Intracerebral metastasis in Ewing's sarcoma.

The evidence of CNS involvement in Ewing's sarcoma is uncommon and very often due to a direct extension of the tumor to extradural space and meninges. We report a case of a 14-year old girl with a primary ischio-pubic localization of Ewing's sarcoma. Body CT scan showed a bone lesion involving the left fronto-temporal region of the skull without affecting the brain. After chemotherapy radiological findings showed complete recovery. Ten months after treatment, the patient had neurological symptoms. Cerebral CT scan evidenced a large tumor mass localized in the right temporal lobe. Following rad

A novel method for acoustic modeling of cranial bone based on the porosity index.

Transcranial ultrasound stimulation (TUS) has emerged as a clinically validated neuromodulation technique. Particularly, phased array ultrasound can be applied in TUS to focus on the cortex or deep brain non-invasively, such as the ventral intermediate thalamic nucleus (VIM) and Precuneus (PCu) region for the treatment of essential tremor (ET) and Alzheimer Disease (AD). Current TUS treatment planning relies on computed tomography (CT)-derived skull porosity measurements, which involve patient radiation exposure and potential registration errors. This study proposes a Porosity Index (PI)-based

Cranial reconstruction evaluation - comparison of European statistical shape model performance on Chinese dataset.

PURPOSE: Morphological variability of the skull is an important consideration for cranioplasty and implant design. Differences in morphology of the skull based on the ethnicity are known. In a previous study we could show the accuracy and benefits of virtual reconstructions based on a statistical shape model (SSM) for neurocranial defects. As the SSM is trained on European data, the question arises how well this model fares when dealing with patients with a different ethnic background. In this study we aim to evaluate the accuracy and applicability of our proposed method when deploying a crani

An ex vivo study of automated motion artefact correction and the impact on cone beam CT image quality and interpretability.

OBJECTIVES: To assess the impact of head motion artefacts and an automated artefact-correction system on cone beam CT (CBCT) image quality and interpretability for simulated diagnostic tasks. METHODS: A partially dentate human skull was mounted on a robot simulating four types of head movement (anteroposterior translation, nodding, lateral rotation, and tremor), at three distances (0.75, 1.5, and 3 mm) based on two movement patterns (skull returning/not returning to the initial position). Two diagnostic tasks were simulated: dental implant planning and detection of a periapical lesion. Three

Impact of endoscopy and related technologies on indications for transsphenoidal pituitary surgery for growth hormone-producing pituitary neuroendocrine tumors in McCune-Albright syndrome: illustrative case.

BACKGROUND: McCune-Albright syndrome (MAS) is frequently associated with growth hormone (GH)-producing pituitary neuroendocrine tumors. However, transsphenoidal surgery is considered difficult because of skull base fibrous dysplasia, making complete tumor removal and endocrine remission challenging. OBSERVATIONS: Eighteen years earlier, a 31-year-old woman was diagnosed with MAS based on findings of GH hypersecretion, a -café--au-lait spot on the back of her right shoulder, and bone lesions in the ribs and craniofacial bones. She had a pituitary tumor, but craniofacial fibrous dysplasia invol

Traumatic Brain Injury in Vulnerable Road Users: Analysis of German In-Depth Crash Data to Inform Targeted Prevention.

Road traffic crashes are a major cause of traumatic brain injury (TBI), particularly among vulnerable road users (VRUs). However, current injury prevention strategies often overlook the heterogeneity of TBI-which include various injury types and severities-leading to an oversimplified approach to evaluating helmets and safety systems in regulations and ratings. To identify priority TBI types and severities in VRUs and to inform targeted prevention strategies, the German In-Depth Accident Study database was analyzed and a pathoanatomic classification system, i.e., Abbreviated Injury Scale, was

Endoscopic endonasal depth EEG-guided resection of a temporal encephalocele resulting in seizure freedom: illustrative case.

BACKGROUND: Temporal encephaloceles (TEs) are increasingly recognized as epileptogenic lesions in drug-resistant temporal lobe epilepsy. However, endoscopic endonasal surgery performed primarily to resect epileptogenic lesions remains extremely rare, largely because defining the optimal extent of resection is challenging without direct electrophysiological confirmation. OBSERVATIONS: A 41-year-old right-handed man presented with drug-resistant focal impaired consciousness seizures. MRI revealed a right TE herniating into the lateral recess of the sphenoid sinus. Scalp EEG suggested seizure on

Pedicled Inferior Turbinate-Nasal Floor and Septal Mucoperiosteal Flap Resurfacing for PRNN.

OBJECTIVES: Postradiation nasopharyngeal necrosis (PRNN) is one of the most severe and famous complications after radiotherapy for nasopharyngeal carcinoma (NPC). Resurfacing with pedicle nasal floor-nasal septum muco-periosteal flap after PRNN debridement is important and effective. However, the flap is limited for recovering large lesions like the parapharyngeal space and cervical vertebra. Here, we introduced a pedicle inferior turbinate-nasal floor and septal mucoperiosteal flap (inferior turbinate-nasal flap, ITNF) for reconstruction. METHODS: From November 2012 to July 2025, 71 PRNN pat

Orbital Apex Syndrome Secondary to Middle Cranial Fossa Meningocele: A Rare Pediatric Case.

BACKGROUND: Orbital apex syndrome (OAS) is a rare, sight-threatening condition caused by injury or compression of neurovascular structures in the orbital apex, which, depending on the underlying etiology, may even pose a life-threatening risk. Prompt recognition and intervention are essential to prevent permanent visual loss and systemic complications. CASE PRESENTATION: We report a 10-year-old child presenting with acute left-sided proptosis, eyelid edema, ophthalmoplegia, and optic disc edema, following a history of orbital cellulitis and meningitis in early childhood.

Delayed-onset cavernous sinus dural arteriovenous fistula after nasoseptal flap reconstruction: illustrative case.

BACKGROUND: Pedicled nasoseptal flap (NSF) is the gold-standard skull base reconstruction method in endoscopic endonasal surgery (EES). Although vascular complications, such as internal carotid artery injury or carotid-cavernous fistula (CCF), are well documented, dural arteriovenous fistula (dAVF) originating from the vascular network of an NSF is exceedingly rare. Herein, the authors report the first case of a cavernous sinus (CS) dAVF supplied by an NSF. OBSERVATIONS: A 50-year-old woman underwent EES with NSF reconstruction for tuberculum sellae meningioma. She experienced delayed hemorrh

Endoscopic Functional Debulking of a Destructive Clival-Sphenoid Benign Notochordal Cell Tumor.

BACKGROUND: Extensive clival and sphenoid lesions may extend into the posterior sinonasal tract and produce progressive nasal obstruction. In selected patients, endoscopic sinus surgery may provide both diagnostic tissue acquisition and symptomatic functional improvement. CASE PRESENTATION: A 30-year-old man initially underwent evaluation for headache, and computed tomography and magnetic resonance imaging demonstrated a large destructive clival-sphenoid lesion. Because no progressive neurological deficits or cranial neuropathies were identified, the lesion was initially managed conservativel

More Than Meets the Ear: Incidental Cerebrospinal Fluid Leak During Myringotomy in a Child With Common Cavity Deformity.

Cerebrospinal fluid (CSF) otorrhea represents a breach of the temporal bone separating the subarachnoid space from the middle ear and carries a significant risk of meningitis. While most cases are associated with trauma or prior skull base surgery, congenital fistulas may remain occult until unmasked by otologic procedures.  We report the case of a five-year-old male with bilateral profound sensorineural hearing loss and previously unknown labyrinthine anomalies who developed high-flow, pulsatile clear otorrhea during routine left ear myringotomy for recurrent otitis media. Immediate concern f

DNA Virus Detection in Olfactory Neuroblastomas Using Targeted Enrichment NGS.

A variety of malignancies can be found in the sinonasal tract including the nasal cavity, paranasal sinuses, and skull base. Many of these are attributed to viruses, for example, carcinomas with the presence of transcriptionally active high-risk human papillomavirus, as well as Epstein-Barr virus associated malignant lymphoepithelial carcinoma and haematolymphoid neoplasias. Olfactory neuroblastoma (ONB) is a rare malignant tumor of the olfactory fossa with an unknown etiology. We present the first comprehensive virus study of ONB. By targeted next-generation sequencing for 41 DNA viruses, we

Sellar Dermoid Cyst Coexistence with Pituitary Adenoma/Pituitary Neuroendocrine Tumor.

INTRODUCTION: Collision tumors are extremely rare in the sellar region, less than 1 to 2% of sellar masses. Intracranial dermoid cyst is a rare, benign, slow-growing lesion accounting for 0.04 to 0.7% of all intracranial tumors. Sellar dermoid cysts are extremely rare; from 1976 to 2024, there were only 59 records of sellar dermoid cyst published in the English language. Coexisting sellar dermoid cyst and pituitary adenoma have not been reported in the literature. CASE PRESENTATION: A 51-year-old man having medical history of left traumatic optic neuropathy and acute lymphoblastic leukemia po

Endoscopic assisted minimally invasive resection of petrous bone epidermoid cyst via a presigmoid burr hole approach: illustrative case.

BACKGROUND: Intracranial epidermoid cysts are rare congenital lesions accounting for approximately 1% of intracranial tumors. Petrous apex involvement is uncommon and surgically challenging because of the lesion's proximity to critical neurovascular structures. Reports describing minimally invasive endoscope-assisted approaches in this location remain limited. OBSERVATIONS: A 32-year-old man presented with progressive right-sided otalgia, pulsatile tinnitus, and hearing loss. Imaging demonstrated a destructive petrous temporal bone lesion with atypical diffusion characteristics, initially sus

Intrapetrous Internal Carotid Artery Pseudoaneurysm Secondary to Malignant Otitis Externa: A Case Report.

Malignant otitis externa (MOE) can rarely progress to skull base osteomyelitis (SBO) with catastrophic vascular complications. We describe a 68-year-old man with uncontrolled diabetes (HbA1c 9%) who presented with left otalgia, purulent otorrhea, and hearing loss. Initial workup showed CRP 23 mg/L, leukocytes 8,000/µL, and ear pus culture positive for Pseudomonas aeruginosa. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated MOE with temporal bone osteomyelitis. Despite prolonged intravenous antibiotics (imipenem, colistin) and topical ofloxacin, symptoms persisted. Thr

Primary Dural-based diffuse large B-cell lymphoma mimicking meningioma in the olfactory groove: An unique case report and brief review of the literature.

Primary central nervous system lymphomas (PCNLs) are malignant non-Hodgkin lymphomas solely affecting the central nervous system (CNS). Primary dural lymphoma, which is rare in the CNS, has not been previously reported in a study of the olfactory groove. We present the clinical course of a 77-year-old immunocompetent male who presented with nonspecific symptoms of fatigue and transient memory loss. Although the patient's neurological examination was unremarkable, cranial imaging identified a lesion highly suspicious for an olfactory groove meningioma. Consequently, the patient underwent a bifr

Transcranial arterial coagulation with cardiac standstill after catastrophic frontopolar artery injury during endoscopic endonasal resection of tuberculum sellae meningioma: illustrative case.

BACKGROUND: Arterial injuries are among the most dangerous complications of endoscopic endonasal skull base surgery (EESBS), posing a risk of massive hemorrhage and requiring immediate management. Although uncommon, such events can be life-threatening. This report describes the first documented case of using a transcranial approach to control an arterial injury sustained during EESBS, with hemostasis achieved via bipolar diathermy. OBSERVATIONS: During endoscopic resection of a tuberculum sellae meningioma, profuse bleeding from the right frontopolar artery occurred. Initial control measures-

Optimizing outcome in radiosurgery for spheno-orbital meningioma: the critical role of planning quality. Illustrative case.

BACKGROUND: Spheno-orbital meningiomas (SOMs) pose significant challenges due to their proximity to critical structures, such as the optic nerve. This report underscores the pivotal role of meticulous planning in achieving favorable outcomes with Gamma Knife radiosurgery (GKRS). OBSERVATIONS: A 54-year-old woman with a left SOM encasing the optic nerve underwent GKRS after being deemed unsuitable for surgery. The radiosurgical plan was meticulously tailored to balance effective tumor control with optic nerve sparing. A prescribed dose of 10 Gy at the 50% isodose line ensured a maximum optic n
