Use of Oral and Emergency Contraceptives After the US Supreme Court's Dobbs Decision.

IMPORTANCE: The US Supreme Court Dobbs v Jackson Women's Health Organization decision allowed states to strengthen restrictions on abortion access, triggering the closure of family planning clinics and leading to confusion about the legality of emergency contraceptives (ECs). OBJECTIVES: To evaluate the association between the Dobbs decision and fills for oral and emergency contraceptives in states that enacted the most restrictive abortion policies after Dobbs. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data on contraceptive fills for women of reproductive age (15-49 years) i

The association between cognitive ability and opioid prescribing in vulnerable older adults with chronic pain in ambulatory care: a secondary data analysis using the Medical Expenditure Panel Survey.

BACKGROUND: Vulnerable older adults living with Alzheimer's disease or Alzheimer's disease and related dementia (AD/ADRD) and chronic pain generally receive fewer pain medications than individuals without AD/ADRD, especially in nursing homes. Little is known about pain management in older adults with AD/ADRD in the community. The aim of the study was to examine opioid prescribing patterns in individuals with chronic pain by levels of cognitive ability in ambulatory care. METHODS: We used the Medical Expenditure Panel Survey (MEPS), years 2002-2017, and identified three levels of cognitive imp

Seasonal patterns of malaria, genital infection, nutritional and iron status in non-pregnant and pregnant adolescents in Burkina Faso: a secondary analysis of trial data.

BACKGROUND: Adolescents are considered at high risk of developing iron deficiency. Studies in children indicate that the prevalence of iron deficiency increased with malaria transmission, suggesting malaria seasonally may drive iron deficiency. This paper examines monthly seasonal infection patterns of malaria, abnormal vaginal flora, chorioamnionitis, antibiotic and antimalarial prescriptions, in relation to changes in iron biomarkers and nutritional indices in adolescents living in a rural area of Burkina Faso, in order to assess the requirement for seasonal infection control and nutrition i

A Cost-Consequence Analysis of Preemptive SLCO1B1 Testing for Statin Myopathy Risk Compared to Usual Care.

There is a well-validated association between SLCO1B1 (rs4149056) and statin-associated muscle symptoms (SAMS). Preemptive SLCO1B1 pharmacogenetic (PGx) testing may diminish the incidence of SAMS by identifying individuals with increased genetic risk before statin initiation. Despite its potential clinical application, the cost implications of SLCO1B1 testing are largely unknown. We conducted a cost-consequence analysis of preemptive SLCO1B1 testing (PGx+) versus usual care (PGx-) among Veteran patients enrolled in the Integrating Pharmacogenetics in Clinical Care (I-PICC) Study. The assessmen

Geographical and socioeconomic disparities in opioid access in Mexico, 2015-19: a retrospective analysis of surveillance data.

BACKGROUND: In 2015, Mexico implemented regulatory changes and an electronic system to improve access to prescription opioids. We aimed to investigate trends in opioid dispensing after the implementation of these changes and assess how opioid dispensing varied geographically and by socioeconomic status. METHODS: In this retrospective analysis of prescription medication surveillance data, we analysed dispensing data for group 1 medications (all opioids, including morphine, methadone, hydromorphone, oxycodone, tapentadol, fentanyl, sufentanil, and remifentanil) obtained from the Federal Commiss

RBE-weighted dose conversions for patients with recurrent nasopharyngeal carcinoma receiving carbon-ion radiotherapy from the local effect model to the microdosimetric kinetic model.

BACKGROUND: We sought to establish a conversion curve to convert the RBE-weighted doses calculated by local effect model I (LEM) (LEM RBE-weighted doses) in patients with locally recurrent nasopharyngeal carcinoma (rNPC) to the RBE-weighted doses calculated by microdosimetric kinetic model (MKM) (MKM RBE-weighted doses). We also converted the LEM dose constraints (RBE-weighted dose constraints in LEM plans) for the brain stem, spinal cord, and optic nerve based on this curve. METHODS: Data from 20 patients with rNPC receiving carbon-ion radiotherapy (CIRT) in our hospital were collected. LEM

Ophthalmic drugs as part of polypharmacy in nursing home residents with glaucoma.

BACKGROUND: Glaucoma comprises age-related neurodegenerative diseases of retinal ganglion cells, the worldwide prevalence of which is increasing. Local pharmacotherapy is the primary treatment option, especially in the elderly. But this therapeutic approach may include risks for adverse drug effects and drug-drug interactions, of particular importance in frail nursing home resident populations. OBJECTIVE: The aim of the present study was to investigate anti-glaucoma pharmacotherapy in nursing home residents in the context of multi-morbidity and related systemic co-medication. METHODS: Data f

Physiotherapeutic care of patients with rupture of the anterior cruciate ligament or meniscus lesion in Germany.

INTRODUCTION: Physiotherapy plays a central role in the rehabilitation of musculoskeletal injuries. While the German catalogue of therapies regulates the type and frequency of treatments in Germany, there is a lack of data on actual prescription practice. The research question of this survey was to examine the current physiotherapeutic care and prescription practices for patients with a rupture of the anterior cruciate ligament (ACL) or a meniscus lesion. METHODOLOGY: An online questionnaire was developed and sent to practitioners who regularly treat patients with knee joint injuries. They we

Physician Billing in Long-Term Care Homes and the Association With the Prescribing of End-of-Life Symptom Management Medications: A Population-Based Retrospective Cohort Study.

OBJECTIVES: Medications are often needed to manage distressing end-of-life symptoms (eg, pain, agitation, dyspnea). We evaluated whether physician billing in long-term care (LTC) was associated with LTC residents' receipt of an end-of-life symptom management medication prescription. DESIGN: Retrospective cohort study using administrative health data. SETTING AND PARTICIPANTS: All LTC decedents in Ontario's 626 publicly funded LTC homes who died between January 1, 2017, and March 17, 2020, were matched to a physician most responsible for their care (MRP) in LTC. METHODS: We measured the prop

Understanding Chronic Opioid Use Risk Associated With Penile Cancer Treatments: A National Analysis.

OBJECTIVE: To characterize risk factors associated with the chronic utilization of narcotics following treatment for penile cancer using a national insurance database. METHODS: The MarketScan Commercial Claims (MCCD) and Medicare supplemental databases (MSD) (2009-2023) were utilized to identify opioid-naive (no narcotic use within 3 months) adults who had undergone treatment for penile cancer. Our primary outcome of chronic opioid use was defined as prescriptions filled 3-6 months after initial treatment. Multivariable regression was utilized to identify independent predictors of chronic opi

Racial disparities in length of hospitalization and systemic medication utilization in patients with psoriasis.

The growing evidence of chronic inflammation and sequelae caused by psoriasis indicates the need for systemic treatment, including biologics and conventional treatments like methotrexate. However, unconscious racial bias may lead to discrepancies in systemic medication prescription. Racial identity is also suspected to affect comorbidity and hospitalization rates in patients with psoriasis. This study examines the impact of race on hospitalization duration and systemic medication prescription patterns in patients with psoriasis in the United States in 2020. The 2020 Healthcare Cost and Utiliza

Gender and Serious Drug Recalls: A Textual Sentiment Analysis of Drug Reviews on WebMD.

Consumers taking prescription drugs have limited ability to ascertain drug quality before taking the drug. After drug use, however, consumers frequently report their personal experiences with prescription drugs on one of the world's largest medical websites: WebMD. Drug reviews on WebMD are a potentially rich source of free-form text that can be utilized to inform firms, consumers, researchers, and the Food and Drug Administration (FDA) about the quality and safety of prescription drugs. Additionally, because men and women communicate in starkly different ways, the gender of the reviewer may p

Pills and the damage done: the opioid epidemic as man-made crisis.

The prescription opioid epidemic has slowly evolved over the past quarter century with increasingly detrimental consequences for public health. Man-made crises are often unforeseen and characterized by a situation without natural causes where - because of human intent, error, negligence, or the failure of manmade systems - the level of needs in the population exceeds available resources to counter the problem. This paper presents the prescription opioid epidemic as a man-made crisis and explores the public health impact of opioid manufacturers and other industries producing commodities with ad

Influences of Software Changes on Oxycodone Prescribing at an Australian Tertiary Emergency Department: A Retrospective Review.

Opioid prescribing and dispensing from emergency departments is a noteworthy issue given widespread opioid misuse and diversion in many countries, contributing both physical and economic harm to the population. High patient numbers and the stochastic nature of acute emergency presentations to emergency departments (EDs) introduce challenges for prescribers who are considering opioid stewardship principles. This study investigated the effect of changes to electronic prescribing software on prescriptions with an auto-populated quantity of oxycodone immediate release (IR) from an Australian terti

Risk of drug-related death associated with co-prescribing of gabapentinoids and Z-drugs among people receiving opioid-agonist treatment: A national retrospective cohort study.

BACKGROUND: Prescribing of gabapentinoids and Z-drug-hypnotics has increased in the population and among people receiving opioid-agonist treatment (OAT) for opioid dependence. Evidence is mixed on whether co-prescribing of sedatives such as gabapentinoids and Z-drugs during OAT increases risk of drug-related death (DRD). METHODS: We conducted a retrospective cohort study of individuals prescribed OAT between 2011 and 2020 in Scotland. Prescribing records were linked to mortality data and other healthcare datasets (sociodemographic, comorbidity). We identified episodes of treatment with gabape
