[Pleuro-pulmonary manifestation of pancreatic diseases].

Non-neoplastic pancreatic disorders may cause pleuro-pulmonary signs. In acute pancreatitis respiratory disorders are very frequent and may cause simply an hypoxia or the adult respiratory distress syndrome (SDRA). There is no single mechanism: mechanical hypoventilation, alteration of the vessel walls and interstitial oedema, stasis of leucocytes, alterations in surfactant and intravascular coagulation may all contribute. These disturbances may be made by direct action of enzymes or by the intermediary of the activation of the kinine system, of complement activation and of coagulation mechani

[Operability of pancreatic carcinoma, surgical experience].

The authors analyse and evaluate the clinical records of 251 patients operated on during the last 14 years for pancreatic cancer, comparing the results with data in the literature. Among the surgical procedures there were 42 resections of the pancreas, 106 palliative bypass operations and 103 explorative laparotomies. The rate of resectability was 16.7% considering the whole period but it was 21.5% for the second seven-year period. The mortality rate of the palliative and resection operations was 14.2% and 14.3 respectively, it was the highest after explorative laparotomies (22.3%). Among the

Endoscopic evaluation of infected pancreatic necrosis.

The most severe form of acute pancreatitis is characterized by necrosis of the pancreatic parenchyma and/or peripancreatic tissue. Secondary infection of this necrotic tissue carries a high mortality because there is little limitation to the spread of this infection in the retroperitoneum. Improvements in the management of infected pancreatic necrosis have been achieved by techniques that allow complete removal of the infected debris. Problems still remain in identifying the extent of necrosis and removing all of it. Intraoperative endoscopy can be used to evaluate tracking of the infected pro

[Iatrogenic Candida infection in pancreatic pseudocyst].

We report the case of a 34-year-old woman, suffering from severe alcoholic hepatitis and an acute recurrence of chronic pancreatitis, who developed a life-threatening candida infection of pancreatic pseudocysts 12 days after ERCP. After percutaneous catheter drainage under ultrasound guidance this infection was healed by a combined intravenous and intracavity therapy with amphotericin B and the final instillation of tetracycline in order to reinforce obliteration of the pseudocysts.

[Cancer of the pancreatic head and the periampullary region].

Between 1976 and 1988, 16 patients underwent a Whipple resection or total pancreatectomy for carcinoma of the head of the pancreas, the ampulla of Vater or the distal common bile duct. Nine patients had carcinoma of the pancreas. Two of these patients are long-term survivors. The tumors in these patients were highly differentiated and localized close to the common bile duct. Four had ampullary lesions, and are all alive today. Three patients had carcinoma of the common bile duct. One of these patients is alive.

Classification and staging of pancreatic nonendocrine tumors.

Pancreatic nonendocrine tumors are classified according to their histogenetic origin. This results in tumor entities that differ in biology and prognosis. Ductal adenocarcinoma is the dominating pancreatic tumor. Although its morphology is well known, our knowledge of its biology is limited. A grading system is presented for a better individual characterization of ductal adenocarcinoma. The tumor's poor prognosis is due to an early spread to retroperitoneal tissues. This most likely accounts for tumor recurrence so frequently seen after "curative" resection. Tumors other than ductal adenocarci

Developmental patterns for pancreatic opioids in the rat.

Developmental patterns for rat pancreatic opioid peptides and islet hormones were studied from gestational day 20 through adulthood. Fetal tissue was obtained as well as pancreas at birth (day 0), and postnatal days 3, 7, 14, and 21, and 7 weeks. The hormones measured included insulin, glucagon, and somatostatin. The opioids measured were beta-endorphin, Met- and Leu-enkephalins, and the high molecular weight enkephalin precursors. Pancreata were pooled as necessary and extracted (acid alcohol, or hot acetic acid), and opioids were further purified on reversed-phase C-18 (Sep-pak) cartridges.

Pancreatic insuloma causing Cushing's syndrome.

The advent of new diagnostic procedures such as petrosal venous sampling and CRF testing has improved the possibilities of precise location of the site of hormonal overproduction in Cushing's syndrome. We report on a case of ectopic ACTH-dependent Cushing's syndrome in which the use of such techniques helped to locate the tumor. A 45-year-old woman with definite ACTH-dependent Cushing's syndrome was referred to our clinic for evaluation. ACTH, cortisol and urinary 17OHCS failed to suppress on high dose dexamethasone oral administration. Computerized tomography showed bilateral adrenal hyperpla

Uneven lipomatosis: a pitfall in pancreatic sonography.

A marked difference in echogenicity between the anterior and posterior parts of the pancreatic head was demonstrated by pancreatic ultrasound in 4 patients. This finding could not be related to local pancreatic or peripancreatic pathology. In all patients the dorsal portion of the head, including the uncinate process, appeared less echogenic than the ventral part of the head, body, and tail. The level of demarcation was identical in all 4 cases, coinciding with the expected fusion line of the embryological dorsal and ventral pancreatic origin. Pathology available in 1 patient showed an uneven

Surgical treatment of chronic pancreatic cholangiopathy.

In a consecutive surgical series of 70 patients with chronic calcifying pancreatitis, 18 presented with fixed stenosis of the terminal common bile duct. Nine patients presented with jaundice and two had a palpable gallbladder. The most relevant laboratory datum in the series was a persistently high serum alkaline phosphatase level. Long tapering of the terminal common bile duct was the characteristic radiological sign in 45 of our patients. In five of the 18 cases compression of the terminal bile duct was due to cephalic pseudocysts. Hepaticojejunostomy-en-Y was the type of drainage chosen in

[Pancreatic pseudocyst. Description of a case in an infant].

A case of pancreatic pseudocyst (PP) in a 22 month old girl is described. The lesion was huge and posed diagnostic difficulties which led to an inadequate procedure. The anatomic lesion consisted of a lymphocytic pancreatitis associated with two previous episodes of "erratic migration" of Ascaris lumbricoides. Physiopathology and clinical picture is discussed and emphasis is made on the need of suspicion of pancreas as the site of origin of disease in spite of short age.

The role of the liver in exocrine pancreatic disease.

Heightened, but unmitigated, free radical activity within the liver, and the intrinsic bioactivity of certain FROPs and drug metabolites, would rationalise those liver aberrations that cannot be explained on the basis of enzyme induction. Thus the increased amounts of copper in bile suggest heightened production of non-biological reactive intermediates--perhaps derived from halogenated hydrocarbons [29]--and the consequent utilisation of glutathione; because it is known that copper in bile is bound to this protein. The increased amount of bilirubin in our patients' bile (Fig. 3), especially af

Pancreatic beta-cell function in CAPD.

Pancreatic beta-cell function was evaluated in uraemic patients by measuring beta-cell peptides in the peripheral blood after intravenous glucagon (1 mg) stimulation. Patients in chronic renal failure, patients on haemodialysis, and both new and established subjects on continuous ambulatory peritoneal dialysis (CAPD) (10 in each group) were studied and compared to 8 healthy controls. Fasting glucose (3.6-4.4 mmol/l) and insulin concentrations (9.5-11.7 mU/l) were normal and did not differ between the uraemic groups, but c-peptide concentrations were markedly increased in uremia (1.84-2.38 nmol

Pancreatic function and rehabilitation after pancreaticoduodenectomy.

Postoperative pancreatic function and rehabilitation were monitored in 149 patients who had had reconstruction of the digestive tract by Child's method, out of a total 151 patients who had undergone pancreaticoduodenectomy. The occurrence of peptic ulcer following Child's method may be prevented by the resection of a wider area of the stomach. Because the source of gut hormone secretion is removed by the pancreaticoduodenectomy, the secretion of this hormone is subsequently reduced and thus metabolic abnormalities also occur. If careful attention however, is paid to the maintenance of residual

Pancreatic abscess associated with pancreas divisum.

Pancreas divisum has been implicated as a cause of pancreatitis. Pseudocyst development in association with chronic pancreatitis has also been observed in a few patients with this anomaly. The association of pancreatic abscess with pancreas divisum has not been observed previously. The case herein reported illustrates a coincidental finding of pancreas divisum in a patient who presented with a pancreatic abscess.

[Pancreatic cancer from the viewpoint of the surgeon].

The incidence of carcinoma of the exocrine pancreas has gone up by factors of something between 2 and 4 in countries of advanced civilization over the past 30 years. Modern diagnostic methods have enabled safe detection of the carcinoma in 85 per cent of all cases. Laparotomy is justified in situations in which the operability of a pancreas tumour cannot be reliably established by other approaches. Radical therapy should include cephalic or total duodenopancreatectomy and romal of regional lymph nodes. Diffuse metastasation or infiltration of the mesenteric superior arteries and veins, the hep

Coexisting pancreatic and breast adenocarcinomas: is there an association?

Two patients are reported with coexisting adenocarcinomas of pancreas and breast. One represents synchronous neoplasms, the other metachronous tumors diagnosed 7 years apart. The significance of these findings is discussed, including possible carcinogenic dietary factors common to both neoplasms. The potential problems associated with the histologic diagnosis of such coexisting tumors are stressed.
