Fenestration of the middle cerebral artery with an associated ruptured aneurysm.

Intracranial arterial fenestrations are uncommon developmental anomalies that may alter local hemodynamic stress patterns and may be associated with intracranial aneurysms. A 75-year-old woman presented with a mild subarachnoid hemorrhage. Cerebral angiography demonstrated a 5 mm aneurysm arising from the middle cerebral artery (MCA) associated with a dramatic fenestration of the proximal M1 segment of the MCA. The patient underwent craniotomy with microsurgical aneurysm clipping. The aneurysm arose immediately distal to the fenestration. Several fine perforating arteries were dissected away f

Skull bone flap fixation--comparative experimental study to assess the reliability of a new grip-like titanium device (Skull Grip) versus traditional sutures: technical note.

INTRODUCTION: After completing a craniotomy, whenever possible, it is crucial to replace and fix the removed bone flap to the cranium; this in order to keep the brain's protection as well as for cosmetic purposes. Visible skull defects might cause patients psychosocial problems and, most importantly, expose the brain to accidental damage. A fixation device should not only provide optimal attachment of the flap to the skull but also allow fast bony healing to avoid possible pseudoarthrosis and/or osteolytic changes. METHODS: After performing 12 different craniotomies on 4 human cadaver heads t

Real-time image guidance for open vascular neurosurgery using digital angiographic roadmapping.

OBJECTIVE: Angiographic roadmapping, commonly used for catheter navigation in endovascular procedures, is the superimposition of a live fluoroscopic image on a previously stored digitally subtracted angiogram. We evaluated this technique for the first time as a method for image-guided navigation during surgical resection of intracranial and spinal vascular lesions. METHODS: After obtaining Institutional Review Board approval, we retrospectively reviewed 38 procedures in 35 patients at two centers performed by one neurosurgeon in which intraoperative roadmapping was used as an image-guided nav

A pterion keyhole approach for the treatment of anterior circulation aneurysms.

The supraorbital keyhole approach is most frequently used in treatment for lesions within the anterior cranial base. However, it has some drawbacks, including cosmetically poor appearance of the scar, forehead deformity, and difficulty in dealing with some kinds of middle cerebral artery (MCA) and internal carotid artery (ICA) aneurysms. Therefore, we have developed a small pterion keyhole approach for an alternative access to treat anterior circulation aneurysms. An oblique skin incision about 3-5 cm in length was made just from 1.0 cm anterior to the superficial temporal artery at the level

The importance of CT scans in planning the removal of orbital-frontal lobe foreign bodies.

PURPOSE: To describe the management of foreign bodies in the orbit and frontal lobe. METHODS: Reports of two cases. RESULTS: Both patients underwent successful removal of an orbital-cerebral foreign body by anterior orbitotomy. CONCLUSION: Computed tomography was useful to confirm preoperatively that the foreign body was not adjacent to cerebral blood vessels and to monitor postoperatively for cerebral hemorrhage. A team approach is necessary in the management of orbital-frontal lobe foreign bodies.

Immediate Perioperative Hypersensitivity and Anaphylaxis in Neuroanesthesia Practice: A Focused Review.

Perioperative hypersensitivity reactions are uncommon but potentially catastrophic. Neurosurgical and spine patients are especially vulnerable, as the injured brain and spinal cord tolerate anaphylaxis-induced hypotension poorly, cerebral and spinal perfusion may fall precipitously, and draping of the head and torso may obscure cutaneous signs. These procedures also rely heavily on topical hemostatic agents. National audits estimate the incidence of perioperative anaphylaxis at roughly 1 in 5000 to 10,000 anesthetics, but estimates vary geographically. No study has reported a denominator-based

A soft electrode array with reconfigurable hydrogel interfaces for high-fidelity neurophysiological monitoring during craniotomy.

Achieving accurate monitoring of electrophysiological activity from the brain cortex is critical for preventing postoperative neurological deficits during craniotomy. However, current technologies face profound challenges in enabling consistent high-quality neural signal acquisition due to complex intracranial environments (e.g., wet/fragile brain surfaces and surgical instrument interference). Here, we develop a soft electrode array with reconfigurable hydrogel interfaces for sustainable high-fidelity monitoring of electrophysiological states from the functional brain cortex throughout neuros

[Mechanisms and morpho-pathophysiological levels of epileptogenesis].

This review elucidates the concept of levels of epileptogenesis, including the effector level of ion channels, the mechanisms that ensure their functionality, and the maintenance of ionic equilibrium, both intracellular, involving specific proteins, and extracellular, encompassing astroglia. Moreover, the discussion extends to the level of mediator systems, including glutamate, gamma-aminobutyric acid (GABA), and acetylcholine, as well as the propagation of excitation to adjacent neurons (plasticity) and the emergence of secondary excited neurons that contribute to the formation of a hyper-exc

Amyloid PET Burden, CSF Biomarkers, and APOE Genotype in People With Iatrogenic and Sporadic Cerebral Amyloid Angiopathy.

BACKGROUND AND OBJECTIVES: Iatrogenic cerebral amyloid angiopathy (iCAA) is a rare form of CAA believed to result from β-amyloid (Aβ) transmission during neurosurgical procedures. We aimed to compare amyloid PET burden, CSF biomarkers, and APOE genotype between iCAA and sporadic CAA (sCAA). METHODS: In this single-center, cross-sectional study, adults fulfilling diagnostic criteria for probable sCAA or iCAA who underwent amyloid PET and CSF assessment between 2021 and 2024 were included. Amyloid burden was quantified using Centiloid scaling of 18Fflutemetamol or 18Fflorbetaben PET scans. CSF

TRPV4 inhibition as a pharmacotherapy for post-hemorrhagic hydrocephalus.

BACKGROUND: Posthemorrhagic hydrocephalus (PHH) is a severe neurological condition characterized by cerebrospinal fluid (CSF) accumulation, ventriculomegaly, and elevated intracranial pressure following a brain hemorrhage. Current treatment is limited to surgical interventions, such as ventriculoperitoneal shunt placement, due to the lack of specific modulators of CSF secretion that can be administered systemically. Transient receptor potential vanilloid 4 (TRPV4) is a Ca2+-permeable ion channel implicated in CSF production, representing a potential pharmacological target. METHODS: Using a wel

Iatrogenic cerebral amyloid angiopathy: Two cases linked to childhood cadaveric dural transplantation for different intracranial pathologies, diagnosed using the simplified Edinburgh computed tomography criteria.

BACKGROUND: Cerebral amyloid angiopathy (CAA) is an age-related condition marked by amyloid-β (Aβ) accumulation in the small cerebral vessels. Iatrogenic cerebral amyloid angiopathy (iCAA) is a distinct form of CAA in younger patients with a history of cranial surgeries involving cadaveric dural transplants. Both iCAA and CAA are linked to recurrent lobar intracerebral hemorrhage (ICH). This article highlights iCAA as a distinct variant, discussing the possibility of using simplified Edinburgh computed tomography (CT) criteria as a possible diagnostic tool for CAA and carefully considering pla

Topical administration of tranexamic acid for prevention of postoperative epidural fibrosis: insights from a rabbit laminectomy model.

Significant advancements in imaging and surgical methodologies have led to more frequent performance of neurosurgical procedures such as laminectomy in both animal and human patients. Epidural fibrosis (EF) is defined as the excessive formation of scar tissue in the epidural space after lumbar laminectomy, often resulting in recurring postoperative pain. Given the association between postoperative hematoma accumulation at the laminectomy site and the development of EF, the present study aimed to evaluate the preventive impact of tranexamic acid (TXA), an antifibrinolytic agent with well-recogn

QEVO®-Assisted Anatomical Inspection of Adjacent Perforators in Microsurgical Clipping-Technical Note.

INTRODUCTION: Aneurysms of brain vessels are life-threatening conditions with various adverse outcomes, some stemming from microsurgical intervention, particularly when major vessel perforators are inadequately protected. The use of endoscopes enhances the approach to aneurysms by providing closer visualization (180-360 degrees) of the local anatomy, potentially reducing accidental damage. To improve visualization and efficiency, a microscope-integrated 45-degree angled microinspection endoscopic tool (QEVO®, Carl Zeiss, OberkochenTM) has been developed and employed in various neurosurgical pr
