Arthroscopic repair of the floating posterior-inferior glenohumeral ligament lesion.

We present our technique for diagnosis and arthroscopic repair of a floating posterior-inferior glenohumeral ligament avulsion injury. A high degree of suspicion based on the patient's history, along with careful examination, should alert the surgeon to the existence of this lesion. By use of an anterolateral viewing portal and precisely placed posterior working portals, the lesion is repaired through suture anchor fixation to the humeral head and glenoid rim in a carefully sequenced fashion to achieve appropriate tension and restore the anatomy.

Iatrogenic external iliac artery disruption during open pelvic lymph node dissection: successful repair with hypogastric artery transposition.

We report the first case of a wide, iatrogenic, proximal disruption of the right external iliac artery, occurring during staging open lymph node dissection for prostate cancer, which was repaired by hypogastric artery transposition. The hypogastric artery was mobilized and rotated anteriorly, and sutured to the distal segment of the external iliac artery. This is a feasible, innovative and safe technique which permits, by a single anastomosis, the secure reconstruction of a vascular axis to the leg when other procedures are not accessible.

[Reposition and suture fixation of dislocated posterior chamber lenses].

We treated 11 patients with posterior chamber intraocular lenses that were dislocated into the vitreous cavity. Using a pars plana approach, vitrectomy was performed and in order to facilitate manipulation, half of the bulbus volume was filled by perfluorodecaline. The IOL was grasped with forceps and repositioned with a special technique. In this way the first haptic is fixed in the ciliary sulcus. Fixation of the second haptic with 9 x 0 polypropylene suture was performed with a similar procedure. Finally, prophylactic cryotherapy of the retina is indicated.

[The apical meatus in surgical treatment of hypospadias].

We present a surgical technique for the correction of hypospadias which has the major advantage of locating the new meatus at the apex of the glans. The prepuce is opened out and fashioned into a pediculated strip which is used to form the new penile and distal portion of the urethra. This pediculated strip is placed longitudinally along a deep cleft created by an incision along the glans. The five surgical stages are described together with the sutures and diversions employed. The results and complications obtained in 132 boys are then analysed.

[Arterio-venous popliteal fistula: a rare late complication of a civilian trauma. A clinical case and review of the literature].

In spite of the relatively high frequency of popliteal injuries following war and civilian traumas, a late development of an arteriovenous fistula can be considered an uncommon complication. We report a case of a chronic arterio-venous popliteal fistula in a young boy, caused five years previously by a blunt trauma, while playing. The tardive onset of symptomatology and the conspicuous enlargement of the vein underline the unusuality of the case. A review of the literature and the technique for fistula repair, using adsorbable suture, are examined.

A modification of the pterygoid plate separation in low-level maxillary osteotomies.

The technical details of a modified procedure for separation of the pterygoid plates in low-level maxillary osteotomies is described. The cut is made through the tuberosity and ends at the posterior aspect of the hard palate, medial and anterior to the pterygomaxillary fissure. This technique provides several advantages over the more classically described posterior vertical cut through the pterygomaxillary suture. The major advantages are that it is anatomically less hazardous and allows greater mobility of the segments. It also permits a larger vascular pedicle to be maintained.

[Enucleation: causes of extrusion of orbital implants (author's transl)].

The frequency and the causes of extrusion of orbital implants have been investigated. Of the 294 patients in which enucleation was done, in 17 cases (5.8%) extrusion of the first implant developed; in 9 cases with second or third implantations another 5 implants have been extruded. The extrusion of implants is caused mainly by the operative technique and only rarely by intolerance reactions. For a reliable healing exact sutures of the muscles and also exact closure of the implant with plenty of covering tissue is important.

Rapid, safe and simple method for grafting whole thymus in the mouse.

A rapid, safe and simple method is presented for obtaining and grafting whole neonatal thymuses in mice. No anesthesia, sutures or incisions are required for implantation. The grafting technique involves no special skills except the ability to inject mice subcutaneously. Thymuses grafted by the method described show normal architecture, survive lethal irradiation and bone marrow reconstitution, repopulate thymic dependent areas of peripheral lymphoid organs and restore normal inflammatory cell responses to thymic deprived mice.

Repair of the Thumb Ulnar Collateral Ligament With Suture Tape Augmentation.

One of the most commonly injured structures of the thumb metacarpophalangeal (MCP) joint is the ulnar collateral ligament (UCL). An acute injury of this ligament is often amenable to primary repair. Despite the favorable outcomes of primary repair, the thumb is often restricted during initial healing of the ligament with immobilization and delayed rehabilitation. We present a novel technique to augment the repair of the UCL with suture tape to provide immediate biomechanical support and strength during the critical time of ligament healing. We describe the surgical technique of suture tape aug

Modified palatal strip grafting for keratinized tissue augmentation: A technical report.

BACKGROUND: Previously reported techniques on strip grafting have utilized either multiple palatal strips or a single palatal strip in conjunction with a collagen matrix, and have primarily been described for increasing the peri-implant keratinized mucosa width. However, the application of a palatal strip graft for keratinized tissue (KT) augmentation around teeth has not been reported to date. This technical report describes a modified approach to augment KT around teeth using a single palatal strip without adjunctive use of biomaterials. METHODS: The modified palatal strip graft (mPSG) tech

Arthroscopic Anterior Labral Repair and Remplissage in a Young, Active Athlete.

BACKGROUND: Arthroscopic remplissage is an effective adjunct for anterior shoulder stabilization in patients with large engaging Hill-Sachs lesions (HSLs) and without significant glenoid bone loss or on-track HSLs with high risk of recurrence. INDICATIONS: The arthroscopic remplissage shown was performed in a patient with a high-risk profile for recurrence (age <25, near track) following a first-time traumatic anterior dislocation event. In this specific case, the remplissage was used to fill a near-track HSL, a significant risk factor for recurrence. TECHNIQUE DESCRIPTION: Following diagnos

A new technique in primary repair of congenital esophageal atresia preventing anastomotic stricture formation and describing the opening condition of blind pouch: plus ("+") incision.

Anastomotic strictures are common and important problems following repair procedures of esophageal atresia. We hereby defined an anastomosis technique that could efficiently prevent this complication in 11 patients with esophageal atresia (EA) and tracheoesophageal fistula (TEF). The proximal end of the atretic esophagus was opened with a plus ("+")-shaped incision providing sufficient anastomosis width. Longitudinal incisions of 2 mm length were made on the anterior and posterior parts of the distal end according to the patients. The two ends were anastomosed with a primary suture at a single

Surgical technique for the correction of postpalatoplasty fistulae of the hard palate.

Postpalatoplasty fistulas of the hard palate remain challenging. The reported recurrence rate after surgical correction ranges between 33 and 37 percent. The authors describe a surgical technique for the correction of postpalatoplasty fistulas by total elevation of the palatal gingivoperiosteum starting at the dental sulcus, excision of the fistula, and watertight separate closure of the nasal and palatal mucosa by means of the transoral approach. This technique has the advantages of providing adequate exposure for closure of the nasal mucosal gap and allowing closure of the palatal defect und

Areolar vertical approach (AVA) mammaplasty: Lejour's technique evolution.

The areolar-vertical approach (AVA) mammaplasty, derived from the Vertical Mammaplasty described by Lejour, offers us the opportunity to achieve good cosmetic results in breast reduction even in larger hypertrophies and makes it available to all patients. In marking, the skin there are certain fixed landmarks, but the final skin design is obtained by dynamic maneuvers (points A, V, and S). They do not follow a rigid pattern. This technique is based on the superior areolar pedicle and parenchymal resection, mostly from the central-lower pole. Shape and final contour rely on breast parechymal su
