HIV Infection: Antepartum Treatment and Management.

Human immunodeficiency virus (HIV) is a retrovirus which became pandemic in the early 1980s. Since its initial characterization, advancements in diagnosis and management have transformed HIV infection from a terminal diagnosis to a chronic, manageable condition. Effective antiretroviral therapy, acting at multiple steps in the viral lifecycle, durably suppresses viral replication, preserves maternal health and prevents mother to child HIV transmission. Here, we review the salient clinical and ethical considerations of managing HIV infection during pregnancy and delivery.

Caesarean hysterectomy--a review of 21 cases in the University Hospital, Kuala Lumpur.

Caesarean hysterectomy is a useful surgical procedure. However, the increased blood supply to the pelvis during pregnancy, distortion of the anatomy caused by the enlarged uterus, fragility of oedematous pelvic tissues and adhesions from prior caesarean sections predispose to poor haemostasis and urinary tract injuries. In this series all the cases were done as an emergency procedure and, despite the multiple obstetric complications, there was no maternal mortality and the incidence of post-operative morbidity was low.

Pregnancy During Surgical Training: Are Residency Programs Truly Supporting Their Trainees?

OBJECTIVE: Despite recent national improvements in family leave policies, there has been little focus on program-level support for surgical trainees. Trainees who may require clinical duty adjustments during pregnancy, who experience pregnancy loss, or who struggle with balancing work obligations with the demands of a new infant may face stigma when seeking schedule accommodations. The aim of this study was to describe program and colleague support of surgical trainees for pregnancy-related and postpartum health needs. DESIGN: Survey questionnaire. Participants responded to multiple-choice qu

Metabolic syndrome before 16 weeks' gestation: prevalence and haemodynamic profile in a socioeconomically disadvantaged pregnancy cohort: baseline findings from Mother's Heart Study.

BACKGROUND: Pregnancy provides an opportunity to identify young women at risk of future cardiometabolic diseases. We aimed to determine the prevalence of metabolic syndrome (MetS) and its components, and to assess haemodynamic profiles in early pregnancy. METHOD: We recruited pregnant women at Lyell McEwin Hospital before 16 weeks' gestation. Haemodynamic parameters were measured using USCOM BP+ device. Blood glucose, lipid profile, liver and renal function were assessed. We included data from 264 women aged 19-44 years in the analyses. MetS was defined using the Harmonizing the MetS criteria

Syphilis Screening Among Commercially-Insured Pregnant Women in the United States.

BACKGROUND: Congenital syphilis (CS) incidence increased by 81.7% in the United States (US) between 2020 and 2024. Syphilis screening is recommended at the first prenatal visit and at delivery, but poor compliance may lead to inadequate screening and preventable CS cases. METHODS: Using a US-wide collection of longitudinal healthcare claims data, we identified a cohort of commercially-insured pregnant women aged 15-44 years. Women were included if they had a pregnancy resulting in a live birth, stillbirth, or spontaneous abortion during 2017-2021. We described patterns of syphilis screening,

Access to and engagement with healthcare services among women with Children's Social Care involvement during the perinatal period who subsequently died: a confidential enquiry.

INTRODUCTION: In the last decade, UK maternal death surveillance data have shown that among the women who died during pregnancy and the year after birth (the perinatal period), the proportion of women with Children's Social Care (CSC) involvement nearly doubled. Parental non-engagement in the context of CSC involvement has been described as a particular professional concern. OBJECTIVE: To explore organisational and system barriers when accessing and engaging with healthcare services experienced by women with CSC involvement who subsequently died during the perinatal period. METHODS: MBRRACE-

Assessing the willingness and preference of pregnant and postpartum women regarding RSV immunization for newborns in western China: A mixed-methods study.

Respiratory syncytial virus (RSV) is a leading cause of pediatric lower respiratory tract infections, yet evidence on vaccine acceptance among Chinese pregnant and postpartum women remains limited. This study examines the willingness, preference and factors influencing neonatal RSV immunization among this population in Lanzhou City, China. The study used a convergent mixed-methods approach, including cross-sectional questionnaires and in-depth interviews, and data were processed through multiple regression analysis and thematic analysis. The results showed that the respondents' knowledge of RS

Efficacy of Single-Dose Intravenous Ferric Derisomaltose for Iron Deficiency Anemia in Pregnancy.

Iron deficiency anemia (IDA) is prevalent in pregnancy, yet oral iron therapies are associated with poor adherence and many intravenous (IV) iron formulations require multiple doses for treatment. Ferric derisomaltose (FDI) is a newer IV iron formulation that is safe and efficacious in non-pregnant individuals, yet pregnancy data is lacking. We evaluated the safety, efficacy, and maternal and neonatal outcomes associated with a single 1000 mg IV dose of FDI in pregnant individuals with IDA. We conducted a prospective, single-arm interventional trial of FDI administered in the second or third t

Social capital and maternal mental health: Findings from secondary data analyses of two linked longitudinal trials in Australia.

PROBLEM: Low social capital has been identified as an important risk factor in the development of postpartum mental illness. BACKGROUND: Evidence suggests that new and expectant mothers with higher levels of support have lower rates of maternal mental illness, yet few studies examine this relationship longitudinally. AIM: This study investigated the association between social capital in late pregnancy and maternal mental health up to 5 years postpartum. METHODS: Secondary analysis of Communicating Healthy Beginnings Advice by Telephone trial data was conducted. Participants included women i

Serum Ferritin Levels in Pregnancy and Their Association with Gestational Diabetes Mellitus: A Prospective Longitudinal Study.

OBJECTIVE: Elevated serum ferritin (SF) levels are associated with oxidative stress (OS) and systemic inflammation in various disorders. However, the changes in SF levels during pregnancy and their relationship with gestational diabetes mellitus (GDM) and blood glucose levels are not well understood. METHODS: This prospective longitudinal study included 390 participants (130 GDM cases and 260 controls) during early pregnancy. We measured SF levels in the1st, 2nd, and 3rd trimesters, as well as plasma malondialdehyde (MDA) and C-reactive protein (CRP) in the 1st trimester, blood glucose levels

Cross one single body 49 tissues single-cell transcriptome reveals detailed macrophage heterogeneity during pig pregnancy.

INTRODUCTION: Pregnancy involves complex physiological adaptations across maternal organs and the immune system to support fetal development. Macrophages play a dual role during pregnancy: defending against pathogens and supporting tissue adaptation. However, comprehensive and in-depth studies of cross-tissue transcriptional heterogeneity of macrophages during healthy pregnancy at the single-cell level remain elusive. METHODS: We performed single-cell RNA sequencing (scRNA-seq) to profile macrophages from a healthy pregnant pig across 49 tissues. Immunofluorescence was performed to verify the

Intravenous immunoglobulin versus placebo in recurrent pregnancy loss: A systematic review and meta-analysis of randomized trials.

Recurrent pregnancy loss (RPL) affects 1-2 % of fertile couples, with immunological factors increasingly implicated in unexplained cases. Currently, no convincing treatment exists. Intravenous immunoglobulin (IVIg) has been explored as a potential treatment, but results have been inconsistent. This study evaluates whether IVIg affects live birth rates in women with RPL in subsequent pregnancies. We searched PubMed, Embase, and Cochrane libraries for relevant randomized controlled trials up to March 20, 2023, and contacted authors for individual patient data. Trials involving women with two or

Nutrition in early life, epigenetics and lifelong health - evidence from cohort and intervention studies.

This review summarises evidence from cohort and intervention studies on the relationships between nutrition in early life, epigenetics and lifelong health. Established links include maternal diet quality with conception rates, micronutrient sufficiency before and during pregnancy with preterm birth prevention, gestational vitamin D intake with offspring bone health, preconception iodine status with child IQ, adiposity with offspring obesity and maternal stress with childhood atopic eczema. Animal studies demonstrate that early-life environmental exposures induce lasting phenotypic changes via

The China Birth Cohort-Environmental and Genetic Factors (CBC-EG) Study: Design, Aims, Protocol, Findings, and Prospects.

Early life, including the embryonic period, is particularly vulnerable to environmental exposures. Previous studies have largely focused on the relationship between individual environmental factors and population health. In reality, individuals are exposed to multiple environmental factors, which together form an "exposome" that reflects the environmental influences on human health. Exposome-based analyses utilizing biological samples have been conducted in various countries to assess the impacts of environmental endocrine disruptors and other pollutants on maternal and fetal health during pre

Maternal Circulatory NAD Precursor Levels and the Yolk Sac Determine NAD Deficiency-Driven Congenital Malformation Risk.

Nicotinamide adenine dinucleotide (NAD) is an essential cofactor in hundreds of cellular processes. Genetic disruption of NAD de novo synthesis causes congenital NAD deficiency disorder (CNDD), characterized by multiple congenital malformations or death in utero. Patient outcomes are highly variable, likely due to differences in the availability of maternal NAD precursors vitamin B3 and tryptophan to the embryo and its extraembryonic tissues. Here, maternal plasma and yolk sac NAD metabolomes, embryonic NAD levels, and pregnancy outcomes were quantified in a CNDD mouse model to determine how m
