Living Arrangements and the Digital Divide Among Older Adults: Evidence from a Survey of Smartphone Use.

Population aging has increased the demand for digital support in later-life care, yet many older adults remain excluded from smartphone-based services. Using a questionnaire survey and interviews with 4,236 older adults conducted in 2018 in Jiangxi Province, China, this study examines how living arrangements are associated with smartphone adoption and usage competence. The study found that co-residence with adult children was significantly positively associated with smartphone use, with the odds ratios of 18.1%, whereas spousal co-residence was positively associated with usage competence, refl

The disproportionate burden of severe obesity in youth with special needs and the role of metabolic and bariatric surgery.

Metabolic and Bariatric Surgery (MBS) has evolved from being historically contraindicated for pediatric special needs populations to be an evidence-based standard of care. As obesity acts as a morbidity multiplier within this cohort, success with MBS is dependent on the strength of support ecosystems available to caregivers of these patients, rather than cognitive capacity. Although specific pathophysiology's such as intractable hyperphagia associated with Prader-Willi syndrome or complete MC4R deficiency may pose long-term durability challenges, many of these patients achieve weight loss and

Mastectomy With Sentinel Lymph Node Biopsy Under Regional Anesthesia in a Patient With a History of Multiple Anaphylactic Episodes.

We describe the case of a 55-year-old woman proposed for an elective unilateral mastectomy with sentinel node biopsy due to breast cancer. The patient has a history of four episodes of anaphylaxis, one of which occurred during the induction of general anesthesia and resulted in cardiorespiratory arrest, which was reversed with advanced life support. The intradermal tests confirmed allergy to propofol, all types of neuromuscular relaxants, ketorolac, acetylsalicylic acid, generic antibiotics, and iodinated contrast agents. Following a multidisciplinary discussion that included anesthesiology, g

Update on the Symptomatic Treatment of Huntington's Disease: From Pathophysiology to Clinical Practice.

Huntington's disease (HD) is the most common autosomal dominant neurodegenerative disorder, characterized by a triad of motor dysfunction, cognitive decline, and psychiatric disturbances. While recent efforts have focused on developing disease-modifying therapies, no treatment has yet demonstrated clinical efficacy. As a result, symptomatic treatment remains the cornerstone of care. However, high-quality evidence from large randomized trials is limited, and therapeutic decisions must rely on clinical expertise and extrapolation from other neurological or psychiatric conditions. This narrative

Recommendations for asthma monitoring in children: A PeARL document endorsed by APAPARI, EAACI, INTERASMA, REG, and WAO.

Monitoring is a major component of asthma management in children. Regular monitoring allows for diagnosis confirmation, treatment optimization, and natural history review. Numerous factors that may affect disease activity and patient well-being need to be monitored: response and adherence to treatment, disease control, disease progression, comorbidities, quality of life, medication side-effects, allergen and irritant exposures, diet and more. However, the prioritization of such factors and the selection of relevant assessment tools is an unmet need. Furthermore, rapidly developing technologies

Therapeutic Drug Monitoring Is a Feasible Tool to Personalize Drug Administration in Neonates Using New Techniques: An Overview on the Pharmacokinetics and Pharmacodynamics in Neonatal Age.

Therapeutic drug monitoring (TDM) should be adopted in all neonatal intensive care units (NICUs), where the most preterm and fragile babies are hospitalized and treated with many drugs, considering that organs and metabolic pathways undergo deep and progressive maturation processes after birth. Different developmental changes are involved in interindividual variability in response to drugs. A crucial point of TDM is the choice of the bioanalytical method and of the sample to use. TDM in neonates is primarily used for antibiotics, antifungals, and antiepileptic drugs in clinical practice. TDM a

Multisystem inflammatory syndrome with refractory cardiogenic shock due to acute myocarditis and mononeuritis multiplex after SARS-CoV-2 infection in an adult.

A 22-year-old male with a typical history of pauci-symptomatic COVID-19 3 weeks earlier, confirmed by positive serology for SARS-CoV-2 (IgG), was admitted to the intensive care unit because of severe myocarditis with refractory cardiogenic shock that required extracorporeal life support. Due to a clinical presentation suggestive of Kawasaki-like disease with coronary aneurysm and severe systemic inflammation, intravenous immunoglobulins were administered in combination with tocilizumab. The initial clinical course was favourable with these treatments. However, the patient subsequently develope

Supporting patients with long-term catheterisation to reduce risk of catheter-associated urinary tract infection.

More than 90 000 of the UK adult population are estimated to have a urinary catheter, with 24% likely to develop symptoms of catheter-associated urinary tract infection (CAUTI). The consequences of having a CAUTI are reduced quality of life, risk of hospitalisation and increased mortality. The authors undertook a literature review of primary research studies to identify how nurses could support patients to maintain effective catheter care to reduce the risk of CAUTI. Four themes emerged: education, knowledge, empowerment and communication. The authors therefore conclude that consistent knowled

Cord blood platelet gel for the treatment of inherited epidermolysis bullosa.

The management of patients affected by epidemolysis bullosa requires an integrated approach involving different specialties. A cornerstone of clinical management is the prevention and treatment of mechanobullous ulcerations of the patient's skin, which significantly impact the quality of life and can be the cause of septic and neoplatic complications. This article describes the preliminary clinical evaluation of the use of allogeneic cord blood platelet gel, a novel blood component obtained from umbilical cord blood of healthy, term neonates, for the treatment of skin ulcers in patients with d

Ominous triad triggered by high-dose glucocorticosteroid therapy.

Glucocorticosteroids (CS) play a key role in the treatment of numerous diseases. Nonetheless, they can be accompanied by several adverse effects. We present the case of a 51-year-old woman who was treated with high-dose CS for a relapse of her multiple sclerosis. After 5 days of treatment, the patient developed severe diabetic ketoacidosis, hypertriglyceridemia and acute pancreatitis-a potentially life-threatening triad which has previously been described, in our case, however, for the first time as a complication of CS therapy. Our patient's condition was further aggravated by a circulatory s

Guidelines for specialized nutritional and metabolic support in the critically-ill patient: update. Consensus SEMICYUC-SENPE: nutritional assessment.

Current parameters to assess nutritional status in critically-ill patients are useful to evaluate nutritional status prior to admission to the intensive care unit. However, these parameters are of little utility once the patient's nutritional status has been altered by the acute process and its treatment. Changes in water distribution affect anthropometric variables and biochemical biomarkers, which in turn are affected by synthesis and degradation processes. Increased plasma levels of prealbumin and retinol -proteins with a short half-life- can indicate adequate response to nutritional suppor

Antenatal depression.

About 20 per cent of pregnant women experience antenatal depression (AD), which not only has deleterious effects on the woman and her baby but also increases the risk of developing postpartum depression. Nurses who understand the prevalence, signs and symptoms, and risk factors associated with AD can help to identify it and prevent the sequelae. The signs and symptoms of depression in pregnancy do not differ from depression at at any other time. However, AD may go undiagnosed because of a focus on maternal and fetal well-being and the attribution of complaints to the physical and hormonal chan

The high-risk liver allograft recipient. Should allocation policy consider outcome?

The Boston Center for Liver Transplantation has accumulated one of the larger series of liver allograft recipients. This review has provided an opportunity to examine recent pronouncements by Medicare regarding patient selection and survival and to question whether the current allocation scheme best utilizes a scarce supply of donor liver allografts. Patients with primary biliary cirrhosis, sclerosing cholangitis, and metabolic derangements have enjoyed excellent survival: in aggregate, 78.9% at 1 year. In contrast, patients suffering from acute hepatic failure, patients requiring life support

Respiratory care in Duchenne muscular dystrophy.

Respiratory complications are amajor cause of morbidity and mortality in Duchenne muscular dystrophy (DMD). Progressive weakness of inspiratory and expiratory muscles leads to ineffective cough, mucus retention, sleep-disordered breathing, nocturnal hypoventilation, and ultimately chronic respiratory failure. Before the advent of mechanical ventilation, respiratory failure was the leading cause of early death. Advances in multidisciplinary care, including noninvasive ventilation (NIV), airway clearance techniques, corticosteroid therapy, scoliosis management, and cardioprotective treatments, h

Optimizing Prepregnancy Cardiovascular Health to Improve Outcomes in Pregnant and Postpartum Individuals and Offspring: A Scientific Statement From the American Heart Association.

This scientific statement summarizes the available preclinical, epidemiological, and clinical trial evidence that supports the contributions of prepregnancy (and interpregnancy) cardiovascular health to risk of adverse pregnancy outcomes and cardiovascular disease in birthing individuals and offspring. Unfavorable cardiovascular health, as originally defined by the American Heart Association in 2010 and revised in 2022, is prevalent in reproductive-aged individuals. Significant disparities exist in ideal cardiovascular health by race and ethnicity, socioeconomic status, and geography. Because
