Arthroscopic synovectomy of the knee in hemophilia: 10-to-15 year followup.

Nine knees in 8 patients with severe hemophilia A and 1 patient with hemophilia B underwent arthroscopic synovectomy during the period of 1980 to 1985. This group of patients has been observed prospectively for the past 10 to 15 years. One complication occurred immediately postoperatively in an 8-year-old boy in whom a severe hemarthrosis developed that required arthroscopic evacuation. His postoperative recovery was compromised leading to significant loss of motion. Recurrent hemarthroses developed in only 1 patient after an injury to the knee. A second arthroscopic synovectomy was performed

Effects of landing instructions on Achilles tendon load: Emphasising hip flexion as the optimal strategy.

PURPOSE: Managing Achilles tendon (AT) load during dynamic activities is essential for preventing and rehabilitating Achilles tendinopathy. While various exercises have been studied, the impact of verbal instructions on landing mechanics remains unclear. This study aimed to identify instructional methods that effectively reduce AT load during drop vertical jumps (DVJs). METHODS: Twenty-three healthy participants performed DVJs under five instructional conditions: (1) natural: no instruction; (2) posterior-centre of pressure (COP): shifting the COP posteriorly; (3) hip-flexion: increasing hip

Long-term Outcomes of Surgically Treated Congenital Dislocation of the Knee.

BACKGROUND: Congenital dislocation of the knee (CDK) may be idiopathic or associated with another condition, such as Larsen syndrome or arthrogryposis. Surgical reduction of type-3 dislocation may require quadricepsplasty (QP) or femoral diaphyseal shortening (FS). Because it is unknown which treatment is more effective, we evaluated long-term outcomes using patient-reported questionnaires and gait analysis, comparing results by surgery type and underlying diagnosis. METHODS: Twelve patients (mean age, 19 mo) were treated surgically for CDK from 1985 to 2015 and studied 9 to 30 years postoper

Semi-extended extra-synovial (SEES) tibial intramedullary nailing technique: Up to 10 year retrospective analysis of outcomes and anterior knee pain rates.

BACKGROUND: Tibial intramedullary nailing is a common method of fixation for fractures of the tibia, with several approaches described. Anterior knee pain is a common complication following nailing, but the reported incidence of knee pain varies in the literature between 10 % and 86 %. There is considerable variation in incidence between nailing techniques, with an exact aetiology still unknown. We investigated the reported incidence of anterior knee pain in patients undergoing tibial nailing using the semi-extended extra synovial (SEES) technique at a Major Trauma Centre (MTC) in the UK. Increased valgus laxity in flexion with greater tibial resection depth following total knee arthroplasty.

PURPOSE: Soft tissue balancing is of central importance to outcome following total knee arthroplasty (TKA). However, there are lack of data analysing the effect of tibial bone cut thickness on valgus laxity. A cadaveric study was undertaken to assess the biomechanical consequences of tibial resection depth on through range knee joint valgus stability. We aimed to establish a maximum tibial resection depth, beyond which medial collateral ligament balancing becomes challenging, and a constrained implant should be considered. METHODS: Eleven cadaveric specimens were included for analysis. The bi

Management of Advanced Tricompartmental Knee Osteoarthritis With Degenerative Meniscal Pathology Using Extracorporeal Magnetotransduction Therapy, Extracorporeal Shockwave Therapy, Microfragmented Adipose Tissue, and Platelet-Rich Plasma.

Advanced tricompartmental knee osteoarthritis (OA) with degenerative meniscal tears and ligamentous insufficiency represents a considerably difficult presentation in non-surgical musculoskeletal medicine. Although total knee arthroplasty is the current standard for surgical intervention, many patients may prefer to first explore non-surgical options or may require interim symptom management prior to surgery. This report describes management of a complex case of advanced knee OA via a staged non-surgical protocol utilizing extracorporeal shockwave therapy (ESWT), extracorporeal magnetotransduct

Effect of Q-angle symmetry on knee joint kinematics in level walking, upslope walking and landing task.

BACKGROUND: The quadriceps angle (Q-angle) is a key biomechanical measure of lower limb alignment. While its asymmetry is linked to knee disorders like patellofemoral pain, its dynamic impact on full six-degree-of-freedom (6DOF) knee kinematics across different tasks remains unclear. METHODS: Forty healthy university students were categorized into Q-angle symmetric (QS, bilateral difference ≤ 3°) or asymmetric (QA, > 3°) groups. Knee 6DOF kinematics were captured during level walking, 10% upslope walking, and landing task using a 3D motion capture system. The International Physical Activity Q

Anterior tibial translation in pediatric ACL-deficient knees under functional orthosis loading: a finite element study.

BACKGROUND: Anterior cruciate ligament (ACL) deficiency increases anterior tibial translation (ATT), predisposing the knee to instability and injury. Functional orthoses are prescribed to limit ATT, yet their biomechanical mechanisms remain poorly quantified. PURPOSE: To develop ACL-deficient pediatric finite element (FE) knee models that evaluate the effectiveness of ACL orthoses in minimizing ATT and to benchmark braced ACL-deficient predictions against unbraced ACL-intact behaviour. METHODS: Twenty-two pediatric FE models representing ACL-deficient knees were developed. Simulations replic

Association between 2D landing biomechanics, isokinetic muscle strength and asymmetry in females using novel, task specific metrics based on ACL injury mechanisms.

This study investigated the relationship between isokinetic muscle strength metrics, landing biomechanics, and their asymmetries, in females. Twenty-three female team sport athletes completed unilateral forward drop landings, and isokinetic muscle strength assessment of the knee extensors and flexors, on both limbs. Discrete two-dimensional kinematics of the trunk, hip, knee, and ankle in the sagittal and frontal plane and peak GRF were recorded during the drop landings. Novel, task-specific isokinetic strength metrics related to the landing task, such as peak concentric and eccentric torque,

Manual Tibial Distraction in Seated Position for Identification of Intra-articular Knee Joint Lesions: A Case Report Presenting a Novel Clinical Special Test.

The meniscus is a weight-bearing, intra-articular, fibrocartilaginous structure that is frequently injured. Traditional special tests for diagnosis of intra-articular lesions of the knee involve provocative tests with compressive or torsional components, often leading to increased pain. With this case study, we aim to introduce the Tibial Distraction Test (TDT), a novel clinical special test for the assessment of intra-articular knee joint lesions, including meniscus injury and osteoarthritis (OA), without provoking pain. A 30-year-old man presented to the clinic with right knee pain and swell

Micro-Computed Tomography Analysis of the Knee in Aged Dunkin-Hartley Guinea Pigs after Intra Articular Injection.

The purpose of this protocol is to guide researchers in performing a palpation-guided technique of intra-articular knee injection in guinea pigs and assessment using micro-computed tomography. Dunkin-Hartley guinea pigs are robust models for osteoarthritis research as they spontaneously develop osteoarthritis in their knees. Intra-articular drug delivery is a common method to study the effects of an investigational drug in vivo. In humans, therapeutic agents administered via intra-articular injection can offer pain relief and delay further progression of osteoarthritis. As with any species, th

Dynamic variation of tibiofemoral compression force during total knee arthroplasty: Implications for soft tissue balance and functional outcomes.

BACKGROUND: Achieving precise alignment and soft tissue balance is crucial for optimal total knee arthroplasty (TKA) outcomes. We aimed to explore how tibiofemoral compression force (TFCF) varies with knee flexion and its correlation with functional outcomes. METHODS: This prospective study included 60 patients undergoing cruciate-retaining TKA (FINE Total Knee System). Sensor-equipped trial inserts were used to measure the TFCF at 15° intervals, from full extension to 90° flexion. Patients were classified into anterior and posterior force groups based on whether the medial TFCF was higher in
