Type A2 influenza viral infections in children.

We retrospectively reviewed the manifestations of influenza A2 in 83 hospitalized young children. Our purpose was to define the spectrum of clinical illness in this age group. Findings included fever (91%), vomiting or diarrhea (49%), pharyngitis (34%), pneumonitis (29%), otitis media (24%), conjunctivitis (13%), croup (13%), and bronchiolitis (6%). Neuromuscular manifestations occurred in 16 patients (19%) and included seizures, apnea, opisthotonos, and myositis. Three children had cerebrospinal fluid pleocytosis. Children younger than 3 months of age had fever less often and gastrointestinal

Preventing unnecessary tympanostomy tube placement in children.

OBJECTIVE: In 2013 the American Academy of Otolaryngology published tympanostomy tube guidelines for children; Action Statement 6 recommends against tube placement without middle ear effusion (MEE) at time of assessment. To date, little research has directly evaluated this recommendation in reducing the need for ear tubes. We evaluated the effectiveness of this recommendation and potential risk factors that influence the success of watchful waiting. METHODS: Retrospective chart review collecting demographics, daycare status, smoking exposure, and time of year of visit. Children aged 6 months

Follow up after middle-ear ventilation tube insertion: what is needed and when?

INTRODUCTION: There is a paucity of evidence to guide the post-operative follow up of patients undergoing middle-ear ventilation tube insertion for the first time. This study was conceived to identify current practice at our institution (Ninewells Hospital, Dundee) and to inform subsequent change in our follow-up procedure. METHODS: Two cycles of data collection and analysis were performed. All paediatric patients undergoing ventilation tube insertion for the first time were identified. Patients who had previously undergone ventilation tube insertion or additional procedures such as adenoidec

Tympanometry in three-year-old children. The 3-year follow-up of a cohort study.

All 3-year-old children in a region were screened by tympanometry in January 1976 (503 children), and the 6-year-olds of the same region had tympanometric and tone audiometric screening in January 1979 (498 children), 70% being included both times. Significant improvement in middle ear status had occurred from the age of 3-6 years, but otherwise there was surprisingly little difference between the two age levels, e.g., about one-third of the tympanograms were abnormal in both age groups. Ears having a middle ear pressure of 0 to -99mm H2O at 3 years showed significantly better impedance audiol

Preventing unnecessary tympanostomy tube placement in children: A 5-year Update.

OBJECTIVE: In 2022 the American Academy of Otolaryngology (AAO) published updated clinical guidelines for pediatric tympanostomy tube placement. Statement 6 recommended that clinicians should not perform tympanostomy tube insertion in children with recurrent acute otitis media (rAOM) who do not have middle ear effusion (MEE) at the time of evaluation. We performed a 5-year update on a study evaluating this recommendation in our pediatric patient population. STUDY DESIGN: Retrospective cohort study. SETTING: University of Texas Medical Branch. METHODS: Retrospective chart review was performe

Contrasting Effects of Pressure Compensation on TEOAE and DPOAE in Children With Negative Middle Ear Pressure.

In children with normal cochlear acuity, middle ear fluid often abolishes otoacoustic emissions (OAEs), and negative middle ear pressure (NMEP) reduces them. No convincing evidence of beneficial pressure compensation on distortion product OAE (DPOAE) has yet been presented. Two studies aimed to document effects of NMEP on transient OAE (TEOAE) and DPOAE. In Study 1, TEOAE and DPOAE pass/fail responses were analyzed before and after pressure compensation in 50 consecutive qualifying referrals having NMEP from -100 to -299 daPa. Study 2 concentrated on DPOAE, recording both amplitude (distortion

Five-year results for use of single-flanged tympanostomy tubes in children.

AIMS: To assess the five-year outcome of the use of single-flanged tympanostomy tubes in children, including the time to extrusion, rate of retained tubes and rate of persistent perforation. MATERIALS AND METHODS: The medical records relating to 640 single-flanged tympanostomy tubes intended for short-term use in paediatric patients were retrospectively reviewed. RESULTS AND ANALYSIS: We found that 36.4 per cent of the tubes had extruded within 12 months and 71.0 per cent within 24 months. Results showed that 14.1 per cent of the tubes had been removed because of prolonged retention, with a

[Pathogenetic validation of optimal antioxidant therapy in suppurative inflammatory otic diseases in children].

The knowledge of cell-molecular characteristics of middle ear inflammation in children allowed the authors to suggest a beneficial effect of antioxidants in this disease. The trial of ascorbic acid, retinol and alpha-tocopherol as components of combined treatment has confirmed this hypophysis in view of high response achieved in children with middle ear purulent inflammation.

[Otologic manifestations presenting and predominating in necrotizing angiitis].

The authors report on 5 observations of necrotising angiitis with a specifically otological onset, namely 4 Wegener's syndromes and one periarteritis nodosa. In all 5 cases, the initial symptomatology consisted of otological signs in a feverish context, producing pictures of serous or sero-purulent otitis. Its resistance to usual forms of therapy led in 3 cases to the installation of transtympanic aerators, and in 2 cases to the performance of an antro-attico-mastoidectomy. One of the observations led to the discovery in the middle ear of a histological aspect specific to the disease, from whi

Otogenic Aspergillus Brain Abscess in an SLE teen: Successful case management from Eastern India.

Neuroaspergillosis is a rare, high-mortality complication in Systemic Lupus Erythematosus (SLE). We report an otogenic Aspergillus fumigatus brain abscess in a 17-year-old female with SLE and seizure disorder, managed with immunosuppressants. She presented with altered sensorium and hemiparesis. Despite initial aspiration and combination antifungal therapy (voriconazole and liposomal amphotericin B), clinical worsening demanding radical excision. Culture and sequencing confirmed A. fumigatus, likely spread via chronic otitis media. The patient recovered and was discharged on oral posaconazole,

High-definition otoscopic device for humanitarian mission: how i do it.

BACKGROUND: Despite a high prevalence of chronic otitis media and related complications, many African dispensaries and clinics lack microscopes or fiberoptic equipment for examining external ear ducts and tympanic membranes. METHOD: An alternative, inexpensive, and readily available device designed for ear wax removal is presented as a clinical ear and anterior nasal cavity examination tool. The device connects to Wi-Fi or cellular networks, providing high-definition images of the ear and anterior nasal cavity through a smartphone interface. Its use, utility, strengths, and limitations are di

Formula Otorrhea in Pediatric Patients With Cleft Palate.

The case series details 2 unusual cases of male newborns with cleft lip and palate (CLP) that later developed formula otorrhea. Both patients underwent bilateral myringotomies with the insertion of pressure equalizing (PE) tubes for chronic otitis media with effusion (OME). Chronic otorrhea associated with feeding occurred post-PE tube insertion and the otorrhea was later confirmed to be due to reflux of formula. Patients were treated with antibiotic ear drops, routine ear cleaning, anti-reflux medication, and reflux precautions. After definite cleft palate repair, formula otorrhea completely

Transient third-degree atrioventricular block during anesthesia in a cat.

BACKGROUND: Third-degree atrioventricular block (AVB) is usually permanent, with transient cases being rare. Cats with transient third-degree AVB. It had been not reported in detail. CASE DESCRIPTION: A 9.3-year-old, male shorthair cat was evaluated for possible nervous disease resulting from otitis interna. Under propofol and isoflurane anesthesia, this cat developed a third-degree AVB. Isoproterenol was administered by continuous infusion to increase its heart rate. During recovery, heart rate returned to sinus bradycardia together with first-degree AVB without medical treatment. The cause

Endophthalmitis due to Delftia acidovorans: An unusual ocular pathogen.

Endophthalmitis is a dreaded postoperative complication of cataract surgery. Delftia acidovorans is usually nonpathogenic and an unusual ocular pathogen. Isolated reports of delftia-associated sepsis, otitis media, endocarditis, keratitis, etc. exist in literature. We report a rare and unique case of delftia-related endophthalmitis in a 67-year-old male diagnosed 2 weeks after uneventful cataract surgery. He was treated successfully with core vitrectomy and intravitreal antibiotics. Microbiological evaluation of vitreous sample identified the causative organism as Delftia acidovorans. Post-vit

The biofilm-associated bacterial infections unrelated to indwelling devices.

Biofilms are microbial communities established in the self-produced extracellular substances that include up to 80% of associated microbial infections. During biofilm formation, bacterial cells shift from the planktonic forms to aggregated forms surrounded by an extracellular polymeric substance. The bacterial biofilm shows resistance against immune reactions as well as antibiotics and is potentially able to cause disorders by both device-related and nondevice-related infections. The nondevice-related bacterial biofilm infections include dental plaque, urinary tract infections, cystic fibrosis

Chronically Discharging Ears: Evalution with High Resolution Computed Tomography.

Chronic suppurative otitis media (CSOM) refers to the middle ear inflammation which is clinically characterized by the discharging ear, hearing deficit, fever and otalgia. Although a clinical diagnosis, imaging is imperative to rule out associated complications which apart from causing hearing deficit, may prove fatal at times. Both high resolution computed tomography (HRCT) and MRI are helpful in evaluating middle ear pathologies, usage being indication specific. Due to its excellent spatial resolution, HRCT is invaluable in assessment of chronically discharging ears, especially to look for b

Tuberculous Otomastoiditis.

Tuberculous otitis media and mastoiditis, or tuberculous otomastoiditis, is a rare but well-described infectious process occasionally affecting individuals in the United States but more frequently seen in countries where tuberculosis is endemic. Infection may be primary and occur through mucus aspirated through the Eustachian tube. Alternatively, organisms may secondarily infect the nasopharynx when expectorated from the lungs and, less frequently, may be hematogenously spread. Chronic otorrhea and hearing loss are common symptoms, and extensive perforation of the tympanic membranes and facial

Interim results of an ecological experiment - Conjugate vaccination against the pneumococcus and serotype replacement.

Streptococcus pneumoniae has more than 95 serotypes, each of which presumably can cause sepsis, meningitis, pneumonia, and acute otitis media. Pneumococcal conjugate vaccines (PCV) targeted against a limited number of serotypes have nonetheless revealed an impressive impact on each manifestation of pneumococcal disease. At the same time, growing evidence of significant non-vaccine type (NVT) replacement disease following implementation of infant PCV programs has raised questions about the long-term viability of PCV immunization strategies and how to optimize PCV formulations. We discuss here t

Community-acquired Pseudomonas meningitis causes acute obstructive hydrocephalus.

INTRODUCTION: Pseudomonas aeruginosa (PS) infection is serious in children and can cause malignant external otitis, endophthalmitis, endocarditis, meningitis, pneumonia, and septicemia (Huang et al. Pediatr Infect Dis J 1). The treatment of Pseudomonas infection requires prompt medical evaluation and appropriate antibiotic treatment. CASE REPORT: We report the case of a 6-month-old boy with an unusual presentation of acute obstructive hydrocephalus owing to Pseudomonas meningitis. Treatment with optimal antibiotic begun immediately after the pathogen was recognized and continued for 4 weeks.
