Multidisciplinary Intervention for Children With Epilepsy and Autism Spectrum Disorder Admitted for EEG: A New Standard of Care.

BACKGROUND AND OBJECTIVES: Approximately one-third of children with epilepsy develop intractable epilepsy and require multiple-day hospital admission for EEG and neuroimaging to determine other interventions for seizure reduction (Phase 1). Of note, children with epilepsy are at increased risk of autism spectrum disorder (ASD); however, prolonged hospitalization may be difficult due to developmental delays, sensory sensitivities, and challenging behavior. Challenging behavior during or reluctance to complete admission may lead to delayed or incomplete information about seizures and interfere w

Hierarchical clustering of clinical and flow cytometry parameters is associated with deterioration in patients with community-acquired pneumonia in the emergency department: A preliminary study.

Anticipating the evolution of septic patients with community-acquired pneumonia (CAP) is challenging for front-line physicians in the Emergency Department (ED). Prognosis depends mainly on early identification, antibiotics, organ support, but also immune status. The objective of this proof-of-concept study was to perform a cluster analysis to investigate whether specific phenotypes, including cellular immunology parameters, are associated with the prognosis in patients with CAP presenting to the ED. We performed an exploratory study in the ED of Limoges university hospital (France) on patients

The association between neutrophil lymphocyte ratio and perihematomal edema in cerebral hemorrhage: a multicenter retrospective study.

BACKGROUND: Perihematomal edema (PHE) after a brain hemorrhage is an increase in the water content of the brain tissue surrounding the haematoma, which can be observed and measured on imaging. PHE is one of the major secondary brain injuries after a brain hemorrhage and is strongly associated with poor patient prognosis. The relationship between the neutrophil-lymphocyte ratio (NLR) and cerebral oedema after cerebral hemorrhage remains unclear. METHODS: Data for this study were obtained from a registry database of hospital admissions at two medical institutions covering the population of sout

Trends in the Evaluation and Treatment of Patients Presenting With Sigmoid Volvulus Over the Past 15 Years.

BACKGROUND: Few studies have assessed changes over time in the evaluation and treatment of patients presenting with acute sigmoid volvulus. We sought to evaluate trends in sigmoid volvulus care at a single center between 2010 and 2024. We hypothesized that there would be increasing rates in the use of computed tomography (CT) scans, lactate levels, laparoscopic surgery, and same-admission surgery. METHODS: A single-center, retrospective study was performed for all adult admissions for acute sigmoid volvulus over 15 years (2010-2024). Each admission was assessed individually for demographic, c

Neutrophil CD64 index as a new early predictive biomarker for infected pancreatic necrosis in acute pancreatitis.

OBJECTIVE: Infectious pancreatic necrosis (IPN) is a serious complication of acute pancreatitis, and early recognition and timely intervention are the keys to improving clinical outcomes. The purpose of this study was to investigate the predictive capacity of the neutrophil CD64 index (nCD64 index) on IPN in patients with acute pancreatitis METHODS: This study comprises two independent cohorts: the training cohort consisted of 202 patients from Hunan Provincial People's Hospital, and the validation cohort consisted of 100 patients from Changsha Central Hospital. Peripheral blood samples were c

Lung Ultrasound to Evaluate Fluid Status and Optimize Early Volume-Expansion Therapy in Children with Shiga Toxin-Producing Escherichia Coli-Haemolytic Uremic Syndrome: A Pilot Study.

Background: Shiga toxin-producing Escherichia coli-haemolytic uremic syndrome (STEC-HUS) can result in kidney and neurological complications. Early volume-expansion therapy has been shown to improve outcomes, but caution is required to avoid fluid overload. Lung ultrasound scanning (LUS) can be used to detect fluid overload and may be useful in monitoring hydration therapy. Methods: This prospective observational pilot study involved children with STEC-HUS who were recruited from a regional paediatric nephrology centre. B-line quantification by LUS was used to assess fluid status at the emerge

Inappropriate therapy of euvolemic hyponatremia, the most frequent type of hyponatremia in SARS-CoV-2 infection, is associated with increased mortality in COVID-19 patients.

INTRODUCTION: Admission hyponatremia, frequent in patients hospitalized for COVID-19, has been associated with increased mortality. However, although euvolemic hyponatremia secondary to the Syndrome of Inappropriate Antidiuresis (SIAD) is the single most common cause of hyponatremia in community-acquired pneumonia (CAP), a thorough and rigorous assessment of the volemia of hyponatremic COVID-19 subjects has yet to be described. We sought to identify factors contributing to mortality and hospital length-of-stay (LOS) in hospitalized COVID-19 patients admitted with hyponatremia, taking volemia i

The Challenge of Predicting Pressure Ulcers in Critically Ill Patients. A Multicenter Cohort Study.

RATIONALE: Pressure ulcers are preventable events. Critically ill patients are particularly vulnerable. The Braden scale has been used to identify hospitalized patients at high risk for the development of pressure ulcers; however, this predictive tool has not been adequately validated for critically ill patients. OBJECTIVES: We aimed to validate and improve the Braden scale for critically ill patients by adding clinical variables to the original scale. METHODS: We conducted a cohort study in 12 intensive care units (ICUs) within a network of hospitals in Brazil during 2013. We excluded patie

Immediate posttraumatic seizures: is routine hospitalization necessary?

OBJECTIVE: A recent Internet survey of pediatric neurosurgeons showed that 86% routinely admitted children with immediate posttraumatic seizures (PTS) for a brief period of observation. We wished to determine whether certain children meeting predefined criteria could instead be safely discharged from the emergency room. METHODS: We reviewed the records of children admitted during the past 5 years with a diagnosis of seizure and head injury. Children with a minor head injury, a PTS occurring within 24 h of injury and no intracranial abnormalities on admission CT scan were included. Children wi

Admission of bone marrow transplant recipients to the intensive care unit: outcome, survival and prognostic factors.

The role of ICU support in BMT patients is controversial. In an era of constrained resources, the use of prognostic factors predicting outcome may be helpful in identifying patients who are most likely (or unlikely) to benefit from this intervention. We attempted to define the survival of patients admitted to ICU following autologous or allogeneic BMT and to identify those factors important in determining patient outcome. A retrospective study of all adult BMT recipients admitted to intensive care over a 6 year study period was performed to determine overall and prognostic indicators of poor o

Is there an increased risk of hip fracture in Parkinson's disease? A nationwide inpatient sample.

OBJECTIVE: The aim of this study was to determine whether people with Parkinson's disease (PD) are overrepresented in a national cohort of hip-fracture admissions. BACKGROUND: Frequent falls, combined with a higher rate of osteoporosis in people with PD, should lead to an increased risk of hip fracture. METHODS: This work was a retrospective cohort analysis from the Nationwide Inpatient Sample from 1988 to 2007, a stratified sample of 20% of U.S. hospital admissions. Admissions with a primary diagnosis of acute hip fracture were identified, as was a subset with a secondary diagnosis of PD.

Hospitalisations during the 30-day period preceding admissions with cardiac arrest.

BACKGROUND: Cardiac arrest (CA) is common but deadly. Prior hospitalisation represents a missed opportunity for prevention and identification of high-risk groups before CA. This study aims to determine the extent of and underlying reasons for hospitalisations during the 30-day period preceding an admission with CA. METHODS: We conducted a retrospective cohort study using the United States National Readmission Database (NRD) during 2018-2020. We evaluated hospitalisations with a diagnosis of CA and admissions in the 30-day period before hospitalisation with CA. Multiple logistic regressions we

The long-term burden of congenital cytomegalovirus: Hospitalisation and mortality in a population-based matched cohort study.

AIM: Congenital cytomegalovirus (cCMV) infection can result in considerable morbidity and mortality. However, the impact of cCMV on health system utilisation beyond infancy is poorly defined. Here we sought to describe the burden of cCMV health service use and mortality using linked population-based datasets. METHODS: The design was a matched cohort study using record-linked administrative datasets. Participants included all children aged 0-15 years identified through the New South Wales (NSW), Australia, Admitted Patient Data Collection who were hospitalised with an admission code for cCMV i

Prospective Phase II trial of drug-eluting bead chemoembolization for liver transplant candidates with hepatocellular carcinoma and marginal hepatic reserve.

BACKGROUND: After a pediatric intensive care unit (PICU) admission, children can experience long-term effects on their growth and development, termed post-intensive care syndrome in pediatrics (PICS-p). The impact of a PICU admission on a family's social health is understudied, especially for families with low socioeconomic status (SES) who may be at greater risk for PICS-p. This study explored how families with low SES experience the social aftermath of a PICU admission, specifically the impacts on aspects of social health, including routines, relationships, and finances. METHODS: We perform

Undetected HIV infection among patients admitted to an alcohol rehabilitation unit.

Of 300 patients admitted to an alcohol rehabilitation unit, serological testing of discarded admission blood samples revealed that 31 (10.3%) had HIV infection. Chart records indicated that four patients were known to be HIV infected on admission, and HIV infection was detected in another three patients during hospitalization; however, 24 (77.4%) of the HIV-positive patients were discharged with their HIV infection still undetected.
