One-stage versus two-stage video-assisted thoracic surgery for synchronous bilateral pulmonary nodules: protocol for a single center, non-randomized clinical trial (OTVATS-1).

BACKGROUND: Previous retrospective studies demonstrated both one-stage and two-stage video-assisted thoracic surgery (VATS) for bilateral pulmonary nodules were safe and feasible in selected patients. However, prospective data is still lacking. The purpose of this trial is to prospectively compare the prognostic and perioperative outcomes between one-stage and two-stage VATS for synchronous bilateral pulmonary nodules. METHODS: We conduct a prospective clinical trial to investigate the surgical outcomes of one-stage and two-stage VATS for patients with synchronous bilateral pulmonary nodules.

Preoperative Urine Testing in Elective Endourology Procedures: Is a Change in Practice the Need of the Hour?

Introduction  Urine testing is routinely done preoperatively in patients undergoing elective endourology procedures to ensure that any asymptomatic bacteriuria is picked up and treated. This practice, in addition to leading to overtreatment, can also lead to unnecessary cancellations of elective procedures. With the emergence of multidrug-resistant bacteria and the implementation of stricter antibiotic stewardship programmes, it is the need of the hour to re-examine this innocuous practice. Patients and methods  One hundred patients undergoing elective endourology surgery were studied retrospe

Smoking and Postoperative Pain: A Systematic Review and Meta-Analysis.

BACKGROUND: Smoking is known to increase the risk of numerous postoperative complications, including increased rates of infection, cardiovascular events, and pulmonary complications. In addition, there may be an effect of smoking on pain in the perioperative period. Through this meta-analysis, we sought to investigate what is known about the relationship between smoking and acute postoperative pain. METHODS: Articles published between January 2003 and December 2023 that compared smokers to nonsmokers and utilized pain scores and/or measured postoperative opioid consumption were selected for r

Hypertension after solid-organ transplantation: special considerations for management.

PURPOSE OF REVIEW: Hypertension is one of the most common comorbidities affecting patients after solid organ transplantation. Here we review the mechanisms leading to hypertension, along with common practices in terms of medical management. Glucagon-like peptide-1 receptor (GLP-1R) agonists, baroreflex therapy, and renal denervation are common interventions utilized prior to heart transplant. Special considerations for their management throughout the perioperative and postoperative period are covered here. RECENT FINDINGS: Most common medical management includes utilization of calcium channel

Clinical Status and Reintervention in Neonates With Symptomatic Tetralogy of Fallot: A Landmark Analysis.

BACKGROUND: In symptomatic neonates with tetralogy of Fallot (sTOF), the initial treatment strategy significantly affects early outcomes, but its long-term impact remains less well defined. OBJECTIVES: The aim of the study was to compare primary (PR) vs staged repair (SR) in sTOF with respect to reintervention (RI) rates and types, clinical and echocardiographic outcomes, and medication use. METHODS: Neonates with sTOF undergoing PR or SR and with >1 year of follow-up after complete repair were included. The primary outcome was cumulative RI incidence; secondary outcomes included mortality a

Risk factors for 30-day mortality in neonates undergoing surgery for esophageal atresia.

Despite advances in neonatal surgery and intensive care, esophageal atresia (EA) continues to carry a substantial risk of early postoperative mortality. The aim of this study was to investigate risk factors for 30-day mortality in neonates undergoing surgery for EA. Following approval by our institutional ethics committee, we conducted a retrospective study from January 1, 2010 to December 31, 2024, in a pediatric surgery department. All neonates (≤28 days) who underwent primary surgery for EA were included. During the 15-year study period, 113 neonates underwent surgery for EA, with 52.2% bei

Infrared thermography for detecting compensatory load in people with haemophilia: a cross-sectional study.

BACKGROUND: Haemophilia, caused by deficiencies in factor VIII or IX, leads to deep tissue bleeding and haemophilic arthropathy. Bleeding, arthropathy, and the perioperative period of surgery for arthropathy are associated with severe pain. To avoid this pain, patients may compensate by overloading other parts of the body, potentially resulting in pain or secondary damage in areas distant from the primary site. This study investigates the use of infrared thermography (IRT) to monitor whole-body surface temperatures in people with haemophilia and to explore the potential of infrared thermograph

A Case of Development and Rapid Increase in the Microbleeds after Pipeline Placement for an Intracranial Unruptured Aneurysm.

Flow diverter stents are used for the treatment of intracranial aneurysms and require long-term antiplatelet therapy, which has been associated with complications such as intracranial hemorrhage. We encountered a case in which rapid increases in the size and number of cerebral microbleeds were observed shortly after flow diverter stent placement. It is known that microthrombosis within a stent can occur after flow diverter stent placement, and this microthrombosis is thought to cause cerebral microbleeds. The patient was a 77-year-old man who underwent flow diverter stent placement for an unru
