From May 1983 to 1992, 15 patients with urinary epithelial squamous cell carcinoma in renal pelvis (7) and in bladder (8) were treated. Eleven of them were complicated by urinary tract stone. Of the 15 patients, 8 were misdiagnosed as having simple urinary stone and 1 renal tuberculosis. It is suggested that active treatment for urinary stones be used to prevent the occurrence of squamous cell carcinoma and that possibility of concomitant epithelial tumor in patients with long-term and large urinary stones be considered.

We present a rare case of a hydatidiform mole that occurred in a 56-year old woman. The main symptom was prolonged vaginal bleeding, and the gynaecological examination revealed a markedly enlarged uterus. Sonography revealed a 9 x 15 cm large cystic tumour in the pelvis. At operation, a suspicion of trophoblastic disease was arosen. Serum human chorion gonadotropin was still markedly elevated on the fifth postoperative day. After removal of the uterus and adnexae the recovery and the 3-year follow-up have been uneventful.

We report on a fetus with multiple pterygia in the popliteal, antecubital, intercrural, axillary, and nuchal region, arthrogryposis, camptodactyly, anal atresia, hypospadias, ambiguous genitalia, and neonatal death. Arthrogryposis was much more pronounced at the left than at the right side. Moreover, there was gross body asymmetry with hypoplasia of the left arm, leg, pelvis, and kidney. As this spectrum of anomalies does not fit any of the known multiple pterygium syndromes, this patient adds another clinical entity to the already wide spectrum of multiple pterygium syndromes.

We present a case of a 65 year-old woman with a late vesicovaginal fistula after colporrhaphy. Too late proper diagnosis of that complication caused perivesical tissue inflammation and led to damage to the urinary bladder, occlusion of the urethra as well as created a massive inflammatory tumor in the minor pelvis, which suggested a cancerous process. Bilateral hydronephrosis forced us to make bilateral nephrostomy. After intensive antiinflammatory therapy, anterior exenteration and Bricker's operation were performed with a finally good effect.

The study describes two cases of polyostotic fibrous dysplasia of the pelvis tending to progress along the lower extremities. The disease is of unknown origin and develops in the period of puberty. Has chronic qualities with remissions which could last for several years. The disease leads to deformities but spontaneous fractures are also possible. It is diagnosed by means of thorough radiologic examinations and histologically. Therapeutical treatment consists of symptomatic therapy and physiatric procedures, while cases of marked deformities and spontaneous fractures require surgical treatment

We reviewed 122 cases of pyeloureterolithotomy. The pyeloureterotomy incision was closed loosely in 70 cases and not closed in 52 cases. The duration of urinary leakage had no difference statistically between no closure and loose closure (p greater than 0.01). The duration of urinary leakage from the renal pelvis and the upper ureter was shorter than that from the lower ureter in both loose and no closure (p less than 0.05). There was no postoperative stricture. In difficult closures it is not necessary to close the pyeloureterotomy incision.

The most dreaded complication of vaginal breech delivery is entrapment of the aftercoming head. When this is due to disproportion, persistent attempts at vaginal extraction are likely to result in a dead or damaged baby. A largely unknown solution in this desperate predicament is to surgically enlarge the pelvis by means of a symphysiotomy. A review of the literature shows that symphysiotomy performed to free the trapped aftercoming head will save at least 80% of babies if the procedure is performed without delay. Invasive transitional cell carcinoma of the bladder is known to extend directly to other organs in the pelvis. When metastases occur they almost always adhere to a rather consistent pattern of dissemination. We present an unusual case of partial obstruction of the sigmoid colon due to a metastatic transitiocellular bladder tumour four months after transurethral resection of the tumour complicated with a perforation into the peritoneal cavity. Direct implantation of the tumour with the resectoscope is the most possible explanation, but seeding on an intact serosa cannot be excluded either.

Two patients with documented anteroposterior compression injuries to the pelvis sustained concomitant injury to the iliofemoral artery and fracture of the acetabulum involving the high anterior column (the portion of the ilium forming the anterior component of the acetabulum). A 38-year-old female patient with a long history of bilateral dysplasia of the hip died during a TEP-reimplantation of a hemorrhage due to a lesion in the left external iliac artery. The real extent of the atrophy in the left acetabular fossa could not be detected on the preoperative X-ray photograph. A conspicious secondary finding were fixation screws in the well adapted Judet prosthesis on the right side that were projecting up to 2 cm into the pelvis. The results of the autopsy and the special juridical questions involved are demonstrated and discussed.

A male premature triplet with healthy siblings was born with characteristic symptoms of a Rossi syndrome: pterygiums at neck, axillae, elbows and in a moderate way at hips and knees with inhibition of extension of the affected joints, dislocation of the right hip, club-feet, cutis laxa et hyperelastica, facial dysmorphism, double pelvis renalis with ureter duplex on the right side, kryptorchidism, hypoplastic genital. The boy died after 38 days because of a haemorrhage of the lungs. An intrauterine immobilisation is discussed as cause of the syndrome.

We performed suprapubic percutaneous urography in two patients to diagnose unusual duplication anomalies of the urinary tract, since preoperative diagnosis was unclear after routine noninvasive studies. In both patients standard suprapubic puncture technique was employed. The procedure was used to confirm the diagnosis of duplication of the bladder in one case and to establish the site of origin of a ureterocele in the other. The technique employed in suprapubic cystography may be extended to the diagnosis of unusual cystic abnormalities in the pelvis that may be of urinary tract origin.

Concern has been expressed regarding the use of ultrasonic lithotripsy in the percutaneous treatment of renal calculi with the suggestion that high rates of retained stones may be experienced. Evaluation of 89 patients with stones confined to the renal pelvis treated both with and without ultrasonic lithotripsy were evaluated with regard to stone size, retained fragments, stone composition and previous renal surgery. An asthenic 30-year-old dentist presented at the orthopaedic clinic complaining of pain in both hip joints. Although venous thrombosis most often occurs in the return circulation of the legs and pelvis, it may also occur in the veins of several organs and compromise venous return. Thus, the clinician, in any field will regularly be confronted with manifestations of venous thrombosis in particular organs. This review summarizes the pathogenesis and the main clinical features of venous thrombosis in the central retinal vein, the cerebral veins and sinuses of the skull, the renal and the portal veins and the hepatic and mesenteric veins as well as priapism. The principles of treatment are outlined.

Four years experience of a haematuria diagnostic service in a District General Hospital, comprising 215 patients, has identified 42 new cases of bladder cancer, 5 of renal cell cancer and 3 of renal pelvis cancer. Urine and blood tests were non-contributory in their diagnosis but intravenous urography was diagnostic in the majority. The period of time between presentation to the general practitioner and treatment of the cancer has been reduced to less than the time normally spent on the Out-Patient waiting list.

AA. are reporting their experience on the subject of the surgical treatment of the rectal prolapse in all his clinical forms, during 12 years of activity that has taken place in the clinical surgery of the University of Ferrara. From their casuistry it is shown that usually this pathology is joined with other morbid forms of the small pelvis which vary according to the degree of the prolapse. They emphasize the excellent results obtained and support the surgical way of laparotomy adopted in the treatment of prolapse of II type (incomplete) and III type (complete).

Demonstration of traumatic lumbosacral root avulsion is a rare clinical entity because the bony pelvis offers support and protection to the nerve roots against stretching and rupture. Major vehicular trauma producing unstable pelvic fractures allows lumbosacral root avulsion to occur, and the 18th well-documented case is reported. Delay in diagnosis of several months is common. Demonstration of traumatic meningoceles is important, and metrizamide offers better visualization of the root than does iodophendylate. Absence of the root in the meningocele is equated with avulsion.
