Impact of Proximal Optimization Technique in Provisional Bifurcation Percutaneous Coronary Intervention: Insights from PROGRESS-BIFURCATION Registry.

The proximal optimization technique (POT) improves proximal main vessel stent expansion and facilitates side-branch access during bifurcation percutaneous coronary intervention (PCI), yet its clinical impact remains unclear. We analyzed data from the PROGRESS-BIFURCATION registry (NCT05100992), a multicenter observational registry including patients treated at 7 centers in the United States, Russia, and Turkey between 2013 and 2025. Procedural characteristics and outcomes were compared between provisional bifurcation PCIs performed with versus without POT. Among 2,679 bifurcation PCIs (2,454 p

Impact of chronic kidney disease on long-term outcomes after left main percutaneous coronary intervention: a propensity score-matched analysis with up to 10 years of follow-up.

INTRODUCTION: Chronic kidney disease (CKD) is a major determinant of adverse cardiovascular outcomes. Patients with CKD undergoing percutaneous coronary intervention (PCI) represent a particularly high-risk population; however, data on the long-term prognostic impact of CKD in patients treated with PCI for left main coronary artery disease (LM CAD) remain limited. OBJECTIVES: This study aimed to evaluate the long-term impact of CKD on outcomes after left main PCI. PATIENTS AND METHODS: This single-center, prospective registry study analyzed 1321 patients who underwent left main PCI. Propensi

Inflammatory Response After Elective PCI and Subsequent Outcomes in Stable Coronary Artery Disease.

Background/Objectives: Inflammation contributes to atherosclerotic progression and may intensify after percutaneous coronary intervention (PCI). In stable coronary artery disease (CAD), whether post-procedural inflammation is prognostically distinct from baseline inflammatory status and post-procedural renal function remains unclear. Methods: We studied 496 consecutive patients with stable CAD undergoing elective PCI. The primary outcome was a composite of death, myocardial infarction, stroke, or repeat revascularization. Post-PCI high-sensitivity C-reactive protein (hs-CRP) and creatinine wer

The Co-Occurrence of Pertrochanteric Fracture and Acute Coronary Syndrome in a Geriatric Patient: A Case Report and Review of the Literature.

BACKGROUND: Managing elderly patients with simultaneous acute cardiovascular and orthopedic emergencies presents a unique challenge. While ST-elevation myocardial infarction (STEMI) requires prompt revascularization and dual antiplatelet therapy (DAPT), pertrochanteric femoral fractures usually necessitate early surgical fixation to reduce morbidity and mortality. However, the combination of these conditions complicates both standard treatment pathways. CASE PRESENTATION: We present the case of an 86-year-old woman admitted after a low-energy fall, with a radiologically confirmed unstable per

Longitudinal Psychosocial and Sleep Assessments for Long-Term Cardiovascular Risk Stratification after First Myocardial Infarction in Rural India.

OBJECTIVES: The risk of myocardial infarction (MI) evolves beyond the acute phase, and in-hospital tools, such as the Global Registry of Acute Coronary Events (GRACE) score, may not fully capture long-term vulnerability. We evaluated whether simple longitudinal clinical and patient-reported measures improved the prediction of major adverse cardiovascular events (MACEs). SUBJECTS AND METHODS: In this prospective cohort study, 351 patients with first MI were enrolled at a rural tertiary center in South India, of whom 319 formed the analytic cohort. MACE was defined as cardiovascular death, non-

Left atrial myopathy predicts cardiovascular outcome in hypertension and diabetes.

Arterial hypertension (AH) and type 2 diabetes mellitus (T2DM) predispose to left atrial (LA) dysfunction and atrial myopathy. We assessed the ability of peak atrial longitudinal strain (PALS) to predict major adverse cardiovascular events (MACEs) in AH and/or T2DM. We retrospectively enrolled patients > 40 years, in sinus rhythm, with AH and/or T2DM. Standard and advanced echocardiographic indices were collected; patients with previous MACEs, cardiac surgery, pacemakers, or moderate-to-severe valvular disease were excluded. The composite endpoint included atrial fibrillation, heart failure ho

Concomitant Budd-Chiari syndrome and coronary artery pseudoaneurysm in Behçet's disease: A case report.

Behçet's disease (BD) is a chronic, relapsing multisystem inflammatory disorder classified as a variable-vessel vasculitis. Venous thrombosis is common, and Budd-Chiari syndrome (BCS) represents one of the most severe manifestations, whereas coronary artery involvement is rare and associated with poor prognosis. Concomitant presentation of BCS and coronary pseudoaneurysm has been rarely reported. A 23-year-old man with a history of BCS caused by thrombotic occlusion of the retrohepatic inferior vena cava (IVC), previously treated with balloon dilatation and anticoagulation, presented with acut

Influence of Chronic Kidney Disease on Outcomes After Paclitaxel-Coated Balloon Angioplasty for Coronary In-Stent Restenosis.

BACKGROUND: Drug-coated balloons (DCB) are superior to conventional balloon angioplasty in coronary in-stent restenosis. The influence of chronic kidney disease (CKD) on outcomes after DCB for in-stent restenosis is not well described. METHODS: A post hoc analysis of the AGENT IDE trial was performed to compare the efficacy and safety of DCB versus balloon angioplasty in patients with CKD (estimated glomerular filtration rate <60 mL/min per 1.73 m2) and those without renal impairment. The primary outcome was target lesion failure, a composite of ischemia-driven target lesion revascularization

Long-Term Survival and Mortality Predictors of Off-Pump vs On-Pump Coronary Artery Bypass Grafting: A 25-Year Cohort Study.

BACKGROUND: Coronary artery bypass grafting (CABG), a key procedure for managing severe coronary artery disease, is performed by 2 primary surgical strategies: off-pump CABG and on-pump CABG. The long-term survival impact of off-pump CABG compared with on-pump CABG remains debated because of conflicting evidence. This study compared 25-year survival rates between off-pump CABG and on-pump CABG and identified key predictors of long-term mortality in CABG patients. METHODS: A retrospective cohort study analyzed 13,626 patients who underwent isolated CABG from January 1996 to September 2023. Pat
