Double-blind trial comparing epidural lignocaine with epidural fentanyl for anaesthesia during extracorporeal shock wave lithotripsy.

A randomised double-blind trial was undertaken to compare epidural lignocaine 1.5% with adrenaline to epidural fentanyl (100 micrograms in saline 10 ml) in forty unpremedicated patients undergoing extracorporeal shock wave lithotripsy without ancillary procedures. Midazolam only was used for sedation. There was no significant difference in intraoperative pain scores with several patients in each group complaining of pain. Decreases in blood pressure occurred in both groups but were more pronounced in the lignocaine groups (36% vs 25% maximum decrease from control, P = 0.004). Heart rate was in

[Effect and side effects of oral morphine, lormetazepam and placebos as premedication].

In 60 patients undergoing a curettage in thiopentone induced inhalation anaesthesia with enflurane and N2O/O2 = 2:1, the effects of oral premedication (2 h before anaesthesia) with 30 mg morphine (MST 30) (n = 21), 1 mg lormetazepam (Noctamid) (n = 19) and placebo (n = 21) on psychological (anxiety, depression and asthenia), physiological (blood pressure, heart and respiratory rate) and pain parameters (visual analogue scale, analgesic consumption) were investigated. The study design was single blind, randomized. Before premedication the three groups did not differ in one parameter and so were

Ultrasound therapy of subacromial bursitis. A double blind trial.

Ultrasound (US) is used widely to treat patients with supraspinatus tendinitis, subacromial bursitis, or adhesive capsulitis (SSA). No double blind studies of US in the treatment of SSA, however, have been conducted. This study was designed to determine whether the addition of US can further decrease pain and increase range of motion in those receiving the usual courses of ROM exercises and nonsteroidal anti-inflammatory drugs (NSAIDs) or ROM exercises in patients with SSA. Twenty patients with SSA were randomized to receive true or sham US three times a week for four weeks. All other aspects

Famotidine: nocturnal administration for gastric ulcer healing. Results of multicenter trials in Austria and Germany.

We conducted an 8-week, double-blind, randomized, placebo-controlled multicenter trial (Austria and Germany) to determine whether famotidine would speed healing or relief of symptoms in patients with benign gastric ulcer. Of the 65 patients who completed the trial, 32 received famotidine 40 mg once at night, and 33 received placebo. At 4, 6, and 8 weeks after entry, ulcers had healed in a larger percentage of patients treated with famotidine than in those treated with placebo (German multicenter trial: 65 vs. 46%, 95 vs. 71%, and 95 vs. 79%; Austrian multicenter trial: 42 vs. 44%, 67 vs. 44%,

Comparison of the analgesic efficacy of flurbiprofen and aspirin for postsurgical dental pain.

The analgesic efficacy of flurbiprofen 25 mg and 50 mg compared with aspirin 650 mg and placebo (lactose) was evaluated. Subjects were 164 dental outpatients undergoing the surgical removal of impacted teeth. Each subject received a single dose of study medication and was evaluated hourly for six hours. Aspirin was superior to placebo in all measures of analgesic efficacy, and both dosages of flurbiprofen were superior to aspirin.

Control of pain with meclofenamate sodium following removal of an impacted molar.

The analgesic effectiveness of meclofenamate sodium (Meclomen) at two dose levels, 200 mg and 100 mg, was compared with the effectiveness of a placebo and aspirin, 600 mg, in a double-blind study of 174 adult outpatients who had undergone removal of impacted third molars. When compared with the placebo, meclofenamate sodium at either dose level produced a significantly greater reduction in pain intensity, greater pain relief, fewer withdrawals for inefficacy, greater percentage of patients who considered their medication effective, and greater percentage of patients considered by the investiga

Peripheral Low Frequency Versus High Frequency Nerve Stimulation: A Case Series.

BACKGROUND: Peripheral nerve stimulation is a therapeutic option for managing chronic neuropathic pain. After a thorough clinical evaluation-including a medical history review and, when appropriate, diagnostic anesthetic injections-a specific nerve associated with the pain can be identified. A peripheral nerve stimulator may then be implanted near this target nerve. The device delivers controlled electrical impulses that disrupt pain signals, offering varying levels of relief. CASE REPORT: This case series compares the clinical efficacy of low frequency vs high frequency peripheral nerve stim

AAAPT Diagnostic Criteria for Acute Neuropathic Pain.

OBJECTIVE: Acute neuropathic pain is a significant diagnostic challenge, and it is closely related to our understanding of both acute pain and neuropathic pain. Diagnostic criteria for acute neuropathic pain should reflect our mechanistic understanding and provide a framework for research on and treatment of these complex pain conditions. METHODS: The Analgesic, Anesthetic, and Addiction Clinical Trial Translations, Innovations, Opportunities, and Networks (ACTTION) public-private partnership with the U.S. Food and Drug Administration (FDA), the American Pain Society (APS), and the American A

Radiofrequency Cingulotomy as a Treatment for Incoercible Pain: Follow-Up for 6 Months.

Incoercible or intractable pain is defined as pain that is refractory to pharmacological treatment to such an extent that opioid and analgesic adverse effects outweigh the therapeutic effects. The anterior cingulate cortex (ACC) is involved in the perception of pain, especially emotional pain, so it is logical that cingulotomy has an effective therapeutic effect. Therefore, we evaluated the effectiveness of cingulotomy for the treatment of incoercible pain. An observational, longitudinal, retrospective, and analytical study was carried out on a series of cases in which bilateral cingulotomy wa

Challenges in drug discovery for overcoming 'dysfunctional pain': an emerging category of chronic pain.

'Dysfunctional pain', a type of chronic pain, is associated with a broad range of clinical disorders, including fibromyalgia, irritable bowel syndrome and interstitial cystitis. It is emerging as a serious issue due to the negative impact of inexplicable pain on quality of life, lack of effective therapies and health care cost. Although drug discovery efforts in pain research have so far focused primarily on inflammatory and neuropathic pain, this editorial attracts attention to dysfunctional pain research and discusses a possible fundamental framework for tackling this difficult issue. While

[Symptomatic approach to musculoskeletal dysfunction and chronic pelvic and perineal pain].

INTRODUCTION: Clinical examination of a patient with chronic pelvic and perineal pain often demonstrates muscle hypertonia or muscle contracture sometimes associated with local tenderness or real muscle trigger points. It is sometimes very difficult to determine whether this muscle pain detected on clinical examination is the cause or a consequence of the pain. The purpose of this article is to review musculoskeletal dysfunction in the context of chronic pelvic and perineal pain. MATERIAL AND METHODS: Review of the literature devoted to musculoskeletal aspects of pelvic and perineal pain. RE

Mixed pain phenotypes in musculoskeletal disorders: mechanisms and management.

Pain is categorized into three pain phenotypes: nociceptive, neuropathic and nociplastic, with overlapping pain referring to the coexistence of more than one pain phenotype. Overlapping pain occurs in more than one-third of individuals with musculoskeletal conditions, including inflammatory arthritis, osteoarthritis, spinal disorders and soft-tissue conditions. Whereas nociceptive pain is associated with identifiable tissue pathology and often responds to anatomically focused treatments, the presence of an additional pain phenotype, most commonly nociplastic pain, contributes to clinical compl

Chronic Post-Treatment Pain in Head and Neck Cancer Survivors: A Descriptive Analysis of Phenotypic Overlap.

BACKGROUND: Chronic post-treatment pain (CPTP) affects up to 42% of head and neck cancer (HNC) survivors, significantly reducing quality of life. Its diverse clinical presentations remain poorly characterized. This study aimed to delineate the distinct painful phenotypes in CPTP following curative HNC treatment. METHODS: A prospective cross-sectional study enrolled 100 adult HNC survivors presenting to the Orofacial Pain Clinic at Shaukat Khanum Memorial Cancer Hospital and Research Centre, Pakistan. Patients with oropharyngeal mucositis, hardware failure, infection, or refractory disease wer

[Symptomatic approach to chronic penile pain].

OBJECTIVE: To describe the aetiologies of non-cancer chronic penile pain. MATERIAL AND METHODS: A review of the literature was performed by searching the Medline database (National Library of Medicine). Search terms were either medical subject heading (MeSH) keywords (pain, penis, penile diseases) or terms derived from the title or abstract. Search terms were used alone or in combinations by using the "AND" operator. The literature search was conducted from 1990 to the present time. RESULTS: Clinical interview and physical examination are essential elements of the assessment of chronic penil

Studies in patients with temporomandibular disorders pain: Can scales of hypnotic susceptibility predict the outcome on pain relief?

Many medical conditions are claimed to benefit when hypnosis is incorporated into their treatment. For some conditions, the claims are largely anecdotal, but the treatment of pain stands out in two ways. First, there is a strong body of evidence that hypnosis can produce clinically useful analgesic effects. Second, since innocuous pain can be induced in the laboratory, the process can be explored rigorously. This idea assumes that experimentally induced pain and clinical pain behave identically. We describe using experimentally induced pain in patients already suffering from temporomandibular

Musculoskeletal Pain in Individuals With Inflammatory Bowel Disease Reflects Three Distinct Profiles.

OBJECTIVES: Pain affects over 70% of individuals with inflammatory bowel disease (IBD), with abdominal and musculoskeletal pain representing the most common symptoms. Musculoskeletal pain in IBD is reported to be associated with multiple clinical features, however the scope and nature of pain is not well understood. Primary aims were to identify subgroups of musculoskeletal pain in individuals with IBD based on clinical features of pain and assess how these subgroups differ in aspects of demographics, comorbidity, and IBD characteristics. METHODS: Cross-sectional online survey was performed o
