Some pharmacological properties of althesin (CT1341) in man.

Althesin in dose ranges of 40, 60 and 80 mul/Kg was administered to six healthy unpremedicated volunteers according to a balanced incomplete block design. Both subjective and objective changes resulting from the administration of the agent were studied and analyzed. In the dose ranges used, Althesin was found to have no deleterious effects on heart rate, respiratory rate or blood pressure and apnoea only occurred in one patient who had an end-expiratory CO2 level lower than normal, due to preanaesthetic hyperventilation. The onset of sleep is prolonged with Althesin at the injection rate of 1

Shade matters: heat stress alleviation in Gyr and Girolando cows through silvopastoral management in tropical conditions.

Silvopastoral systems integrating tree cover into pasturelands offer promising strategies to mitigate heat stress in tropical livestock production. This study evaluated the effects of shade from Eucalyptus urograndis on thermal comfort indices in Gyr and Girolando dairy cattle in the Brazilian Cerrado. Forty-eight lactating cows (24 Gir, 24 Girolando) were monitored over two years in two contrasting environments: no shade (full sun) and shade (silvopasture). Thermal indices, including rectal and surface temperatures (measured via infrared thermography), respiration rates, and panting scores, w

Effect of Wearing a Surgical Mask or N95 Mask in Patients With Stable Chronic Hypercapnic Respiratory Failure.

BACKGROUND: Surgical and N95 masks are commonly used in the prevention of respiratory virus transmission. The safety of wearing masks is well documented in healthy individuals but is unclear in people with chronic hypercapnia. In addition, masks are often poorly tolerated in this population. The present study assessed the effect of surgical and N95 masks in people with stable chronic hypercapnia using noninvasive ventilation (NIV). METHODS: A randomized crossover trial was performed. SpO2 and transcutaneous CO2 (PtcCO2) were measured in participants wearing a surgical mask, an N95 mask, and n

Comorbidity patterns of respiratory symptoms and diseases and their impact on all-cause and respiratory mortality: Prospective cohort study.

BACKGROUND: Respiratory symptoms and diseases often co-occur and may increase mortality risk. OBJECTIVE: This study aims to identify patterns of respiratory multimorbidity and assess their links with mortality. METHODS: We conducted a prospective cohort study using data from the National Health and Nutrition Examination Survey (NHANES) 2007-2011, linked to mortality records through 2019. Participants aged ≥40 years were classified using latent class analysis based on respiratory symptoms, diseases, and spirometry patterns. Cox models were used to estimate the relationship between respiratory

Monitoring of breathing effort through pressure time product measurement using airway occlusion pressure (PTPPOCC study).

BACKGROUND: Monitoring respiratory effort is crucial to prevent ventilator-induced lung injury and diaphragmatic dysfunction, but esophageal manometry (the gold standard) is invasive an can be cumbersome. The airway occlusion pressure (ΔPOCC) correlates with inspiratory effort but does not account for inspiratory time (Ti) or respiratory rate (RR). We aimed to validate a novel non-invasive index (PTPPOCC) that combines ΔPOCC, Ti, and RR to estimate the esophageal pressure-time product (PTPESO). METHODS: We conducted a prospective, multicenter, physiological, crossover study. Three levels of i

Repeated High-dose Fentanyl Administration in Rats Reveals Minimal Tolerance to Unconsciousness, Bradycardia, Muscle Rigidity, and Respiratory Depression.

BACKGROUND: Fentanyl is a synthetic opioid that is widely used in anesthesiology, but its illicit use is rapidly increasing. At high doses, fentanyl induces unconsciousness and muscle rigidity, the mechanisms of which are poorly understood. Since animal models are needed to study these effects, the aim of this study was to establish a rat model of fentanyl abuse and investigate the effects of repeated high-dose fentanyl injections on loss of righting reflex, heart rate, respiratory depression, muscle, and brain activity. METHODS: Male and female Sprague-Dawley rats were studied (n = 40). A bo

Increased circulating cytokeratin 19 fragment levels in preterm neonates receiving mechanical ventilation are associated with poor outcome.

Invasive mechanical ventilation and oxygen toxicity are postnatal contributors to chronic lung disease of prematurity, also known as bronchopulmonary dysplasia (BPD). Cyfra 21-1 is a soluble fragment of cytokeratin 19, which belongs to the cytoskeleton stabilizing epithelial intermediate filaments. As a biomarker of structural integrity, Cyfra 21-1 might be associated with airway injury and lung hypoplasia in neonates. Serum Cyfra 21-1 concentrations for 80 preterm and 80 healthy term newborns were measured within 48 h after birth. Preterm infants with the combined endpoint BPD/mortality had s

Diffusing Capacity Is an Independent Predictor of Outcomes in Pulmonary Hypertension Associated With COPD.

BACKGROUND: Patients with COPD who experience pulmonary hypertension (PH) have worse mortality than those with COPD alone. Predictors of poor outcomes in COPD-PH are not well-described. Diffusing capacity of the lung (Dlco) assesses the integrity of the alveolar-capillary interface and thus may be a useful prognostic tool among those with COPD-PH. RESEARCH QUESTION: Using a single center registry, we sought to evaluate Dlco as a predictor of mortality in a cohort of patients with COPD-PH. STUDY DESIGN AND METHODS: This retrospective cohort study analyzed 71 COPD-PH patients from the Johns Ho

Effects of High-Flow Nasal Cannula on End-Expiratory Lung Impedance in Semi-Seated Healthy Subjects.

BACKGROUND: High-flow nasal cannula (HFNC) enables delivery of humidified gas at high flow while controlling the FIO2 . Although its use is growing in patients with acute respiratory failure, little is known about the impact of HFNC on lung volume. Therefore, we aimed to assess lung volume changes in healthy subjects at different flows and positions. METHODS: This was a prospective physiological study performed in 16 healthy subjects. The changes in lung volumes were assessed by measuring end-expiratory lung impedance by using electrical impedance tomography. All the subjects successively bre

Enhancement of capnogram waveform in the presence of chest compression artefact during cardiopulmonary resuscitation.

BACKGROUND: Current resuscitation guidelines emphasize the use of waveform capnography to help guide rescuers during cardiopulmonary resuscitation (CPR). However, chest compressions often cause oscillations in the capnogram, impeding its reliable interpretation, either visual or automated. The aim of the study was to design an algorithm to enhance waveform capnography by suppressing the chest compression artefact. METHODS: Monitor-defibrillator recordings from 202 patients in out-of-hospital cardiac arrest were analysed. Capnograms were classified according to the morphology of the artefact.

Impact of hypothermia on implementation of CPAP for neonatal respiratory distress syndrome in a low-resource setting.

BACKGROUND: Neonatal hypothermia is widely associated with increased risks of morbidity and mortality, but remains a pervasive global problem. No studies have examined the impact of hypothermia on outcomes for preterm infants treated with CPAP for respiratory distress syndrome (RDS). METHODS: This retrospective analysis assessed the impact of hypothermia on outcomes of 65 neonates diagnosed with RDS and treated with either nasal oxygen (N = 17) or CPAP (N = 48) in a low-resource setting. A classification tree approach was used to develop a model predicting survival for subjects diagnosed with

Functional t1ρ imaging in panic disorder.

BACKGROUND: Abnormal brain pH has been suggested to play a critical role in panic disorder. To investigate this possibility, we employed a pH-sensitive magnetic resonance (MR) imaging strategy (T1 relaxation in the rotating frame [T1ρ]) and conventional blood oxygen level-dependent (BOLD) imaging. METHODS: Thirteen panic disorder participants and 13 matched control subjects were enrolled in the study. T1ρ and BOLD were used to study the functional response to a visual flashing checkerboard and their relationship to panic symptoms assessed using the Beck Anxiety Inventory. RESULTS: In respons

Diaphragm pacing and noninvasive respiratory management of amyotrophic lateral sclerosis/motor neuron disease.

INTRODUCTION: Although it is known that continuous noninvasive ventilation (CNIV) can prolong life in amyotrophic lateral sclerosis/motor neuron disease (ALS/MND), in this study we explore similar claims for diaphragm pacing (DP). METHODS: NIV and DP users' vital capacities (VCs) over time and duration of NIV and CNIV dependence were analyzed for 354 non-DP and 8 DP ALS/MND patients. RESULTS: Patients had a higher rate of monthly VC decline before NIV use (5.1 ± 7.6%) than during NIV use (2.5 ± 3.6%) (P < 0.01, 95% CI 0.84-4.5); the decline for 4 DP users was 3.7-20%.
