a dramatic regression 1 week after subconjunctival injection of bevacizumab. The epithelial defect slowly healed, no complications were observed, and no recurrence observed after 4 months of follow-up. In conclusion, bevacizumab may be valid complementary treatment

Toxic anterior segment syndrome after cataract surgery.

We report a case of toxic anterior segment syndrome (TASS) that developed after cataract surgery. A 78-year-old woman had uneventful phacoemulsification via a clear corneal incision with implantation of an acrylic intraocular lens. One day postoperatively, diffuse corneal edema and anterior chamber inflammation were evident. Topical antibiotic and steroid eyedrops were prescribed. The anterior chamber reaction decreased considerably, but corneal edema persisted. After a thorough investigation, the antiseptic solution used to soak surgical instruments before subsequent surgery was identified as

Anti-inflammatory protein TSG-6 reduces inflammatory damage to the cornea following chemical and mechanical injury.

Previous reports demonstrated that adult stem/progenitor cells from bone marrow (multipotent mesenchymal stem cells; MSCs) can repair injured tissues with little evidence of engraftment or differentiation. In exploring this phenomenon, our group has recently discovered that the therapeutic benefits of MSCs are in part explained by the cells being activated by signals from injured tissues to express an anti-inflammatory protein TNF-α-stimulated gene/protein 6 (TSG-6). Therefore, we elected to test the hypothesis that TSG-6 would have therapeutic effects in inflammatory but noninfectious disease

Course of auditory impairment in Cogan's syndrome.

PURPOSE: Cogan's syndrome (CS), characterized by interstitial keratitis, hearing loss, and vestibular impairment, rarely occurs in children. Hearing loss is commonly bilateral and permanent in 37%-67% of patients. To date, long-term evaluation of hearing impairment in children with CS has been reported in only 3 patients. We describe the 35-month course of hearing impairment in a teenaged boy with Cogan's syndrome. MATERIALS AND METHODS: The medical record of a 15-year-old boy with Cogan's syndrome was reviewed. Data included relevant history and physical examination, diagnostic workup, and m

Genetic heterogeneity of KID syndrome: identification of a Cx30 gene (GJB6) mutation in a patient with KID syndrome and congenital atrichia.

Connexins are integral membrane proteins forming aqueous gap junction channels that allow the diffusional exchange of ions and small metabolites between cells, thus coordinating metabolic activities in multicellular tissues. Dominant mutations in the Cx26 gene GJB2 have been shown to cause keratitis-ichthyosis-deafness (KID) syndrome, palmoplantar keratoderma associated with hearing loss, and Vohwinkel syndrome. Missense mutations in the closely related Cx30 gene GJB6 underlie Clouston syndrome (autosomal dominant hidrotic ectodermal dysplasia). We report a 6-y-old boy with phenotypic characte

[Corneal lesions caused by caterpillar hairs: four case studies].

Caterpillar hairs disseminated by the wind can cause serious ocular problems in humans. We present 4 cases of corneal lesions caused by caterpillar hairs. Intense functional signs observed in exposed subjects should bring this diagnosis to mind. The severity of this affection is due to the possibility of intraocular migration, even many years after the first accident. Treatment consists of removing the caterpillar hair associated with an antibiotic and local anti-inflammatory treatment.

Airbags and bilateral eye injury: five case reports and a review of the literature.

We report five cases of bilateral eye injuries from airbag deployment in motor vehicle crashes and review the world's literature on ocular injuries associated with airbags. The cases in the literature were identified by cross-referencing Medline searches from airbags and ocular injuries. Additional cases were identified after review of references from each article in the search. An additional 89 cases from the literature were identified and are included for discussion. Patients were treated individually in a noncontrolled, nonrandomized fashion according to the nature of each injury with regul

Transconjunctival orbital decompression in Graves' ophthalmopathy: lateral wall approach ab interno.

AIMS: A modified surgical technique is described to perform a one, two, or three wall orbital decompression in patients with Graves' ophthalmopathy. METHODS: The lateral wall was approached ab interno through a "swinging eyelid" approach (lateral canthotomy and lower fornix incision) and an extended periosteum incision along the inferior and lateral orbital margin. In addition, the orbital floor and medial wall were removed when indicated. To minimise the incidence of iatrogenic diplopia, the lateral and medial walls were used as the first surfaces of decompression, leaving the "medial orbita

Topical treatment of experimental acute corneal inflammation by dexamethasone and nonsteroidal drugs.

In this study, we have correlated the relative effectiveness of nonsteroidal anti-inflammatory drugs versus corticosteroids in the topical treatment of acute corneal inflammation. The authors have studied the effect of 1 drop every 6 h of flurbiprofen 0.03%, indomethacin 1%, diclofenac 0.1% and dexamethasone 0.1% in an experimental model of corneal alkali burn. The objective parameters used to evaluate the corneal inflammatory response were Luminol-enhanced chemiluminescence, ultrasonic pachymetry and computer-assisted analysis of the area of the corneal ulceration. Indomethacin 1% showed stat

New approach for early, simple, and rapid detection of acute eye infections.

Several methods are known for the diagnosis of infections all of which require the use of well equipped laboratories and skilled personnel. For example: 1. Isolation and culturing of microorganisms from samples taken from body fluids. This method is complicated and time consuming. 2. Detection of antibodies. The main disadvantage of this method is the fact that the change in the level of antibodies occurs only after a few days from the onset of the disease. In the course of our work we found that already in the early stages of acute external eye infections, mucopolysaccharide degrading enzymes

Systemic tetracycline hydrochloride as adjunctive therapy in the treatment of persistent epithelial defects.

Recent evidence shows tetracyclines have anticollagenolytic activity that may be clinically effective in disease treatment. This led us to use systemic tetracycline (oral tetracycline hydrochloride 1 g daily divided into 4 doses) as adjunctive therapy in treating 18 patients with persistent corneal epithelial defects. Fourteen of 18 patients healed their defects; within 48 hours (9 patients) and within two weeks (the other 5 responding patients). Two patients showed no effect and two showed disease progression.
