Twenty years of chalazion, big data analysis.

PURPOSE: To describe large-scale epidemiological patterns of chalazia and assess treatment outcomes. METHODS: We conducted a retrospective cohort study using the Clalit Health Services electronic medical record database, analyzing demographic data, medical history, treatments, procedures, and outcomes between January 2003 and December 2022. RESULTS: A total of 611,993 chalazion diagnoses were recorded in 412,365 patients. Invasive procedures were required in 56,840 cases (9.3%). After topical therapy, 11.2% of cases progressed to surgical management (18.3% with neomycin/polymyxin-B/dexametha

Serum Homocysteine as a Potential Dynamic Biomarker for Staging and Monitoring Progression in Alzheimer's Disease.

BACKGROUND: Diffuse unpatterned alopecia (DUPA) is an uncommon and incompletely characterized variant of androgenetic alopecia (AGA) marked by diffuse follicular miniaturization across the entire scalp, including the occipital region, which distinguishes it from classic patterned AGA and often precludes hair transplantation. SUMMARY: In this review of the available literature on DUPA, we discuss its clinical presentation, diagnostic features, competing pathophysiologic hypotheses, and management considerations. Current evidence suggests DUPA typically presents with diffuse thinning across all

Recommendations for radiotherapy in patients with lung cancer and interstitial lung disease: A Delphi consensus process of the Italian association of radiotherapy and clinical oncology (AIRO).

AIM: Interstitial lung diseases (ILD) are a group of disorders affecting the lung parenchyma. ILD patients have a higher risk of lung cancer and worse prognosis, with increased susceptibility to radiotherapy (RT)-related toxicities due to ILD exacerbations. There are no clear guidelines for managing lung cancer patients with ILD undergoing RT, alone or with systemic therapies. This work aims to establish a consensus on ILD assessment, patient selection, risk/benefit analysis, and clinical management for those eligible for thoracic RT. METHODS: Using the Estimate-Talk-Estimate method, a board

Management and Outcomes of Renal Artery Aneurysms in Older Adults.

Renal artery aneurysms (RAAs) are rare but clinically significant vascular abnormalities. The management of RAA in older adults poses unique challenges related to their baseline comorbidities and age-related vascular changes. Traditionally, open surgery has been the gold standard for large or complex RAA. However, recent data and guidelines support a more conservative threshold for intervention and broader adoption of endovascular therapies, which offer lower perioperative morbidity, shorter hospital stays, and overall faster recovery. General indications for repair include symptomatic aneurys

Heart failure with reduced ejection fraction.

Heart failure is a complex clinical syndrome affecting around 70 million individuals globally. It has a prevalence of 2% in Europe and North America and approximately 1% in Asia and South America. Accurate diagnosis relies on the presence of typical signs and symptoms, elevated natriuretic peptide concentrations, and evidence of cardiac structural or functional abnormalities using cardiac imaging techniques. Approximately half of all heart failure cases are attributed to reduced left ventricular systolic function-classified as heart failure with reduced ejection fraction (HFrEF). Current guide

Prescriber Specialty Involvement in Medicare Patients With Chronic Hepatitis B.

Chronic hepatitis B (CHB) is a major cause of liver-related morbidity and mortality in the United States. Patients with CHB require long-term antiviral treatment and consistent follow-up, but often face numerous barriers to accessing care and medications. In this study, we used the Medicare Part D database and the Rural-Urban Continuum code to explore specialty and geographic characteristics of healthcare providers that manage Medicare patients with CHB. Between 2013 and 2021, more than 7000 prescribers prescribed over 2.4 million 30-day prescriptions of these CHB therapies, of which 2 million

Perioperative Care of Transgender and Gender-Diverse Patients: A Biopsychosocial Approach.

Transgender and gender-diverse (TGD) people endure numerous physical and mental health disparities secondary to lifelong stigma and marginalization, which are often perpetuated in medical spaces. Despite such barriers, TGD people are seeking gender-affirming care (GAC) with increased frequency. GAC facilitates the transition from the sex assigned at birth to the affirmed gender identity and is comprised of hormone therapy (HT) and gender-affirming surgery (GAS). The anesthesia professional is uniquely poised to serve as an integral support for TGD patients within the perioperative space. To pr

Abdominal Aortic Aneurysm.

Abdominal aortic aneurysm is a pathologic condition with progressive abdominal aortic dilatation of 3.0 cm or more that predisposes the abdominal aorta to rupture. Most abdominal aortic aneurysms are asymptomatic until they rupture, although some are detected when an imaging study is performed for other reasons. The risk factors for abdominal aortic aneurysm include hypertension, coronary artery disease, tobacco use, male sex, a family history of abdominal aortic aneurysm, age older than 65 years, and peripheral artery disease. Abdominal ultrasonography is the preferred modality to screen for

High risk and low prevalence diseases: Tubo-ovarian abscess.

INTRODUCTION: Tubo-ovarian abscess (TOA) is a rare but serious condition that carries with it a high rate of morbidity and even mortality. OBJECTIVE: This review highlights the pearls and pitfalls of TOA, including diagnosis, initial resuscitation, and management in the emergency department (ED) based on current evidence. DISCUSSION: TOA is associated with pelvic inflammatory disease (PID) as well as intrauterine devices, uterine procedures, multiple sexual partners, diabetes mellitus, and immunocompromised states. While usually arising from a gynecologic infection, TOA can be associated wit

Abnormal uterine bleeding: managing endometrial dysfunction and leiomyomas.

Abnormal uterine bleeding refers to any change in the regularity, frequency, heaviness or length of menstruation. There are several potential causes for bleeding disturbance, the two most common being primary endometrial dysfunction and fibroids. Management of abnormal uterine bleeding involves both medical and surgical options and will largely depend on a patient's fertility plans. The use of levonorgestrel-releasing intrauterine devices for heavy menstrual bleeding is increasing in Australia, and they are considered first-line medical management for women accepting of hormonal therapies. Sedation scales: Do they capture the concept of opioid-induced sedation?

AIM: The purpose of this study was to explore the concept of opioid-induced sedation and how nurses define and measure sedation in the hospital setting. BACKGROUND: Opioid medications are the primary treatment for acute pain in the postoperative setting. One of the most serious side effects of opioid therapy is excessive sedation and respiratory depression. Nurses have the responsibility of providing effective pain management, while keeping the patient safe from adverse sedation and respiratory depression. Thus, the assessment of sedation becomes an integral part of the nurses' responsibiliti

Medical Expulsive Therapy: Is It Time to SUSPEND Judgement?

Medical expulsive therapy (MET) has been a matter of debate for many years. A number of randomised controlled trials (RCTs) and meta-analyses have been conducted, but outcomes have been varied. This makes it challenging to determine the benefit of using MET as an adjunct in patients with expectantly managed ureteric stones. This article aimed to summarize both the historic and the more contemporary literature in this area by focusing on published meta-analyses and recent RCTs. Studies of interest were those comparing either an α-blocker or a calcium channel blocker with a control. Outcome meas

[Treatment of hand ischemia with arterialization of the venous system of the hand: report of three cases].

INTRODUCTION: Hand ischemia is a rare phenomenom of various etiologies among which can be found Raynaud's phenomenon and autoimmune arteritis. The limitations of symptomatic medical treatment and the frequent plurisegmental and distal affection of arteries make the management of chronic hand ischemia difficult. The arterialization of the superficial venous system, which consists in redirecting the arterial flow to the fingertip through the dorsal venous system, could be an interesting alternative to the disappointing results of medical therapy and surgical sympathectomy. Behavioral intervention for cancer treatment side effects.

The use of increasingly aggressive methods of cancer treatment during the last 20 years has brought clinical attention to the need for more effective management of pain, nausea, and other aversive side effects of state-of-the-art cancer therapy. One of the most promising approaches to effective management is nonpharmacologic intervention based on behavioral research and theory. The purpose of this review is to examine the effectiveness of behavioral intervention methods in the control of aversive side effects of cancer treatments. Fifty-four published studies using a variety of research design

Extra-uterine pregnancy following assisted conception treatment.

Ectopic pregnancy may be the only life-threatening disease in which prevalence has increased as mortality has declined. The most prominent theory to explain this phenomenon involves increased sensitivity of serum beta-human chorionic gonadotrophin (HCG) immunoassay and improved quality of transvaginal ultrasound, combined with a heightened awareness and increased suspicion of the condition among clinicians which has allowed early detection of ectopic pregnancy. Laparotomy, once the standard treatment of ectopic pregnancy, has been replaced almost entirely by operative laparoscopy. This is asso

First-Line Treatment of Symptomatic Subocclusive Large-Vessel Stroke: Results of a Nationwide StrokeNet Survey.

BACKGROUND: Limited evidence is available for the treatment of acute symptomatic sub-occlusive lesions in ischemic stroke. We sought to identify current treatment patterns of stroke teams at academic health centers. METHODS: We conducted an email survey of the National Institutes of Health (NIH) StrokeNet regional coordinating centers (RCCs). Each RCC principal investigator (PI) was asked to nominate a local stroke interventionalist, or neurologist if the RCC PI was an interventionalist, most aligned with the typical practice pattern of the RCC's lead hospital, to receive a survey. The survey
