Factors affecting long-term survival after aortic valve replacement.

BACKGROUND AND AIM: To evaluate long-term outcomes of surgical aortic valve replacement (AVR) due to significant aortic stenosis (AS) and assess changes in factors affecting survival during a 10-year period in patients referred for surgery from a single centre. METHODS: We evaluated 1143 patients (478 women, 665 men; mean age 61 ± 5 years) treated in the Department of Valvular Heart Disease at the Institute of Cardiology in Warsaw who were referred for AVR due to significant AS in 1998-2008 and survived the surgery and the initial 30-day postoperative period. We assessed long-term survival in

Free coracoacromial ligament graft for augmentation of massive rotator cuff tears treated with mini-open repair.

OBJECTIVES: Repair of massive rotator cuff tears is a challenging and complex procedure. The tissue at the end of the torn tendons is often friable and weak, because ruptures are old and degenerated, and thus the ends must be sutured effectively and strongly to achieve a satisfactory outcome. We aimed to evaluate the effectiveness of free total or partial coracoacromial ligament (CAL) graft to protect and augment the repair of massive rotator cuff tears. METHODS: Forty six patients ((32 females, mean age 54.3 years (range 39-66 years)) operated for massive rotator cuff tears between January 2

Surgical management of brainstem hemangioblastomas in patients with von Hippel-Lindau disease.

OBJECT: Hemangioblastomas of the brainstem constitute 5 to 10% of central nervous system (CNS) tumors in patients with von Hippel-Lindau (VHL) disease. At present, optimal management of brainstem hemangioblastomas associated with VHL disease is incompletely defined. In an attempt to clarify some of the uncertainty about the operative treatment of these lesions and its outcome, the authors reviewed all cases of VHL disease in which resection of brainstem hemangioblastomas was performed at the National Institutes of Health during a 10-year period. METHODS: Twelve consecutive patients with VHL d

Surgical treatment of prosthetic valve endocarditis.

From 1975 through 1992, we reoperated on 146 patients for the treatment of prosthetic valve endocarditis. Prosthetic valve endocarditis was considered to be early (< 1 year after operation) in 46 cases and active in 103 cases. The extent of the infection was prosthesis only in 66 patients, anulus in 46, and cardiac invasion in 34. Surgical techniques evolved in the direction of increasingly radical débridement of infected tissue and reconstruction with biologic materials. All patients were treated with prolonged postoperative antibiotic therapy. There were 19 (13%) in-hospital deaths. Univaria

EEG monitoring, selective shunting and patch graft angioplasty in carotid endarterectomy. Early and longterm results.

From March 1980 to July 1986 at the Department of Vascular Surgery of the University of Padua, 182 patients underwent 210 carotid revascularizations for atherosclerotic stenosis involving the carotid bifurcation (28 operations were bilateral). Carotid endarterectomies (CE) and patch graft angioplasty totalled 192 (166 patients); an enlarging patch graft angioplasty of the internal carotid artery (ICA) without CE was performed in 14 cases (13 patients); in the remaining four surgical procedures (3 patients), for technical reasons prohibiting CE, the operation consisted of a great saphenous vein

Connectivity alteration in thalamic nuclei and default mode network-related area in memory processes in mesial temporal lobe epilepsy using magnetoencephalography.

This work aimed to investigate the involvement of the thalamic nuclei in mesial temporal lobe epilepsy (MTLE) and identify the influence of interictal epileptic discharges on the neural basis of memory processing by evaluating the functional connectivity (FC) between the thalamic nuclei and default mode network-related area (DMNRA) using magnetoencephalography. Preoperative datasets of nine patients with MTLE with seizure-free status after surgery and those of nine healthy controls were analyzed. The FC between the thalamic nuclei (anterior nucleus [ANT], mediodorsal nucleus [MD], intralaminar

The perioperative management of patients maintained on medications used to manage opioid addiction.

PURPOSE OF REVIEW: The substantial increase in prescription and illicit opioid abuse observed over the last 2 decades has significantly increased the number of patients in recovery from addiction and now maintained on opioid replacement or agonist therapy. These patients present unique challenges to perioperative pain management. RECENT FINDINGS: Standard opioid-based analgesic techniques are often not sufficient when patients are maintained on medications used to manage opioid addiction. The current recommendations to support perioperative pain management plans in this population are based o

Nasal encephalocele: endoscopic excision with anesthetic consideration.

OBJECTIVE: Nasal encephalocele may presents as a nasal mass, its treatment is surgical and it should be done early in life. When removal is indicated, there are multiple surgical approaches; including lateral rhinotomy, a transnasal approach and a coronal flap approach. However, the treatment of a basal intranasal encephalocele using transnasal endoscopic approach could obviates the possible morbidity associated with other approaches. The aim of this study was to evaluate the efficacy of endoscopic removal of intranasal encephalocele, also to document the role of anesthetist in the operative a

Predicting acute kidney injury after radical cystectomy for muscle-invasive bladder cancer.

INTRODUCTION: Radical cystectomy (RC) with urinary diversion remains the gold standard for the treatment of muscle invasive tumors. RC is a major surgery associated with a high morbidity rate. Postoperative acute kidney injury (AKI) is associated with severe morbidity and mortality. In this study we attempted to identify risk factors for postoperative AKI in patients undergoing RC. PATIENTS AND METHODS: A multicenter retrospective analysis was conducted among patients treated with radical cystectomy for muscle-invasive bladder cancer between 2015 and 2024. AKI was defined by Kidney Disease Im

The Rate and Timing of Revision Carpal Tunnel Release with Long-Term Follow-Up.

PURPOSE: Although carpal tunnel release (CTR) is the most common upper-extremity orthopedic procedure in the United States, revision surgery can be indicated when symptoms of carpal tunnel syndrome persist, recur, or worsen after primary surgery. Understanding the revision surgery profile after CTR is important for preoperative counseling and treatment decision-making, yet published rates of revision surgery vary widely. In our study, we aimed to establish the rate of revision CTR and describe temporal patterns of revisions after primary surgery for patients with long-term follow-up at our ins

Factors That Influence Resource Utilization Following Primary Lower Extremity Total Joint Arthroplasty.

BACKGROUND: In an effort to reduce unnecessary emergency department (ED) visits, we established a "Total Joint Hotline" (TJH) for patients undergoing primary total knee arthroplasty (TKA) or total hip arthroplasty. This 24-hour hotline was designed to give patients direct access to the adult reconstruction care team during the postoperative period. The purpose of this study was to determine which patient- and procedure-related factors drove utilization of the TJH. METHODS: We prospectively collected data on 372 patients who underwent primary unilateral total joint arthroplasty (Total Joint Ar

Postoperative hypotension following acute hip fracture surgery is a predictor of 30-day mortality.

AIMS: Hip fractures are some of the most common fractures encountered in orthopaedic practice. We aimed to identify whether perioperative hypotension is a predictor of 30-day mortality, and to stratify patient groups that would benefit from closer monitoring and early intervention. While there is literature on intraoperative blood pressure, there are limited studies examining pre- and postoperative blood pressure. METHODS: We conducted a prospective observational cohort study over a one-year period from December 2021 to December 2022. Patient demographic details, biochemical results, and haem

From the perspective of pseudo-progression rather than treatment failure, how long should we wait before considering treatment failure if large cystic enlargement occurs after Gamma Knife radiosurgery for vestibular schwannoma?: insight into pseudoprogression based on two case reports.

Gamma knife radiosurgery (GKRS) has been accepted as a safe and effective treatment for vestibular schwannoma (VS). However, during follow-up, tumor expansion induced by irradiation can occur, and diagnosis of failure in radiosurgery for VS is still controversial. Tumor expansion with cystic enlargement causes some confusion regarding whether further treatment should be performed. We analyzed more than 10 years of clinical findings and imaging of patients with VS with cystic enlargement after GKRS. A 49-year-old male with hearing impairment was treated with GKRS (12 Gy; isodose, 50%) for a lef
