Activity of cefepime against nosocomial blood culture isolates.

Cefepime is a new aminothiazolylacetamido cephalosporin with a wider spectrum and greater potency than many currently available cephalosporins. Since the blood culture isolates from patients of the study centre in Saudi Arabia are significantly more resistant to antimicrobial agents in clinical practice, we evaluated the in-vitro activity of cefepime and six other beta-lactam antibodies against 390 and 273 isolates of Gram-negative and Gram-positive bacteria, respectively. Cefepime had a broad spectrum of activity against the Enterobacteriaceae (MIC50 < 0.12 mg/L), Pseudomonas aeruginosa, Acin

The clinical and molecular epidemiology of bacteremias at a university hospital caused by pneumococci not susceptible to penicillin.

To determine the epidemiology of bacteremias due to pneumococci not susceptible to penicillin (PNSP) at a university hospital, active microbiologic surveillance of bacteremias due to PNSP was done for 28 months. Controls were bacteremias caused by penicillin-susceptible pneumococci. Antimicrobial susceptibilities for alternative antibiotics were determined. Pulsed-field gel electrophoresis (PFGE) and serotyping were used as markers of strain identity. Of 113 pneumococcal isolates, 14 (13%) were intermediate or resistant to penicillin (MIC > or = 0.1 microgram/mL). Twelve PNSP were resistant to

Corynebacterium striatum: a diphtheroid with pathogenic potential.

Although nondiphtherial corynebacteria are ubiquitous in nature and commonly colonize the skin and mucous membranes of humans, they rarely account for clinical infection. Corynebacterium striatum has rarely been reported to be a pathogen, causing pleuropulmonary infections and bacteremia in only immunocompromised or anatomically altered patients. We noted C. striatum to be the infecting pathogen or copathogen in six patients. To our knowledge, this report describes the first cases of C. striatum causing infection of exist sites of central venous catheters, thrombophlebitis associated with cent

Survey of Bacteroides fragilis group susceptibility patterns in Canada.

The in vitro activities of penicillin, clindamycin, chloramphenicol, metronidazole, piperacillin, piperacillin-tazobactam, ticarcillin, ticarcillin-clavulanate, ampicillin-sulbactam, cefoxitin, ceftizoxime, cefotetan, moxalactam, and imipenem against 348 Bacteroides fragilis group isolates collected from six Canadian cities during 1990 were determined by the National Committee for Clinical Laboratory Standards (NCCLS) agar dilution technique. All isolates were susceptible to chloramphenicol, metronidazole, piperacillin-tazobactam, and imipenem. For the other antibiotics tested, the following r

Anaerobic infections. The basics for primary care physicians.

Anaerobic bacteria constitute a major portion of the normal human microflora, and some of them can cause disease in contiguous body parts, especially if there is a mucosal break. Most anaerobic infections are polymicrobial. Because anaerobes are difficult to culture, diagnosis is often made on the basis of clinical clues. Thus, knowledge of the common sites, predisposing conditions, and other representative features of anaerobic infections is critical. For anaerobic infections above the diaphragm, where Bacteroides fragilis is not a common isolate, high-dose penicillin G therapy is usually suf

Acidic drug transport in vivo through the blood-brain barrier. A role of the transport carrier for monocarboxylic acids.

The relationship of the transports between acidic drugs and monocarboxylic acids through the blood-brain barrier (BBB) was examined using the carotid artery injection technique in rats. The BBB uptakes of [3H]acetic acid and [14C]salicylic acid were significantly reduced by the presence of the respective unlabeled compounds, valproic acid, lactic acid, benzoic acid, nicotinic acid or beta-lactam antibiotics (benzylpenicillin, propicillin and cefazolin), but was not reduced by choline, phenylalanine and a basic drug, eperisone. A remarkable pH dependency was observed for the BBB uptake of [14C]

Systemic antimicrobial therapy of nosocomial pneumonia: monotherapy versus combination therapy.

The treatment of nosocomial pneumonia is difficult because of the absence of a precise microbial etiology in a sizable fraction of cases and the severity of predisposing conditions in patients who develop these infections. Staphylococcus aureus and aerobic gram-negative rods continue to be the most common isolates in these infections. Therapy of nosocomial pneumonia is often empiric, with initial antimicrobial therapy based upon local experience and sensitivity patterns, immune competence of the patient and disease severity. Combination therapy with an aminoglycoside and a beta-lactam has long

Biliary concentrations of temocillin.

Temocillin is a new semisynthetic beta-lactam antibiotic with a spectrum of activity against most aerobic Gram-negative bacteria. We investigated the biliary excretion of temocillin after administration of 1g either intravenously or intramuscularly to 15 patients with normal liver and renal function who were undergoing elective cholecystectomy for gallstones. Very high concentrations of temocillin (up to 8 times the corresponding serum concentrations) were found in common bile duct bile and gallbladder bile of functioning gallbladders 1 to 2 hours after administration. However, no temocillin w

Endemic Serratia marcescens infection in a neonatal intensive care nursery associated with gastrointestinal colonization.

Serratia marcescens (SM) produced a prolonged outbreak in a neonatal intensive care unit of high level gastrointestinal colonization (10(9) SM/g feces) which in the early part of the outbreak predisposed to respiratory infection. The early outbreak featured a strain of SM carrying a 54 X 10(6) dalton conjugative plasmid which mediated resistance to gentamicin, tobramycin and beta-lactam agents. The second part of the outbreak involved primarily gastrointestinal colonization with SM strains that were plasmid-free. Acquisition of SM was related to very low birth weight (less than 1500 g). Among

[Antibiotic sensitivity of Branhamella catarrhalis isolated from respiratory tract infections].

The efficacy of antibiotics including some newly developed penicillins and cephems on clinically isolated Branhamella catarrhalis was evaluated in vitro. Among 44 strains of B. catarrhalis which were isolated from expectorated sputum of patients with respiratory tract infections 36 strains were proved to produce beta-lactamase. MICs of penicillins for beta-lactamase positive strains ranged from less than or equal to 0.05 to 3.13 micrograms/ml, while those for beta-lactamase negative strains were all less than 0.05 microgram/ml. MICs of cephems, on the other hand, ranged more variably, especial

Gram-negative anaerobic bacilli: their role in infection and patterns of susceptibility to antimicrobial agents. I. Little-known Bacteroides species.

The role in infection and the patterns of susceptibility to antimicrobial agents of 17 infrequently reported species of Bacteroides were studied. The findings of this research are summarized, and the literature dealing with recovery of these species from both the normal flora and infections of humans is reviewed. During a six-year period, 22% of 679 specimens from infections due to anaerobic bacteria that were studied by the authors yielded one or more of these "other Bacteroides species" (OBS). The most commonly isolated OBS were Bacteroides ruminicola subspecies brevis (63 patients), Bactero

Antibiotic treatment of abscesses of the central nervous system.

Samples of intracranial pus and serum from 32 patients were assayed to determine the concentrations reached in them of penicillin, ampicillin, cloxacillin, cephaloridine, gentamicin, chloramphenicol, fusidic acid, and lincomycin. Metronidazole had not been given. Penicillin penetrated abscesses reasonably well, but other beta-lactam antibiotics did not. The penetration of chloramphenicol was erratic. Aminoglycosides penetrated poorly, but lincomycin and fusidic acid penetrated well. Assay of sulphonamides and co-trimoxazole in pus was unreliable. These studies indicate that treatment of absces

[Isolation and characterization of two strains of Streptomyces able to metabolize natural polysaccharides including mannan (author's transl)].

Two strains of aerobic and mesophilic microorganisms were isolated from palm-tree plantation sand. They grew on insoluble polysaccharides: mannan, cellulose, chitin as only source of carbon. This lytic activity was used for the purification of the two strains. The morphology of the organisms and the presence of LL-diaminopimetic acid in their cell-wall are characteristic of the genus Streptomyces. Investigations lead to: 1) the characterization of their specific polysaccharidase activity toward insoluble and natural beta-and alpha-glycans (mannan, cellulose, chitin, pectine and starch) and the

Structural insights into Nirmatrelvir (PF-07321332)-3C-like SARS-CoV-2 protease complexation: a ligand Gaussian accelerated molecular dynamics study.

Coronavirus 3C-like protease (3CLpro) is found in SARS-CoV-2 virus, which causes COVID-19. 3CLpro controls virus replication and is a major target for target-based antiviral discovery. As reported by Pfizer, Nirmatrelvir (PF-07321332) is a competitive protein inhibitor and a clinical candidate for orally delivered medication. However, the binding mechanisms between Nirmatrelvir and 3CLpro complex structures remain unknown. This study incorporated ligand Gaussian accelerated molecular dynamics, the one-dimensional and two-dimensional potential of mean force, normal molecular dynamics, and Krame

Phenotypic and genotypic profiling reveals a high prevalence of methicillin-resistant Staphylococcus aureus isolated from hospitals, houseflies and adjacent informal food retailers in Botswana.

The increasing occurrence of methicillin-resistant Staphylococcus aureus (MRSA) in the environment, food and healthcare systems is a global public health concern. MRSA is reported to cause food poisoning, osteomyelitis and pyogenic infections of the skin, and consequently has been categorized as a high-priority pathogen by the World Health Organization. Here, we determined the presence of MRSA in clinical (n=56), food (n=150) and housefly samples (n=970) collected from two hospitals in Botswana.
