[Peripheral neuropathy in large granular lymphocytic leukemia].

A 16-year old woman with LGL leukemia developed peripheral neuropathy. She showed virus-associated hemophagocytic syndrome (VAHS)-like signs including high fever, liver dysfunction, huge splenomegaly, hepatomegaly and pancytopenia. The presence of chronic active EB virus infection was proved by marked high titers for IgG and IgA antibodies to the Epstein-Barr viral capsid and early antigens and low titers of antibody to Epstein-Barr nuclear antigens. She showed dysesthesia and paresthesia of bilateral lower extremities with marked swelling and tenderness, and later developed muscular weakness

Morphological changes in the peripheral nervous system in the case of congenital malformations of the spinal cord.

A 21-year-old oligophrenic man developed after upper respiratory tract infection, quadriplegia with sphincter and respiratory disturbances. Lumbar punction revealed subarachnoid bleeding and elevated cerebrospinal protein level. Guillain-Barré syndrome and subarachnoid hemorrhage were diagnosed. At autopsy intraspinal angioma (C2-D6) and diastematomyelia (D11-lumbar segments) were found. Beside, intraspinal hemorrhage was present. Morphological examination of posterior and anterior spinal roots as well as peripheral nerves was done. Spheroids, axonal degeneration and prominent loss of myelinat

Electrodiagnostic medicine.

Electrodiagnostic testing examines the physiologic integrity of the peripheral nervous system. However, such testing should represent only one part of an electrodiagnostic consultation in which the entire clinical context, including the history, physical examination, laboratory studies, and electrodiagnostic testing, is considered as a whole. Although each electrodiagnostic laboratory establishes its own normal values for nerve conduction studies and needle EMG, these values should not be used in isolation. The electrodiagnostic consultation can help narrow an otherwise broad differential diag

Epitope map of neurofilament protein domains in cortical and peripheral nervous system Lewy bodies.

A subset of demented elderly patients exhibit large numbers of cortical intraneuronal inclusions similar to the neurofilament (NF)-rich Lewy bodies (LB) found in pigmented subcortical neurons of patients with Parkinson's disease (PD). Because these cortical inclusions may contribute to the emergence of cognitive impairments in afflicted individuals, the authors mapped the distribution of NF epitopes in these so-called cortical LBs. This was done using ethanol-fixed tissues and a large library of monoclonal antibodies (MAbs) with well-characterized binding specificities to various regions of ea

Large and small fibre type sensory dysfunction in patients with cancer.

Quantitative sensory testing was used to assess the prevalence of sensory dysfunction in patients with cancer, carefully screened for other risk factors for neuropathy. Large fibre type sensory function was evaluated using vibration threshold (VT) determinations while small fibre type sensory function was assessed by thermal threshold (TT) determinations. Mean VT and TT were significantly elevated in the toes but not the fingers of cancer patients. VT elevations in the toes occurred in 31% of cancer patients and in 6% of control subjects. TT elevations in the toes occurred in 43% of cancer pat

Somatosensory evoked potentials of the medial and lateral plantar and calcaneal nerves.

The ideal electrodiagnostic procedure to assess possible plantar neuropathies continues to elude investigators. Motor studies are rarely abnormal, pure sensory studies may be difficult to obtain, needle electromyography can demonstrate membrane instability in normal feet. Mixed nerve plantar studies may be more diagnostically valuable than the other techniques but they also have shortcomings. In this report, a technique utilizing somatosensory evoked potentials to assess the medial and lateral plantar and calcaneal nerves is demonstrated. Normative data with respect to latencies, amplitudes, a

Axonal degeneration of the peripheral nerves and postganglionic anhidrosis in a patient with multiple sclerosis.

A 36-year-old woman had, since the age of 24, numerous episodes of visual loss and spinal symptoms and signs at various levels, and was diagnosed as multiple sclerosis(MS). CSF myelin-basic-protein was increased. Neurological and electrophysiological investigations suggested the peripheral nerve involvement. Sural nerve biopsy performed about six years after the onset, revealed severe loss of both myelinated and unmyelinated fibers. Subsequently, histamine skin reaction was defective in the lower limbs. Tests on sudomotor and pupillary functions indicated deficits of both central and postgangl

Use of the pudendo-anal reflex in the treatment of neurogenic faecal incontinence.

An electrical stimulator has been devised to treat neurogenic faecal incontinence caused by pudendal nerve neuropathy and works on the basis of repeated stimulation of the pudendo-anal reflex arc. Although conduction in the pudendo-anal reflex arc may be prolonged, and is so in neurogenic faecal incontinence, it must be shown to be present before the method can be used. This stimulation results in an immediate rise in the pressure in the anal canal and a significant increase in the electromyographic activity of the external anal sphincter. Maintenance of the stimulus over a two month period ra

Outcome and treatment of electrical injury with immediate median and ulnar nerve palsy at the wrist: a retrospective review and a survey of members of the American Burn Association.

Electrical injury to the upper extremity with immediate median and ulnar nerve palsy at the wrist is uncommon but devastating. When it does occur, the immediate clinical questions are (1) will the nerves recover, and (2) should the carpal tunnel and Guyon's canal be released? Our review of the literature did not answer these two questions. Therefore, we reviewed our experience with such patients and surveyed approximately 10% of the physician members of the American Burn Association. We reviewed approximately 80 patients with electrical injuries treated between January 1983 and September 1988,

Peripheral neuropathy profile in various groups of diabetics.

The results of applying a battery of peripheral nerve function tests in three groups of diabetics are reported. The tests examined the integrity of all the major subgroups of nerve fibres. The diabetics were grouped according to the clinical severity of their neuropathy: Group I--11 patients with long standing diabetes but clinically insignificant neuropathy, Group II--27 patients with mild neuropathy and Group III--23 patients with neuropathic foot lesions. As expected the incidence of abnormality of all functions increased from Group I to III, but within a group there was no clear pattern of

Pathological changes in peripheral nerves in experimental fetal alcohol syndrome.

The peripheral nerve pathology is described in experimental fetal alcohol syndrome. The marked retraction and shrinkage of axons with vacuolar spaces in the periaxonal area are the striking features. The direct toxicity of ethanol or its metabolites on the nerve fibers is considered to be the cause for pathological changes. Additional biochemical factors, pyridoxal phosphate dependent enzyme-depletion in the genesis of nerve damage, is proposed.

[Sensory nerve involvement in X-linked bulbospinal amyotrophy (Kennedy syndrome). Contributions of electrophysiologic and histologic data].

Two new cases of X-chromosome linked bulbospinal muscular atrophy associated with gynecomastia (described by Kennedy in 1968) are reported. In one patient, an electrophysiological study and a muscle nerve biopsy were performed. Motor nerve conduction studies were normal but sensory action potentials were small or unrecordable in the absence of clinical sensory loss. Superficial peroneal nerve biopsy showed axonal lesions. These findings suggest that this disease is not purely due to degeneration of spinal motor neurons and is also associated with an axonopathy.

[Evaluation of digital vibration threshold for asymptomatic ulnar neuropathy in patients with carpal tunnel syndrome].

Quantitative assessment of vibration threshold at 125 Hz of index and little finger tips together with measurements of median and ulnar sensory nerve conduction velocity (SNCV) was performed for 25 limbs of 15 patients with carpal tunnel syndrome (CTS), 25 limbs of 13 disease controls with neurological symptoms similar to CTS and the nearly same number of limbs of normal controls. There was no significant difference in ulnar SNCV among 3 groups. However, ulnar SNCV at wrist was significantly reduced in patients with CTS as compared with normal value. In 17 limbs with CTS, vibration threshold o

Swelling of the cauda equina in patients who have herniation of a lumbar disc. A possible pathogenesis of sciatica.

Morphological changes in the cauda equina in patients who have herniation of a lumbar disc have not been previously reported, to our knowledge. Using computed tomographic myelography, we found swelling of nerve-root elements in the cauda equina that corresponded to the affected root in seventeen of twenty-eight patients who had proved herniation of a lumbar disc. All seventeen patients had severe sciatica. The swelling of the nerve-root elements gradually returned to normal after surgical decompression, which was also associated with improvement of the sciatica. Patients who did not have sever

Peripheral neuropathy associated with acquired immunodeficiency syndrome. Prevalence and clinical features from a population-based survey.

We prospectively studied 40 hospitalized patients who had well-established diagnoses of acquired immunodeficiency syndrome. Patients with confounding risk factors for neuropathy were excluded; none of the study patients had known vitamin deficiency, alcoholism, or any metabolic, drug, or toxic factor. Clinical and electrophysiologic evidence of a distal symmetric polyneuropathy was found in 35% (13/37) of the patients. Symptoms and signs of neuropathy were usually mild, and painful dysesthesias were uncommon. Amplitude reduction of sural nerve action potentials distinguished all patients with

Subclinical brachial plexopathy following median sternotomy.

In order to detect subclinical cases of brachial plexus injury following open heart surgery through median sternotomy, 15 patients were included in this prospective study. Physical and electrophysiologic examinations were performed preoperatively and postoperatively. The axillary F-loop latency was used to diagnose the brachial plexopathy. No clinical symptoms were found in this group postoperatively. Of the 15 patients, 13 (87%) were found to have a postoperative subclinical brachial plexopathy, at least on one side, and in eight of the patients, it was present on both sides.

A controlled trial of continuous lumbar traction in the treatment of back pain and sciatica.

A controlled trial of continuous lumbar traction in the treatment of back pain and sciatica showed similar improvements in both the treated group (weighted traction) and the control group (simulated traction). The findings of this study question the justification of admitting patients with back pain into hospitals for purposes of traction alone.

Polyclonal plasma cell proliferation with systemic capillary hemangiomatosis, endocrine disturbance, and peripheral neuropathy.

An autopsy case of so-called plasma cell dyscrasia (PCD) of a 37-year-old woman was reported. This case showed typical clinical symptoms of PCD, except for no abnormality in serum immunoglobulins. Besides the typical histological findings often observed with PCD, the autopsy revealed an additional finding of systemic angiomatous proliferation of small blood vessels throughout the body, especially in the skin, kidneys, brain, various lymph nodes, and soft tissue. A peculiar glomerular lesion in this case, similar to membranoproliferative glomerulonephritis, was likely to be a result of prolifer
