The role of subchondral bone damage in post-traumatic osteoarthritis.

Osteoarthritis (OA), the most common musculoskeletal disease in the United States, is characterized by cartilage breakdown, pain, and restricted movement. Post-traumatic OA (PTOA) occurs subsequent to traumatic joint injury, such as anterior cruciate ligament (ACL) rupture, and makes up 12% of the overall disease burden, with healthcare costs of approximately $3 billion/year. The current paradigm for PTOA is based on the observation that joint injury affects multiple tissues, all of which may contribute to subsequent joint failure. Subchondral bone plays a significant role in PTOA, as shown by

New developments in the pathogenesis of obesity-induced hypertension.

Obesity is a disorder that develops from the interaction between genotype and environment involving social, behavioral, cultural, and physiological factors. Obesity increases the risk for type 2 diabetes mellitus, hypertension, cardiovascular disease, cancer, musculoskeletal disorders, chronic kidney and pulmonary disease. Although obesity is clearly associated with an increased prevalence of hypertension, many obese individuals may not develop hypertension. Protecting factors may exist and it is important to understand why obesity is not always related to hypertension. The aim of this review

Proteomic analysis of ligamentum flavum from patients with lumbar spinal stenosis.

Lumbar spinal stenosis (LSS) is a syndromic degenerative spinal disease and is characterized by spinal canal narrowing with subsequent neural compression causing gait disturbances. Although LSS is a major age-related musculoskeletal disease that causes large decreases in the daily living activities of the elderly, its molecular pathology has not been investigated using proteomics. Thus, we used several proteomic technologies to analyze the ligamentum flavum (LF) of individuals with LSS. Using comprehensive proteomics with strong cation exchange fractionation, we detected 1288 proteins in these

Heterotopic ossificans in chronic venous insufficiency: a new consideration for clinical, aetiology, anatomy and pathophysiology staging.

OBJECTIVES: Heterotopic ossification is defined as the abnormal formation of true bone within extra-skeletal soft tissues. It may be associated with a variety of clinical conditions, but is most frequently seen with musculoskeletal trauma, neurologic injury or genetic abnormalities. It has also been described in patients with chronic venous insufficiency; however, it often goes underdiagnosed due to chronic ulceration that masks exam findings. To date, few reports of heterotopic ossification due to chronic venous disease exist within the literature with the most recent dating back to the 1970s

Update on treatment of psoriatic arthritis.

Some of this past year's key papers or abstracts on psoriatic arthritis (PsA) assessment and treatment are reviewed in this paper. Treatment begins with identification of the PsA patient. Several screening questionnaires have been developed to be used in dermatology and primary care settings to identify which patients with psoriasis have developed PsA as opposed to other common musculoskeletal problems, such as osteoarthritis and fibromyalgia, thus increasing case-finding and targeting referral. PsA can present in a heterogeneous manner, involving arthritis, enthesitis, dactylitis, spondylitis

Bread mold osteomyelitis in the femur.

Rhizopus osteomyelitis is an uncommon and often fatal infection that usually occurs in immunocompromised patients. The infection is commonly referred to as "bread mold." The usual course of treatment is Amphotericin B, debridement, and, if needed, amputation of the affected limb. This article details a rare case of postoperative Rhizopus osteomyelitis in an otherwise healthy patient. The patient originally presented at another institution for anterior cruciate ligament repair after a ski injury. Postoperatively, he developed clinical evidence of infection. He was referred to our institution wi

A rare case of Paget's disease of the bone in the Philippines.

A 40-year-old Filipina presented with a 2-year history of progressive bilateral inguinal and hip pain with limited range of motion. She had a bad fall which led to widespread body pains for 6 months partially relieved on multiple analgesic and anti-inflammatory agents. There was progressive weight loss and anemia. Focused musculoskeletal examination revealed moderate degree of generalized muscle atrophy, pain-limited range of motion of bilateral hip and shoulder joints, with equivocal muscle strength of both proximal and distal muscle groups of all extremities. Metastatic bone disease was rule

Innovative therapies in osteoarthritis.

Until recently, osteoarthritis (OA) was classified as a mechanical wear-and-tear disorder of articular cartilage, for which only pain-modifying therapies such as nonaddictive analgesics were prescribed. Little scientific attention had been focused on the patient with OA, who typically was seen as a frail elderly person hobbling down the street with a cane. With the demographic change that is facing medical policy makers, musculoskeletal disability will decrease the quality of life of the elderly population. By way of analogy, the medical establishment viewed osteoporosis as a similar disease p

Pelvic reconstruction for severe periacetabular metastatic disease.

The functional outcome of acetabular reconstruction in 21 patients with severe acetabular deficiency was examined. The surgical indications were pain, immobility, and loss of indepedence. Surgical reconstruction included curettage, placement of an acetabular roof ring with screws crossing the defect, cementation of the defect, and cemented hip arthroplasty. Although three patients died within the first month, the mean postoperative survival was 14.5 +/- 4.0 months, with a 10.4 +/- 3.0-month period of independent living. Statistically significant postoperative improvements were seen in pain and

HLA-B27-associated spondyloarthritis and enthesopathy in childhood: clinical, pathologic, and radiographic observations in 58 patients.

HLA-B27 typing of all arthritic children helped to identify and focus attention on a subset whose disease was pathogenetically related to and demonstrated clinical features of ankylosing spondylitis and Reiter syndrome, but only rarely fulfilled current diagnostic criteria for those disorders (spondyloarthritis). In contrast to other forms of childhood arthritis, enthesopathy (inflammation at the sites of attachment of ligaments and tendons to bone) was a prominent feature in 75%; a family history of similar arthritis was obtained from 60%; boys were more frequently affected (2:1); urethritis,

Chronic pain in the aging adult with sickle cell disease.

Improved survival in sickle cell disease (SCD) has led to a growing population of older adults who experience a high burden of chronic pain. With aging, pain often shifts from episodic vaso-occlusive crises to persistent, multifactorial syndromes driven by cumulative tissue injury, central sensitization, and progressive organ damage. This evolution presents significant clinical challenges, particularly in the context of comorbidities, polypharmacy, and age-related physiological changes. This narrative review examines the epidemiology, pathophysiology, and management of chronic pain in aging ad

Nutrition in Osteoarthritis care: expanding the physical therapist's role in multimodal management.

Osteoarthritis (OA) is one of the leading causes of pain, disability, and reduced quality of life worldwide and has traditionally been managed through exercise, physical activity, and symptom-focused rehabilitation. However, growing evidence suggests that OA is not solely a mechanical "wear-and-tear" condition but a multifactorial disease influenced by chronic low-grade inflammation, metabolic dysfunction, obesity, and lifestyle behaviors. This evolving understanding highlights nutrition as a potentially important, yet underutilized, component of comprehensive OA management. Physical therapist

Vision-based work posture assessment: monocular pose estimation and fuzzy REBA methods.

With the implementation of China's National Occupational Disease Prevention Plan (2021-2025), workers' health has gained growing attention. To reduce incidence of work-related musculoskeletal disorders (MSDs), this study developed a contactless, vision-based system for posture risk assessment. An MMPose-based monocular pose estimation model was designed to extract skeletal landmarks for joint-angle computation, and it demonstrated the accuracy superior to that of Kinect V2 in challenging industrial environments. Experimental validation demonstrated that the proposed model achieved an average j

Anatomical reduction for focal fasciculations in peroneus brevis spondylolisthesis: a case report suggesting a mechanism of peripherally derived tremor.

CASE: A 9-year-old girl presented with visible, rhythmic fasciculations in the peroneal muscle groove of her right ankle at rest, accompanied by pain and mild movement limitation. Dynamic ultrasonography showed persistent intrasheath subluxation of the peroneus brevis tendon, with fasciculations correlating with tendon dislocation. After excluding motor neuron disease and metabolic disorders, she underwent surgery involving fibular groove deepening and retinaculum repair. At the 6-month follow-up, ultrasound demonstrated tendon stability and no recurrence of fasciculations at rest or during ex

Hypercortisolism: Causes, Consequences and Clinical Significance - A Review of Pathophysiology.

Hypercortisolism or Cushing syndrome is a heterogeneous clinical spectrum caused by chronic glucocorticoid excess, ranging from exogenous Cushing syndrome to rare endogenous aetiologies and the increasingly recognised entity of mild autonomous cortisol secretion (MACS). Physiological cortisol production is tightly regulated by the hypothalamic-pituitary-adrenal axis, and disruption at any level may lead to clinically significant cortisol excess. Endogenous hypercortisolism is broadly classified into corticotropin-dependent and corticotropin-independent forms. The clinical phenotype varies acco

Chikungunya: From Hypothesis to Evidence of Increased Severe Disease and Fatalities.

Chikungunya virus infection often manifests as an acute, self-limiting febrile illness, with arthralgia and musculoskeletal symptoms being the most commonly reported. Arthralgia can persist for months or even years, and approximately 50% of cases progress to chronic conditions. However, recent outbreaks have revealed a rising number of severe cases and fatalities. This review examines evidence from the past decade that suggests a higher incidence of severe chikungunya virus (CHIKV) infections and increased mortality rates, challenging official reports and guidelines from many countries. The li

An annotated heterogeneous ultrasound database.

Ultrasound is a primary diagnostic tool commonly used to evaluate internal body structures, including organs, blood vessels, the musculoskeletal system, and fetal development. Due to challenges such as operator dependence, noise, limited field of view, difficulty in imaging through bone and air, and variability across different systems, diagnosing abnormalities in ultrasound images is particularly challenging for less experienced clinicians. The development of artificial intelligence (AI) technology could assist in the diagnosis of ultrasound images. However, many databases are created using a

Hydrogen Sulfide in Balneology: Physiology, Evidence, and Clinical Translation.

This review integrates the biology and clinical translation of hydrogen sulfide (H2S) in balneology. It frames H2S as a gasotransmitters with dual chemical and biological actions and summarizes the H2S/HS- equilibrium as a function of pH, temperature, and oxygenation, which governs bioaccessibility in sulfurous waters. Endogenous and exogenous sources, transport, and mitochondrial catabolism are outlined, together with core cellular mechanisms: protein persulfidation; activation of Nrf2/ARE; modulation of NF-κB; regulation of ion channels; and engagement of PI3K/Akt, MAPK/ERK, and Wnt pathways
