Gray matter volume decreases in elderly patients with schizophrenia: a voxel-based morphometry study.

BACKGROUND: Aged patients (>50 years old) with residual schizophrenic symptoms differ from young patients. They represent a subpopulation with a more unfavorable Kraepelinian course and have an increased risk (up to 30%) for dementia of unknown origin. However, our current understanding of age-related brain changes in schizophrenia is derived from studies that included less than 17% of patients who were older than 50 years of age. This study investigated the anatomical distribution of gray matter (GM) brain deficits in aged patients with ongoing schizophrenia. METHODS: Voxel-based morphometry

Electroencephalographic characterization of spike-wave discharges in cortex and thalamus in WAG/Rij rats.

PURPOSE: The waveform of spontaneous spike-wave discharges (SWD) in the electroencephalogram (EEG) was delineated in the WAG/Rij rat model of absence epilepsy according to the definitions of clinical electroencephalography. We defined four elements in SWD based on the schema of Weir (1965): Spike 1 and 2, Positive Transient (PT), and Wave. The EEG patterns of generalized type I and local type II SWD in cortical and thalamic areas were analyzed. METHODS: EEGs were recorded in freely moving rats epidurally from different cortical regions and with deep electrodes from the specific and reticular

Localized cerebral blood flow reductions in patients with heart failure: a study using 99mTc-HMPAO SPECT.

BACKGROUND AND PURPOSE: Reduced resting global cerebral blood flow has been previously detected in association with heart failure (HF), but it is not clear whether there are brain regions that could be specifically affected by those brain perfusion deficits. The authors used a fully automated, voxel-based image analysis method to investigate, across the entire cerebral volume, the presence of resting regional cerebral blood flow (rCBF) abnormalities in HF patients compared to healthy controls. METHODS: rCBF was evaluated with 99mTc-single-photon emission computed tomography in 17 HF patients

Selective spatial attention in vision and touch: unimodal and multimodal mechanisms revealed by PET.

Two positron-emission tomography (PET) experiments explored the neural basis of selective spatial attention in vision and touch, testing for modality-specific versus multimodal activations due to attended side. In the first study, either light flashes or finger vibrations were presented bilaterally. Twelve healthy volunteers were scanned while sustaining covert attention on the left or right hemifield within each modality. The main effect for attending right minus left, across both modalities, revealed bimodal spatial attention effects in the left intraparietal sulcus and left occipitotemporal

Language mapping in pretreatment planning of patients with cerebral arteriovenous malformation: a PET study.

PURPOSE: Three patients who had cerebral arteriovenous malformations underwent language mapping by positron emission tomography (PET) to identify the dominant hemisphere for language and the spatial relation of lesions to language areas. METHODS: Mapping was performed to assess the risk that surgery could cause neurologic deficits and to plan other therapeutic strategies. RESULTS: The information obtained by PET language mapping changed the course of management for these patients. Two patients were treated with stereotactic radiosurgery and one patient with surgical resection. CONCLUSIONS:

Quantitative 1H MRS of the human brain in vivo based on the stimulation phantom calibration strategy.

Normal metabolite concentrations were determined in five different brain regions of healthy adult volunteers using proton magnetic resonance spectroscopy (1H MRS) in vivo. The absolute in vivo concentrations of N-acetylaspartate (NAA), creatine and phosphocreatine (Cre), and choline containing compounds (Cho) were quantified from measurements obtained with a head-shaped simulation phantom. Scanner performance and calibration accuracy were assessed by phantom experiments. Localized spectra were acquired on clinical 1.5 T systems using the PRESS localization sequence with frequency selective wat

Calvarial Pyramid Presenting as Occult Metastasis of Follicular Thyroid Carcinoma.

Follicular thyroid carcinoma (FTC) is a malignant follicular cell-derived neoplasm with prominent hematogenous spread. Metastases may be the presenting symptom; however, scalp metastasis is uncommon. A 66-year-old woman presented with progressive scalp swelling since 4 years accompanied by bilateral lower limb weakness and incontinence. On examination, there was a large 10-cm parietal scalp mass. Fine-needle aspiration smears prepared from the pyramidal scalp swelling showed microfollicular arrangement of uniform tumor cells. Diffuse positivity for pancytokeratin, PAX 8, and thyroglobulin on c

Neuroimaging evidence for a dopamine-independent association between motor cortex microstructure and Parkinson's disease severity.

We determined brain microstructure alterations in early-stage Parkinson's disease (PD) using diffusion tensor imaging and neurite orientation dispersion and density imaging (NODDI). Additionally, dopamine transporter (DAT) PET in PD was also performed to evaluate whether microstructural changes and dopaminergic losses within the brain contribute independently to PD motor severity. Mean diffusivity was significantly higher in PD in many mid-brain, nigrostriatal, sub-cortical, cortical and white matter regions. However, Viso (NODDI outcome for cerebrospinal-fluid volume fraction) in the motor co

Paradoxical Embolic Stroke via Pulmonary Arteriovenous Malformation Complicated by Bubble-Study-Induced Non-ST-Segment Elevation Myocardial Infarction.

Pulmonary arteriovenous malformations (PAVMs) are uncommon vascular anomalies that predispose patients to paradoxical embolic phenomena, including stroke. Contrast echocardiography is a key diagnostic tool, but complications are rarely reported. We describe a 44-year-old woman with no significant past medical history who presented with acute ischemic stroke involving the right superior parietal lobe. Initial workup with echocardiography and bubble study demonstrated right-to-left shunting, considered to be of intracardiac origin, typically a patent foramen ovale. However, this was complicated

Acute Type A Aortic Dissection with Concomitant Intracerebral Haemorrhage: A Therapeutic Dilemma Regarding the Timing of Surgery.

Acute type-A aortic dissection is a life-threatening emergency requiring prompt surgical intervention. However, the management of patients presenting with concomitant intracerebral haemorrhage remains challenging, as the risk of neurological deterioration during surgery must be balanced against the risk of fatal aortic rupture. We report the rare case of a 68-year-old patient diagnosed with acute intracerebral haemorrhage involving the parietal lobe and right frontal region in association with acute type-A aortic dissection. Due to the neurological condition, a delayed surgical strategy was ch

ASSOCIATIONS BETWEEN EPILEPSY-RELATED POLYGENIC RISK AND BRAIN MORPHOLOGY IN CHILDHOOD.

Temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) is associated with a complex genetic architecture, but the translation from genetic risk factors to brain vulnerability remains unclear. Here, we examined associations between epilepsy-related polygenic risk scores for HS (PRS-HS) and brain structure in a large sample of neurotypical children, and correlated these signatures with case-control findings in in multicentric cohorts of patients with TLE-HS. Imaging-genetic analyses revealed PRS-related cortical thinning in temporo-parietal and fronto-central regions, strongly anchored to di

From Correlation to Causation: Understanding Episodic Memory Networks.

Episodic memory, our ability to recall past experiences, is supported by structures in the medial temporal lobe (MTL) particularly the hippocampus, and its interactions with fronto-parietal brain regions. Understanding how these brain regions coordinate to encode, consolidate, and retrieve episodic memories remains a fundamental question in cognitive neuroscience. Non-invasive brain stimulation (NIBS) methods, especially transcranial magnetic stimulation (TMS), have advanced episodic memory research beyond traditional lesion studies and neuroimaging by enabling causal investigations through ta

Contrast subgraphs catch patterns of altered functional connectivity in autism spectrum disorder.

Despite the breakthrough achievements in understanding structural and functional alterations of brain connectivity in autism spectrum disorder (ASD), the exact nature and type of such alterations are not yet clear due to conflicting reports of hyper-connectivity, hypo-connectivity, and-in some cases-combinations of both. In this work, we bring order to the debate using a network comparison technique to capture mesoscopic-scale differential patterns of functional connectedness. In particular, we leverage recent algorithmic advances in extracting contrast subgraphs to identify maximally differen

Cross-sectional associations between multisensory impairment and brain volumes in older adults: Baltimore Longitudinal Study of Aging.

Sensory impairment and brain atrophy is common among older adults, increasing the risk of dementia. Yet, the degree to which multiple co-occurring sensory impairments (MSI across vision, proprioception, vestibular function, olfactory, and hearing) are associated with brain morphometry remain unexplored. Data were from 208 cognitively unimpaired participants (mean age 72 ± 10 years; 59% women) enrolled in the Baltimore Longitudinal Study of Aging. Multiple linear regression models were used to estimate cross-sectional associations between MSI and regional brain imaging volumes. For each additio

Multi-scale hierarchical brain regions detect individual and interspecies variations of structural connectivity in macaque monkeys and humans.

Macaques are representative animal models in translational research. However, the distinct shape and location of the brain regions between macaques and humans prevents us from comparing the brain structure directly. Here, we calculated structural connectivity (SC) with multi-scale hierarchical regions of interest (ROIs) to parcel out human and macaque brain into 8 (level 1 ROIs), 28 (level 2 ROIs), or 46 (level 3 ROIs) regions, which consist of anatomically and functionally defined level 4 ROIs (around 100 parcellation of the brain). The SC with the level 1 ROIs showed lower individual and int

Medial temporal lobe atrophy patterns in early-versus late-onset amnestic Alzheimer's disease.

BACKGROUND: The medial temporal lobe (MTL) is hypothesized to be relatively spared in early-onset Alzheimer's disease (EOAD). Yet, detailed examination of MTL subfields and drivers of atrophy in amnestic EOAD is lacking. METHODS: BioFINDER-2 participants with memory impairment, abnormal amyloid-β and tau-PET were included. Forty-one amnestic EOAD individuals ≤65 years and, as comparison, late-onset AD (aLOAD, ≥70 years, n = 154) and amyloid-β-negative cognitively unimpaired controls were included. MTL subregions and biomarkers of (co-)pathologies were measured. RESULTS: AD groups showed smal

Long-Term Bridge Training Induces Functional Plasticity Changes in the Brain of Early-Adult Individuals.

The aim of this study was to investigate the impact of extended bridge expertise on rapid perceptual processing and brain functional plasticity in early adulthood, utilizing functional magnetic resonance imaging (fMRI). In this investigation, we compared 6 high-level college bridge players with 25 college students lacking bridge experience, assessing their intelligence and working memory. Additionally, we scrutinized behavioral performance and whole-brain activation patterns during an image perceptual judgment task. Findings indicated significant group and interaction effects at the behavioral

The spatiotemporal dynamics of semantic integration in the human brain.

Language depends critically on the integration of lexical information across multiple words to derive semantic concepts. Limitations of spatiotemporal resolution have previously rendered it difficult to isolate processes involved in semantic integration. We utilized intracranial recordings in epilepsy patients (n = 58) who read written word definitions. Descriptions were either referential or non-referential to a common object. Semantically referential sentences enabled high frequency broadband gamma activation (70-150 Hz) of the inferior frontal sulcus (IFS), medial parietal cortex, orbitofro

Hyperosmolar hyperglycaemic state as a stroke cause or stroke mimic: an illustrative case and review of literature.

Hyperosmolar hyperglycaemic state (HHS) is a life-threatening metabolic complication of type 2 diabetes (T2DM) that often presents with neurological symptoms. A 74-year-old man with known T2DM presented to the emergency department with collapse, left-sided weakness and slurred speech (National Institutes of Health Stroke Scale (NIHSS) 3) and a biochemical profile consistent with HHS. When he further deteriorated (NIHSS 20), he was managed for concurrent ischaemic stroke. All his symptoms fully resolved after 24 hours, which coincided with establishment of normoglycaemia.
