[Subglottal stenosis as the initial manifestation in Wegener's disease].

A 21-year-old man with 3-month history of hoarseness and dyspnea was suspected to have Wegener's granulomatosis with initially appearing in the trachea. In spite of unspecific biopsies, the clinicopathological picture and the high titles of anticytoplasmic autoantibodies (ACPA) confirmed the diagnosis. The early treatment with cyclophosphamide impeded the evolution to graver systemic forms of the disease.

[Multiparameter analysis of dysphonia using the equipment Physiologia].

The purpose of the multiparametric analysis of the vocal phenomenon is to evidence the instability of the signal in amplitude and in frequency, together with the aerodynamic phenomena related to the glottic air leakage. Statistical analysis has made it possible to establish a correlation between the data of the apparatus, as from 9 variables, and the clinical classification. The main diagnostic categories are relatively close, evidencing the various biomechanical anomalies of the laryngeal vibrator. A certain number of issues remain to be addressed: the physiopathology of the vibrator, the cho

Laryngo-tracheo-oesophageal cleft: a plea for early diagnosis.

Congenital laryngeal clefts are rare. This paper reports on the management of 3 patients with Type 2 laryngo-tracheo-oesophageal cleft. One patient died following tracheopexy after successful cleft closure. Prognosis is related to the presence of other major anomalies, the early diagnosis of the lesion and institution of appropriate respiratory and nutritional care prior to correction of the defect. A greater awareness of the condition combined with aggressive diagnostic endoscopy should result in early diagnosis and improved survival.

[Laryngomalacia: a review of 42 cases].

In the period from 1981 to 1990, in the Children's Hospital from Sevilla were performed 220 direct laryngoscopies and bronchoscopies to newborns and infants that showed as a main finding: stridor voice change or aspiration syndromes. In the 30 percent the main cause was a congenital laryngeal anomalies. Laryngomalacia is the most common congenital laryngeal anomaly with 42 cases. For it frequency, the otolaryngologist should be familiar with the clinical findings, diagnosis and actual management.

Airway injury after tracheotomy and endotracheal intubation.

Iatrogenic airway injury after tracheotomy and endotracheal intubation continues to be a serious clinical problem. Endotracheal tubes cause pressure injury to the glottis and may result in severe commissural scarring that is difficult to treat. Tracheotomy tubes may result in severe stomal stenosis in the trachea or subglottic region, which is more amenable to surgical treatment. Both methods of airway intubation may result in pressure necrosis from the tube cuff that can be prevented by careful monitoring of inflation pressures. The technique of laryngotracheal resection and reconstruction ha

Spasmodic dysphonia, whether and where: results of seven years of research.

Two questions are addressed in this presentation and integration of experimental data from seven years of research. They are (a) is spasmodic dysphonia (SD) a neurologic disorder; and (b) if so, where in the nervous system does the dysfunction have its locus? The conclusion is that SD is a supranuclear movement disorder primarily, but not exclusively, affecting the larynx. For over 50% of the subjects, isolated, multifocal, cortical lesions can be identified, specifically in left frontal/temporal cortex (perisylvian region), medial frontal cortex, and right posterior temporal/parietal cortex.

Surgical anatomy of structures adjacent to the thyroid apex and post-operative voice change (a review including dissection).

This paper considers the anatomy of structures contiguous to the upper pole of the thyroid gland. It also describes the findings of an anatomical dissection confined to the muscular and carotid triangles of the neck with the nerves therein innervating the cricothyroid and infrahyoid muscles. The segmental nature of the latter is described. The variable course of the cricothyroid artery with regard to the sternothyroid muscle is noted. A superficially placed artery is accompanied by branches of the ansa cervicalis; a deeply placed one is related to the external laryngeal nerve. In addition to t

Evaluation of a stented laryngoplasty for correction of cranial glottic stenosis in four dogs.

After laryngeal surgery, cranial glottic stenosis secondary to laryngeal web formation was diagnosed as the cause of upper airway obstruction in 4 dogs. Initial treatment of 3 dogs by peroral laryngeal web excision resulted in rapid recurrence of clinical signs. Subsequently, these 3 and the fourth dog were treated by scar excision, mucosal flap coverage, and placement of a silicone intraluminal laryngeal stent for 14 days. By 5 months (average) after surgery, on the basis of clinical evaluation of exercise intolerance, respiratory distress, and stridor, all dogs had alleviation of upper airwa

Electromyographic findings in focal laryngeal dystonia (spastic dysphonia).

Spastic dysphonia is a clinical speech disorder characterized by spasms of the laryngeal muscles during phonation, producing a broken pattern of speech sometimes termed laryngeal stuttering. Fourteen patients with the diagnosis of spastic dysphonia based on voice quality were referred for evaluation; detailed clinical and electrophysiologic evaluations were performed. Laryngeal electromyographic (EMG) testing failed to demonstrate any spontaneous activity in the 14 patients tested. Seven patients (50%) had normal number and amplitude of motor unit potentials. Four of these had disparate amplit

Chevalier Jackson lecture. Respiratory and alimentary relations in staged human embryos. New embryological data and congenital anomalies.

The early development of the digestive and respiratory systems is summarized in relation to embryonic staging. It has frequently been emphasized that the digestive and respiratory tubes do not arise from a common chamber, that they pursue separate courses as soon as the lung bud appears, that a mesenchymal septum comes to intervene between them, and that the two tubes rapidly acquire independent outer coats. Some commonly held views such as the supposed caudorostral separation of the trachea from the esophagus, have been shown to be incorrect. These ideas often arose from the use of unstable l

Fatal choking in infants and children.

Food asphyxiation in infants/children follows a different pattern from the adult "cafe coronary." In the absence of ethanol intoxication, infants/children are prone to mishandling nonfriable, firm, slippery foods/objects with a rounded contour. The Dade County Medical Examiner's files were searched from 1956 to mid-1983 for accidental pediatric choking deaths. Seventeen food and six foreign body asphyxiations were found. The male/female ratio was 1.4/1 and 1/1, respectively. The white/black ratio was 0.9/1 for food asphyxiation whereas no black victims were encountered choking upon foreign obj

Serial dilation compared to elective laryngo-tracheoplasty in the treatment of acquired subglottic stenosis in children.

Acquired incomplete subglottic stenosis in children has historically been managed by serial bouginage of the strictured region sometimes accompanied by injection or systemic administration of corticosteroid medications. The forceful stretching of the scar whilst awaiting growth of the region to compensate for scar contracture, however, dooms that child to a prolonged period of waiting for an unpredictable result with a tracheotomy in place. Recently, laryngo-tracheoplasty has been undertaken in younger age groups and in a review of 30 children, with very similar types of stenosis, it was noted

[Phonatory airflow in the supraglottal space].

The phonatory airflow can be measured by means of a hot wire tube placed in the supraglottic space without tying down the tongue. The velocity of airflow above the glottis reaches values around c = 50 to 150 cm/s. The variations in airflow oscillations were recorded. The voice of the person under examination was picked up by a condenser microphone (Bruel & Kjaer No. 2112). According to D'Alembert's wave equation, the sound intensity is related to the velocity of the phonatory air stream. The validity of this statement has been confirmed by repeated testing. The fundamental frequency of voice a

Treatment of chronic aspiration as a complication of cerebrovascular accident.

Chronic aspiration remains one of the most difficult complications to treat in the patient recovering from brain stem infarction. Aspiration in these patients is often due to an incompetent glottis in association with dysfunction of the pharyngeal phase of swallowing. A systematic approach to the evaluation and treatment of these patients is presented with illustrative case presentations. Such individual treatment protocols allow for optimum rehabilitation of the post-stroke victim.

Canine model of subglottic stenosis secondary to prolonged endotracheal intubation.

Acquired subglottic stenosis is usually a consequence of prolonged endotracheal intubation. The present study describes a canine model of subglottis stenosis which is congruent with the known and suspected pathogenesis of the disease in man. Eighteen young pups (Canis familiaris) were used. A modified cuffed endotracheal tube was placed within the subglottis and secured by inflating the cuff. The capillary tube supplying the cuff was sealed and cut so as to lie below the epiglottis. Three animals died of tracheal rupture or collapse prior to or on the thirteen day following intubation. In the

Acute upper airway obstruction in the adult. 2. Causative events.

Acute obstruction of the upper airway can be caused by several events. With some, such as anaphylaxis and trauma, obstruction may not be immediate and the physician may be able to take steps to avert it. Intubation is a common cause of upper airway injury. The most serious complication of this procedure is chronic subglottic stenosis. Vocal cord paralysis may result from tumor or neck trauma but especially from neck or mediastinal surgery, and the vocal cords should always be examined before and after such surgery.

[Association of posterior laryngeal cleft and atresia of the oesophagus. Two cases treated successfully (author's transl)].

Two rare cases of oesophageal atresia associated with a posterior laryngeal cleft were treated successfully: a so-called "O" interarytenoid cleft recognised secondarily to the type II atresia, not treated surgically; a so-called II cricoid cleft recognised before the treatment of type I atresia was closed in a first stage by an anterior transl-laryngeal approach. A bibliographical review is followed by a discussion of the diagnosis aspects and therapeutic possibilities of laryngeal cleft, the approach, the timing of surgery on the basis of its type and the type of oesophageal atresia.
