Hypertension in blacks: clinical overview.

Although the decline in stroke and other cardiovascular morbid and mortal events has been occurring since the 1940s, the steeper decline since 1968 has been attributed to improved hypertension awareness, treatment, and control. However, in spite of this encouraging trend from the population in general, surveys from the 1970s and our more recent survey from the Maryland Hypertension Program indicate that hypertension control among blacks remains unacceptably poor, particularly in view of the high prevalence. Of special concern are black men, who have the highest prevalence of any group and the

Engagement and Intersectionality in Digital Self-Management Interventions for Asthma and Chronic Obstructive Pulmonary Disease: Scoping Review.

BACKGROUND: Asthma and COPD are long-term respiratory conditions that require active self-management to improve quality of life and reduce health care burdens. Digital health interventions (DHIs) are increasingly used to support behavior change, symptom monitoring, and medication adherence, offering new opportunities for personalized care and real-time feedback. Understanding patient engagement with digital tools is essential for optimizing intervention design, improving clinical outcomes, and addressing potential inequalities in access and effectiveness. This review is informed by a novel con

Eye Care Quality and Accessibility Improvement in the Community (EQUALITY) for adults at risk for glaucoma: study rationale and design.

BACKGROUND: Primary open angle glaucoma is a chronic, progressive eye disease that is the leading cause of blindness among African Americans. Glaucoma progresses more rapidly and appears about 10 years earlier in African Americans as compared to whites. African Americans are also less likely to receive comprehensive eye care when glaucoma could be detected before irreversible blindness. Screening and follow-up protocols for managing glaucoma recommended by eye-care professional organizations are often not followed by primary eye-care providers, both ophthalmologists and optometrists. There is

Expert opinion on the optimal use of sevelamer in clinical practice in Southeast Asia.

Chronic kidney disease (CKD)-mineral bone disorder (MBD) is a prevalent and serious complication in patients with advanced CKD, particularly in Southeast Asia where the disease burden is high. This condition significantly increases the risk of cardiovascular events and mortality. This review synthesizes mechanistic insights, clinical trial evidence, guideline recommendations, and expert perspectives to evaluate the role of sevelamer, a non-calcium-based phosphate binder, in the management of CKD-MBD. Regional epidemiological and clinical practice data from Southeast Asia were also examined. Se

Iodinated contrast media (ICM)-induced thyroid dysfunction: a review of potential mechanisms and clinical management.

Iodinated contrast media (ICM) are extensively utilized in medical imaging to enhance tissue contrast, yet their impact on thyroid function has attracted increasing attention in recent years. ICM can induce thyroid dysfunction, with reported prevalence ranging from 1 to 15% and a higher incidence observed in individuals with pre-existing thyroid conditions or other risk factors like age, gender, underlying health issues, and repeated ICM exposure. This review summarized the classification of ICM and the potential mechanisms, risk assessment, and clinical management of ICM-induced thyroid dysfu

The impact of gut microbiota on the occurrence, treatment, and prognosis of ischemic stroke.

Ischemic stroke (IS) is a cerebrovascular disease that predominantly affects middle-aged and elderly populations, exhibiting high mortality and disability rates. At present, the incidence of IS is increasing annually, with a notable trend towards younger affected individuals. Recent discoveries concerning the "gut-brain axis" have established a connection between the gut and the brain. Numerous studies have revealed that intestinal microbes play a crucial role in the onset, progression, and outcomes of IS. They are involved in the entire pathophysiological process of IS through mechanisms such

Clinical and Disease Burden of Patients with Generalized Pustular Psoriasis: A Review of Real-World Evidence.

Generalized pustular psoriasis (GPP) is a chronic, rare, and potentially life-threatening disease. There is limited understanding of patient characteristics in GPP and their correlation with disease progression or healthcare resource utilization. Our review aims to examine real-world evidence on these characteristics and the associated disease burden as related to economic and quality of life factors. Results showed that most patients with GPP experienced flares once a year, lasting from 2 weeks to 3 months, with > 80% of patients having residual disease post-flare, with/without treatment, ind

Review: Mitigating the risks posed by intensification in livestock production: the examples of antimicrobial resistance and zoonoses.

Major shifts in how animals are bred, raised and slaughtered are involved in the intensification of livestock systems. Globally, these changes have produced major increases in access to protein-rich foods with high levels of micronutrients. Yet the intensification of livestock systems generates numerous externalities including environmental degradation, zoonotic disease transmission and the emergence of antimicrobial resistance (AMR) genes. Where the process of intensification is most advanced, the expertise, institutions and regulations required to manage these externalities have developed ov

Psychotic and Bipolar Disorders: Antipsychotic Drugs.

Antipsychotic drugs block dopamine receptors and are used to manage psychosis as well as other mental illnesses that may or may not have psychotic features, such as bipolar disorders and major depressive disorder. First-generation antipsychotic drugs are more likely to cause adverse effects such as extrapyramidal symptoms and tardive dyskinesia. Adverse effects of second-generation antipsychotic drugs typically are related to metabolic abnormalities such as weight gain, abnormal blood glucose levels, and elevated lipid levels. Neuroleptic malignant syndrome is a rare but serious adverse effect

Postoperative cognitive dysfunction, Alzheimer's disease, and anesthesia.

Although aging itself is not a disease, there are many comorbidities that become more common with aging. Heart disease, cancer, and other chronic illnesses are either more common or more severe in aging patients. Approximately 5.5 million people in the United States have Alzheimer's disease (AD), with the principal risk factor being age. It is estimated that the incidence of AD diagnosis doubles every 5 years after the age of 65. Therefore, as the population ages, the impact of AD on the healthcare landscape will increase. Understanding how to manage patients with AD is critical as we begin to

The case for routine HIV screening and impact of managed care.

An estimated 230,000 people in the United States are unaware that they are infected with the human immunodeficiency virus (HIV). Targeted testing strategies are likely to miss a significant proportion of the HIV-infected population, and routine screening could help identify individuals who are likely to experience complications or transmit the virus. Recent advances in treatment, including the introduction of highly active antiretroviral therapy, have dramatically extended the life expectancy of HIV-infected individuals and allowed many patients to have their condition managed in a manner simi

Overcoming barriers to evidence-based diabetes care.

Despite evidence-based guidelines that advocate aggressive management of hyperglycemia, hypertension, and hyperlipidemia, patients with diabetes continue to suffer from high rates of cardiovascular and microvascular complications and can expect a lifespan reduction of 10 to 15 years. Our current inability to effectively and widely translate clinical evidence into usual practice represents a major barrier to reducing the burden of diabetes and its complications. Diabetes care represents a complex interaction between patients (and their families and communities), physicians (and other providers)

Development of agroenvironmental indicators to evaluate the hygienic pressure of livestock production on human health.

Infections by enteropathogenic microorganisms linked to agroenvironmental contamination represent a significant threat to urban and rural communities. To better characterize and manage this risk, it is necessary, not only to accurately describe enteric illnesses occurring over time or across regions, but also to correctly assess exposure attributable to this environmental pollution. New agroenvironmental hygienic pressure indicators (AHPIs) were developed to synthesise relevant data expressing this exposure. They were derived from a conceptual framework for developing sustainable agriculture i

Rationale and Design of the NPAC-India Study: A Prospective Multicenter Observational Study of Pregnant Women With Heart Disease.

Cardiovascular disease (CVD) is a leading and preventable cause of maternal mortality in low and middle-income countries (LMICs), yet most management guidelines rely on data from high-income countries. Robust, nationally representative data on pregnant women with heart disease (PWHD) are limited in India, underscoring the need for locally relevant evidence to guide clinical practice and policy. The National Pregnancy and Cardiac Disease Study in India (NPAC-India) is a multiphase national initiative, and this paper describes the protocol for Phase 1, a prospective multicenter observational stu
