Subthalamic nucleus stimulation restores glucose metabolism in associative and limbic cortices and in cerebellum: evidence from a FDG-PET study in advanced Parkinson's disease.

Deep brain stimulation of the subthalamic nucleus (STN-DBS) is a highly effective surgical treatment in patients with advanced Parkinson's disease (PD). Because the STN has been shown to represent an important relay station not only in motor basal ganglia circuits, the modification of brain areas also involved in non-motor functioning can be expected by this intervention. To determine the impact of STN-DBS upon the regional cerebral metabolic rate of glucose (rCMRGlc), we performed positron emission tomography (PET) with 18-fluorodeoxyglucose (FDG) in eight patients with advanced PD before sur

Proton MR spectroscopic evaluation of suspicious brain lesions after stereotactic radiotherapy.

BACKGROUND AND PURPOSE: The radiologic assessment of suspicious brain lesions after stereotactic radiotherapy of brain tumors is difficult. The purpose of our study was to define parameters from single-voxel proton MR spectroscopy that provide a probability measure for differentiating neoplastic from radiation-induced, nonneoplastic lesions. METHODS: Seventy-two lesions in 56 patients were examined using a combined MR imaging and MR spectroscopy protocol (point-resolved spectroscopy, TE = 135 ms). Signal intensities of cholines, creatines, N-acetyl aspartate, and the presence of lactate and l

Automated quality assurance of rigid brain CT/MR image registration using a 3D convolutional neural network.

BACKGROUND: Accurate CT/MR registration is important in stereotactic brain radiotherapy, where small spatial errors may affect target localization and treatment planning. In many clinical settings, registration quality is still assessed primarily through manual visual inspection. However, time and workflow constraints may limit the consistency and depth of manual evaluation. PURPOSE: This study aimed to develop a 3D convolutional neural network (3D CNN)-based quality assurance (QA) method for rigid brain CT/MR registration using paired sub-volumes and a registration-level mean probability sco

Adaptive-driven CT simulation-free multi-fraction SBRT radiotherapy: Initial clinical experience.

INTRODUCTION: Using diagnostic CT for radiotherapy (RT) planning can bypass traditional CT simulation but introduces challenges in patient positioning and Hounsfield unit (HU) fidelity, affecting dose accuracy. Here a Varian Ethos adaptive-driven CT direct-to-treatment (DtT) multi-fraction stereotactic body radiation therapy (SBRT) workflow is presented. METHODS: This study employed institutional diagnostic PET-CT images to generate an initial reference Ethos treatment plan. HU and dosimetric accuracy were validated for PET-CT, Ethos CBCT images (with and without Hypersight (HS), and the gold

A phase I study of the combination of atezolizumab, tiragolumab, and stereotactic body radiation therapy in patients with metastatic multiorgan cancer.

BACKGROUND: Immunotherapy targeting the PD-1/PD-L1 pathway is a standard of care in a number of metastatic malignancies, but less than a fifth of patients are expected to respond to ICIs (Immune Checkpoint Inhibitors). In a clinical trial, combining the anti-TIGIT (T cell immunoreceptor with Ig and ITIM domains) Mab (monoclonal antibody) tiragolumab with atezolizumab improved outcomes in non-small cell lung cancer. In preclinical models, SBRT (Stereotactic Body Radiation Therapy) could increase expression levels of the inhibitory co-receptors TIGIT and PD-L1. We aim to assess the combination o

A case of endocrine mucin-producing sweat gland carcinoma with distant metastasis.

Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is a rare cutaneous adnexal neoplasm typically arising on the face of older individuals, most commonly around the eyelids. Histopathologic features include a circumscribed proliferation of low-grade epithelioid cells with areas of cystic and cribriform growth, foci of intracytoplasmic and extracellular mucin, and coexpression of endocrine, neuroendocrine, and cytokeratin markers by immunohistochemistry. Given histopathologic and immunohistochemical similarities, EMPSGC is often likened to solid papillary carcinoma of the breast and endoc
