Osteitis Condensans Ilii.

Osteitis Condensans Ilii (OCI) is a benign cause of axial low back pain. Although no clear etiology has been identified, the prevailing theory is that mechanical strain affects the auricular portion of the ilium and causes premature arthritis. The location of the sclerosis has been traditionally confined to the ilium and may give the false impression of sacro-iliac joint involvement. Clinicians must be guided by history, radiographic findings, and laboratory studies in differentiating OCI with other disorders; furthermore additional causes of low back pain including metastatic disease and anky

A case of infliximab-induced psoriasis.

Anti-tumor necrosis factor (anti-TNF-alpha) are a group of new drugs able to inhibit the action of this cytokine. Although systemic side effects have been well described, cutaneous adverse reactions have not yet been clearly elucidated. The authors report a case of a 29-year-old man affected by Crohn disease and ankylosing spondylitis who developed psoriatic lesions after IV infusion of infliximab 5 mg/Kg. The patient underwent cyclosporine treatment after interruption of biological therapy, and had complete resolution of cutaneous lesions. The reason for this phenomenon is not clear, Obviousl

Sarcoidosis, role of tumor necrosis factor inhibitors and other biologic agents, past, present, and future concepts.

Tumor necrosis factor is a potent cytokine involved in the inflammatory process of many diseases. Agents that block tumor necrosis factor have been used in the treatment of various immune-mediated diseases, including rheumatoid arthritis, Crohn disease, psoriatic arthritis, and ankylosing spondylitis. Sarcoidosis is an immune-mediated inflammatory disorder of unknown etiology characterized by the formation of noncaseating granulomas. Tumor necrosis factor plays a major role in the inflammatory process seen in sarcoidosis. Sarcoidosis therapies with activity against tumor necrosis factor and sp

Rheumatologic disease and associated ocular manifestations.

BACKGROUND: Ocular involvement is a common manifestation of rheumatic disease. Recognition of associated systemic findings strengthens our diagnostic armamentarium. METHODS: The previous ophthalmic and medical literature was reviewed and synthesized into a focused review of rheumatoid arthritis, systemic lupus erythematosus, polyarteritis nodosa; the seronegative spondyloarthropathies, including ankylosing spondylitis, Reiter's syndrome, psoriatic arthritis, ulcerative colitis and Crohn's disease. RESULTS: New and improved diagnostic and therapeutic measures have added a great deal to our un

[Initial findings using dynamic magnetic resonance tomography in the diagnosis of inflammatory diseases of the sacroiliac joint].

Up to now 42 patients with clinically suspected diagnosis of sacroiliitis (including 19 patients with confirmed spondylitis ankylosans) and 12 healthy volunteers were examined via magnetic resonance tomography. In oblique sections parallel to the plane of the sacrum the signal intensity patterns from normal and pathological tissues of the sacroiliac articulations were evaluated by plain T1-weighted spin-echo sequences and T2*-weighted FLASH sequences. In addition we used a dynamic FLASH measurement following intravenous application of gadolinium-DTPA for quantitative calculation of signal-time

Late complications after a Yersinia enterocolitica epidemic: a follow up study.

Ninety four Finnish conscripts were affected by a Yersinia enterocolitica epidemic in 1973. Thirteen years later 75 men completed a questionnaire about their present health. One half had no health problems, and the most common complaints in the other subjects were musculoskeletal disorders. Sixteen men wanted to be re-examined. In three cases a chronic connective tissue disease was diagnosed. Two men had ankylosing spondylitis. The most notable results of this study were (a) the fairly low number of late complications, (b) the close correlation between complications and the HLA-B27 antigen, an

Disseminated visceral infection with Mycobacterium fortuitum in a hemodialysis patient.

A 28-year-old patient with chronic renal failure on maintenance hemodialysis developed fever, granulomatous hepatitis, pulmonary infiltrates, sacroiliitis, and spondylitis which were unresponsive to conventional antituberculous treatment. Cultures of the sputum gave repeated growth of Mycobacterium fortuitum. This organism was resistant 'in vitro' to most antituberculous drugs and sensitive to some aminoglycosides and doxycycline. No mycobacteria were found in the water used for dialysis. The patient was successfully treated with amikacin and doxycycline. Nontuberculous mycobacterial infection

Monocyte cytotoxicity in connective tissue diseases. Correlation with disease groups.

The proportion of antibody-dependent cytotoxic plaque-forming cells (PFC) with monocyte characteristics was estimated in peripheral blood mononuclear leukocyte suspensions from 40 patients with rheumatoid arthritis (RA), (29 seropositive and 11 seronegative), 7 with ankylosing spondylitis (AS), 14 with juvenile rheumatoid arthritis (JRA) and 7 with systemic lupus erythematosus (SLE). In RA and also in AS patients, higher mean proportions of PFC were found when compared with those of normal adult blood donors. Seropositive RA patients had a significantly higher proportion of PFC than seronegati

Arthritis in myasthenia gravis.

Seven patients with myasthenia gravis developed clinical signs of arthropathy. In two patients, the symptoms were due to a deforming rheumatoid arthritis and the myasthenic symptoms appeared as a transitory phase during the course of the disease. Muscle antibodies of IgG class were demonstrated with sera from both patients. Autoreactivity between muscle antibodies and rheumatoid factor was detected in one patient. Both patients died from sudden cardiac failure. Necropsy was performed in one and revealed a spotty myocardial necrosis. One patient had juvenile rheumatoid arthritis. Two patients h

[Degenerative changes of the cervical spine. Imaging].

Standard radiography of the cervical spine is still the first imaging procedure applied in degenerative disease of the cervical spine. Spondylosis deformans, osteochondrosis and uncovertebral arthrosis are well depicted by such images. Whether or not additional imaging is needed depends on the clinical symptoms. Special projections (anteroposterior oblique radiographs, lateral radiographs in flexion and extension) may be the next step. For many abnormalities, mainly of the intervertebral discs and the myelon, MR imaging has become the standard procedure, largely because of its multiplanar capa

Diagnosis and management of axial spondyloarthritis.

Low back pain is among the most common conditions treated by physician associates in family medicine, internal medicine, and emergency medicine clinics. Inflammatory low back pain is the most frequent manifestation of axial spondyloarthritis (axSpA). AxSpA has a prevalence of 1.0% to 1.4% in the United States. The diagnosis of axSpA includes both clinical and radiographic components. With the advancement of biologic pharmacologic agents, the American College of Rheumatology, Spondylitis Association of America, and Spondyloarthritis Treatment Network have provided updated recommendations for th

Axial Spondyloarthritis: Updates in Concepts and Reporting in Radiology.

Axial spondyloarthritis (axSpA) is a chronic inflammatory disease primarily affecting the axial skeleton, historically termed "human leukocyte antigen B27 positive spondyloarthropathy" or ankylosing spondylitis. Improved understanding of the underlying pathologic mechanisms has led to an ongoing redefinition of this disease state, a recognition of the substantial phenotypic and biological variation, and an emphasis on earlier detection. Magnetic resonance imaging has become central to identifying and confirming early axSpA, critical to fulfilling the promise of slowing disease progression and

[Q Fever: what's new?].

Q fever is caused by Coxiella burnetii, a Gram-negative, intracellular bacterium. Sheep and goats are the main reservoirs of this zoonosis, which is transmitted either by aerosols, over long distances via the wind or through milk or dairy products. Acute Q fever may be asymptomatic, or present as a flu-like syndrome, atypical pneumonia or hepatitis. Chronic Q fever manifests in a wide variety of ways, including blood culture-negative endocarditis, chronic hepatitis, osteoarticular involvement (spondylitis, arthritis, osteitis) or uveitis. Autoimmune diseases can be sometimes induced by chronic

[Traumatic injuries in ankylosing spinal diseases].

Ankylosing spinal diseases, such as ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH), are highly important in spinal traumatology and are therefore specifically considered in the AO Spine Classification of spinal injuries. These diseases make the spine extremely susceptible to injury and also complicate the diagnosis and treatment, leading to an increased mortality. Concomitant neurological injuries are frequent. The treatment of such injuries requires great attention and careful preparation. Early and precise diagnostics using computed tomography (CT) and magnet

A Review of the Safety of Interleukin-17A Inhibitor Secukinumab.

Secukinumab is an anti-interleukin (IL)-17A IgG1-κ monoclonal antibody approved for psoriasis, psoriatic arthritis, and ankylosing spondylitis. Its efficacy is well documented, but the complete safety profile of secukinumab, especially on long-term use, needs to be studied. IL-17 inhibitors increase the risk of infections, especially respiratory tract infections and candidiasis, and inflammatory bowel disease; the causal relationships are well described. However, evidence regarding the other adverse events is scarce, and causal associations between the adverse events and the biologic remain un

Histopathology of the gut in rheumatic diseases.

The gastrointestinal tract regulates the trafficking of macromolecules between the environment and the host through an epithelial barrier mechanism and is an important part of the immune system controlling the equilibrium between tolerance and immunity to non-self-antigens. Various evidence indicates that intestinal inflammation occurs in patients with rheumatic diseases. In many rheumatic diseases intestinal inflammation appears to be linked to dysbiosis and possibly represents the common denominator in the pathogenesis of different rheumatic diseases. The continuative interaction between dys

Soft tissue infections with atypical mycobacteria in two patients with inflammatory rheumatic diseases using TNF-inhibitors and/or leflunomide.

Infections of the soft tissues due to atypical mycobacteria are relatively uncommon. We describe two cases. A 61-year-old woman with rheumatoid arthritis (RA) who was treated with the combination of the TNF inhibitor etanercept (Enbrel) and leflunomide (Arava), developed paronychia and cellulitis of the index finger due to Mycobacterium chelonae/abscessus complex. The patient was successfully treated with clarithromycin and surgical debridement. A second case describes a 50-year-old man with ankylosing spondylitis, receiving infliximab (Remicade) and low dose corticosteroids, who developed a g

MR imaging assessment of the spine: infection or an imitation?

Magnetic resonance (MR) imaging is a powerful diagnostic tool that can be used to help evaluate spinal infection and to help distinguish between an infection and other clinical conditions. In most cases of spinal infection, MR images show typical findings such as vertebral endplate destruction, bone marrow and disk signal abnormalities, and paravertebral or epidural abscesses. However, it is not always easy to diagnose a spinal infection, particularly when some of the classic MR imaging features are absent or when there are unusual patterns of infectious spondylitis. Furthermore, noninfectious

Awake tracheal intubation using the Airtraq laryngoscope: a case series.

The Airtraq laryngoscope (AL) is a new single use indirect laryngoscope designed to facilitate tracheal intubation in anaesthetised patients either with normal or difficult airway anatomy. It is designed to provide a view of the glottis without alignment of the oral, pharyngeal and tracheal axes. We report four cases of successful awake tracheal intubation using the AL. The first case is a patient with severe ankylosing spondylitis and the other three cases with anticipated difficult airway. An awake intubation under sedation and topical airway anaesthesia was chosen. We consider that the AL c

[Treatment of rheumatic patients in a warm climate abroad].

Adult rheumatic and psoriatic patients in Norway have been offered state-funded treatment in a warm climate since 1976. The offer has since then been extended to other patient groups with chronic diseases. We here present a program, which mainly consists of intensive physical treatment in a warm, sunny and dry climate, for adults with chronic rheumatic disease. The article is based on available statistics and literature found by searching Medline, PubMed and Cochrane. Patients are selected for the program according to diagnosis, physical function and disease severity and activity. The majority
