Affective instability as rapid cycling: theoretical and clinical implications for borderline personality and bipolar spectrum disorders.

OBJECTIVES: The Diagnostic and Statistical Manual of Mental Disorders guidelines provide only a partial solution to the nosology and treatment of bipolar disorder in that disorders with common symptoms and biological correlates may be categorized separately because of superficial differences related to behavior, life history, and temperament. The relationship is explored between extremely rapid switching forms of bipolar disorder, in which manic and depressive symptoms are either mixed or switch rapidly, and forms of borderline personality disorder in which affective lability is a prominent sy

Latent structure of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition personality disorder criteria.

AIMS: This study investigated the internal construct validity of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) personality disorders and explored alternative models to characterize the personality disorder traits. The relationship between the obtained personality disorder dimensions and measures of functioning and disability was examined. METHODS: The subjects in the study were 742 community-residing individuals who participated in the Hopkins Epidemiology of Personality Disorders Study. The presence of DSM-IV personality disorder traits was assessed by ps

Modulation of clinical expression of plaque-induced gingivitis: effects of personality traits, social support and stress.

BACKGROUND: Studies have shown an association between an acute stressful event and gingivitis. However, the possible effects of personality traits associated with stress resistance/susceptibility and current level of stress on the clinical expression of plaque-induced inflammation remain to be examined. The aim of this study was to characterize the subject-based clinical behaviour of the gingiva during experimental gingivitis in relation to personality profile, psychological stress and coping behaviour. METHODS: Ninety-six systemically and periodontally healthy subjects (mean age: 23.6+/-1.7

Impact of trait impulsivity and state aggression on divalproex versus placebo response in borderline personality disorder.

OBJECTIVE: The authors' goal was to determine whether specific pretreatment clinical characteristics differentially predict favorable treatment response to divalproex versus placebo for impulsive aggression in patients with borderline personality disorder. METHOD: Fifty-two outpatients with DSM-IV borderline personality disorder were randomly assigned to receive divalproex (N=20) or placebo (N=32), double-blind, for 12 weeks. Trait impulsivity symptoms were determined by using the Barratt Impulsiveness Scale, and state aggression symptoms were determined by using the Overt Aggression Scale mo

Personality disorder and social anxiety predict delayed response in drug and behavioral treatment of panic disorder.

BACKGROUND: The aim of this study was to analyze the impact of pretreatment characteristics and personality disorders on the onset of response in the treatment of panic disorder. METHODS: The data of 73 out-patients with panic disorder who had completed at least 6 weeks of a randomized trial of 24 weeks of either paroxetine only or paroxetine combined with cognitive group-therapy were analyzed in a Cox proportional hazards model. RESULTS: The likelihood of having responded to treatment (defined by a CGI rating of improvement) was more than twice as high for patients without a personality dis

Interrater reliability of the structured interview for DSM-IV personality in an opioid-dependent patient sample.

We examined the interrater reliability of the Structured Interview for DSM-IV Personality (SIDP-IV) in an opioid-dependent patient sample at the criterion as well as at the diagnostic level for both categorical and dimensional data. At the criterion level (Cohen's kappa ranging from 0.76 to 0.93 and intraclass correlation coefficient ranging from 0.67 to 0.97) as well as at the diagnostic level (Cohen's kappa ranging from 0.66 to 1.00 and intraclass correlation coefficient ranging from 0.88 to 0.99), the reliability was excellent. The results suggest the SIDP-IV to be an adequate instrument fo

Association between brain-derived neurotrophic factor 196 G/A polymorphism and personality traits in healthy subjects.

Several lines of evidence suggest that a certain type of personality or temperament is at risk for developing neuropsychiatric diseases, and that brain-derived neurotrophic factor (BDNF) might be involved in pathophysiology of neuropsychiatric diseases such as mood disorders. Considering the role of BDNF and personality traits in the neuropsychiatric diseases, it is of interest to examine the association between BDNF gene polymorphism and personality test scores. In this study, we examined the association between 196 G/A the polymorphism in the coding region of the BDNF gene and personality tr

Predicting borderline and antisocial personality disorder features in nonclinical subjects using measures of impulsivity and aggressiveness.

This study examines impulsivity and aggressiveness dimensions as predictors of borderline (BPD) and antisocial (ASPD) personality disorder symptoms in nonclinical subjects. A total of 747 undergraduate university students were administered the Personality Diagnostic Questionnaire-4+, the Barratt Impulsiveness Scale-11, and the Buss-Durkee Hostility Inventory. Hierarchical regression analysis showed that motor impulsiveness, irritability, resentment, and guilt predicted BPD symptoms among university students after controlling for the effect of ASPD and depressive symptoms. ASPD symptoms were pr

A failure to replicate alcohol-induced laboratory aggression among college men without evidence of personality disturbance.

The effect of acute alcohol intoxication on laboratory-induced aggression among men has been fairly well established. The present study hypothesized that alcohol effects on Point Subtraction Aggression Paradigm (PSAP) responding would not be replicated among "low-risk' college men distinguished by their absence of personality disorder features. Participants were assigned to either Alcohol (n=18), Placebo (n=7), or Time (n=8) comparison groups with each completing 25-min. sessions during the baseline, ascent, peak (70 mg%), and descent (40 mg%) phases of absorption and elimination process. Dissociative symptoms in schizophrenia: a comparative analysis of patients with borderline personality disorder and healthy controls.

The aim of this study was to investigate the occurrence of dissociative symptoms in patients with a schizophrenic disorder. The pattern of dissociative experiences was examined in a group of patients with a diagnosis of the schizophrenia spectrum disorder (n = 26; mean age 27.9 years), and a group of patients with a diagnosis of borderline personality disorder (n = 26; mean age 24.0 years) was compared with normal controls of the general population (n = 1,056; mean age 18.7 years). The degree of self-reported dissociative symptoms was measured using the German version of the Dissociative Exper

[Flight into the imaginary world and dependence: personality trait or opportunist defence mechanism in the transsexual].

While analysing the psychological functioning of sex reassignment candidates, it appeared that they frequently use the mechanism which consist in taking refuge in passivity and fantasies so that they can avoid displeasing situations, responsibilities and decisions making. This unconscious process, identified by Exner in the Rorschach as the Snow-White Syndrome seems to take place in a specific way regarding the real situation faced by the sex reassignment candidates: the achievement of their aim (the sex reassignment) actually depends on the medical crew (doctors, psychologists, surgeons). Personality disorders, coping strategies, and posttraumatic stress disorder in women with histories of childhood sexual abuse.

Using a treatment-seeking sample of adult female survivors of childhood sexual abuse, the relationships between coping strategies, personality disorders (PD) and Posttraumatic Stress Disorder (PTSD) were explored. A variety of PDs were found to exist in this population, with avoidant, antisocial, dependent PDs having higher frequencies than borderline PD. Avoidant coping and PTSD severity significantly correlated with many of these PDs. PTSD severity and avoidant coping were also significantly correlated. Additionally, women with PTSD displayed higher rates of avoidant and dependent PDs, as we

Early-onset alcoholism with conduct disorder: go/no go learning deficits, working memory capacity, and personality.

BACKGROUND: Two studies were conducted to investigate the disinhibitory mechanisms that (1) discriminate early-onset alcoholism (EOA) with conduct disorder (CD; antisocial EOA) from a non-antisocial subtype of EOA and (2) are associated with novelty-seeking and low harm avoidance. METHODS: Young adults with antisocial EOA (n = 96), with non-antisocial EOA (without CD; n = 80), with CD alone (n = 50), and controls (n = 125) were given two go/no go tasks (one with monetary loss and the other with shock punishment), the Digit Span test (working memory capacity), and personality measures of harm

Are personality traits familial risk factors for substance use disorders? Results of a controlled family study.

OBJECTIVE: The well-documented association between maladaptive personality traits and substance use disorders has given rise to diverse explanatory models. In this investigation the authors examined one such explanation, that certain personality traits are familial risk factors for the development of substance abuse or dependence. METHOD: Data were collected from a controlled family study using direct diagnostic interviews. The Multidimensional Personality Questionnaire was used to assess the personality traits of 325 probands, 205 of whom had diagnoses of substance abuse or dependence, and 2

Self-harm behaviors across the life cycle: a pilot study of inpatients with borderline personality disorder.

The purpose of this study was to explore, throughout the life cycle, the prevalence of self-harm behaviors among psychiatric inpatients with and without borderline personality disorder (BPD). Psychiatric inpatients with BPD (n = 43) were compared to those without BPD (n = 40) with regard to self-reported self-harm behaviors during their lifetime. The mean number of self-harm behaviors, including high-lethal behaviors (i.e., suicide attempts, cutting oneself, overdosing), among those with BPD dramatically increased between the ages of 18 and 24 years and was sustained through ages 50 to 59 year

Comorbidity of dependent personality disorder and separation anxiety disorder in addictive disorders and in healthy subjects.

The aim of the study was to determine the comorbidity of dependent personality disorder (DPD) and separation anxiety disorder (SAD) in large nonclinical and clinical samples. Using a cross-sectional method, the relationship between these disorders was studied in 784 healthy subjects and in 708 subjects presenting with various addictive behaviors. DPD was significantly associated with SAD in healthy subjects, alcoholics, drug abusers and anorectic and bulimic patients. The comorbidities of SAD and DPD suggest that specific therapeutic strategies could be discussed. Moreover, longitudinal studie

Psychopathology and personality characteristics in relation to blood serotonin in Tourette's syndrome and obsessive-compulsive disorder.

Family studies suggest an interrelationship between Gilles de la Tourette Syndrome (GTS) and some forms of obsessive-compulsive disorder (OCD). Some authors consider GTS to be part of a serotonergically mediated cluster of OCD spectrum disorders. The present study was undertaken to compare measures of psychopathology, personality and blood serotonin between GTS and OCD (without tics), and to investigate whether an OCD spectrum hypothesis is supported for GTS. Fifteen GTS without OCD subjects, 21 tic with (+) OCD subjects, 15 OCD without tic subjects and 26 controls (all without serotonergic me
