Differences in Ankle ROM Between Young Men and Women With a History of Ankle Sprain.

BACKGROUND: Ankle injuries are the most common injuries in sports. However, very little is known about sex differences in ankle range of motion (ROM) between ankle joints with an episode of ankle sprain (ASE) and those without an episode of ankle sprain (non-ASE). This study aimed to investigate the differences of ankle ROM between men and women with and without ASE. METHODS: 26 ASE had at least one episode of ankle injury in the last 6 to 12 months. 35 non-ASE served as a control group. All subjects were examined in supine position and the ankle ROM was calculated by the sum of inversion (IV

Pediatric Disorders of Orthostatic Intolerance.

Orthostatic intolerance (OI), having difficulty tolerating an upright posture because of symptoms or signs that abate when returned to supine, is common in pediatrics. For example, ∼40% of people faint during their lives, half of whom faint during adolescence, and the peak age for first faint is 15 years. Because of this, we describe the most common forms of OI in pediatrics and distinguish between chronic and acute OI. These common forms of OI include initial orthostatic hypotension (which is a frequently seen benign condition in youngsters), true orthostatic hypotension (both neurogenic and

Angular acceleration and angular jerk of elbow extension-flexion movement as parameters for discriminating a sequential transform of spontaneous movements in early infants.

Objective: We assessed the root mean square (RMS) of angular acceleration and the RMS of an angular jerk as expressions of the transformation of spontaneous movements in early infancy. Methods: During 36-56 weeks post-menstrual age (PMA), 15 premature infants (6 male, 9 female; 36 weeks PMA>) were measured every 4 weeks. A three-dimensional motion analyzer (Fastrak system; Polhemus Inc.) was used to measure spontaneous movements of the upper right limb in the supine infants. Upper limb position data were used to calculate the RMS of angular acceleration and the RMS of angular jerk at the elbow

MRI to measure cervical sagittal parameters: a comparison with plain radiographs.

INTRODUCTION: The inability to visualize the sternum tip and T1 vertebra on radiographs may decrease the accuracy of the measurements of cervical sagittal parameters. The object of this study is to investigate the value of MRI to measure cervical sagittal parameters and to compare the data of cervical sagittal parameters on radiographs and MR images. MATERIALS AND METHODS: Fifty-two asymptomatic adults were recruited. Each subject had both an MRI and radiographs of the cervical spine. Data, including C2-7 lordosis (CL), T1 slope (T1S), thoracic inlet angle (TIA), neck tilt (NT), and T1S minus

Myoclonus subtypes in tertiary referral center. Cortical myoclonus and functional jerks are common.

OBJECTIVE: To evaluate the accuracy of clinical phenotyping of myoclonus patients and to determine differentiating clinical characteristics between cortical (CM), subcortical (SCM), spinal (SM), peripheral (PM) myoclonus, and functional jerks (FJ). METHODS: Clinical notes for all patients with myoclonus over an 8-year period (2006-2014) were reviewed retrospectively. We used the conclusion of electrophysiological testing as definite diagnosis of myoclonus or FJ. RESULTS: 85 patients were identified suffering from CM (34%), SCM (11%), SM (6%), PM (2%), and 47% FJ. The clinical diagnosis of my

Cardiovascular autonomic dysfunctions in elderly patients with essential tremor: comparison with healthy controls.

Questionnaire-based analyses show that patients with essential tremor (ET) may have several autonomic dysfunctions, especially in the cardiovascular and genitourinary domains; yet the laboratory correlates of autonomic dysfunction in ET are unknown and have not been studied. Herein, we explored whether sympathetic and parasympathetic functions differed between control subjects and patients with ET. Seventy-five elderly patients with ET were enrolled in this study, along with 25 age-matched controls. Orthostatic vital signs, ambulatory 24-h blood pressure monitoring and 24-h Holter monitoring v

Early diagnosis and treatment of children with skull deformations. The challenge of modern medicine.

Skull deformations affect approximately 45% of newborn babies, the most common ones being: plagiocephaly, brachycephaly and scaphocephaly. The first symptoms can be observed 4 to 8 weeks after birth. The causes of skull deformation in newborns can be divided into congenital ones and those acquired after birth. An increase in the incidence of acquired head deformations can be attributed to the "BACK TO SLEEP" campaign, carried out in 1992 by the American Academy of Pediatrics (AAP), which was aimed to reduce the frequency of sudden infant death syndrome (SIDS) by placing babies to sleep in the

Does evaluator experience have an impact on the diagnosis of lumbar spine instability in dynamic MRI? Interobserver agreement study.

ObjectivesWe aimed to evaluate interobserver agreement in the definition of spine instability among spine neuroradiologists with or without experience in dynamic magnetic resonance imaging (MRI).Material and methodsTwo expert neuroradiologists and two residents retrospectively evaluated the pre-operative dynamic MRI examinations of patients with vertebral instability. Segmental motion, defined as excessive (more than 3 mm) translational motion from supine to upright, was investigated in 103 subjects (309 segments) using kinetic MRI. Radiographic parameters which can help indicate segmental ins

Heart rate variability and arterial oxygen saturation response during extreme normobaric hypoxia.

The primary purpose of this study was to assess the response of autonomic cardiac activity and changes in the arterial oxygen saturation (SpO2) during normobaric hypoxia and subsequent recovery. Heart rate variability (HRV) and SpO2 were monitored in a supine position during hypoxia (FiO2=9.6%) for 10min, and normoxic recovery in 29 subjects. Spectral analysis of HRV quantified the autonomic cardiac activity by means of low frequency (LF) (0.05-0.15Hz) and high frequency (HF) (0.15-0.50Hz) power transformed by natural logarithm (Ln). Based on the SpO2 response to hypoxia, the subjects were div

The effect of different ranges of motion on local dynamic stability of the elbow during unloaded repetitive flexion-extension movements.

Local dynamic stability (LDS) of movement is controlled primarily by active muscles, and is known to be influenced by factors such as movement speed and inertial load. Other factors such as muscle length, the length of the target trajectory, and the resistance of passive tissues through ranges of motion (ROM) may also influence LDS. This study was designed to examine the effect of ROM, which impacts each of the aforementioned factors, on LDS of the elbow. 16 participants performed 30 unloaded, repetitive, flexion-extension movements of the elbow with varying (1) angular displacement magnitudes

[Laparoscopic robot-assisted partial nephrectomy with total ureterectomy in a symptomatic complete duplicated system: advantages of transperitoneal approach].

We report the case of a laparoscopic robot assisted left upper polar partial nephrectomy with total ureterectomy performed in a teenager. A 14 year-old girl was referred to our institution for stress urinary incontinence. The morphological assessment (ultrasound scan and uro-MRI) showed a double collecting system with a complete ureteral duplication complicated by a dysplasia of the upper moiety of the duplex left kidney and a mega ureter. The surgery started on a lateral decubitus position by the upper polar partial nephrectomy and the ureter section behind superior polar renal vessels. The p

Monitoring of oesophageal pressure.

PURPOSE OF REVIEW: Studies in patients with acute respiratory distress syndrome (ARDS) have been unable to demonstrate a survival advantage with higher levels of positive end-expiratory pressure (PEEP) to open atelectatic lung regions or prevent their cyclic collapse. This review will discuss the challenges of accurately measuring pleural pressure with balloon-tipped catheters in the oesophagus, and the utility of such pressure monitoring to set PEEP and assess lung mechanics, focusing on patients with ARDS. RECENT FINDINGS: Recent investigations have suggested that the monitoring of oesophag

Catheterized uroflowmetry as a noninvasive test for detrusor acontractility.

OBJECTIVE: To evaluate the efficacy of uroflowmetry performed through an indwelling catheter on the differential diagnosis of detrusor acontractility. PATIENTS AND METHODS: 50 men aged between 51 and 85 years (mean 66 years) presenting to the outpatient urology department with indwelling catheters due to urinary retention were included in the study. In the supine position, 300 ml of saline was instilled into the bladder and the catheter was blocked; with the patient standing by the flowmeter, the catheter was opened, allowing the patient to void through the catheter. The evaluation continued

A mini-anterior approach to the hip for total joint replacement: optimising results: improving hip joint replacement outcomes.

Direct anterior approaches to the hip have gained popularity as a minimally invasive method when performing primary total hip replacement (THR). A retrospective review of a single institution joint registry was performed in order to compare patient outcomes after THR using the Anterior Supine Intermuscular (ASI) approach versus a more conventional direct lateral approach. An electronic database identified 1511 patients treated with 1690 primary THRs between January 2006 and December 2010. Our results represent a summary of findings from our previously published work. We found that patients tha

Limited utility of plain abdominal radiographs in evaluating intussusceptions secondary to long indwelling feeding tubes.

Adult intussusception (AI) is relatively rare and can be a difficult clinical diagnosis, often requiring cross-sectional imaging for confirmation. Unfortunately, intussusceptions in the setting of indwelling long enteral feeding tubes have been predominantly characterized in the pediatric population with minimal investigation in adults. We report three cases of AI in patients with long feeding catheters serving as anatomic lead points leading to intussusception diagnosed on cross-sectional imaging. We highlight the limited utility of the supine plain films for detection of AI, and it behooves

Venlafaxine-induced orthostatic hypotension in a geriatric patient.

Venlafaxine is not usually associated with risk of orthostatic hypotension. A 65-year-old US Caucasian female taking 225 mg/day of venlafaxine extended-release developed symptomatic orthostatic hypotension. The systolic and diastolic blood pressure dropped by 25 and 18 mm Hg, respectively, from supine position to standing position within 3 minutes. The patient was otherwise healthy and the orthostatic hypotension resolved with venlafaxine discontinuation. This was a probable venlafaxine adverse drug reaction according to the Naranjo scale. This case contributes to the scarce literature that in

Cerebral blood flow dynamics and head-of-bed changes in the setting of subarachnoid hemorrhage.

Head-of-bed (HOB) elevation is usually restricted in patients with aneurysmal subarachnoid hemorrhage (SAH). The goal of this study is to correlate HOB changes (0° and 90°) with cerebral blood flow using transcranial Doppler (TCD) and thermal diffusion probe in SAH patients. Thirteen patients with SAH were prospectively enrolled in the study. Eight patients underwent placement of a thermal diffusion probe for regional CBF measurement. CBF values were measured with the patients in flat (0°) and upright sitting positions (90°) at days 3, 7, and 10. The average increase in blood flow velocity whe

Foot disorders, foot posture, and foot function: the Framingham foot study.

INTRODUCTION: Foot disorders are common among older adults and may lead to outcomes such as falls and functional limitation. However, the associations of foot posture and foot function to specific foot disorders at the population level remain poorly understood. The purpose of this study was to assess the relation between specific foot disorders, foot posture, and foot function. METHODS: Participants were from the population-based Framingham Foot Study. Quintiles of the modified arch index and center of pressure excursion index from plantar pressure scans were used to create foot posture and f

Intracranial hypotension in the setting of concurrent perineural cyst rupture and subarachnoid hemorrhage.

Although most patients with intracranial hypotension typically present with headaches, the rest of the clinical spectrum is characteristically non-specific and often quite variable. In a patient with concurrent pathologies that can produce a similar clinical picture, a high index of suspicion must be maintained to achieve the correct diagnosis. The authors report a patient with intracranial hypotension in the setting of concurrent perineural cyst rupture and subarachnoid hemorrhage. A 63-year-old woman with a family history of ruptured intracranial aneurysms presented after a sudden thundercla
