Correlates of suicidal ideation among HIV-positive persons.

OBJECTIVES: The present investigation sought to determine the extent to which demographic characteristics, illness-related burdens, alcohol and other substance use, and psychosocial factors are independently associated with suicidal ideation in HIV-positive individuals. DESIGN: HIV-positive individuals in four US cities (San Francisco, Los Angeles, Milwaukee, and New York City) were screened between July 2000 and January 2002 for recruitment into a randomized behavioral prevention trial. Utilizing data from this screening visit, rates and correlates of suicidal ideation were examined in a div

Nirmatrelvir-ritonavir targeting viral persistence in post-COVID-19 condition (long COVID) in the USA (RECOVER-VITAL): a randomised, double-blind, placebo-controlled, phase 2 trial.

BACKGROUND: Post-acute sequelae of SARS-CoV-2 infection, more commonly known as long COVID, has emerged as a major health problem. The pathogenesis of long COVID is unknown, but among the leading hypotheses is viral persistence. We aimed to investigate whether the use of the SARS-CoV-2 antiviral nirmatrelvir-ritonavir improved long COVID symptoms. METHODS: We conducted a double-blind, placebo-controlled, randomised trial involving adults who had developed persistent symptoms (≥12 weeks) associated with three major symptom phenotypes (cognitive, autonomic, or exercise) after acute SARS-CoV-2 i

The Hidden Costs of ICU Caregiving: Economic, Mental Health and Spiritual Consequences.

Background: Family caregivers of intensive care unit (ICU) patients face a double burden: the psychological toll of critical illness and the economic and occupational disruptions that often accompany prolonged caregiving. While prior research has examined caregiver distress, few studies have systematically integrated economic, psychological and spiritual domains over long-term follow-up. Methods: This study presents a cross-sectional analysis conducted at long-term follow-up examining economic, occupational, psychological, and spiritual correlates among family caregivers of former ICU patients

"No One Can Tell Me How Parkinson's Disease Will Unfold": A Mixed Methods Case Study on Palliative Care for People with Parkinson's Disease and Their Family Caregivers.

BACKGROUND: Palliative care for persons with Parkinson's disease (PD) is developing. However, little is known about the experiences of patients with PD in the palliative phase and of their family caregivers. OBJECTIVE: To explore needs of patients with PD in the palliative phase and of their family caregivers. METHODS: A mixed methods case study design. Health care professionals included patients for whom the answer on the question "Would you be surprised if this patient died in the next 12 months?" was negative. At baseline, and after six and twelve months, we conducted semi-structured inte

Fragmented and failed swallows on esophageal high-resolution manometry associate with abnormal reflux burden better than weak swallows.

BACKGROUND: Association between proportions of hypomotile swallows on esophageal high-resolution manometry (HRM) and esophageal reflux burden remains incompletely understood. We investigated relationships between hypomotility, acid exposure time (AET), and mean nocturnal baseline impedance (MNBI) on ambulatory reflux monitoring. METHODS: Clinical data, HRM, and ambulatory pH-impedance studies (performed off acid suppression) from patients with persisting reflux symptoms were reviewed from five international centers. AET (abnormal > 6%) and MNBI (abnormal < 2292 ohms) were extracted from pH-im

Effects of an amino acid mixture on alcohol metabolism and alcohol-related symptoms in healthy adults.

Alcohol consumption is associated with metabolic burden and a range of alcohol-related symptoms. Amino acids have been suggested to support hepatic metabolism and recovery following alcohol consumption. This study investigated the effects of Amino Liver (AL), an amino acid mixture, on alcohol metabolism and alcohol-related symptoms in healthy adults. In a randomized, double-blind, placebo-controlled, crossover clinical trial, healthy adults consumed AL or placebo prior to alcohol consumption. Blood alcohol and acetaldehyde concentrations were measured at predefined time points up to 13 h after

Analgesia through FKBP51 inhibition at disease onset confers lasting relief from sensory and emotional chronic pain symptoms.

Chronic pain affects 20 to 30% of the population and imposes a significant socioeconomic burden as it is often accompanied by substantial emotional comorbidities such as anxiety and depression. Yet, the mechanisms underlying the interactions between the sensory and emotional aspects of chronic pain remain poorly understood. Here, we investigated the role of FKBP51, a regulator of the stress response, in mediating both sensory and emotional symptoms of chronic pain. Inhibition of FKBP51, via genetic deletion or pharmacological blockade, in persistent joint pain reduced fast-onset sensory, funct

Regional and gender disparities in depression and late life expectancy in Chile.

BACKGROUND: Mental health disorders, including depression, are among the top 10 causes of the Global Burden of Disease in 2021. Chile is among the countries where mental health is a public concern due to a significant increase in depression rates in the older adult population in recent years. Considering the accelerated aging process, this study analyses the potential years of depressive symptom-free life expectancy in later life, including long-term conditions, from territorial and gender perspectives. METHOD: Panel data were used to track depressive symptomatology in 2,263 older individuals

Comparison of question wording for self-reported traumatic brain injuries among adults: findings from the 2023 SummerStyles survey.

BACKGROUND: Surveillance of traumatic brain injury (TBI) in the USA has historically relied on healthcare administrative datasets, but these methods likely undercount the true burden of TBI. Self-report measures may help obtain more comprehensive estimates. A fundamental question is whether TBI prevalence estimated through self-report varies by whether and how TBI signs and symptoms are ascertained in the survey question(s). METHODS: Self-report data were collected from 4303 adults in the summer wave of Porter Novelli's 2023 ConsumerStyles survey. Respondents were randomised to receive a ques

Biofluid-based predictors of post-concussion symptoms: a narrative review of mild traumatic brain injury biomarkers.

Mild traumatic brain injury can disrupt brain function and is associated with high morbidity and healthcare utilization. While many individuals recover from mild traumatic brain injury, a significant proportion experience long-term sequelae, collectively known as post-concussion syndrome. Symptoms of post-concussion syndrome include headache, dizziness, insomnia, cognitive processing difficulties and mental health disturbances. The disease burden is augmented by the current lack of objective measures to accurately predict long-term symptoms and deficits, providing an opportunity to utilize bio

Gastrointestinal symptoms in neuronal ceroid lipofuscinoses (NCLs): an observational study on prevalence, timing of progression, and impact on quality of life.

Neuronal ceroid lipofuscinoses (NCLs) are rare, inherited lysosomal storage disorders (LSDs) of the brain and retina. Gastrointestinal (GI) manifestations are reported in clinical practice but remain largely underexplored. Lysosomal dysfunction affects both the central and the peripheral nervous systems, including the enteric nervous system (ENS) where accumulation of storage material and neuronal loss may directly contribute to GI dysmotility. These insights, combined with emerging gene therapies targeting both brain and ENS pathology, underscore the need for validated GI endpoints in NCL res

Definitions and symptoms of the post-COVID syndrome: an updated systematic umbrella review.

Post-COVID syndrome (PCS) describes a persistent complex of symptoms following a COVID-19 episode, lasting at least 4 to 12 weeks, depending on the specific criteria used for its definition. It is often associated with moderate to severe impairments of daily life and represents a major burden for many people worldwide. However, especially during the first two years of the COVID-19 pandemic, therapeutic and diagnostic uncertainties were prominent due to the novelty of the disease and non-specific definitions that overlooked functional deficits and lacked objective assessment. The present work c
