Clinical and radiographic disease course of hand osteoarthritis and determinants of outcome after 6 years.

OBJECTIVE: To investigate the long-term clinical and radiographic disease course of hand osteoarthritis (OA) and determinants of outcome. METHODS: Clinical and radiographic measures were obtained at baseline and after 6 years in 289 patients with hand OA (mean age 59.5 years, 83.0% women). Clinical outcomes were self-reported pain and functional limitations assessed with the Australian/Canadian Osteoarthritis Hand Index (AUSCAN). Poor clinical outcome was defined as a follow-up score not fulfilling the Patient Acceptable Symptom State. Radiographic outcome was assessed by osteophytes and join

Symptom predictability in gastroesophageal reflux disease and role of proton pump inhibitor test.

There are problems with the definition, assessment, and measurement of gastroesophageal reflux disease (GERD). The Reflux Disease Questionnaire and the GERD questionnaire are patient-reported outcome (PRO) measures for use in a primary care setting, which are easy to use and are validated. There is no widely accepted definition of a proton pump inhibitor test and performance of the test in the clinical setting is not standardized. The use of the PRO measures in primary care with predetermined cutoff values may help to reduce the cost of diagnosing GERD and increasing rates of response for eval

Patient-Centered Diabetes Care Through an Ethical Lens: Associations with Patient Satisfaction and Patient-Reported Outcomes in Saudi Primary Healthcare.

Background: Patient-centered care (PCC) is central to diabetes management; unfortunately, evidence concerning its relationship with patient-reported outcome measures (PROM) in primary healthcare facilities in Jeddah is limited. This study examined the association between validated PCC scores and patient-reported outcomes and explored how PCC domains could be interpreted through four ethical principles. Methods: A cross-sectional survey was conducted on 594 adults with diabetes recruited from 47 primary care facilities between July and August 2023. The primary exposure was the validated 36-item

How is pain associated with pelvic mesh implants measured? Refinement of the construct and a scoping review of current assessment tools.

BACKGROUND: Recommendations for the management of pain related to pelvic mesh implants are still under development. One limitation that has impeded progress in this area is that mesh-related pain has not been consistently defined or measured. Here, we reviewed the ways in which pain associated with pelvic mesh implants has been measured, and mapped the ways in which these existing measures capture the construct. METHODS: First, we reviewed existing accounts of the pain associated with pelvic mesh implants to develop a multifaceted construct definition, which includes aspects related to pain i

Clinical outcomes and predictors of dissatisfaction following revision surgery after distal clavicle excision.

BACKGROUND: Distal clavicle excision (DCE) is a well-established treatment for acromioclavicular (AC) joint pain and osteoarthritis, with generally favorable outcomes after primary surgery. However, persistent pain or instability may require revision surgery. The literature on outcomes of revision AC surgery after DCE is limited. The purpose of this study was to evaluate clinical outcomes after revision surgery for persistent pain or instability following DCE and to identify predictors associated with patient satisfaction, subjective instability, and complications. METHODS: A retrospective co

A 360 Degree Qualitative Ethical Assessment of Reporting Patient Reported Outcomes.

While guidelines exist for returning genetic, biomarker, and radiologic research results, there is little guidance regarding the return of patient-reported outcome (PRO) research results. As a supplement to an ongoing trial of palliative care for Parkinson's disease, we conducted thirty qualitative, semi-structured interviews of persons with Parkinson's disease, their carepartners, community neurologists, primary care physicians, and researchers to elicit their perceptions of returning PRO research data to persons with Parkinson's disease and carepartners. Interviews were audio-taped, transcri

Impact of patient-reported taxane-induced peripheral neuropathy on dose reductions or worsening quality of life in Black women with breast cancer: Analysis from ECOG-ACRIN EAZ171.

BACKGROUND: Black patients with breast cancer experience higher rates of taxane-induced peripheral neuropathy (TIPN), which may compromise chemotherapy dose delivery and health-related quality of life (HRQOL). Using the prospective ECOG-ACRIN EAZ171 trial, the authors examined whether patient-reported TIPN predicts dose reductions and longer-term declines in HRQOL and physical function. METHODS: EAZ171 enrolled 249 Black patients planned for (neo)adjuvant taxane; 239 with available patient-reported outcome (PRO) data were analyzed. TIPN was assessed during treatment using the Functional Asses

Association of area-level income with patient reported long-term disability outcomes post-traumatic brain injury.

BACKGROUND: Traumatic Brain Injury (TBI) affects 64-74 million people annually, often causing long-term disability. The influence of social determinants of health (SDOH), particularly neighborhood and built environments, on functional outcomes post-TBI remains underexplored. This study examines the association between census tract-level median household income- a proxy for area income- and self-reported functional outcomes in TBI-patients seen in a Southern California TBI clinic. METHODS: A retrospective cohort study of Neurology TBI & Concussion Clinic data (9/2022-1/2025) included patients

Patient-reported outcomes from the TBCRC 022 study of neratinib and ado-trastuzumab emtansine for HER2-positive breast cancer brain metastases.

PURPOSE: In TBCRC 022 (NCT01494662), neratinib and ado-trastuzumab emtansine (T-DM1) demonstrated intracranial activity among patients with HER2-positive breast cancer brain metastases. However, gastrointestinal (GI) toxicities-particularly diarrhea-were common, potentially impacting quality of life. Clinician-reported adverse event (CTCAE) grading may underestimate the patient experience. We report GI toxicities using patient-reported outcomes (PROs) in TBCRC's neratinib-T-DM1 cohort. METHODS: Patients received neratinib (160 mg daily) and T-DM1 (3.6 mg/kg IV every 21 days). A pre-planned an

Implications of Shame for Patient-Reported Outcomes in Bowel Disorders of Gut-Brain Interaction.

BACKGROUND & AIMS: Bowel disorders of gut-brain interaction (DGBIs) are a highly stigmatized group of disorders. Although the link between stigma and shame is acknowledged overall, few studies focusing on health-related stigma mention shame, and research on shame in bowel DGBIs is nonexistent. The aim of this study was to examine the implications of shame in bowel DGBIs. METHODS: Two Swedish population-based samples were included: a bowel DGBIs sample (n = 537) and a matched comparative sample without bowel symptoms (n = 1881). Participants completed an online survey with validated self-repor

Insulin-like growth factor-I and symptoms of acromegaly according to time since somatostatin receptor ligand injection.

BACKGROUND: Somatostatin receptor ligands (SRLs) are the mainstay of pharmacotherapy for acromegaly. Patients report symptoms toward the end of the injection cycle. However, the correlation between insulin-like growth factor-I (IGF-I) and patient-reported outcomes (PROMs) has not been evaluated in this context. OBJECTIVE: To determine IGF-I, PROMs, and SRL concentration variability in patients with acromegaly receiving long-acting SRLs. METHODS: Quality of life surveys, early and late phase (weeks 1 and 4 after SRL injection) biochemical markers, and SRL concentrations were obtained in 20 pa
