Using electronic medical records to identify patients at risk for underlying cardiac amyloidosis.

BACKGROUND: Identification of transthyretin cardiac amyloidosis (ATTR-CA) patients is largely based on pattern recognition by providers, and this can be automated through electronic medical systems (EMR). METHODS: All patients in a large academic hospital with age > 60, ICD-10 code for chronic diastolic heart failure and no previous diagnosis of any amyloidosis were included. An Epic EMR scoring logic assigned risk scores to patients for ICD-10 and CPT codes associated with ATTR-CA, as follows: carpal tunnel syndrome (score 5), aortic stenosis/TAVR (5), neuropathy (4), bundle branch block (4)

How to distinguish spinocerebellar ataxia 27B from late onset cerebellar ataxia: insights from a case-control study.

BACKGROUND: Spinocerebellar ataxia 27B is the most common genetic late onset cerebellar ataxia (LOCA). However, it commonly overlaps with other genetic LOCA as with the cerebellar form of multiple system atrophy (MSA-C). OBJECTIVES: To pinpoint which clinical signs and symptoms best discriminate between FGF14 + from FGF14 - patients at symptoms' onset. METHODS: Twenty SCA27B (≥ 250 GAA repeat expansion) patients were retrospectively matched by gender and age at disease onset with 20 negative FGF14 (-) LOCA patients and with 20 MSA-C patients. Clinical features were ranked based on their cont

Improving outcomes of patients with advanced prostate cancer through a better understanding of clinical factors contributing to financial toxicity: a qualitative study.

PURPOSE: Patients with advanced prostate cancer (aPC) often face significant treatment-related financial hardship. Given the critical role non-physician clinicians play in helping patients access prescribed treatments, we sought to explore their perspectives on factors contributing to treatment-related financial toxicity. METHODS: We conducted semi-structured interviews with non-physician clinicians who care for patients with aPC. We used purposive sampling to capture diverse perspectives across practice types (academic, community), and settings (rural, urban, suburban). Participants describe

Hemophagocytic inflammatory syndrome in ADA-SCID: report of two cases and literature review.

Hemophagocytic inflammatory syndrome (HIS) is a rare form of secondary hemophagocytic lymphohistiocytosis caused by an impaired equilibrium between natural killer and cytotoxic T-cell activity, evolving in hypercytokinemia and multiorgan failure. In the context of inborn errors of immunity, HIS occurrence has been reported in severe combined immunodeficiency (SCID) patients, including two cases of adenosine deaminase deficient-SCID (ADA-SCID). Here we describe two additional pediatric cases of ADA-SCID patients who developed HIS. In the first case, HIS was triggered by infectious complications

Unusual histopathological findings in appendectomy specimens with clinical diagnosis of acute appendicitis: A retrospective cohort analysis.

INTRODUCTION: While appendicitis is considered one of most common acute surgical conditions, several studies have reported abnormal histopathological findings in appendectomy specimens; however, sending all appendices to histopathology is not yet routinely done. Here we report many unusual findings. Those unusual findings played a role not only in confirming acute appendicitis as a cause of the presentation in some cases but also discovering etiologies that mimic it with great impact on its management. METHODS: Between January 2011 and December 2017, a total of 1510 patients were operated with

Risk factor analysis of long-term outcomes in patients with large intracerebral and intraventricular hemorrhage treated primarily with combined external ventricular drainage and intraventricular thrombolysis.

PURPOSE: Surgical outcomes are typically poor for patients with spontaneous large intracerebral hemorrhage (ICH) complicated by intraventricular hemorrhage (IVH). The optimal treatment strategy remained undetermined -either external ventricular drainage (EVD) alone or in combination with craniotomy. Here, we evaluated shunt-related complications of such patients treated primarily with EVD combined with intraventricular thrombolysis, aiming to provide evidence that may lead to future treatment guidelines. MATERIALS AND METHODS: From January 2010 to December 2021, a total of 114 patients with s

Comparative analysis of demographic and clinical features in common variable immunodeficiency and selective immunoglobulin G deficiency.

Background: Common variable immunodeficiency (CVID) and selective immunoglobulin G (IgG) deficiency (sIgGD) are primary antibody deficiencies with divergent clinical trajectories. CVID is characterized by recurrent infections and systemic noninfectious complications, whereas sIgGD typically follows a milder course. Despite sharing clinical features such as recurrent infections, comparative data on complications and mortality remain limited, which hinders risk-stratified management. Objective: This study compared the prevalence, clinical impact, and mortality associated with noninfectious compl

Revertant mosaic fibroblasts in recessive dystrophic epidermolysis bullosa.

BACKGROUND: Revertant mosaicism has been described previously in recessive dystrophic epidermolysis bullosa (RDEB), manifesting as regions of skin with normal mechanical and biological characteristics. Here we report the discovery of revertant dermal fibroblasts, unique in that all other documented cases of revertant mosaicism occur in epidermal keratinocytes. OBJECTIVES: To determine the cause of revertant mosaicism found in a patient with RDEB from isolated epidermal keratinocytes and dermal fibroblasts in blister and mosaic skin regions. METHODS: Skin biopsies were taken from blister and

SLC39-driven zinc influx orchestrates pleiotropic tumor-immune system cross-talk to establish immune suppression in colorectal cancer.

Tumor metabolic programs shape immune evasion; however, the mechanism by which nutrient-transport systems rewire tumor-immune cell interactions remains unknown. We integrated bulk multi-omics, single-cell transcriptomics, spatial profiling, and functional assays for colorectal cancer-associated zinc-driven immunosuppressive circuit identification. Multi-omics analysis of 258 patients revealed three solute carrier transporter-centered archetypes, with an SLC39-enriched subtype aligned with consensus molecular subtype 2-like immune-desert tumors. SLC39-mediated zinc influx activated CDX2, which
