The Behavioral Health Treatment Engagement and Retention Learning Collaborative to Improve Receipt and Continuity of Medication Treatment for Opioid Use Disorder.

Medicaid is the largest insurer for patients with opioid use disorder (OUD) and finances a disproportionate share of medication treatment for OUD (MOUD). Within Medicaid, initiation and continuity of MOUD are associated with improved outcomes, but substantial inequities across populations and care settings remain. The Medicaid Outcomes Distributed Research Network (MODRN) has supported Medicaid agency efforts to improve MOUD receipt and continuity of care. In the context of MODRN-Examining Quality Improvement for Medicaid Programs, the Behavioral Health Treatment Engagement and Retention Learn

Opioid Dependence Is Associated With Increased Odds for Acute Periapical Abscesses.

AIMS: This study aims to assess the odds ratio for acute periapical abscesses (PAs) in opioid-dependent patients and compare it to non-users. METHODS: Data aggregated from 1 799 122 patients visiting the medical center were recorded. Data from the corresponding diagnosis codes for opioid dependence and acute PAs was retrieved by searching the appropriate query in the database. The odds ratio (OR) of acute PAs and their association with opioid dependence were calculated and analyzed statistically. RESULTS: Opioid-dependent patients showed significantly more acute PAs than non-users. The OR fo

Barriers to opioid replacement therapy in pregnant women with opioid use disorder: a systematic review.

BACKGROUND: Opioid use disorder (OUD) during pregnancy has increased dramatically, paralleling epidemic trends in the general population. Pregnant women with OUD face considerable risks of adverse outcomes. While medications for opioid use disorder (MOUD) is the mainstay of treatment, substantial barriers limit its implementation. Despite the critical need for effective interventions, a comprehensive synthesis of the challenges to MOUD during pregnancy remains lacking. This review examines these barriers to improve treatment access and outcomes for this vulnerable population. METHODS: This re

Comparative analysis of methods for identifying multimorbidity patterns among people with opioid use disorder: A retrospective single-cohort study.

BACKGROUND: Multimorbidity, the presence of two or more (2+) chronic conditions, presents significant challenges for healthcare delivery, particularly among populations with opioid use disorder (OUD). Multimorbidity patterns among individuals with OUD are not well established, and minimal research exists examining the impact of clustering methods on identifying these patterns. OBJECTIVE: Our study aimed to assess multimorbidity prevalence, explore associated sociodemographic and clinical characteristics, and determine multimorbidity patterns using hierarchical cluster analysis (HCA) and K-mea

Making long-acting treatment work: Tracing connections with extended-release buprenorphine depot through time.

INTRODUCTION: How people connect with opioid agonist treatment is an ongoing concern. Extended-release buprenorphine depot (BUP-XR) has been designed with 'retention' in mind. It is important to consider what makes a difference to clients in helping them to stay connected to treatment over time. METHODS: We report findings from the third wave of in-depth interviews with participants (n = 26) in the Community Long-Acting Buprenorphine (CoLAB) study, tracing accounts of connection, disconnection and reconnection with BUP-XR since initiation into treatment. RESULTS: Changing situations in treat

Exploring how pregnant people in recovery from opioid use disorder describe their child during pregnancy.

OBJECTIVE: Understanding parenting strengths and challenges among pregnant people in recovery from opioid use disorder supports effective intervention delivery. How parents think and feel about their child prenatally has implications for postnatal parenting. In our past work, greater use of positive compared to negative or neutral prenatal descriptors of the child during pregnancy was associated with higher sensitivity, warmth, and engagement during caregiver-infant interactions 12-months postpartum. We analyzed descriptions of the child among pregnant people in recovery compared to a non-subs

Association of state medicaid prescription cap policies with trajectories of buprenorphine use for opioid use disorder.

BACKGROUND: Buprenorphine and other medications for opioid use disorder (OUD) can reduce opioid-related morbidity and mortality. It is unknown whether state Medicaid prescription cap policies that restrict the monthly number of covered prescription fills affect the duration of buprenorphine use. OBJECTIVE: To identify trajectories of buprenorphine use and determine the association of caps with trajectory group membership among individuals with OUD. METHODS: Using 10 states' Medicaid claims data from 2010 to 2015, we employed group-based trajectory models to identify patterns of buprenorphine

Exploring the Roots of Stigma: A Qualitative Investigation of the American Public's Attitudes Toward Individuals with Opioid Use Disorder.

The ongoing opioid crisis underscores the urgent need to understand and address stigma toward individuals with opioid use disorder (OUD). Applying message framing theory, this study examines how different segments of the public conceptualize OUD. Through in-depth interviews with 26 U.S. adults, segmented by their OUD experiences (personal, family/friend, or none) and stigma scores (high or low), we explored the underlying reasons for stigmatizing attitudes toward individuals with OUD. Our analysis revealed three key themes that differentiate perspectives between participants with higher and lo

Exploring social impairment in those with opioid use disorder: linking impulsivity, childhood trauma, and the prefrontal cortex.

BACKGROUND: Challenges with social functioning, which is a hallmark of opioid use disorder (OUD), are a drawback in treatment adherence and maintenance. Yet, little research has explored the underlying mechanisms of this impairment. Impulsivity and corresponding neural alterations may be at the center of this issue. Childhood adversity, which has been linked to both impulsivity and poorer treatment outcomes, could also affect this relationship. This study explores the relationship between impulsivity, social functioning, and their neural correlates in the prefrontal cortex, while examining the

Evaluating remyelination compounds for new applications in opioid use disorder management.

Opioid use disorder (OUD) is associated with a reduction in brain white matter, affecting critical areas involved in decision-making, impulse control, and reward processing. The FDA has approved several drugs and natural compounds that enhance myelination, targeting oligodendrocyte progenitor cells (OPCs), directly enhancing oligodendrocyte (OL) function, or acting as cofactors for myelin production. This retrospective case study aimed to assess whether current clinical evidence supports the use of myelin-enhancing agents to promote remission in OUD. We evaluated a range of compounds with demo

Retention among Medicaid beneficiaries in opioid treatment programs in California.

BACKGROUND: Continuing medications for opioid use disorders (MOUD) for an extended period is important to achieve good outcomes, but many patients do not remain in treatment for even six months. The goal of this study was to develop and describe case-mix adjusted retention rates among California Opioid Treatment Programs (OTP). METHODS: We developed measures of 30-, 90-, and 180-day retention using data from the California Outcomes Management System for Medicaid beneficiaries admitted as an outpatient to an OTP between July 1, 2021 and June 30, 2022. We applied case-mix adjustment to the meas

Chronic overlapping pain conditions in individuals with active opioid use disorder: a descriptive study of syringe program participants.

BACKGROUND: Comorbidity between opioid use disorder (OUD) and chronic pain is substantial. Pain has been shown to be a motivator for OUD onset, maintenance, relapse, and treatment delay. A cluster of pain conditions known as chronic overlapping pain conditions (COPCs), also now referred to in contemporary ICD classification as primary pain conditions, are particularly refractory to traditional forms of pain treatment, and likely adversely impact comorbid OUD. OBJECTIVE: This cross-sectional descriptive study sought to obtain a better understanding of the prevalence of COPCs among individuals

Housing Insecurity among Emergency Department Patients with Opioid Use Disorder.

INTRODUCTION: Emergency departments (ED) have increasingly engaged in screening and treatment initiation for patients with opioid use disorder (OUD). Patients with OUD, however, may also be impacted by significant social need, including housing insecurity. We sought to consider the incidence of homelessness and housing insecurity in patients engaged in an ED-initiated medication for opioid use disorder (MOUD) program. METHODS: We performed a secondary analysis, with specific consideration of housing status, on data obtained from a prospective, ED-initiated MOUD study conducted at an urban, ac

Effect of wildfire on the prevalence of opioid misuse through anxiety among young adults in the United States: a modeling study.

BACKGROUND: Exposure to climate change events like wildfires can lead to health and mental health problems. While conceptual frameworks have been hypothesized describing the potential relationship between disaster exposure and substance use, the association remains under-researched and unquantified. METHODS: We constructed a quantitative portrayal of one proposed conceptual framework that focuses on the intermediary role of anxiety. We used the Monte Carlo simulation to estimate the impact of wildfire exposure on opioid misuse outcomes through increased anxiety. We searched for and extracted

How post overdose response efforts can address social determinants of health among people who use drugs: perspectives from the New York State Department of Health (NYSDOH) Post Overdose Response Team (PORT).

Individuals who have survived an overdose often have myriad needs that extend far beyond their drug use. The social determinants of health (SDOH) framework has been underutilized throughout the opioid overdose crisis, despite widespread acknowledgment that SDOH are contributors to the majority of health outcomes. Post Overdose Response Teams (PORTs) engage with individuals who have experienced 1 or more nonfatal overdoses and bear witness to the many ways in which overdose survivors experience instability with healthcare, housing, employment, and family structure. Employing a harm reduction mo

Mortality in adolescents and young adults following a first presentation to the emergency department for alcohol.

BACKGROUND AND METHODS: We conducted a population-based, retrospective cohort study of first-time emergency department (ED) visits in adolescents and young adults (AYA) due to alcohol and compared mortality to AYA with nonalcohol ED visits between 2009 and 2015 using standardized all-cause mortality ratios (age, sex, income, and rurality). We described the cause of death for AYA and examined the association between clinical factors and mortality rates in the alcohol cohort using proportional hazard models. RESULTS: A total of 71,776 AYA had a first-time ED visit due to alcohol (56.1% male, me

Early Remission of Opioid Use Disorder in an Adolescent Using Buprenorphine Extended - Release Subcutaneous Injection: A Case Report.

Opioid use disorder (OUD) continues to be a major public health crisis, with the current epidemic being driven by synthetic opioids such as illicitly manufactured fentanyl. While medications exist to treat OUD, only sublingual and subdermal buprenorphine formulations are approved for patients aged 16-17 years. Furthermore, almost all pediatric patients who are diagnosed with OUD do not receive medication as treatment. This case describes the innovative use of buprenorphine extended-release subcutaneous injection in a 17-year-old with OUD who has achieved early remission after four months of tr

Elucidating the molecular symphony: unweaving the transcriptional & epigenetic pathways underlying neuroplasticity in opioid dependence and withdrawal.

The persistent use of opioids leads to profound changes in neuroplasticity of the brain, contributing to the emergence and persistence of addiction. However, chronic opioid use disrupts the delicate balance of the reward system in the brain, leading to neuroadaptations that underlie addiction. Chronic cocaine usage leads to synchronized alterations in gene expression, causing modifications in the Nucleus Accumbens (NAc), a vital part of the reward system of the brain. These modifications assist in the development of maladaptive behaviors that resemble addiction. Neuroplasticity in the context
