Proximal Tibiofibular Joint Instability: Outcomes After Combined Tibiofibular Joint and Lateral Collateral Ligament Reconstruction.

BACKGROUND: Proximal tibiofibular joint (PTFJ) instability is a relatively rare clinical condition, and the establishment of an evidence-based standard of care remains an ongoing challenge because of its low incidence in the literature. Current reports in the literature focus on isolated PTFJ reconstruction, and there remains to be any report on outcomes of PTFJ reconstruction performed concurrently with procedures for associated knee injuries, such as lateral collateral ligament (LCL) insufficiency coupled with tibiofibular instability. PURPOSE: To assess subjective and clinical outcomes aft

Wearable sensor technology and machine learning for anterior cruciate ligament injury diagnosis: an exploratory feasibility study.

OBJECTIVES: To evaluate the feasibility of using wearable inertial measurement units (IMUs; small body-worn sensors that capture linear acceleration and angular velocity) combined with machine learning (ML) to identify anterior cruciate ligament (ACL) injuries during clinical knee joint laxity assessments. DESIGN: Prospective exploratory feasibility study using a case-control design. A feed-forward neural network classifier was trained on time-normalised IMU signals recorded during Lachman and Anterior Drawer tests to distinguish ACL-injured from healthy knees. Model performance was assessed

Transposition of the local extensor fascia for elbow lateral collateral ligament reconstruction: Results in a group of competitive athletes.

INTRODUCTION: The lateral ulnar collateral ligament (LUCL) is a primary elbow stabilizer against posterolateral rotatory instability (PLRI). Direct repair is the treatment of choice in the acute setting, whereas reconstruction is the best option in patients with chronic PLRI. Several autograft/allograft solutions can be used to perform LUCL reconstruction, including a local extensor fascia septum autograft. METHODS: In this retrospective multicenter study, 38 patients (26 males and 12 females) with chronic PLRI of the elbow underwent LUCL reconstruction with a local extensor fascia septum aut

Reconstruction of the medial patellofemoral ligament using a double-bundle semitendinosus tendon graft and a single suture anchor.

BACKGROUND: The double bundle medial patellofemoral ligament reconstruction (MPFL-R) is a well-established surgical technique for the management of patellar instability. While double anchor screw fixation on the patellar side is currently favored by many surgeons, this article presents a modified surgical approach involving minor technical modifications that may decrease dependence on internal implants. METHODS: From July 2016 to August 2023, the same surgeon performed two distinct MPFL-R procedures on patients with recurrent patellar dislocation. The novel single-anchor MPFL-R technique invo

Psychological Readiness and Kinesiophobia Are Not Associated With Effusion-Synovitis in Early Recovery After Anterior Cruciate Ligament Reconstruction.

CONTEXT: Effusion-synovitis commonly develops following anterior cruciate ligament reconstruction (ACLR) and may influence physical and psychological recovery. We examined the association between effusion-synovitis severity and psychological readiness to return to sport and kinesiophobia at 4 to 6 months post-ACLR. DESIGN: Cross-sectional. METHODS: We assessed 52 participants (age = 18.4 [4.0] y; 27 females; time since surgery = 5.0 [0.9] mo) between 4 and 6 months post-ACLR. We graded effusion-synovitis using the Outcome Measures in Rheumatology ordinal scale (0 = absent to 3 = severe) from

Cellular and molecular landscapes of inflammation in anterior cruciate ligament rupture patients are independent on concurrent meniscal injury.

BACKGROUND: Anterior cruciate ligament (ACL) rupture greatly increases the risk of developing knee osteoarthritis (OA), especially when accompanied by meniscal injury. Therefore, we examined biological processes in the knee joint of ACL rupture patients with or without concurrent meniscal injury. The aim of this study was to explore whether concurrent meniscal injury leads to distinct inflammatory cellular or molecular landscapes and patient reported outcome measures (PROMs), like knee function and pain, compared to an isolated ACL rupture with intact menisci. METHODS: Synovium, synovial fluid

Progressive Bilateral Alterations in Gait Biomechanics Within 2 Years After Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Bilateral gait biomechanics change over time after anterior cruciate ligament reconstruction (ACLR) and have been suggested to contribute to the development of osteoarthritis. However, few studies have investigated the longitudinal changes in bilateral gait biomechanics in the same cohort within 24 months after ACLR. PURPOSE: To evaluate the longitudinal changes in bilateral gait biomechanics compared with the healthy control cohort within 24 months after ACLR. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 24 patients who underwent primary unilateral ACLR surger

Anatomic Risk Factors Predisposing Female Handball Players to Non-contact Anterior Cruciate Ligament Injury.

Handball involves high-intensity movements that increase the risk of anterior cruciate ligament (ACL) injury, making the identification of anatomical risk factors essential. Female handball players are more vulnerable to non-contact ACL injury than male and key contributors include coronal and sagittal knee alignment abnormalities, such as increased Q-angle, valgus alignment and knee hyperextension, which predispose players to the valgus-collapse mechanism frequently observed during cutting and landing. Bony morphology in females, including wider pelvis, narrow intercondylar notch width, alter

Joint-specific mechanical work during overground running in athletes with anterior cruciate ligament reconstruction history.

BACKGROUND: Individuals with anterior cruciate ligament reconstruction (ACLR) history show altered running mechanics. Mechanical work analysis is useful for identifying compensatory strategies. Negative work represents energy absorption and positive work represents energy generation. The purpose of this study was to quantify and compare the mechanical work distribution across the hip, knee, and ankle joints during running in individuals with ACLR, uninvolved limb, and healthy control group. METHODS: Twenty-two male recreational athletes with unilateral ACLR and 22 healthy-male controls ran ov

Differential Quadriceps Muscle Activation Preoperatively and at the Time of Return to Sport After Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: Patients with patellofemoral instability (PFI) often demonstrate altered movement patterns and strength deficits. The vastus medialis plays an important role in patellar stabilization as a dynamic medial force vector on the patella. HYPOTHESIS: It was hypothesized that limb-to-limb asymmetries in muscle activity would be present preoperatively and at the time of return to sport (RTS). STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A total of 25 adolescents with unilaterally treated PFI were evaluated before arthroscopic assisted medial patellofemoral ligamen

Intrapulmonary Bronchogenic Cyst Adjacent to the Inferior Pulmonary Ligament Mimicking a Paraesophageal Mediastinal Cyst: A Case Report.

Bronchogenic cysts are rare congenital foregut malformations that most commonly arise in the mediastinum. Intrapulmonary bronchogenic cysts are less frequent and may present diagnostic challenges when located adjacent to mediastinal structures. We report a case of an intrapulmonary bronchogenic cyst that closely mimicked a paraesophageal mediastinal cyst on preoperative imaging. A 72-year-old man was referred to our department after a cystic lesion was incidentally detected during a routine health examination. CT demonstrated a 37-mm well-circumscribed cystic lesion in the right lower thorax.

Right inguinal canal endometriosis mimicking a groin mass: a rare case report of round-ligament involvement.

INTRODUCTION AND IMPORTANCE: Endometriosis is a chronic, estrogen-dependent disorder affecting over 10% of women, but extrapelvic involvement - particularly within the inguinal canal and round ligament - is rare and often mimics common groin pathologies, leading to diagnostic delays when cyclical symptoms are absent. CASE PRESENTATION: A 31-year-old nulliparous woman presented with a gradually enlarging, painful right inguinal mass over 4 months. She had regular menses and no pelvic symptoms or prior abdominal or gynecologic surgery. Examination revealed a firm, non-tender mass without cough
