Sagittal spinal balance after lumbar spinal fusion: the impact of anterior column support results from a randomized clinical trial with an eight- to thirteen-year radiographic follow-up.

STUDY DESIGN: Randomized clinical trial. OBJECTIVE: To analyze the long-term clinical impact of anterior column support on sagittal balance after lumbar spinal fusion. SUMMARY OF BACKGROUND DATA: Several investigators have stressed the importance of maintaining sagittal balance in relation to spinal fusion to avoid lumbar 'flat back,' accelerated adjacent segment degeneration, pain, and inferior functional outcome. Only limited evidence exists on how sagittal alignment affects clinical outcome. Anterior lumbar interbody fusion combined with posterolateral fusion has been proved superior to p

[Spread of hyperbaric local anesthetics in a spinal canal model. The influence of Trendelenburg position and spinal configuration].

BACKGROUND: The influence of Trendelenburg positions and variations in spinal canal configuration on the spread of hyperbaric spinal anesthetics was examined in two models of the subarachnoid space. METHODS: Both models included simulations of the spinal cord, filum terminale and cerebrospinal fluid. Model I had a straight shape, thus omitting replications of lumbar lordosis and thoracic kyphosis. It allowed the evaluation of fluid dynamics and the spread of 0.5% hyperbaric bupivacaine and 4% hyperbaric mepivacaine in 0° (supine position), 5° and 10° head-down tilt positions. Model II include

Spine deformities in patients with Ehlers-Danlos syndrome, type IV - late results of surgical treatment.

BACKGROUND: Spinal deformities in Ehlers-Danlos syndrome are usually progressive and may require operative treatment. There is limited number of studies describing late results of surgery in this disease. METHODS: This is a retrospective study of the records of 11 patients with Ehlers-Danlos syndrome type IV, treated surgically between 1990 and 2007. All patients underwent surgical treatment for spinal deformity. Duration of operation, type of instrumentation, intraoperative blood loss, complications and number of additional surgeries were noted. Radiographic measurement was performed on stan

Three-column reconstruction through single posterior approach for the treatment of unstable thoracolumbar fracture.

STUDY DESIGN: A prospective study was performed. OBJECTIVE: To assess a technique, three-column reconstruction through single posterior (TRSP) approach, in treatment of highly unstable thoracolumbar fracture. SUMMARY OF BACKGROUND DATA: The goal of surgical intervention for treatment of unstable thoracolumbar fractures is to decompress the neural elements, restore vertebral body height, correct angular deformity, and stabilize the columns of the spine. Operative approaches remain disputed. Common opinions include short-segment posterior fixation, multiple-segment posterior fixation, stand-al

Long-term results of thoracolumbar and lumbar burst fractures after short-segment pedicle instrumentation, with special reference to implant failure and correction loss.

OBJECTIVE: To assess the long-term results of short-segment pedicle instrumentation for thoracolumbar and lumbar burst fractures. METHODS: From February 1987 to June 1995, 89 patients with thoracolumbar or lumbar burst fracture were treated with short-segment pedicle instrumentation, and 68 (76.4%) of them were followed up for an average of 8.0 years (range, 5-13 years). Radiographs were taken pre- and post- operatively, before implant removal and at final follow-up. Computerized tomography (CT) scans of the fractured vertebrae were done on 18 patients, with their consent, at final follow-up.

Intermediate-term outcome of the simplified laparoscopic antegrade continence enema procedure: less is better.

PURPOSE: The Malone antegrade continence enema procedure revolutionized the surgical management of fecal incontinence. Open and laparoscopic antegrade continence enemas are often performed with cecoplication and mesenteric manipulation. Since our initial laparoscopic antegrade continence enema description, we have simplified our technique. We present our series of laparoscopic antegrade continence enema procedures, discuss technique and outcomes, and review the literature. MATERIALS AND METHODS: We retrospectively reviewed children who underwent laparoscopic antegrade continence enema between

The effect of prenatal treatment with steroids and preterm delivery in a model of myelomeningocele on the rabbit foetus.

Damage of neural elements (spinal cord and encephalus) in myelomeningocele (MMC) seems to be progressive during gestation because of amniotic fluid chemical contact and continuous leakage of CSF. We studied the effect of preterm delivery and steroid treatment in a model of MMC in the rabbit foetus. Twelve New Zealand White rabbits underwent laparotomy and hysterotomy at 23 days of gestation. Fifty-nine out of 107 foetuses underwent lumbar laminectomy (three to four levels). Dura was opened to expose the neural elements to the amniotic fluid. Six rabbits underwent caesarean section on gestation
