Routine cholangiography is not warranted during laparoscopic cholecystectomy.

The role of intraoperative cholangiography during laparoscopic cholecystectomy was prospectively evaluated in 514 patients undergoing laparoscopic cholecystectomy. Before surgery, all patients were assigned to one of three groups depending on the likelihood of their having common bile duct stones. Stratification was based on objective historical, laboratory, or radiologic criteria. In 453 patients deemed unlikely to have stones, laparoscopic cholecystectomy was performed without cholangiography. Of these patients, four had retained stones (0.9%). In 25 patients likely to have stones, preoperat

Assessment of myocardial perfusion during CABG surgery with two-dimensional transesophageal contrast echocardiography.

No reliable, quantifiable index of tissue perfusion is currently available to assess the efforts of coronary artery bypass graft (CABG) surgery. We used two-dimensional transesophageal contrast echocardiography with sonicated Renografin-76 microbubbles to determine the distribution of myocardial blood flow during coronary artery bypass graft surgery in 15 patients. Sonicated Renografin-76 contrast agent was injected into the aortic root of all patients after institution of cardiopulmonary bypass and application of the aortic occlusive clamp. Eight patients had contrast agent injected directly

Textbook outcome after robotic liver surgery at a high-volume Scandinavian HPB center: an IWATE-based difficulty-stratified assessment of 200 cases.

BACKGROUND: Textbook outcome (TO) provides a patient-centered composite measure of perioperative quality in liver surgery. Although robotic liver surgery (RLS) is increasingly adopted, data applying internationally harmonized TO definitions to RLS remain limited, particularly when interpreted according to procedural difficulty. METHODS: We reviewed 200 consecutively performed RLSs at a high-volume hepatopancreatobiliary center in Denmark. TO after RLS (TORLS) was assessed by applying the internationally established Delphi consensus definition of textbook outcome in liver surgery to the RLS co

Single-center longitudinal experience with MRgLITT in pediatric neurosurgery: technical evolution, expanding indications, and advanced MRI-driven planning.

OBJECTIVE: The objective of this study is to evaluate the 6-year clinical and technological evolution of MR-guided Laser Interstitial Thermal Therapy (MRgLITT) in a pediatric and young adult cohort. We specifically analyze the paradigm shift from "rescue therapy" to a "first-line" intervention for complex, deep-seated lesions, enabled by the transition from manual stereotaxis to robotic and intraoperative MRI-guided workflows. The primary objective was to evaluate how technological evolution influenced safety in high-risk anatomical targets. METHODS: We retrospectively analyzed 63 procedures

Lumbar Spine Surgery in a Patient With Recent Monkey Bite Receiving Rabies Immunoglobulin: A Case Report on Perioperative Anesthetic Challenges.

Rabies is a universally fatal acute zoonotic disease caused by rhabdovirus, and postexposure prophylaxis (PEP) remains the mainstay of prevention. Patients who sustain animal bites may require urgent or elective surgical interventions for unrelated conditions while simultaneously undergoing rabies prophylaxis. The perioperative team often faces uncertainty regarding the safety of anesthesia and surgery in patients receiving rabies vaccines or rabies immunoglobulin (RIG). This report discusses such a scenario in a patient posted for elective lumbar spine surgery. We report the case of a male in

NuPOWER (Nuwiq for Perioperative management Of patients With haemophilia A on Emicizumab Regular prophylaxis): protocol for an open-label, single-arm, multicentre study.

INTRODUCTION: Despite the known haemostatic action of emicizumab (Hemlibra) in haemophilia A patients, its role in the prevention and control of bleeding in high-demand haemostatic situations, such as major surgery, remains to be determined. Patients receiving regular emicizumab prophylaxis often require concomitant factor VIII (FVIII) therapy during major surgery to prevent uncontrolled bleeding and to promote postoperative healing. However, there are limited prospective surgical data relating to concomitant FVIII and emicizumab use. Simoctocog alfa (Nuwiq) is a B-domain deleted recombinant F

Prediction model for postoperative pulmonary complications after thoracoscopic surgery with machine learning algorithms and SHapley Additive exPlanations.

BACKGROUND: Postoperative pulmonary complications (PPCs) are common and have a negative impact on postoperative morbidity and mortality, with associated medical resource use and cost care plan. Management of preoperative and intraoperative risk factors has been shown to reduce the occurrence of PPCs. Therefore, this study aimed to develop a risk prediction model for PPCs based on explainable machine learning (ML) methods and evaluate its predictive performance in order to enhance the prevention and intervention for PPCs. METHODS: In this study, the medical records of 1,629 patients who underw

Staged short-term modified halo-pelvic traction and posterior spinal fusion versus posterior vertebral column resection for severe rigid kyphoscoliosis: a multicenter comparative study.

BACKGROUND CONTEXT: Surgical correction of severe rigid kyphoscoliosis remains technically challenging and is associated with high complication rates. Posterior vertebral column resection (pVCR) is often required for satisfactory correction but entails substantial surgical risks. Halo-pelvic traction (HPT) has been proposed as a safer alternative that may reduce the need for high-grade osteotomy by partially correcting the deformity preoperatively. PURPOSE: To compare the clinical outcomes of HPT combined with posterior spinal fusion (HPT+PSF) versus pVCR for severe rigid kyphoscoliosis, focu

Effect of raw electroencephalogram-guided anesthesia administration on postoperative outcomes in elderly patients undergoing abdominal major surgery: a randomized controlled trial.

BACKGROUND: EEG monitoring techniques are receiving increasing clinical attention as a common method of reflecting the depth of sedation in the perioperative period. The influence of depth of sedation indices such as the bispectral index (BIS) generated by the processed electroencephalogram (pEEG) machine to guide the management of anesthetic depth of sedation on postoperative outcome remains controversial. This research was designed to decide whether an anesthetic agent exposure determined by raw electroencephalogram (rEEG) can influence anesthetic management and cause different EEG patterns

5-Fluorouracil in primary, impending recurrent and recurrent pterygium: Systematic review of the efficacy and safety of a surgical adjuvant and intralesional antimetabolite.

Pterygium is an ultraviolet-related disease characterized by an aberrant, wing-shaped and active wound-healing process. There is nothing quite as disheartening for the surgeon or patient as the recurrence of pterygium, and various adjuvants have been studied to ameliorate this. This systematic review provides a comprehensive summary of the efficacy and safety of 5-Fluorouracil (5-FU) as an antimetabolite agent for pterygium management. An appraisal of electronic searches of six databases identified 34 clinical studies reporting recurrence outcomes of 5-FU use in primary, impending recurrent an

Fluid Extravasation in Shoulder Arthroscopic Surgery: A Systematic Review.

BACKGROUND: Arthroscopic surgery of the shoulder joint has become increasingly more common given its advantages over open surgery; however, one rare but potentially life-threatening complication is fluid extravasation into the surrounding tissues, causing edema, respiratory compromise, abnormal results on laboratory blood tests, and possibly death. Currently, no systematic review exists that summarizes the existing clinical research on this topic. PURPOSE: To perform a systematic review on fluid extravasation as a complication of shoulder arthroscopic surgery, specifically assessing clinical
