Benefits and Challenges of Telemedicine Utilization in PD Management in Low-Resource Settings.

BACKGROUND: Peritoneal dialysis (PD) is a cost-effective and home-based kidney replacement therapy, particularly relevant in low- and middle-income countries (LMICs). However, outcomes remain suboptimal due to limited access to specialized care, delayed complication detection, and workforce constraints. Telemedicine has emerged as a potential strategy to address these challenges. SUMMARY: This integrative narrative review aimed to synthesize current evidence on telemedicine interventions in PD management, focusing on clinical effectiveness, implementation strategies, and applicability in reso

Exercise attitudes and practices among adults listed for kidney transplantation: a survey of a diverse patient cohort.

BACKGROUND: Given the increasing age and frailty of kidney transplant candidates, there is an emerging drive to optimise patients before transplantation. Lack of exercise has been linked with poor outcomes at all stages of the transplant pathway. The aim of this study was to evaluate the attitudes and perception to exercise in such patients and assess how these practises vary by demographics. METHODS: A single-centre, prospective, survey-based study was conducted on consecutive adult patients being assessed for activation on the deceased-donor kidney transplant waiting list. RESULTS: A total

Addressing Renal Sodium Avidity in Chronic Heart Failure: There Is Always More than One Way to Skin a Cat.

BACKGROUND: Renal sodium avidity is a defining characteristic of chronic cardiorenal syndrome (CRS), leading to persistent sodium retention despite significant fluid overload. This phenomenon exacerbates volume overload, contributes to hemodynamic instability, and accelerates the decline of cardiac and renal function. Conventional diuretic therapies, though effective in acute settings, often fail chronically due to neurohormonal activation, renal tubular adaptations, and diuretic resistance. SUMMARY: This review explores innovative approaches to overcoming natriuresis and diuresis resistance

Economic cost of end-stage renal disease in Africa: a systematic review.

BACKGROUND: Chronic kidney disease (CKD) is a progressive and irreversible condition that contributes substantially to global morbidity and mortality. In Africa, the burden of CKD is compounded by limited healthcare resources, poverty, and the dual challenge of communicable and non-communicable diseases. The economic impact of end-stage renal disease (ESRD) is considerable, yet evidence from the region remains scarce. METHODS: This systematic review, conducted in accordance with PRISMA 2020 guidelines, examined the economic burden of ESRD in Africa. Data were extracted on study characteristic

Diagnostic and management challenges of a case of N-acetylglutamate synthase deficiency in a resource-limited healthcare setting in Tanzania: a case report.

BACKGROUND: Inborn errors of metabolism represent a significant cause of childhood morbidity and mortality. These conditions are frequently missed in low-resource settings due to their anticipated rarity and similarity of symptoms to conditions such as sepsis. We present a case of a neonate with N-acetylglutamate synthase deficiency whose diagnosis and management at our facility were complicated by limited healthcare resources. CASE REPORT: A three-day-old male of South Asian origin born to consanguineous parents presented with lethargy, hypothermia and respiratory distress. He was initially

Challenges of Vaccine-Induced Thrombotic Thrombocytopenia-Related Kidney Transplantation: Venous Graft Thrombosis in a Pediatric Recipient.

During the COVID-19 pandemic, adenoviral vaccines drew attention owing to a potential life-threatening coagulation disorder, the vaccine-induced immune thrombotic thrombocytopenia (VITT). Patients deceased of VITT have been accepted as organ donors despite safety concerns regarding the transmission of VITT to recipients. The outcome of adult kidney graft recipients was reported favorable in most cases; however, (thrombotic) complications were observed more frequently. We present 2 pediatric recipients of kidney grafts from VITT-deceased patients. Both donors (38 and 34 years old, male, body ma

The association of hepcidin, reticulocyte hemoglobin equivalent and anemia-related indicators on anemia in chronic kidney disease.

Hepcidin is an essential regulator of iron homeostasis in chronic kidney disease (CKD) anemia, reticulocyte hemoglobin equivalent (RET-He) can be used to evaluate the availability of iron for erythropoiesis. Previous research has found that hepcidin indirectly regulates RET-He. This study aimed to investigate the association of hepcidin, RET-He and anemia-related indicators on anemia in chronic kidney disease. A total of 230 individuals were recruited, including 40 CKD3-4 patients, 70 CKD5 patients without renal replacement therapy, 50 peritoneal dialysis patients, and 70 hemodialysis patients

Refractory ascites induced by mycophenolate in a pediatric kidney transplant patient.

Common side effects of mycophenolate mofetil (MMF) are diarrhea, leukopenia, and infectious complication. The polymorphisms of enzymes affecting MMF clearance could be related to MMF toxicity, and in vitro study revealed that high MMF levels might cause endothelial dysfunction. A 7-year-old Korean male with end-stage renal disease on peritoneal dialysis due to mesangial proliferative glomerulonephritis received a kidney transplantation (KT) from a deceased donor, and immunosuppressive medications including MMF, tacrolimus, and methylprednisolone were started after KT. The patient developed oli

False positive finding of F18-FDG in non-Hodgkins lymphoma; an inflamed appendix.

BACKGROUND: Fluid overload is a common problem in peritoneal dialysis (PD) patients. Cardiothoracic ratio (CTR) and vascular pedicle width (VPW) in routine chest radiograph are useful indicators of intravascular volume status and may represent important prognostic factors of PD patients. METHODS: We measured VPW and CTR in 286 unselected prevalent PD patients. VPW was further adjusted for the thoracic diameter (VPWR). One-year actuarial survival, technique survival, and duration of hospitalization were analyzed. RESULTS: The mean values of VPW, CTR, VPWR were 47.31 ± 4.73 mm, 0.542 ± 0.074,

End-stage renal disease in Japanese children: a nationwide survey during 2006-2011.

BACKGROUND: End-stage renal disease (ESRD) in children is considered a rare, but serious condition. Epidemiological and demographic information on pediatric ESRD patients around the world is important to better understand this disease and to improve patient care. The Japanese Society for Pediatric Nephrology (JSPN) reported epidemiological and demographic data in 1998. Since then, however, there has been no nationwide survey on Japanese children with ESRD. METHODS: The JSPN conducted a cross-sectional nationwide survey in 2012 to update information on the incidence, primary renal disease, ini
