Panic disorder - acceptance of the diagnostic entity and treatment modalities preferred by German health professionals.

A survey of the pharmacological and psychological treatment of panic disorder and agoraphobia (PDA) was conducted among 103 physicians, psychologists and psychotherapists. It revealed that the treatments for PDA preferred by the majority of the respondents are inconsistent with the recommendations given in the international literature. Non-psychiatric physicians most frequently proposed herbal preparations as a possible treatment (46% of non-psychiatric physicians), followed by homoeopathic formulations (32%). Tricyclic antidepressants which are recommended as first-line treatment in panic dis

Efficacy of large language models in detecting postoperative delirium from unstructured clinical notes: A retrospective cohort study.

Early identification of postoperative delirium (POD) remains challenging. This retrospective observational study compared the performance of large language models (LLMs), Llama-3-70B and GPT-4o, and physicians in predicting clinically significant POD, defined as either requiring antipsychotics or diagnosis of delirium by neurologists following consultation for delirium-related symptoms. The c-statistics of Llama-3-70B and GPT-4o were 0.74 and 0.76, respectively. LLMs showed higher sensitivity (Llama-3-70B, 0.900; GPT-4o, 0.868; physicians, 0.723) and lower specificity (0.463, 0.547, and 0.814,

A preliminary investigation of the effects of a provider network on costs and lost-time in workers' compensation.

OBJECTIVE: We sought to compare lost-time days and average and median workers' compensation claims costs between injured workers managed by OMNET Gold (OG) physicians and those managed by physicians not participating in OG. OG is a statewide health care provider network coordinated by occupational medicine physicians and established by the Louisiana Workers' Compensation Corporation (LWCC) to manage the medical care of injured workers. METHODS: We identified and contrasted 158 lost-time claims managed by OG physicians and 1,323 claims managed by physicians not participating in OG during the f

Continuing medical education for AIDS: an organizational response.

The Management of the Southern California Kaiser-Permanente Medical Group mandated a day-long program of continuing medical education on AIDS for all staff physicians at one facility. Presentations dealt with cognitive (transmission/diagnosis), skills (risk factors, history taking), and affective (discomfort/attitudes) domains. All physicians participated in 2 small-group workshops on sexual history taking and ethical issues. An evaluation compared primary care physicians' attitudes and practices before and after the program at the experimental site (Medical Center), a comparison site (written

Barriers to exercise prescription among Australian general practitioners and endocrinologists: a cross-sectional survey.

BACKGROUND: Physical inactivity is a leading preventable cause of chronic disease in Australia. Exercise prescription by physicians increases patients' physical activity, but implementation is inconsistent, and barriers among Australian physicians are not well characterised. AIMS: To investigate barriers and facilitators to exercise prescription among Australian general practitioners (GPs) and endocrinologists and to identify physician characteristics associated with prescription frequency. METHODS: Cross-sectional survey of 118 Australian GPs (n = 59) and endocrinologists (n = 59) recruited

Assessment of practices in screening and treating women with bacteriuria.

INTRODUCTION: Evidence-based screening and treatment for bacteriuria is crucial to prevent increasing antibiotic resistance. The Infectious Disease Society of America (IDSA) previously released guidelines on the management of asymptomatic bacteriuria (ASB) and uncomplicated urinary tract infections (UTIs) in women. The study's objective was to assess physicians' practices in managing women with bacteriuria relative to these guideline recommendations. MATERIALS AND METHODS: Cross-sectional data from physicians were collected using an anonymous questionnaire. Multivariable logistic regression a

Management of obesity in primary care.

Obesity is one of the most common presenting chronic medical conditions in primary care, yet it is not adequately treated. Physicians are often reluctant to counsel patients because of their limited training in treating chronic weight problems and negative attitudes toward obese patients. This study evaluated the feasibility of training physicians to provide weight control counseling to their patients. Eleven physicians were randomly assigned to either an obesity-counseling skills training group or to a control group. Physicians in the counseling skills group received training in behavioral an

Use of injectable progestin (medroxyprogesterone acetate) in adolescent health care.

To assess medroxyprogesterone acetate (DMPA) prescription practices in adolescent health care practices, an anonymous questionnaire was distributed to 160 American and Canadian physicians attending the 1991 meetings of either the Society for Adolescent Medicine or the North American Society for Pediatric and Adolescent Gynecology. Of 54 (33.7%) physicians who responded, 33 (61.1%) reported prior prescription of an injectable progestin (DMPA) as a form of birth control. Of those, 15 (45.5%) had prescribed it to 10 or more adolescents. Twelve (57%) pediatricians but only 3 (25%) gynecologists re

Guiding individual decisions: a randomized, controlled trial of decision analysis.

In early 1983, all 1,280 faculty and resident physicians at one hospital who were eligible to be vaccinated against hepatitis B were divided randomly into three groups: Group 1 physicians received general information about the risks and benefits of alternative vaccine decisions; Group 2 physicians were additionally invited to provide personal information for an individualized decision analysis (12.6 percent responded); and Group 3 physicians, who served as controls, were not contacted. In one year's follow-up, 20 percent of physicians were screened for hepatitis B antibody or vaccinated. More

Effectiveness evaluation of standardized patient training system on emergency department residents in disposing patients with acute chest pain.

BACKGROUND: Rapid triage and effective treatment of patients with acute chest pain are critical. Enhancing the clinical competency of emergency resident physicians in managing these patients has emerged as a key challenge in contemporary clinical education. METHODS: Emergency department resident physicians were divided into two groups in 2022-2023. After receiving either traditional lecture-based training or standardized patient (SP) training, we assessed whether there was an improvement in the physicians' clinical competency in managing chest pain patients, and whether the time-to-treatment

Opioid maintenance therapy in Switzerland: an overview of the Swiss IMPROVE study.

BACKGROUND/AIMS: Switzerland's drug policy model has always been unique and progressive, but there is a need to reassess this system in a rapidly changing world. The IMPROVE study was conducted to gain understanding of the attitudes and beliefs towards opioid maintenance therapy (OMT) in Switzerland with regards to quality and access to treatment. To obtain a "real-world" view on OMT, the study approached its goals from two different angles: from the perspectives of the OMT patients and of the physicians who treat patients with maintenance therapy. The IMPROVE study collected a large body of d

Managing quality in hospital practice.

BACKGROUND: While routine clinical decision-making has a substantial effect on quality, most practising physicians do not routinely examine their outcomes. OBJECTIVES: To set up a practical process for identifying problems in hospital practices of primary care physicians, examine their causes, and develop a quality improvement process that intimately involves practising physicians in problem-solving. DESIGN: All hospital admissions to the Primary Care Service were screened over a 14-month period using simple pre-specified criteria. Quality problems were verified by medical record reviews car

How Following Medical Artificial Intelligence Advice Can Mitigate Malpractice Liability: Cross-National Insights from a Randomized Trial.

Artificial intelligence (AI) increasingly influences clinical decision-making, yet its recommendations may diverge from standard care. Although malpractice concerns are thought to discourage physicians from following AI advice, experimental evidence from the United States suggests the opposite: lay jurors are more likely to hold physicians liable when they reject AI recommendations. Whether this pattern extends to systems in which court-appointed experts, not lay jurors, determine liability remains unknown. Methods: To examine how physicians and laypeople in expert-based and lay-juror legal sy

Women in pain medicine.

In the UK more women than men are practicing medicine, and for the first time in the history of the Royal College of Anaesthetists (RCoA), the president of the RCoA, Dean of the Faculty of Pain Medicine, and Dean of the Faculty of Intensive Care Medicine are all women. However, within the subspecialty of pain medicine, there are significantly more men practicing than women, with the most recent UK estimates identifying that only 26.7% of current pain physicians are women. Both historical and modern perspectives illustrate how women often prefer to be cared for by other women, highlighting the

Who can help me? Understanding the antecedent and consequence of medical information seeking behavior in the era of bigdata.

INTRODUCTION: The advent of bigdata era fundamentally transformed the nature of medical information seeking and the traditional binary medical relationship. Weaving stress coping theory and information processing theory, we developed an integrative perspective on information seeking behavior and explored the antecedent and consequence of such behavior. METHODS: Data were collected from 573 women suffering from infertility who was seeking assisted reproductive technology treatment in China. We used AMOS 22.0 and the PROCESS macro in SPSS 25.0 software to test our model. RESULTS: Our findings
