Posterior realignment of irreducible atlantoaxial dislocation with C1-C2 screw and rod system: a technique of direct reduction and fixation.

BACKGROUND CONTEXT: Treatment of chronic and irreducible atlantoaxial dislocation (AAD) with ventral compression is challenging for surgeons. The main procedures are occipitocervical/C1-C2 fusion after transoral odontoidectomy or release of the periodontoid tissues. These surgical procedures, which are performed simultaneously or intermittently, have many disadvantages that may discount their effectiveness. Therefore, a more effective way to achieve surgical reduction and to keep solid stability with only a single procedure is needed. PURPOSE: We describe a technique to reduce chronic and irr

[Arthroscopy of the knee--"surgery without complications"].

INTRODUCTION: Beside other advantages that arthroscopic surgery has, comparing to the open surgery, there is one more--the risk of complications is present but a very little. The aim of this work is to present complications that we have had and to point at possibilities how to prevent and treat them. MATERIAL, METHODS AND RESULTS: In the period from September 1990 to December 1998, 1071 arthroscopic surgeries were performed in our hospital. 817 patients (76%) were male and 254 (24%) female, average age 30.63 (12-69). Left knee was involved in 560, while the right in 511 cases. Among all arthr

Short- and Long-Term Outcomes following Severe Traumatic Lower Extremity Reconstruction: The Value of an Orthoplastic Limb Salvage Center to Racially Underserved Communities.

BACKGROUND: Previous studies have demonstrated that nonwhite race and disadvantaged socioeconomic status negatively impact outcomes following lower extremity reconstruction. The authors sought to characterize differences in outcomes between racial groups in patients necessitating traumatic lower extremity reconstruction at an orthoplastic limb salvage center. METHODS: A retrospective review between 2002 and 2019 was conducted of patients who underwent free flap lower extremity reconstruction at an orthoplastic limb salvage center. Patient demographics were identified, and permanent addresses

A prospective study of coracoclavicular ligament reconstruction with autogenous peroneus longus tendon for acromioclavicular joint dislocations.

BACKGROUND: Anatomic coracoclavicular (CC) ligament reconstruction (ACCR) provides good outcomes for Rockwood type III and VI acromioclavicular (AC) joint dislocations. Various grafts have been used, but complications from graft harvesting are not uncommon. This study examined the clinical and radiographic outcomes of patients with AC joint dislocations repaired with the autogenous anterior half of the peroneus longus tendon (AHPLT) to achieve ACCR. METHODS: Patients with a Rockwood type III to V AC joint dislocation and magnetic resonance imaging of the disruption of the CC ligaments, as wel

Triple Achilles Tendon Rupture: Case Report.

We present a case report with 1-year follow-up data of a 57-year-old male soccer referee who had sustained an acute triple Achilles tendon rupture injury during a game. His triple Achilles tendon rupture consisted of a rupture of the proximal watershed region, a rupture of the main body (mid-watershed area), and an avulsion-type rupture of insertional calcific tendinosis. The patient was treated surgically with primary repair of the tendon, including tenodesis with anchors. Postoperative treatment included non-weightbearing for 4 weeks and protected weightbearing until 10 weeks postoperative,

Characterizing the Diagnosis and Treatment of Allergic Contact Dermatitis to 2-Octyl Cyanoacrylate Used for Skin Closure in Elective Orthopedic Surgery.

BACKGROUND: While studies have shown that postoperative wound complications can predispose to deep infection following orthopedic surgery, the best form of skin closure has not been elucidated. Furthermore, the unique risks and benefits of each type of wound closure have not been studied. The goal of this study is to present the diagnosis and treatment of patients with allergic contact dermatitis (ACD) from 2-octyl cyanoacrylate, a skin adhesive commonly used in wound closure. METHODS: Twenty-nine patients with ACD to 2-octyl cyanoacrylate (Prineo, Ethicon, NJ) following elective orthopedic s

Chronic scapholunate instability treated with temporary screw fixation.

PURPOSE: To determine the efficacy of scapholunate (SL) temporary screw fixation to maintain the reduction of SL interval after ligament debridement, repair, or reconstruction. METHODS: A total of 36 patients who had chronic SL instability were included. Arthroscopic debridement was performed for dynamic instability, and primary repair or reconstruction of the SL ligament was performed for static instability. We then used SL screw fixation for the protection of the repair or reconstruction. Screws were removed when loosening was noted on x-ray. RESULTS: The average patient age was 43 years.

Percutaneous adhesiolysis in the management of chronic low back pain in post lumbar surgery syndrome and spinal stenosis: a systematic review.

BACKGROUND: Low back pain after either post lumbar surgery syndrome or spinal stenosis in the absence of surgery is a vexing problem. Post lumbar surgery syndrome can occur in any age group, while low back and radicular pain from spinal stenosis is a disease of aging. As the population ages, the incidence of symptomatic spinal stenosis will increase. There are currently limited treatment options for either group. Further surgery is not uniformly effective in relieving pain after previous surgery. While therapies are being developed to treat pain due to spinal stenosis, no therapy other than ad

Osteoid osteoma of the mobile spine: surgical outcomes in 81 patients.

STUDY DESIGN: A restrospective review of 81 cases of osteoid osteoma of the mobile spine submitted to surgical treatment. OBJECTIVE: Analyze pro and contras of different techniques (conventional and minimally invasive) for the treatment of osteoid osteoma. SUMMARY OF BACKGROUND DATA: Intralesional excision has been considered the standard treatment for spinal osteoid osteoma. The high success rate of minimally invasive surgery in the treatment of a variety of spinal disorders lead us to believe this technique can also be applicable for the treatment of osteoid osteoma of the spine. METHODS:

A comparison of 2 drilling techniques on the femoral tunnel for anterior cruciate ligament reconstruction.

PURPOSE: This cadaveric study was undertaken to characterize the femoral tunnel geometry resulting from commonly used drilling techniques in anterior cruciate ligament reconstruction. METHODS: We randomized 10 matched-pair cadaveric knees (20 knees) into 2 groups with right and left matched pairs from each cadaver. Of the knees, 10 underwent transtibial femoral tunnel drilling from a far-medial starting point on the tibia (group 1) and 10 had the femoral tunnel drilled from a medial arthroscopic portal (group 2). The dimensions and size of the apertures, the volume and length of the tunnels,

The muscular and the new osteomuscular composite peroneus brevis flap: experiences from 109 cases.

BACKGROUND: Pedicled peroneus brevis muscles are proven flaps for defect coverage around the ankle, foot, and distal lower leg. Some of these defects--such as postosteitic hollowed out calcaneal bones--may profit from a simultaneous vascularized bone transplantation. The authors' experiences with 109 successive muscular and newly developed osteomuscular peroneus flaps are outlined for small to medium combined defects in this region. METHODS: One hundred nine patients with various soft-tissue and skeletal defects underwent reconstruction with proximally (n = 10) or distally pedicled (n = 98) o

Comparison of a double bundle arthroscopic inlay and open inlay posterior cruciate ligament reconstruction using clinically relevant tools: a cadaveric study.

PURPOSE: The ideal surgical management of isolated and combined posterior cruciate ligament (PCL) injuries is controversial. One active debate concerns the advantages of an open tibial inlay technique versus an arthroscopic transtibial tunnel technique. Therefore, the goal of this study is to biomechanically compare arthroscopic and open double-bundle PCL tibial inlay procedure with and without posterolateral corner (PLC) deficiency. METHODS: Ten matched pairs of cadaveric knees were randomly assigned to undergo PCL sectioning and reconstruction using either the open inlay or arthroscopic inl

Less invasive Achilles tendon reconstruction.

BACKGROUND: The optimal management of chronic ruptures of the Achilles tendon is surgical reconstruction. Reconstruction of the Achilles tendon using peroneus brevis has been widely reported. Classically, these procedures involve relatively long surgical wounds in a relatively hypovascular area which is susceptible to wound breakdown. RESULTS: We describe our current method of peroneus brevis reconstruction for the Achilles tendon using two para-midline incisions. CONCLUSION: This technique allows reconstruction of the Achilles tendon using peroneus brevis preserving skin integrity over the

[The multidisciplinary approach to reconstructive surgery of the extremities-considerations for trauma and orthopedic surgery].

Interdisciplinary reconstructive surgery of the extremities involves a variety of indications ranging from IIIB/C open fractures with major segmental loss of bone and soft tissue, to arterial vessel injury necessitating vascular repair, and to biological, plastic reconstruction following resection of musculoskeletal tumors. The multidisciplinary approach involving trauma/orthopedic surgery combined with vascular, plastic, and neuro- resp. microsurgery has significantly increased the rate of limb-sparing operations and improved morbidity, function, quality of life, and long-term oncological out
