Rapid Radiographic Improvement in Large Multilobulated Shoulder Calcific Tendinitis After Ultrasound-Guided Prolotherapy and Adjunctive Nutritional and Metabolic Support: A Case Report.

Calcific tendinitis of the shoulder is a common disorder characterized by calcium hydroxyapatite deposition within or near the rotator cuff tendons. Although the condition is often self-limited, large multilobulated calcific deposits may be less responsive to conservative treatment and can prompt consideration of more invasive procedures. We report a case of a 49-year-old man who presented with acute severe right shoulder pain, sleep disturbance, and marked functional limitation. At presentation, pain was 10/10 on the visual analog scale, and the shoulder range of motion could not be formally

Biceps brachii regional shear modulus following elbow flexion exercises at different muscle lengths.

The biceps brachii (BB) muscle, due to its biarticular nature, undergoes substantial length changes during exercise. Muscle elongation, particularly when the shoulder is extended, increases passive tension and may impair force production due to passive insufficiency. The distal BB appears vulnerable to mechanical strain and injury during eccentric contractions. This study investigated regional changes in BB shear modulus (μ) through shear wave elastography before and after elbow flexion exercises performed in three shoulder positions: flexion (45°), neutral (0°), and extension (-45°). Eighteen

A biomechanical investigation of the surface strains on the acromion and scapular spine during simulated ex-vivo arm motion.

While several biomechanical investigations have measured acromion and scapular spine strains for various pathological conditions to better understand the risk factors for fracture, no study has measured strains in the native shoulder. The objective of this study was to use an ex-vivo shoulder motion simulator to measure principal strain during continuous, unconstrained, muscle-driven motion of the native shoulder. Eight cadaveric specimens (57 ± 6 years) were used to simulate scapular plane abduction (27.5 to 80° of humerothoracic elevation), forward flexion (27.5 to 72.5° of humerothoracic el

Extracorporeal Shock Wave Therapy Combined with Complex Decongestive Therapy in Patients with Breast Cancer-Related Lymphedema: A Systemic Review and Meta-Analysis.

Breast cancer-related lymphedema (BCRL) is one of the most significant complications seen after surgery. Several studies demonstrated that extracorporeal shock wave therapy (ESWT), in addition to conventional complex decongestive therapy (CDT), had a positive effect on BCRL in various aspects. The systematic review and meta-analysis aim to explore the effectiveness of ESWT with or without CDT on BRCL patients. We searched PubMed, Embase, PEDro, Cochrane Library Databases, and Google Scholar for eligible articles and used PRISMA2020 for paper selection. Included studies were assessed by the PED

The oblique fracture of the manubrium sterni caused by a seatbelt--a rare injury? Treatment options based on the experiences gained in a level I trauma centre.

INTRODUCTION: Sternal fractures are rare with 3-8 % out of the total number of trauma cases mostly caused by direct impact to the anterior chest wall. Most cases described are due to motor vehicle crash either caused by direct impact to the steering wheel or by the seat belt. Fractures mainly occur to the sternal body. Only rarely are cases of manubrium fractures described in literature, for example, in relationship with a direct impact to the shoulder which caused an oblique fracture near to the sternoclavicular joint. Three patients with profoundly dislocated oblique manubrium fracture were

Passive contribution of the rotator cuff to abduction and joint stability.

PURPOSE: The purpose of this study is to compare shoulder joint biomechanics during abduction with and without intact non-functioning rotator cuff tissue. METHODS: A cadaver model was devised to simulate the clinical findings seen in patients with a massive cuff tear. Eight full upper limb shoulder specimens were studied. Initially, the rotator cuff tendons were left intact, representing a non-functional rotator cuff, as seen in suprascapular nerve paralysis or in cuff repair with a patch. Subsequently, a massive rotator cuff tear was re-created. Three-dimensional kinematics and force require

Arthroscopic Resection of Distal Clavicular Osteochondroma: A Case Report.

CASE: A 23-year-old boy presented with a 3-month history of right shoulder pain. Radiographs and computed tomography showed an exostosis on the inferior aspect of the distal clavicle. It was osteotomized and removed by arthroscopy. The histological examination confirmed the diagnosis of osteochondroma. The patient was found asymptomatic with full range of motion in the last clinical control at 1 year of follow-up. CONCLUSION: This case report shows an arthroscopic resection of distal clavicular osteochondroma achieving complete resection of the tumor with an excellent clinical outcome. The te

Reconstruction Using Frozen Autograft for Disseminated Phosphaturic Mesenchymal Tumor of the Humerus after a Pathological Fracture.

Phosphaturic mesenchymal tumors (PMTs) that cause tumor-induced osteomalacia (TIO) occur commonly in the bone with a small and nonaggressive appearance. Here, we report the case of a 67-year-old man with disseminated PMT in the humerus after a pathological fracture. Liquid nitrogen was used as an adjuvant therapy after curettage of the tumor, and the frozen autograft, using a pedicle freezing method, conserved the function of the shoulder joint. To our knowledge, this is the first case of a disseminated PMT in the bone that was treated with a frozen autograft, and this treatment method may be

Rehabilitation: Common Problems and Solutions.

There are numerous complications that can occur following a musculoskeletal injury or surgery in the sporting population. Prevention of the most frequent complications is the key in any successful rehabilitation program, but occasionally problems do occur. A thorough well-designed postoperative or postinjury rehabilitation program may prevent these problems. However, if complications do arise, a team approach among the parties involved in the process to develop an evidenced-based treatment program designed for the underlying complication can successfully treat these issues. The authors discuss
