The International Agency for Research on Cancer: from global evidence to national action.

As we mark the International Agency for Research on Cancer (IARC) 60th anniversary, this is an opportunity to reflect on our contribution to cancer evidence and action. Global cancer incidence continues to rise, with the largest proportional increases projected in countries that are the least equipped to respond. However, the central challenge is no longer only to produce evidence, but to understand why evidence is translated into policy and practice in some settings more effectively than in others. Drawing on the International Agency for Research on Cancer Impact in Practice series-30 country

"I need to talk, to talk to people": a qualitative study of service user and carer views and priorities for social functioning in schizophrenia-spectrum disorders.

Improving social functioning for people with schizophrenia-spectrum disorders is a priority for service users, carers and broader society. While many face challenges with social functioning, a proportion do obtain a job, a partner, and other valued social goals. Informal carers are key stakeholders, support recovery and may support service users' social functioning, though some research suggests carer and service user priorities may differ. Research suggests item content in measures of social functioning may not fully capture people's social world. The aim of this study was to explore service

Healthcare utilization patterns and medication burden in post-COVID syndrome: a cross-sectional analysis reveals four phenotypes.

Post-COVID syndrome has emerged as a major public health challenge, yet comprehensive characterization of healthcare utilization patterns and medication burden within single cohorts remains limited. This cross-sectional analysis examined baseline data from 76 patients (49 ± 12 years, 62% female) enrolled in a randomized trial of psychosomatic treatment interventions for functional post-COVID syndrome. Participants completed the Health Care Utilization Questionnaire (HCU) assessing physician visits, hospitalizations, alternative healing methods, and other healthcare services over the preceding

Planning and feasibility of phase III surgical trials in gynecologic oncology.

Surgical randomized controlled trials (RCTs) have fundamentally reshaped gynecologic oncology, yet surgical interventions remain far less likely than medical therapies to be supported by prospective evidence. This disparity arises not from a deficit of scientific ambition or a scarcity of clinically meaningful surgical questions but rather from the methodological constraints intrinsic to surgical research: operative variability across institutions and surgeons, operator-dependent outcome heterogeneity, and the difficulty of standardizing complex intraoperative decision-making. This article, th

Evaluating the impact of OMOP-CDM on data quality insight generation in respiratory disease management.

The increasing volume and heterogeneity of patient care data present significant challenges for comprehensive analysis and the generation of insights, particularly in specific areas such as respiratory diseases. Standardizing diverse health data is crucial for enabling large-scale observational research and ensuring data readiness. The Observational Medical Outcomes Partnership (OMOP) Common Data Model (CDM) provides a widely adopted standard for harmonizing such data. However, evaluating the quality of data transformed into the OMOP CDM format is a critical step before its use in research or

Parents' Perspectives on XR Distraction During Pediatric Needle Procedures.

Parents play a crucial role in supporting their children during pediatric hospital visits, where needle-related procedures are commonly performed. Both children and parents may experience distress related to needles, which can make these procedures more challenging. Adequate distraction, such as extended reality, may help to mitigate these potential difficulties. The aim of this study was to explore parents' perspectives on their child's use of extended reality as a distraction during a needle-related procedure. In this exploratory and prospective mixed-methods study, a total of 30 parents com

Clinical application of PIRCHE scores: reclassifying immunologic risk in patients with medium eplet mismatch.

BACKGROUND: Molecular histocompatibility assessment using eplet mismatch and Predicted Indirectly ReCognizable HLA Epitopes (PIRCHE) scores has improved immunologic risk stratification in kidney transplantation, but its clinical implementation remains limited, particularly for donor-recipient pairs with medium eplet mismatch. METHODS: We conducted a single-center retrospective cohort study of 493 adult kidney transplant recipients (2021-2024). Eplet mismatch was categorized based on Wiebe/Nickerson criteria, and medium mismatch pairs were further stratified using PIRCHE-T2 and PIRCHE-B scores

From Burden to Balance: Trends in Documentation Workload with Ambient Artificial Intelligence Scribe Adoption.

BACKGROUND: Physician burnout, often driven by documentation burden in electronic health records, remains a major challenge in clinical care. Ambient artificial intelligence (AI) scribes generate draft notes from clinical encounters, offering a scalable approach to reduce administrative workload, yet population-level evaluations remain limited. OBJECTIVES: The objective of this study is to assess changes over time in clinician perceptions of workload, well-being, and care delivery following implementation of ambient AI scribes. METHODS: We conducted a prospective repeated cross-sectional sur

Topical anesthetic cream vs. lidocaine infiltration for superficial laceration repair in the emergency department: a randomized clinical trial.

BACKGROUND AND IMPORTANCE: Superficial skin lacerations requiring local anesthesia before suture are common in emergency departments (EDs). Lidocaine infiltration is effective but involves needle use, which may cause pain, discomfort, and increase patient anxiety. Topical anesthetic cream offers a needle-free alternative, yet comparative evidence in adult ED populations remains limited. OBJECTIVES: To compare the analgesic efficacy of topical anesthetic cream with lidocaine infiltration for anesthesia during the repair of superficial skin lacerations in adult ED patients. DESIGN: Randomized,

High-Frequency Chest Wall Oscillation in Critical Care: A Systematic Review.

INTRODUCTION: Airway secretion retention represents a major clinical challenge in critically ill patients, promoting gas exchange impairment and atelectasis, precipitating failure of noninvasive respiratory support, and prolonging invasive mechanical ventilation, with an associated increase in respiratory-related complications and mortality. High-frequency chest wall oscillation (HFCWO) is a noninvasive airway clearance technique widely used in chronic respiratory diseases, but its role in the intensive care unit (ICU) remains unclear. This systematic review aimed to evaluate the available evi

A Systematic Review and Meta-Analysis of Postoperative Opioid Consumption, Pain and Complications Following Intraoperative Autonomic Nervous System-Based Nociception Monitoring.

BACKGROUND: Autonomic nervous system-based nociception monitors have been proposed to optimise intraoperative opioid administration. While they reduce intraoperative opioid use, evidence on postoperative outcomes remains limited. We conducted a systematic review and meta-analysis to evaluate their effects on postoperative pain, opioid consumption and complications. METHODS: MEDLINE, EMBASE, LILACS, Google Scholar, Cochrane and ClinicalTrials.gov were searched for randomised controlled trials comparing intraoperative nociception monitoring with standard care and reporting postoperative outcome

Taxonomy-free approach overcomes the gaps in ecological knowledge: The case of foraminiferal metabarcoding applied to environmental impact assessment.

Environmental DNA (eDNA) metabarcoding has emerged as a cost- and time-efficient alternative to traditional morpho-taxonomic methods for benthic foraminiferal inventories. However, the prevalence of soft-walled monothalamous taxa of unknown ecology, which are largely underrepresented in the barcode reference database, limits the accuracy of eDNA studies. Taxonomy-free approaches, which bypass species-level assignment, may overcome these constraints. Here, we use the case study of the Armida gas platform to confirm the potential of the taxonomy-free framework for ecological quality assessment.
