Prostate cancer, the most frequent non-cutaneous malignancy in aging men, is a growing medical problem, representing the second leading cause of male cancer deaths. Despite its high morbidity, the etiology of prostate cancer remains largely unknown. Several studies have documented hormonal imbalance, such as alteration in androgens and estrogens, obesity, family history and growth factors, as risk factors in the pathogenesis of prostate cancer. Insulin is a growth-promoting hormone that is reported to be involved in the pathogenesis of various malignancies, such as breast and bladder cancers.

Numerous drugs have been associated with destructive thyroiditis (subacute thyroiditis). Sunitinib, a tyrosine kinase inhibitor employed in renal cell carcinoma and gastrointestinal stromal tumors, has recently been linked to destructive thyroiditis with resultant hypothyroidism. Dermatitis herpetiformis (DH) is a chronic, pruritic, papulovesicular dermatosis on extensor surfaces that is characterized by a neutrophilic infiltrate and granular immunoglobulin A deposition at the dermal papillae. Although the presence of immunoglobulin A in the skin and the severity of DH are known to be associated with gluten intake, few drugs have been implicated in the induction of DH. We report a case of DH triggered by intramuscular injections of leuprolide acetate, a gonadotropin-releasing hormone analog, in a patient with a history of prostate cancer.

Indwelling urethral catheters are bothersome for patients following radical retropubic prostatectomy. At Indiana University, to alleviate postoperative discomfort, early removal of urethral catheters has become commonplace. In our series of patients, complications were infrequent and removal improved patient mobility and reduced discomfort.

OBJECTIVE: To develop a convenient technique for dividing the bladder neck during radical retropubic prostatectomy.
METHOD: Before opening the bladder, we created a plane that separates the anterior surface of the seminal vesicles from the posterior wall of the bladder and ran an umbilical tape through the plane. The posterior bladder neck wall was later divided using this tape as a guide mark.
RESULTS AND CONCLUSION: The posterior bladder neck was divided in a single, easy, and swift maneuver (less than 2 min) in 350 patients with no subsequent complications.

Experience had with radical prostatectomy in patients presenting localized prostate carcinoma is shared. A modified technique for new bladder-neck formation contributing to continence preservation is used in five patients, yielding very good postoperative results.
