[The transillumination technique. An alternative to conventional intubation?].

The technique of light-guided intubation is based on the principle that a source of light brought into the trachea results in clearly visible and defined transcutaneous illumination, while no illumination can be observed with the light source in the oesophagus (Fig. 1-7). The Trachlight is a reintroduced instrument for this alternative intubation technique. The essential developments are: a length-adjustable stylet with a removable internal metal wire, a brighter light source, a stable handle with tight fixation of the endotracheal tube, and a time-dependent warning device to avoid extended in

BaTiO3-based Piezoelectric Wireless Neuromodulation: From Molecular Mechanisms to Clinical Translation Challenges.

Neurological injuries remain difficult to treat due to the complex microenvironment and the limited intrinsic regenerative capacity of neural tissues. Recently, wireless neuromodulation based on piezoelectric nanomaterials, particularly barium titanate (BaTiO3), has emerged as a promising alternative to conventional implantable electrical stimulation. Polar BaTiO3 ceramics generally have higher piezoelectric charge coefficients than conventional Polyvinylidene Fluoride (PVDF) and Poly(vinylidene fluoride-co-trifluoroethylene) (P(VDF-TrFE)) films, are more resistant to degradation than ZnO, and

Analysis of Functional Outcomes Following Surgical Management of Neglected Multiple Carpometacarpal Dislocations: A Series of four Cases and Review of Literature.

INTRODUCTION: Dislocation of the carpometacarpal (CMC) joint is a rare injury that results following high-velocity trauma. Although there are a few anecdotal reports of isolated CMC joint dislocations, there is scarce literature concerning dislocations involving multiple joints, especially those that are neglected and present late after injury. The injury is often missed when the patient presents to the surgeon since the swelling obscures the characteristic deformity, and the fracture may not be apparent on radiographs. When missed at the initial presentation, they can frequently result in pai

Nerves in Continuity Following Hand Trauma: A Descriptive Report.

BACKGROUND: Understanding the clinical presentation of nontransected nerve injuries in acute hand trauma/wrist trauma will help in early intervention, which is vital for maximizing return of function in patients. This retrospective study evaluated patients who experienced traumatic hand injuries with nerve in continuity within the zone of injury. METHODS: This was a single-center retrospective chart review of 20 patients with hand or wrist trauma resulting in damage to bone, tendon, or soft tissues within Zones II to V. Patients were 18 to 70 years of age and had documented visualization of a

Healing an ischial tuberosity pressure ulcer in a patient with neurogenic heterotopic ossification: a case report.

Neurogenic heterotopic ossification (NHO) is widely recognised as an aberrant bone formation in soft tissue following central nervous system injury. It is most frequently associated with pain and limited movement, especially in the hip. However, it may be neglected in patients with paraplegia with a pressure ulcer (PU). We report the case of an 18-year-old male patient who presented with a hard-to-heal ischial tuberosity PU and who had undergone three operations at other hospitals during the previous six months, which had failed to repair the PU. There was a history of paraplegia as a conseque

Bosworth ankle fracture-dislocation: current concept review.

Bosworth fracture (BF) is a special type of locked ankle fracture-dislocation, characterized by displacement of a fragment of the fractured fibula from the fibular notch behind the posterior surface of the distal tibia. BF is a complex injury affecting multiple structures of the ankle joint, which is still frequently misjudged even today, potentially leading to severe complications. CT examination, including 3D reconstructions, should be the diagnostic standard in BF, as it provides a complete picture of the fracture pathoanatomy, most prominently the morphology of the frequently associated po

Medial Patellar Retinaculum Cable Transfer With Anchoring Suture Reinforcement and Medial Tightening to Correct Patellar Dislocation in Skeletally Immature Patients.

Surgical treatment of patellar dislocation poses a challenge in skeletally immature patients. The traditional approach of lateral release and medial imbrication carries a high probability of recurrence. Medial patellofemoral ligament reconstruction recently has become a popular procedure for patellofemoral instability, but its broad application in immature skeletal systems is limited by the risk of injury to epiphyseal plate. Herein, we describe an alternative surgical strategy whereby medial patellar retinaculum cable transfer is done in conjunction with anchoring suture reinforcement and med

Endoscopic Release of the Ulnar Nerve at the Elbow: Technique Description and Case Series.

Objective The endoscopic release of the ulnar nerve reproduces a simple ( in situ ) procedure with smaller incisions, less soft tissue damage, and higher preservation of nerve vascularization. Endoscopy allows the clear visualization of the entire path of the nerve and surrounding noble structures. Moreover, it reveals any signs of compression and allows a safe release of 10 cm distally or proximally to the medial epicondyle. Methods A retrospective survey revealed that 15 subjects (1 with a bilateral injury) underwent an ulnar nerve compression release at the elbow using the endoscopic techni

The role of the perioperative prone position in the low recurrence of pressure injuries in the pelvic region.

OBJECTIVE: A pressure injury (PI) is a localised area of damage to the skin and/or underlying soft tissue as a result of a sustained mechanical loading. There are three key aetiological mechanisms to PI formation-direct cell deformation, inflammatory oedema and ischaemic damage-which are typically activated sequentially to drive a spiral of injury. This article discusses the role of the perioperative prone position as a rational approach to reducing the recurrence of pelvic PI after reconstructive surgery. METHOD: Patients with deep PI in the pelvic region, who were operated on from 2011 to 2

An All-Arthroscopic Technique for Complex Posterolateral Corner Reconstruction.

Injuries to the posterolateral corner (PLC) often result in lateral, rotational, and dorsal instability, which need appropriate and differentiated treatment. Besides posterior cruciate ligament reconstruction for posterior instability, the technique according to LaPrade et al. efficiently stabilizes posterolateral rotational and lateral instability as described in Fanelli type B or C injuries. This technique has been exclusively used as an open procedure. In this article, we present an all-arthroscopic technique for the posterolateral stabilization procedure. To achieve this, 5 different arthr

Asymptomatic pellet migration to the heart: report of a case and update on proper management.

We report a case of a 42-year-old male patient who was transferred to our emergency department suffering from a gunshot wound in his left lateral thigh. The patient was haemodynamically stable, and the physical examination of the abdomen and thorax was unremarkable. There was no obvious exit point and there were no other injuries. The radiologic control of the left thigh showed an intact femur and multiple pellets within the adjacent soft tissues. Routine X-ray evaluation of the thorax revealed a small-sized round object of metal density-possibly a migrated pellet-in the proximity of the right

Compartmental anatomy of the superficial fibular nerve with an emphasis on fascial release operations of the leg.

Compartment syndrome is a common cause of lower extremity pain via an increased intra-compartmental pressure. Fasciotomy is currently the mainstay for surgical treatment. Fasciotomy can be performed either with classical open or minimally invasive techniques including endoscopically assisted or semi blind subcutaneous releases. Incompletely released fascial compartments, soft tissue damage, and neurovascular injury, which includes superficial fibular nerve injury are common complications. The aim of this study was to investigate the localization of the superficial fibular nerve (SFN) in the la

Occipito-atlantal dislocation: an uncommon case of cervical spine injury.

Occipito-atlantal dislocation (OAD) is a rare but often lethal type of spinal injury that can be found in up to 35% of traffic fatalities. Improved pre-hospital care, diagnosis, and immobilization of these patients have led to an increase in reported cases of survivors. Significant ligamentous disruption in OAD results in craniocervical instability. While the vast majority of patients will not survive, those who do present with a wide variety of neurological findings ranging from no deficit to quadriparesis with cranial nerve palsies. When the appropriate landmarks can be identified OAD can be

[Locked nailing of fractures of the long bones. 1. Fractures of the leg].

The authors present an account of their initial experience with the use of a method of locked nailing of fractures of the leg, applying the technique described by Grosse and Kempf. They use original implants of Howmedica Co. They indicated for osteosynthesis closed fractures of the leg in the middle three fifths of the bone length, without major damage of soft tissues (grade 0 and 1 of Tscherne's classification). In injuries with imminent compartment syndrome they operated after a delay, in all others they operated early after injury. Using the described method, the authors operated 16 patient

Femoral shaft fracture treated with Hoffmann external skeletal fixator in young teens.

External skeletal fixation has been proved to be an effective method in treating complex femoral shaft fracture in adult patients with multiple trauma, extensive soft tissue injury or vascular injury which need reconstructive procedures. This method has the advantages of less extensive surgical exposure, easy wound access, and early ambulation. Twenty-two cases of femoral shaft fracture, between the ages of 10 and 15, were treated with the Hoffmann external skeletal fixator in Chang Gung Memorial Hospital from 1981 to 1985. The average time for union and removal of fixator was 72 +/- 8 days (f

Effect of temporary vascular shunting as a previous intervention on lower extremity arterial injury: Single center experiences in the Syrian Civil War.

BACKGROUND: The goal of this retrospective study was to clarify the effect of using temporary vascular shunt (TVS) as a previous intervention. METHODS: A total of 96 cases with war-related lower extremity arterial injury and surgically treated between October 2013 and March 2016 were included in the study. The patients were divided into two groups: those in which TVS was performed as a previous intervention on admission (TVS group, n=24) and those in which compression, tourniquet, and ligation/clampage were performed as a previous intervention on admission (non-TVS group, n=72). RESULTS: In
