[Appropriate extent of lymph node dissection for clinical I a stage non-small cell lung cancer].

BACKGROUND & OBJECTIVE: There is no agreement on the appropriate extent of lymph node dissection for lung cancer, especially for early non-small cell lung cancer (NSCLC). This study was to explore the appropriate extent of lymph node dissection for early NSCLC by investigating the metastasis regulations of hilar and mediastinal lymph nodes. METHODS: Forty-one clinical Ia stage NSCLC patients received lobectomy and systematic mediastinal lymphadenectomy in Tongji Hospital from Jan. 2002 to Dec. 2004. The removed lymph nodes were subjected for pathologic examination. The metastasis regulations

Lymph Node Sampling Patterns and Completeness of Staging During Systematic Mediastinal Lymph Node Staging in Patients with Locally Advanced Non-Small-Cell Lung Cancer: A Post Hoc Analysis from the SEISMIC Study.

Introduction: Systematic endoscopic mediastinal lymph node (LN) staging in early stage non-small-cell lung cancer (NSCLC) is widely recommended for optimal pre-operative staging due to imperfect accuracy of PET/CT. The SEISMIC study utilized synoptic reporting of findings during systematic endoscopic staging to inform radiotherapy planning in patients with locally advanced NSCLC and established a significant rate of discordance between PET-identified extent of disease and observed findings following EBUS. International guidelines recommend systematic LN staging in patients with clinical nodal

Long-term complete remission in a patient with high-risk primary mediastinal B-cell lymphoma and iatrogenic symptomatic bradycardia after only two courses of DA-EPOCH-R followed by chemo-free treatment.

Most patients with Primary Mediastinal B-Cell Lymphoma (PMBCL) are cured by rituximab and doxorubicin-based immunochemotherapy, with or without radiotherapy. In cases with relapsed and refractory (RR) disease the prognosis was historically poor. Recently, immune checkpoint-based strategies have been shown to be highly effective in patients with RR-PMBCL. We report the case of a 23-year-old woman who, due to recurring episodes of symptomatic chemotherapy-induced sinus bradycardia, was unable to receive the planned six courses of immunochemotherapy, mediastinal radiotherapy, and autologous trans

High-risk patients with locally advanced non-small cell lung cancer treated with stereotactic body radiation therapy to the peripheral primary combined with conventionally fractionated volumetric arc therapy to the mediastinal lymph nodes.

INTRODUCTION: A very narrow therapeutic window exists when delivering curative chemoradiotherapy for inoperable locally advanced non-small cell lung cancer (NSCLC), particularly when large distances exist between areas of gross disease in the thorax. In the present study, we hypothesize that a novel technique of stereotactic body radiation therapy (SBRT) to the primary tumor in combination with volumetric arc therapy (VMAT) to the mediastinal lymph nodes (MLN) is a suitable approach for high-risk patients with large volume geographically distant locally advanced NSCLC. PATIENTS AND METHODS: I

Life-threatening massive bleeding in the pulmonary trunk adjacent to the right ventricular outflow tract during the resection of a large mediastinal germ cell tumor: proposed safety measures in the absence of cardiovascular surgeons: a case report.

This report presents an unusual case of life-threatening massive bleeding in the pulmonary trunk adjacent to the right ventricular outflow tract during resection of a large primary mediastinal nonseminomatous germ cell tumor (PMNSGCT) in the absence of cardiovascular surgeons. The patient was a 21-year-old male whose large mediastinal tumor was diagnosed as an extragonadal PMNSGCT, which was a mixture of a yolk sac tumor and an immature teratoma. Generally, chemotherapy causes extensive peripheral tumor necrosis of PMNSGCTs, thus enabling their complete resection. In this case, surgeons consid

The value of endobronchial ultrasound-guided transbronchial needle aspiration, 18-fluorodeoxyglucose positron emission tomography/computed tomography, and ultrasonography imaging techniques in the diagnosis of mediastinal and/or hilar malignant, anthracotic, and other benign lymph nodes.

INTRODUCTION: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a simple, reliable, minimally invasive and effective procedure. However, a surgical technique may be required, if the results are negative. Therefore, there is a need for new studies to increase the diagnostic value of EBUS-TBNA and provide additional information to guide the biopsy in performing the procedure. Here, we aimed to investigate the diagnostic value of EBUS-TBNA and 18-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in diagnosis of hilar and/or mediasti

Diagnostic yield of endobronchial ultrasound-guided transbronchial needle aspiration to assess tumor-programmed cell death ligand-1 expression in mediastinal lymph nodes metastasized from non-small cell lung cancer.

PURPOSE: We investigated the utility of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) to evaluate programmed cell death ligand-1 (PD-L1) expression in patients with advanced non-small cell lung cancer (NSCLC). METHODS: A retrospective chart review of patients who underwent EBUS-TBNA between April 2017 and April 2019 was conducted. Among patients diagnosed with NSCLC, we investigated the rate of successful evaluation of tumor PD-L1 expression, compared the relevant factors between patients with evaluable and those with unevaluable PD-L1 expression, and examined t

Assessment of nodal involvement in non-small-cell lung cancer with 18F-FDG-PET/CT: mediastinal blood pool cut-off has the highest sensitivity and tumour SUVmax/2 has the highest specificity.

OBJECTIVES: Lymph node involvement in non-small-cell lung cancer (NSCLC) is a major factor in determining management and prognosis. We aimed to evaluate the accuracy of fluorine-18-fluorodeoxyglucose-PET/computed tomography (CT) for the assessment of nodal involvement in patients with NSCLC. PATIENTS AND METHODS: In this retrospective study, we included 61 patients with suspected or confirmed resectable NSCLC over a 2-year period from April 2013 to April 2015. 221 nodes with pathological staging from surgery or endobronchial ultrasound-guided transbronchial needle aspiration were assessed usi

The method and efficacy of support vector machine classifiers based on texture features and multi-resolution histogram from (18)F-FDG PET-CT images for the evaluation of mediastinal lymph nodes in patients with lung cancer.

OBJECTIVES: In clinical practice, image analysis is dependent on simply visual perception and the diagnostic efficacy of this analysis pattern is limited for mediastinal lymph nodes in patients with lung cancer. In order to improve diagnostic efficacy, we developed a new computer-based algorithm and tested its diagnostic efficacy. METHODS: 132 consecutive patients with lung cancer underwent (18)F-FDG PET/CT examination before treatment. After all data were imported into the database of an on-line medical image analysis platform, the diagnostic efficacy of visual analysis was first evaluated w

Late valvular and other cardiac diseases after different doses of mediastinal radiotherapy for Hodgkin disease in children and adolescents: report from the longitudinal GPOH follow-up project of the German-Austrian DAL-HD studies.

BACKGROUND: To analyze the impact of mediastinal irradiation on the incidence of cardiac late effects in long-term survivors of pediatric Hodgkin disease (HD). METHODS: The study cohort comprised 1,132 survivors of HD who received treatment before 18 years of age in consecutive trials between 1978 and 1995. They had maintained remission without secondary malignancy for 3.1-29.4 years. The cumulative doxorubicin dose was uniformly 160 mg/m(2), the mediastinal radiation dose (MedRD) was 36, 30, 25, 20, or 0 Gy. Follow-up questionnaires complemented by additional contacts served to collect infor

Evaluation of traumatic aortic injury: does dynamic contrast-enhanced CT play a role?

To investigate the value of 5-mm contrast material-enhanced computed tomography (CT) in patients with moderate to low probability of aortic laceration after a substantial deceleration injury, scans were obtained through the upper mediastinum in 160 consecutive patients. Thoracic angiography and aortography were performed in patients with evidence of mediastinal hemorrhage at CT. There was no evidence of mediastinal hemorrhage in 132 patients with normal admission chest radiographs. In the 28 patients with abnormal admission chest radiographs, CT helped exclude mediastinal hemorrhage in 22 pati

[Surgical treatment of upper mediastinum involvement following thyroid cancer surgery].

Four cases with mediastinal nodal involvements after surgery for thyroid cancer were treated by tumor extirpation and mediastinal lymph nodes dissection. The removal were performed via the median sternotomy in three cases and partial resection of the clavicle in one case. The transsternal approach made it possible to remove the tumor with mediastinal lymph nodes safely and easily. It is generally sufficient to carry out dissection as far as the brachiocephalic vein. However, mediastinal nodal involvements extending below the brachiocephalic vein were seen in one of 4 cases. All patients are al

Axillary-coronary artery bypass using a vein graft via the suprasternal route.

Redo coronary artery bypass grafting after an in-situ right internal thoracic artery graft to the left anterior descending artery is challenging. For such a 52-year-old male patient with a history of mediastinitis, we performed redo bypass grafting of the right coronary artery using a saphenous vein graft and xiphoid resection via the suprasternal route with the left axillary artery as the inflow source and the graft were patent. The axillary artery is an inflow source for patients with inaccessible aorta. Combining the suprasternal route with a xiphoid resection provides a bail-out option.

Pictorial review of computed tomography and magnetic resonance imaging findings of cardiovascular manifestations of IgG4-related disease.

This study includes a series of 5 cases demonstrating cardiovascular manifestations of findings of IgG4-related disease. Two cases demonstrate peri-aortic soft tissue thickening in the infrarenal abdominal aorta and bilateral ostio--proximal common iliac artery. In 2 cases there were circumferential soft tissue lesions around the arch of the aorta. One of the cases showed coexistent, biopsy-proven Riedel's thyroiditis and infiltrative soft tissue along the right atrial wall and interatrial septum. In one case there was a partly calcified mass in the left hemi-thorax consistent with a diagnosis

Transcervical pharyngeal pouch stapling.

BACKGROUND: Pharyngeal pouches are generally treated with surgery, via either an endoscopic or an external approach. Large pouches extending to the mediastinum carry an increased risk of post-operative mediastinitis in the event of a leak following external approach surgery, and may not always be amenable to endoscopic stapling. METHODS: Transcervical stapling is a newly described technique that uses the endoluminal stapling approach but in which the diverticuloscope is inserted into the pharynx via a neck incision. CONCLUSIONS: This technique has been used successfully on three patients wit

Bronchoscopical Repair of Tracheal Laceration Using Conventional Instrumentation: A Novel Technique.

Postintubation tracheobronchial laceration is a rare complication of endotracheal intubation. It requires early serial treatment in cases of pneumomediastinum with difficulty in ventilation to prevent mediastinitis and stricture. The surgical access to the posterior tracheal wall depends on the site of the laceration. A right posterolateral thoracotomy is performed for lacerations of the middle-distal trachea and a transcervical approach for the upper third. Either of them is associated with specific postoperative morbidities. We describe a novel technique of exclusively intraluminal repair of

Retrosternal salvage reconstruction of esophageal discontinuity for a necrotic gastric tube after esophagectomy: A novel procedure.

In patients with esophageal cancer, esophageal conduit necrosis is a catastrophic complication of esophagectomy that requires surgical restoration. Because such patients are generally fatigued, less-invasive surgery is encouraged whenever possible. Therefore, we trim the sternum minimally above the healthy part of the gastric conduit, expose its surface, and then make anastomoses between the remnant esophagus and the exposed gastric conduit using a free jejunal graft through a retrosternal-subcutaneous route. The risk involved with this procedure is low, because we avoid manipulation of the he

Esophageal necrosis after endovascular repair for ruptured aortic dissection.

A 78-year-old man presented with back pain and shock and was transferred to our hospital. Computed tomography showed a ruptured aortic dissection in which the false lumen was thrombosed with an ulcer-like projection, and the mid-esophagus was shifted to the right due to a mediastinal hematoma. He underwent emergency thoracic endovascular aortic repair of the descending thoracic aorta. One week later, esophageal necrosis occurred, and he died of mediastinitis and sepsis on postoperative day 16. Although esophageal necrosis is a rare and fatal complication after thoracic endovascular aortic repa
